BACKGROUND Cerebral infarction(CI)is a major subtype of ischemic stroke,with high incidence,recurrence rate,disability rate,and mortality rate in the elderly.Elderly CI patients are prone to peptic ulcer due to long-t...BACKGROUND Cerebral infarction(CI)is a major subtype of ischemic stroke,with high incidence,recurrence rate,disability rate,and mortality rate in the elderly.Elderly CI patients are prone to peptic ulcer due to long-term use of antiplatelet drugs and stress factors,which further leads to gastrointestinal dysfunction,seriously affecting nutritional intake and rehabilitation.Naloxone,an opioid receptor antagonist,has neuroprotective potential,and enteral nutrition(EN)can improve gastrointestinal function.This study aims to explore the intervention effect of their combination.AIM To investigate the efficacy of naloxone combined with EN on gastrointestinal dysfunction in elderly patients with CI and peptic ulcer.METHODS In this randomized controlled trial,110 elderly patients with CI and peptic ulcer were equally assigned to a control group(receiving standard therapy combined with EN)or a combination group(receiving standard therapy,EN,and naloxone).The National Institute of Health Stroke Scale(NIHSS)scores,Barthel Index(BI),Chinese version of the MOS 36-Item Short Form Health Survey(SF-36)scores,gastrointestinal function scores,and serum levels of motilin(MTL),gastrin(GAS),and somatostatin(SS)were compared.Adverse reactions were also recorded.RESULTS Compared with the control group,the combination group had lower NIHSS scores(5.88±1.07 vs 8.73±1.14,P<0.001)and gastrointestinal dysfunction scores(5.14±1.27 vs 8.62±1.31,P<0.001),higher BI(87.36±4.42 vs 72.29±3.23,P<0.001)and SF-36 scores(76.26±3.85 vs 70.21±3.66,P<0.001),increased serum MTL(201.31±12.35 ng/L vs 186.81±11.18 ng/L,P<0.001)and GAS levels(94.46±9.64 pg/mL vs 103.15±10.52 pg/mL,P<0.001),and decreased SS levels(5.69±1.25 pg/mL vs 8.31±1.19 pg/mL,P<0.001).No significant difference in the incidence of adverse reactions was found between the two groups.CONCLUSION The combination of naloxone and EN is a safe and efficacious therapeutic approach for ameliorating gastrointestinal dysfunction and promoting overall recovery in elderly individuals with CI and peptic ulcer.展开更多
The Three Gorges Reservoir(TGR)is one of the largest hydroelectric projects in the world,with significant impacts on the hydrology and ecology of the Yangtze River Basin.Understanding the effects of TGR operation on s...The Three Gorges Reservoir(TGR)is one of the largest hydroelectric projects in the world,with significant impacts on the hydrology and ecology of the Yangtze River Basin.Understanding the effects of TGR operation on surrounding water systems,especially the Jingjiang Reach and Dongting Lake,is crucial for local water resources management and flood control.This study evaluated the impact of the TGR on water diversion in the Jingjiang Reach and outflow from Dongting Lake using observed data and sedimentation patterns before and after TGR operation.A coupled one-wo-demensional hydrodynamic model was developed to simulate hydrological processes.The relationship between TGR scheduling and Dongting Lake inflow and outflow across different periods was quantified.The results indicated that after TGR operation began,riverbed erosion significantly lowered tributary water levels under equivalent main stream flow.Lake inflow through the three Jingjiang Reach outlets increased during drawdown and water supplement periods but decreased during flood and impounding periods.Lake outflow increased during drawdown,flood,and water supplement periods but declined significantly during the impounding period.The contributions of factors varied considerably.Reservoir scheduling accounted for 328.86% of inflow changes at the Taiping outlet during the drawdown period but only 20.72% during the flood season.River topography changes contributed 157.41% to lake outflow changes during the drawdown period,but only 1.85% during the water supplement period.These findings enhance our understanding of river-lake system evolution and support improved management strategies.展开更多
