BACKGROUND Spinal cord ischemia-reperfusion injury(IRI),which can occur as a result of temporary aortic occlusion during resection of thoracoabdominal aneurysms,can be an unpredictable and devastating complication of aortic surgery.There are no specific medications or guidelines for the prevention and treatment of spinal cord IRI(SCIRI).It is now known that IRI not only exacerbates local tissue damage when blood flow is restored but also affects distant organs,such as the liver,lungs,and brain,through mediators released into the systemic circulation.Studies show that ozone pretreatment reduces oxidative damage by increasing antioxidant capacity,promotes anti-inflammatory signaling,improves blood circulation,and ameliorates IRI.Our study is one of the first to examine the effects of ozone on remote organs(liver,lung,and brain)when administered via different routes(intrathecal,intraperitoneal,rectal)in a spinal cord ischemia-reperfusion model.AIM To determine whether ozone administered by different routes offers multiorgan protection in SCIRI.METHODS Thirty adult Wistar albino rats were randomly divided into five groups(n=6,each):A control(C group),an IR group,an IR rectal ozone(IRRO)group,an IR intrathecal ozone(IRITO),and an IR intraperitoneal ozone(IRIPO).In the IR groups,the spinal cord IR models(a 30-minute ischemia period was applied to the infrarenal abdominal aorta using an atraumatic vascular clamp,and then a 120-minute reperfusion period was applied by removing the clamp)were applied.An ozone-oxygen mixture of 1 mg/kg(50μg/mL)was administered by rectal insufflation to the IRRO group,0.7 mg/kg(50μg/mL)via the peritoneum to the IRIPO group,and 20μL(20μg/mL)intrathecally to the IRITO group 30 minutes before midline laparotomy.At the end of the reperfusion procedure,histopathological and biochemical analyses of liver,lung,and brain tissues were performed.RESULTS Liver tissue malondialdehyde(MDA)levels were significantly lower,and catalase(CAT)enzyme activities were significantly higher in the IRRO,IRITO,and IRIPO groups than in the IR group.Histopathologically,we had favorable results from all three ozone applications compared to the IR group.Lung tissue MDA levels were significantly lower,and CAT enzyme activities were significantly higher in the IRITO and IRIPO groups than in the IR group.We had more positive results in the IRRO group than in the IR group,but the difference was not found to be significant.Histopathologically,we obtained significantly more positive results in the IRITO and IRIPO groups compared with the IR group regarding all the criteria we evaluated.Our results in the IRRO group were also positive.Brain tissue MDA levels were significantly lower and CAT enzyme activities significantly higher in the IRITO and IRIPO groups than in the IR group.We had positive results in the IRRO group compared with the IR group.Histopathologically,we obtained significantly more positive results in the IRITO group regarding all the criteria we evaluated.CONCLUSION We observed histopathologically that single-dose ozone pretreatment administered intrathecally,intraperitoneally,or rectally had positive effects on liver,lung,and brain tissues compared to the IR group in an SCIRI model in rats due to its antioxidant effect.The best histopathological results were obtained with intrathecal,intraperitoneal,and rectally administered ozone,in that order,in all three tissues. 展开更多

BACKGROUND Non-steroidal anti-inflammatory drugs(NSAIDs)are widely used for pain management,but they can cause nephrotoxicity,particularly in patients with existing renal impairment.Cerium oxide(CeO2),an antioxidant nanoparticle,may provide protection against renal damage caused by NSAIDs.AIM To assess the protective effects of CeO2 against ibuprofen-induced renal injury in a rat model of unilateral ureteral obstruction(UUO).METHODS Thirty male Wistar albino rats,weighing between 300 g and 425 g,were randomly divided into five groups(n=6 each):Control(sham),UUO only,UUO+ibuprofen,UUO+ibuprofen+CeO2,and UUO+CeO2.UUO was surgically induced in all groups except the control group.Treatments were administered for a duration of 21 days.Serum levels of blood urea nitrogen,creatinine,malondialdehyde,and nitric oxide were measured.In addition,histopathological analysis was performed on both kidneys to assess parameters such as glomerular vacuolization(GV),tubular dilation,vascular hypertrophy,tubular necrosis,Bowman’s space dilation,hyaline casts,lymphocyte infiltration,and tubular desquamation.RESULTS In the right(non-obstructed)kidneys,the UUO+CeO2 group exhibited significantly lower levels of GV,tubular hyaline casts,and tubular desquamation compared to the UUO+ibuprofen group(P<0.05).In the left(obstructed)kidneys,the CeO2-treated group also exhibited significant reductions in GV,vascular hypertrophy,and tubular hyaline casts(P<0.05).Furthermore,malondialdehyde levels were markedly decreased in the CeO2 groups,whereas nitric oxide levels were elevated in the ibuprofen group.Blood urea nitrogen and creatinine levels increased across all UUO groups,but no significant differences were observed between the treatment groups.CONCLUSION CeO2 attenuates ibuprofen-induced renal injury in rats with UUO,likely through antioxidative mechanisms.These findings suggest that CeO2 could serve as a potential nephroprotective adjunct during NSAID therapy in patients with obstructive renal conditions. 展开更多

