BACKGROUND Gallbladder stones combined with common bile duct stones are frequently encountered.Laparoscopic choledocholithotomy is an effective treatment,but the optimal bile duct closure method primary closure vs T-tube drainage remains controversial.Primary closure may offer faster recovery and fewer tube-related complications,yet its safety and advantages over T-tube drainage require further validation.This study compared the effectiveness and safety of the two methods.AIM To assess the comparative effectiveness and safety of primary ductal closure vs conventional T-tube drainage in patients undergoing laparoscopic choledocholithotomy for gallbladder and common bile duct stones.METHODS Clinical data of 102 patients who underwent surgery in the First Department of General Surgery,Peking University Shougang Hospital,from October 2023 to October 2025,were retrospectively analyzed.The primary closure group consisted of 52 patients,and the T-tube drainage group had 50 patients.Factors for comparison were stone number and size,common bile duct diameter,stone clearance rate,liver function variables,operation time,drainage volume,indwelling time,postoperative hospital stay duration,total costs,and postoperative complications.Multivariate logistic regression was used to adjust for baseline differences in common bile duct diameter.RESULTS The two groups did not differ significantly in stone characteristics or clearance rates(P>0.05),although common bile duct diameter distribution differed between groups(P<0.05).After adjusting for common bile duct diameter and other covariates,primary closure remained associated with significantly shorter operative time,less drainage from the Winslow foramen,faster tube removal,shorter hospital stay,and lower costs(all P<0.05).Postoperative day 3 liver function parameters(total bilirubin,direct bilirubin,alanine aminotransferase,aspartate aminotransferase,alkaline phosphatase)were significantly lower in the primary closure group,but these differences should be interpreted with caution due to potential confounding by intraoperative factors.While bile leakage did not differ significantly between the two methods,the primary closure group suffered from significantly fewer electrolyte imbalances(P<0.05);the majority of electrolyte disturbances in the T-tube group were hypokalemia and hyponatremia,possibly related to biliary fluid loss.CONCLUSION Primary closure appears to be a safe and effective alternative to T-tube drainage during laparoscopic choledocholithotomy,with potential benefits in recovery and resource utilization.However,these findings are limited by the retrospective,single-center design and residual confounding;prospective studies are needed. 展开更多

BACKGROUND Endoscopic retrograde cholangiopancreatography(ERCP)is widely used technique for the diagnosis and treatment of pancreaticobiliary diseases.However,post-ERCP pancreatitis(PEP)has become the main issue restricting the extensive application of this technique,and there is an urgent need for more precise and targeted preventive strategies in clinical practice.AIM To explore the effect of indomethacin plus pancreatic duct stent implantation on high-risk populations for PEP.METHODS This prospective study included 386 patients who underwent ERCP in Peking University Shougang Hospital between April 2023 and March 2025 and were at high risk of pancreatitis.They were randomly divided into the indomethacin(rectally administered with indomethacin suppositories,n=129),stent(implanted with pancreatic duct stents,n=128),and combined(indomethacin plus pancreatic duct stent implantation,n=129)groups.Inflammatory factors,preoperative and postoperative immune function,incidence of postoperative pancreatitis and severe pancreatitis,symptom remission time,and hospital stay were compared.RESULTS Preoperatively,no significant differences were found in the levels of inflammatory factors and immune function among the three groups(P>0.05).Postoperatively,the levels of inflammatory factors,including interleukin-6,interleukin-10,and C-reactive protein,were lower in the combined group than in the indomethacin and stent groups.Meanwhile,CD4+and CD4+/CD8+levels were significantly higher in the combined group than in the indomethacin and stent groups,whereas the CD8+level was significantly lower(P0.05).The pain remission time and hospital stay duration were significantly shorter in the combined group than in the indomethacin and stent groups(P<0.05).CONCLUSION Indomethacin plus pancreatic duct stent implantation can mitigate the inflammatory response in high-risk populations for PEP,enhance immune function,and shorten the postoperative pain remission time and hospital stay. 展开更多