This study utilized a sequential mediating model to examine the role of motivation to learn and transfer selfefficacy in the relationships between perceived content validity,mentoring function,continuous learning work...This study utilized a sequential mediating model to examine the role of motivation to learn and transfer selfefficacy in the relationships between perceived content validity,mentoring function,continuous learning work culture and intention to transfer learning.The sample comprized 429 final-year apprentices in Guangdong province,China(females=69.9%,Engineering&Medicine=69%,mean age=20.99,SD=1.60).The apprentices completed standardized measures of motivation to learn,transfer self-efficacy perceived content validity,mentoring function,and continuous learning work culture.Structural equation modeling was used to analyze the data.Results showed perceived content validity,mentoring function,continuous learning culture to predict intention to transfer learning.Of these factors,perceived content validity was the strongest predictor of intention to transfer learning.Of these factors,perceived content validity was the most influential predictor of intention to transfer learning.The motivation to learn and transfer self-efficacy sequentially mediated the relationship between mentoring function and intention to learning transfer to be stronger than by either alone.Although perceived content validity and continuous learning culture exhibited no significant direct effects on intention to transfer learning,they demonstrated positive indirect associations with intention to transfer via motivation to learn and transfer self-efficacy.These study findings extend the applications of the learning transfer framework to individuals undergoing apprenticeship training which also would apply to other a long-term work-based learning programs.展开更多
Background Patients with Parkinson’s disease(PD)undergoing long-term levodopa therapy are prone to develop levodopa-induced dyskinesia(LID).Amantadine is the main drug recommended for the treatment of LID by current ...Background Patients with Parkinson’s disease(PD)undergoing long-term levodopa therapy are prone to develop levodopa-induced dyskinesia(LID).Amantadine is the main drug recommended for the treatment of LID by current guidelines,but it is far from meeting clinical needs.Tianqi Pingchan Granule(TPG),a compound Chinese herbal medicine,has been developed to relieve symptom of LID.Objective This randomized controlled trial evaluated the efficacy and safety of the combination of TPG and amantadine for LID.Design,setting,participants and interventions This is a randomized double-blind placebo-controlled trial,conducted from January 2020 to August 2021 at 6 sites in Jiangsu,Zhejiang and Shanghai,China.One hundred PD patients with≥0.5 h of LID were randomly assigned to either the TPG plus amantadine group(TPG group)or the placebo plus amantadine group(placebo group),and treated for a period of 12 weeks.To ensure unbiased results,all study participants,investigators and sponsors were unaware of group allocations.Additionally,the data analysts remained blinded until the analysis was finalized.Main outcome measures The primary outcome was assessed using the Unified Dyskinesia Rating Scale(UDysRS)(Range 0–104).The key secondary end point was improvement of motor and non-motor symptoms.Safety analyses included all enrolled patients.Results One hundred patients were enrolled and randomized into the two treatment groups.The changes in UDysRS at week 12 were–11.02 for the TPG group and–4.19 for the placebo group(treatment difference–6.83[–10.53 to–3.12];P=0.0004).Adverse events were reported for 2 of 50 patients(4.0%)in each of the groups.Conclusion This study indicated that a 12-week treatment of amantadine plus TPG effectively reduced UDysRS scores and was well tolerated,demonstrating the efficacy and safety of TPG for the treatment of LID in PD.展开更多
AIM:To explore the effect of miR-184 and miR-205 on the proliferation and metastasis of conjunctival mucosa associated lymphoid tissue(MALT)lymphoma.METHODS:Tissue of tumor and adjacent normal control from 5 patients ...AIM:To explore the effect of miR-184 and miR-205 on the proliferation and metastasis of conjunctival mucosa associated lymphoid tissue(MALT)lymphoma.METHODS:Tissue of tumor and adjacent normal control from 5 patients with conjunctival MALT was included.RPMI8226 cell line was selected to verify the effect of mi RNAs in B cells.The function of micro RNA on the RPMI8226 cell apoptosis,migration and invasion was evaluated by apoptosis assay and Transwell assay.The m RNA and protein