BACKGROUND Testicular ischemiaeperfusion(I/R)injury may affect distant organs.During I/R,metabolites formed in the affected organ are released into the circulation;thus,non-ischemic organs,such as the kidneys and lungs,become involved in the inflammatory process,even if they are not primarily affected.Damage from testicular torsion/detorsion(T/D)models has been shown to affect kidneys.There is a need to examine the nephroprotective effect of boric acid(BA),which is known to have antioxidant effects,on kidney damage.AIM To investigate kidney damage in a rat testicular T/D model and the effectiveness of BA on this distant organ damage.METHODS Twenty-four male Wistar albino rats were divided into four groups(n=6 each)-control(group C),BA(group BA),T/D(group I/R),BA-T/D(group BA-I/R).In all groups,the scrotum was cut longitudinally,and the testis was dissected.No medication was administered to groups C and group I/R.Groups BA and group BA-I/R were administered 50 mg/kg BA intraperitoneally half an hour before the surgical procedure.The testicular T/D model was applied to groups I/R and group BA-I/R.The malondialdehyde(MDA)level,catalase,glutathione S-transferase,and paraoxonase-1(PON-1)enzyme activities were measured in kidney tissue.Additionally,sections were taken and evaluated for histopathological examination.RESULTS An increase in MDA levels and a decrease in PON-1 activity were detected in the I/R group compared to the control group(P=0.001,P=0.002,respectively).MDA levels were found to be significantly lower in the BA-I/R group than in the I/R group(P=0.041).PON-1 enzyme activity was found to be significantly higher in the BA-I/R group than in the I/R group(P=0.048).The light microscopy examination showed an increase in glomerular vacuolization,tubular dilatation,vascular vacuolization and hypertrophy(VVH),and tubular cell degeneration and necrosis damage scores in the I/R group compared to the control group(P=0.003,P=0.005,P=0.008,and P<0.001,respectively).In the I/R group treated with BA,tubular dilatation and VVH scores decreased compared to the I/R group(P=0.037 and P=0.048,respectively).CONCLUSION These findings show that the testicular T/D model in rats causes kidney damage,and BA administration reduces this damage in the kidneys. 展开更多

In this editorial,we comment on an article by Wang et al.Recent literature shows an increase in research on pelvic organ prolapse(POP).Although the true incidence of POP remains uncertain,its impact on quality of life is substantial.Anatomical studies report high incidence rates,surpassing those observed in symptom-based surveys.Weakness of the endopelvic fascia is a primary anatomical risk factor for POP.Additionally,gestational diabetes mellitus(GDM)has emerged as a growing concern,as poor glycemic control increases complications for both mother and fetus.GDM and POP are interconnected,with factors like maternal obesity,macrosomia,and hormonal changes exacerbating pelvic floor dysfunction.Modifiable risk factors,such as obesity and chronic hyperglycemia,along with multiparity,instrumental deliveries,and obstetric trauma,further increase susceptibility.For patients with GDM,gynecological exams,Pelvic Organ Prolapse Quantification staging,and pelvic floor ultrasonography are valuable diagnostics,with proctological exams and magnetic resonance defecography aiding in multi-compartment prolapse diagnoses.Imaging,though uncomfortable during pregnancy,is safe in the early postpartum period.This editorial emphasizes the need for further research on the pathophysiology of GDM-related POP and offers recommendations for improving diagnosis and clinical management of patients with GDM. 展开更多