To address issues such as poor initial population diversity, low stability and local convergence accuracy, and easy local optima in the traditional Multi-Objective Artificial Hummingbird Algorithm (MOAHA), an Improved MOAHA (IMOAHA) was proposed. The improvements involve Tent mapping based on random variables to initialize the population, a logarithmic decrease strategy for inertia weight to balance search capability, and the improved search operators in the territory foraging phase to enhance the ability to escape from local optima and increase convergence accuracy. The effectiveness of IMOAHA was verified through Matlab/Simulink. The results demonstrate that IMOAHA exhibits superior convergence, diversity, uniformity, and coverage of solutions across 6 test functions, outperforming 4 comparative algorithms. A Wilcoxon rank-sum test further confirmed its exceptional performance. To assess IMOAHA’s ability to solve engineering problems, an optimization model for a multi-track, multi-train urban rail traction power supply system with Supercapacitor Energy Storage Systems (SCESSs) was established, and IMOAHA was successfully applied to solving the capacity allocation problem of SCESSs, demonstrating that it is an effective tool for solving complex Multi-Objective Optimization Problems (MOOPs) in engineering domains. 展开更多

Cunninghamia lanceolata is integral to soil conservation,climate regulation,and biodiversity maintenance,yet its ecological functions are threatened by drought.Functional trait trade-offs underpin hydraulic safety,but the plasticity of hydraulic strategies in C.lanceolata remains poorly understood.Here,the historical intensity of drought across different C.lanceolata regions were quantified using the Temperature Vegetation Dryness Index(TVDI)derived from satellite remote sensing.Seedlings sourced from these regions were subjected to drought under greenhouse conditions,and then water status,physiological traits,and metabolic responses were assessed to elucidate hydraulic strategies.The results revealed that TVDI effectively captured regional drought patterns,with the Dechang region experiencing the highest drought intensity,followed by the Hongya and Shiyan regions.The seedlings exhibited distinct hydraulic responses under drought stress.The highest drought-originated seedlings maintained a stable leaf water status,imposed stricter regulation of stomatal and biomass,and accumulated higher levels of antioxidants and defense compounds,indicative of a conservative strategy.In contrast,the low drought-originated seedlings showed greater fluctuations in leaf water content and potential,retained more aboveground biomass,and accumulated fewer defense compounds,reflecting an acquisitive strategy.The moderate drought-originated seedlings adopted an intermediate strategy,balancing growth and antioxidant accumulation.Overall,as the intensity of the drought increased across the provenances,C.lanceolata shifted from an acquisitive to a conservative hydraulic strategy.By linking provenance-specific drought regimes with physiological and metabolic responses,this study provides new insights into drought resistance mechanisms and informs species selection and forest management under climate change. 展开更多

In the present study,we aimed to investigate whether anlotinib reverses osimertinib resistance by inhibiting the formation of epithelial-mesenchymal transition(EMT)and angiogenesis.In a clinical case,anlotinib reversed osimertinib resistance in non-small cell lung cancer(NSCLC).Therefore,we performed immunohistochemical analyses on tumor tissues from three NSCLC patients with osimertinib resistance to analyze alterations in the expression levels of EMT markers and vascular endothelial growth factor A(VEGFA)before and after the development of osimertinib resistance.The results revealed the downregulation of E-cadherin,coupled with the upregulation of vimentin and VEGFA in tumor tissues of patients exhibiting osimertinib resistance,compared with those in tissues from patients before receiving osimertinib.Subsequently,we established osimertinib-resistant(Osi-R)cell lines and found that the Osi-R cells acquired EMT features.Next,we analyzed the synergistic effects of the combination therapy to verify whether anlotinib could reverse osimertinib resistance by inhibiting EMT.The expression levels of VEGFA and tube formation were analyzed in the combination group in vitro.Finally,we determined the reversal of osimertinib resistance by the combination of osimertinib and anlotinib in vivo using 20 nude mice.The combined treatment of osimertinib and anlotinib effectively prevented the metastasis of Osi-R cells,inhibited tumor growth,exerted antitumor activity,and ultimately reversed osimertinib resistance in mice.The co-administration of osimertinib and anlotinib demonstrated synergistic efficacy in inhibiting EMT and angiogenesis in three NSCLC patients,ultimately reversing osimertinib resistance. 展开更多