expression were examined by quantitative RT-PCR and Western blotting.The effect of micro RNA on regulation of downstream gene expression was evaluated by luciferase report assay.RESULTS:A decreased level of miR-184 and miR-205 was observed in MALT lymphoma tissue.Exogenous miR-184 and miR-205 analogues promoted apoptosis,and inhibited the survival,migration,and invasion of RPMI8226 cells.miR-184 and miR-205 inhibitor reversed the process.The RNA and protein level of Ras L10 B and TNFAIP8 were downregulated in MALT lymphoma tissue.The exogenous of miR-184 and miR-205 promoted the expression of Ras L10 B and TNFAIP8.Meanwhile,inhibition of miR-184 and miR-205 repressed the expression of target gene,Ras L10 B and TNFAIP8.CONCLUSION:miR-184 and miR-205 suppresses the tumorigenesis of conjunctival MALT lymphoma through regulating Ras L10 B and TNFAIP8.展开更多

Here, we investigated the expression and localization of 11β-HSD in rat testis during postnatal development, and the regulation of these genes by luteinizing hormone (LH) and androgens, mRNA and protein levels were analyzed by quantitative real-time-polymerase chain reaction and western blotting, respectively, in testes collected from rats at postnatal day (PND) 7, 14, 21, 35, and 90, and from rats treated with LH, 7α.methyl-19-nortestosterone (MENT) and testosterone at PND 21 and PND 90. Immunohistochemical staining was used to identify the localization of the 11β-HSD in rat testis at PND 7, 14, and 90. We found that 11β-HSD1 expression was restricted to the interstitial areas, and that its levels increased during rat testis development. In contrast, whereas 11β-HSD2 was expressed in both the interstitial areas and seminiferous tubules at PND 7, it was present only in the interstitial areas at PND 90, and its levels declined during testicular development. Moreover, 11β-HSD1 mRNA was induced by LH in both the PND 21 and 90 testes and by MENT at PND 21, whereas 11β-HSD2 mRNA was induced by testosterone and MENT in the PND 21 testis and by LH in the PND 90 testis. In conclusion, our study indicates that the 11β-HSD1 and 11β-HSD2 genes have distinct patterns of spatiotemporal expression and hormonal regulation during postnatal development of the rat testis.展开更多
BACKGROUND Visceral obesity is increasingly prevalent among adolescents and young adults and is commonly recognized as a risk factor for type 2 diabetes.Estrogen[17β-estradiol(E2)]is known to offer protection against...BACKGROUND Visceral obesity is increasingly prevalent among adolescents and young adults and is commonly recognized as a risk factor for type 2 diabetes.Estrogen[17β-estradiol(E2)]is known to offer protection against obesity via diverse me-chanisms,while its specific effects on visceral adipose tissue(VAT)remain to be fully elucidated.AIM To investigate the impact of E2 on the gene expression profile within VAT of a mouse model of prediabetes.METHODS Metabolic parameters were collected,encompassing body weight,weights of visceral and subcutaneous adipose tissues(VAT and SAT),random blood glucose levels,glucose tolerance,insulin tolerance,and overall body composition.The gene expression profiles of VAT were quantified utilizing the Whole Mouse Genome Oligo Microarray and subsequently analyzed through Agilent Feature Extraction software.Functional and pathway analyses were conducted employing Gene Ontology and Kyoto Encyclopedia of Genes and Genomes analyses,respectively.RESULTS Feeding a high-fat diet(HFD)moderately increased the weights of both VAT and SAT,but this increase was mitigated by the protective effect of endogenous E2.Conversely,ovariectomy(OVX)led to a significant increase in VAT weight and the VAT/SAT weight ratio,and this increase was also reversed with E2 treatment.Notably,OVX diminished the expression of genes involved in lipid metabolism compared to HFD feeding alone,signaling a widespread reduction in lipid metabolic activity,which was completely counteracted by E2 adminis-tration.This study provides a comprehensive insight into E2's local and direct protective effects against visceral adiposity in VAT at the gene level.CONCLUSION In conclusion,the present study demonstrated that the HFD-induced over-nutritional challenge disrupted the gene expression profile of visceral fat,leading to a universally decreased lipid metabolic status in E2 deficient mice.E2 treatment effectively reversed this condition,shedding light on the mechanistic role and therapeutic potential of E2 in combating visceral obesity.展开更多