Graph Similarity Learning Based on Learnable Augmentation and Multi-Level Contrastive Learning 认领 引用

Graph similarity learning aims to calculate the similarity between pairs of graphs.Existing unsupervised graph similarity learning methods based on contrastive learning encounter challenges related to random graph augmentation strategies,which can harm the semantic and structural information of graphs and overlook the rich structural information present in subgraphs.To address these issues,we propose a graph similarity learning model based on learnable augmentation and multi-level contrastive learning.First,to tackle the problem of random augmentation disrupting the semantics and structure of the graph,we design a learnable augmentation method to selectively choose nodes and edges within the graph.To enhance contrastive levels,we employ a biased random walk method to generate corresponding subgraphs,enriching the contrastive hierarchy.Second,to solve the issue of previous work not considering multi-level contrastive learning,we utilize graph convolutional networks to learn node representations of augmented views and the original graph and calculate the interaction information between the attribute-augmented and structure-augmented views and the original graph.The goal is to maximize node consistency between different views and learn node matching between different graphs,resulting in node-level representations for each graph.Subgraph representations are then obtained through pooling operations,and we conduct contrastive learning utilizing both node and subgraph representations.Finally,the graph similarity score is computed according to different downstream tasks.We conducted three sets of experiments across eight datasets,and the results demonstrate that the proposed model effectively mitigates the issues of random augmentation damaging the original graph’s semantics and structure,as well as the insufficiency of contrastive levels.Additionally,the model achieves the best overall performance. 展开更多

AIM: To establish a scoring system to predict clinicallyrelevant postoperative pancreatic fistula(CR-POPF)after pancreaticoduodenectomy(PD).METHODS: The clinical records of 921 consecutive patients who underwent PD between 2008 and 2013 were reviewed retrospectively. Postoperative pancreatic fistula(POPF) was defined and classified by the international study group of pancreatic fistula(ISGPF).We used a logistic regression model to determine the independent risk factors of CR-POPF and developed a scoring system based on the regression coefficient of the logistic regression model. The optimal cut-off value to divide the risk strata was determined by the Youden index. The patients were divided into two groups(low risk and high risk). The independent sample t test was used to detect differences in the means of drain amylase on postoperative day(POD) 1, 2 and 3. The optimal cut-off level of the drain amylase to distinguish CR-POPF from non-clinical POPF in the two risk strata groups was determined using the receiver operating characteristic(ROC) curves.RESULTS: Grade A POPF occurred in 106(11.5%)patients, grade B occurred in 57(6.2%) patients,and grade C occurred in 32(3.5%) patients. A predictive scoring system for CR-POPF(0-6 points) was constructed using the following four factors: 1 point for each body mass index ≥ 28 [odds ratio(OR) = 3.86;95% confidence interval(CI): 1.92-7.75, P = 0.00],soft gland texture(OR = 4.50; 95%CI, 2.53-7.98, P =0.00), and the difference between the blood loss and transfusion in operation ≥ 800 mL(OR = 3.45; 95%CI,1.92-7.75, P = 0.00); and from 0 points for a 5 mm or greater duct diameter to 3 points for a less than 2 mm duct(OR = 8.97; 95%CI: 3.70-21.77, P = 0.00). The ROC curve showed that the area under the curve of this score was 0.812. A score of 3 points was suggested to be the best cut-off value(Youden index = 0.485). In the low risk group, a drain amylase level ≥ 3600 U/L on POD3 could distinguish CR-POPF from non-clinicalPOPF(the sensitivity and specificity were 75% and85%, respectively). In the high risk group, the best cutoff was a drain amylase level of 1600(the sensitivity and specificity were 77 and 63%, respectively).CONCLUSION: A 6-point scoring system accurately predicted the occurrence of CR-POPF. In addition, a drain amylase level on POD3 might be a predictor of this complication. 展开更多

Alternate path(AP) method is the most widely used method for the progressive collapse analysis,and its application in frame structures has been well proved.However,the application of AP method for other structures,especially for cable-stayed structures,should be further developed.The four analytical procedures,i.e.,linear static,nonlinear static,linear dynamic,and nonlinear dynamic were firstly improved by taking into account the initial state.Then a cable-stayed structure was studied using the four improved methods.Furthermore,the losses of both one cable and two cables were discussed.The results show that for static and dynamic analyses of the cable-stayed bridges,there is large difference between the results obtained from simulations starting with either a deformed or a nondeformed configuration at the time of cable loss.The static results are conservative in the vicinity of the ruptured cable,but the dynamic effect of the cable loss in the area farther away from the loss-cable cannot be considered.Moreover,the dynamic amplification factor of 2.0 is found to be a good estimate for static analysis procedures,since linear static and linear dynamic procedures yield approximately the same maximum vertical deflection.The results of the comprehensive evaluation of the cable failure show that the tread of the progressive failure of the cable-stayed bridges decreases when the location of the failed cables is closer to the pylon. 展开更多

BACKGROUND Upper gastrointestinal endoscopy is critical for esophageal squamous cell carcinoma(ESCC)detection;however,endoscopists require long-term training to avoid missing superficial lesions.AIM To develop a deep learning computer-assisted diagnosis(CAD)system for endoscopic detection of superficial ESCC and investigate its application value.METHODS We configured the CAD system for white-light and narrow-band imaging modes based on the YOLO v5 algorithm.A total of 4447 images from 837 patients and 1695 images from 323 patients were included in the training and testing datasets,respectively.Two experts and two non-expert endoscopists reviewed the testing dataset independently and with computer assistance.The diagnostic performance was evaluated in terms of the area under the receiver operating characteristic curve,accuracy,sensitivity,and specificity.RESULTS The area under the receiver operating characteristics curve,accuracy,sensitivity,and specificity of the CAD system were 0.982[95%confidence interval(CI):0.969-0.994],92.9%(95%CI:89.5%-95.2%),91.9%(95%CI:87.4%-94.9%),and 94.7%(95%CI:89.0%-97.6%),respectively.The accuracy of CAD was significantly higher than that of non-expert endoscopists(78.3%,P<0.001 compared with CAD)and comparable to that of expert endoscopists(91.0%,P=0.129 compared with CAD).After referring to the CAD results,the accuracy of the non-expert endoscopists significantly improved(88.2%vs 78.3%,P<0.001).Lesions with Paris classification type 0-IIb were more likely to be inaccurately identified by the CAD system.CONCLUSION The diagnostic performance of the CAD system is promising and may assist in improving detectability,particularly for inexperienced endoscopists. 展开更多

AIM To explore the value of three-dimensional(3 D) visualization technology in the minimally invasive treatment for infected necrotizing pancreatitis(INP). METHODS Clinical data of 18 patients with INP, who were admitted to the PLA General Hospital in 2017, were retrospectively analyzed. Two-dimensional images of computed tomography were converted into 3 D images based on 3 D visualization technology. The size, number, shape and position of lesions and their relationship with major abdominal vasculature were well displayed. Also, percutaneous catheter drainage(PCD) number and puncture paths were designed through virtual surgery(percutaneous nephroscopic necrosectomy) based on the principle of maximum removal of infected necrosis conveniently.RESULTS Abdominal 3 D visualization images of all the patients were well reconstructed, and the optimal PCD puncture paths were well designed. Infected necrosis was conveniently removed in abundance using a nephroscope during the following surgery, and the median operation time was 102(102 ± 20.7) min. Only 1 patient underwent endoscopic necrosectomy because of residual necrosis. CONCLUSION The 3 D visualization technology could optimize the PCD puncture paths, improving the drainage effect in patients with INP. The phytohormone auxin plays crucial roles in nearly every aspect of plant growth and development.Auxin signaling is activated through the phytohormone-induced proteasomal degradation of the Auxin/INDOLE-3-ACETIC ACID(Aux/IAA)family of transcriptional repressors.Notably,many auxin-modulated physiological processes are also regulated by nitric oxide(NO)that executes its biological effects predominantly through protein S-nitrosylation at specific cysteine residues.However,little is known about the molecular mechanisms in regulating the interactive NO and auxin networks.Here,we show that NO represses auxin signaling by inhibiting IAA17 protein degradation.NO induces the S-nitrosylation of Cys-70 located in the intrinsically disordered region of IAA17,which inhibits the TIR1-IAA17 interaction and consequently the proteasomal degradation of IAA17.The accumulation of a higher level of IAA17 attenuates auxin response.Moreover,an IAA17C70Wnitrosomimetic mutation renders the accumulation of a higher level of the mutated protein,thereby causing partial resistance to auxin and defective lateral root development.Taken together,these results suggest that S-nitrosylation of IAA17 at Cys-70 inhibits its interaction with TIR1,thereby negatively regulating auxin signaling.This study provides unique molecular insights into the redox-based auxin signaling in regulating plant growth and development. 展开更多

The effect of thermal exposure at 350 ℃ for 200 h on microstructure and mechanical properties was investigated for Al-Si-Cu-Ni-Mg alloy, which was produced by permanent mold casting(PMC) and high pressure die casting(HPDC). The SEM and IPP software were used to characterize the morphology of Si phase in the studied alloys. The results show that the thermal exposure provokes spheroidization and coarsening of eutectic Si particles. The ultimate tensile strength of the HPDC alloy after thermal exposure is higher than that of the PMC alloy at room temperature. However, the TEPMC and TEHPDC alloys have similar tensile strength around 67 MPa at 350 ℃. Due to the coarsening of eutectic Si, the TEPMC alloy exhibits better creep resistance than the TEHPDC alloy under studied creep conditions. Therefore, the alloys with small size of eutectic Si are not suitably used at 350 ℃. 展开更多

The evolution of microstructure and mechanical properties of Al-0.4 Cu-0.14 Si-0.05 Mg-0.2 Fe(wt.%)alloys, micro-alloyed with Zr, Ti and Sc, were investigated. The addition of 0.2%Zr to base alloy accelerates the precipitation of Si-rich nano-phase in α-Al matrix, which plays an important role in improving the mechanical properties of an alloy. The tensile strength increases from 102 MPa for the base alloy to 113 MPa for the Zr-modified alloy. Adding 0.2%Zr + 0.2%Ti to base alloy effectively refines a-Al grain size and accelerates the precipitation of Si and Cu elements, leading to heavy segregation at grain boundary.By further adding 0.2%Sc to Zr + Ti modified alloy, the segregation of Si and Cu elements is suppressed and more Si and Cu precipitates appeared in α-Al matrix. Accompanied with the formation of coherent Al;Sc phase, the tensile strength increases from 108 MPa for the Zr + Ti modified alloy to 152 MPa for the Sc-modified alloy. Due to excellent thermal stability of Al;Sc phase, the Sc-modified alloy exhibits obvious precipitation hardening behavior at 350℃, and the tensile strength increases to 203 MPa after holding at 350 ℃ for 200 h. 展开更多

The relationship between the as-cast microstructure and mechanical properties of the Al-12Si-3.SCu- 2Ni-0.8Mg alloys produced by permanent mold casting (PMC) and high pressure die casting (HPDC) is investigated. The alloys in both PMC and HPDC consist of Al, Si, A15 Cu2 MgsSi6, Al3 CuNi, and Al Cu4Ni phase. However, the microstructure of the HPDC alloy is significantly refined. Compared to the PMC alloy, the ultimate tensile strength of the HPDC alloy is significantly increased from 244 MPa to 310 MPa, while the elongation shows a reverse trend at room temperature. At low stress and temperature range, slight variations of stress exponent and activation energy indicate that the minimum creep rate is controlled by the grain boundary creep. Then the minimum creep rate is higher for the specimen with the smaller grain size, where grain boundary creep is the dominant creep mechanism. At high stress region, the stress exponent for the PMC alloy and HPDC alloy is 5.18 and 3.07, respectively. A systematic study on how Cu content affects the microstructure and mechanical properties of rheo-diecasting Al-6Zn-2Mg-xCu alloys during solution treatment and ageing heat treatment was conducted.The swirled enthalpy equilibrium device(SEED)was adopted to prepare the semi-solid slurry of Al-6Zn-2Mg-xCu alloys.The microstructure development and mechanical properties were studied using optical microscopy(OM),scanning electron microscopy(SEM),X-ray diffraction(XRD),differential scanning calorimetry(DSC),as well as hardness and tensile testing.The grain boundary and shape factor were calculated using image processing software(Image-Pro Plus 6.0).Results show that the alloys are composed of typical globular primaryα-Al grains,eutectic phases,and smaller secondaryα-Al grains.After solution and ageing heat treatment,the eutectic phases are dissolved into Al matrix when the Cu content is lower than 1.5wt.%,while some eutectic phases transform into Al2CuMg(S)phases and remain at grain boundaries when Cu content reaches 2wt.%.T6 heat treatment significantly enhances the mechanical properties of rheo-diecasting Al-6Zn-2Mg-xCu alloys.When Cu concentration is 0.5wt.%-1.5wt.%,the ultimate tensile strength,yield strength and elongation of T6 treated alloys rise to around 500 MPa,420 MPa,and 18%,respectively. 展开更多

In order to quantify within-and between-population variation in seed and seedling traits of Juglans mandshurica and reveal the relationship among genetic and environmental variations and select elite families,samples of 50 J.mandshurica families from five natural populations in Liaoning Province,northeast China,were taken to measure seed and seedling traits.The results show that all seed traits varied significantly among families within the population,but only kernel weight and kernel rate showed significant variations among populations.Average values of single seed weight,length,width,lateral diameter,and average size,and kernel weight and rate were 10.1 g,43.0 mm,29.2 mm,28.1 mm,33.4 mm,and 2.2 g and 22.5%,respectively.Significant variations were observed in seedling height and root collar diameter among families and interaction between families and blocks,but the block effects on height and root collar diameter were insignificant.Average values of height and root collar diameter were 94.0 cm and 8.7 mm,respectively.Family heritability of traits ranged from 0.6 gm(kernel weight)to 0.9 mm(seedling height).Correlation analysis showed a strong relationship among seed traits but a weak correlation between seed and seedling traits.Cluster analysis grouped the five natural populations of J.mandshurica into three significant clusters with different characteristics.The general combining ability analysis showed that most traits in one family(WD11)were higher,suggesting parental traits were excellent for selecting elite parent clones.Using the comprehensive evaluation method,five families with better seed traits and five families with better seedling traits were selected as elite materials with 10%selection rate.The genetic gains of these elite materials for seed weight,kernel weight,average seed size,kernel rate,seedling height,and root collar diameter were 13.1%,10.3%,4.1%,2.4%,29.7%,and 21.1%,respectively. 展开更多

This paper reviewed the development of optically pumped GaSb based semiconductor disk lasers (SDLs) emission at 2 μm wavelength region from the aspects of wavelength extending, power scaling, line-width narrowing and short-pulse generation. Most recently, the wavelength of GaSb based SDLs has been extended to 2.8 μm. The highest output power of the GaSb based SDLs has been reached to 17 W at the temperature of 20 ℃. By using active stabilization, the GaSb based SDL with line-width of 20 kHz and output power of 1 W was realized. Moreover, the shortest pulse obtained fromthe GaSb based SDLs was generated as short as 384 fs by incorporating semiconductor saturable absorber mirrors(SESAM) in the cavity. 展开更多

The epitaxial growth conditions and performance of a diode-pumped GaSb-based optically pumped semiconductor disk laser(SDL) emitting near 2.0 μm in an external cavity configuration are reported. The high quality epitaxial structure,grown on Te-doped(001) oriented GaSb substrate by molecular beam epitaxy, consists of a distributed Bragg reflector(DBR), a multi-quantum-well gain region, and a window layer. An intra-cavity SiC heat spreader was attached to the gain chip for effective thermal management. A continuous-wave output power of over 1 W operating at 2.03 μm wavelength operating near room temperature was achieved using a 3% output coupler. 展开更多