Background Pulmonary thromboembolism is rare in children,but can be life-threatening.Timely diagnosis and treatment of pulmonary thromboembolism are crucial for reducing mortality associated with pulmonary thromboembo...Background Pulmonary thromboembolism is rare in children,but can be life-threatening.Timely diagnosis and treatment of pulmonary thromboembolism are crucial for reducing mortality associated with pulmonary thromboembolism in children.While guidelines for pulmonary thromboembolism in adults are available,guidelines for standardized diagnosis and management of pulmonary thromboembolism in children are not.This expert consensus aims to provide recommendations for the management of pulmonary thromboembolism in children based on the current best available evidence.Data sources Following the World Health Organization Handbook for Guideline Development,the expert panel consisted of 30 members from different clinical areas.The panel identified clinical questions through systematic reviews and expert discussions,systematically reviewed evidence on pulmonary thromboembolism in children,and evaluated the quality of the evidence using the Grading of Recommendations Assessment,Development,and Evaluation(GRADE)approach.Using the GRADE Evidence to Decision Framework,the panel made recommendations,considering the effects of interventions,resource use,values and preferences,equity,acceptability,and feasibility.Results The epidemiology,classification,and pathophysiology characteristics are summarized.The expert panel developed33 recommendations addressing 20 questions related to diagnosis steps,treatment approaches such as anticoagulant therapy,thrombolysis therapy,catheter-based interventional therapy,surgical embolectomy,multidisciplinary team,and treatment of patients with comorbidities,prognosis,education,as well as follow-up.Among these,18 are weak recommendations based on very low quality evidence,and 15 are good practice statements.Conclusions The expert panel provided recommendations for pulmonary thromboembolism in children based on available evidence,which was generally low in quality and volume.The panel urges further research on early identification and diagnosis strategies,preventive and therapeutic regimens,and long-term management for pulmonary thromboembolism.展开更多
Background and Objectives:To evaluate the potential benefits of Bacteroides fragilis 839(BF839),a nextgeneration probiotics,in reducing myelosuppression and gastrointestinal toxicity associated with chemotherapy in br...Background and Objectives:To evaluate the potential benefits of Bacteroides fragilis 839(BF839),a nextgeneration probiotics,in reducing myelosuppression and gastrointestinal toxicity associated with chemotherapy in breast cancer patient.Methods and Study Design:40 women with early breast cancer were randomly assigned to the BF839(n=20)or placebo(n=20)during the administration of adjuvant chemotherapy(4 cycles of epirubicin 100mg/m2 and cyclophosphamide 600mg/m2).Myelosuppression and gastrointestinal adverse effects were monitored in both groups.Results:Throughout the four treatment cycles,the percentage of patients experiencing myelosuppression was 42.5%in the BF839 group,significantly lower than the 66.3%observed in the control group(p=0.003).Two patients in the BF839 group and three patients in the placebo group received recombinant human granulocyte colony-stimulating factor(rhG-CSF)due to leukopeniaeutropenia.When considering an ITT analysis,which included all patients regardless of rhG-CSF treatment,the BF839 group exhibited less reduction from baseline in white blood cells(-0.31±1.19 vs-1.15±0.77,p=0.012)and neutrophils(0.06±1.00 vs-0.84±0.85,p=0.004)compared to the placebo group.The difference became even more significant when excluding the patients who received rhG-CSF injections.Throughout the four treatment cycles,compared to the placebo group,the BF839 group had significantly lower rates of 3-4 grade nausea(35.0%vs 71.3%,p=0.001),vomiting(20.0%vs 45.0%,p=0.001),and diarrhea(15.0%vs 30.0%,p=0.023).Conclusions:These findings suggest that BF839 has the potential to effectively mitigate myelosuppression and gastrointestinal toxicity associated with chemotherapy in breast cancer patients.展开更多

摘要Background Patients with Parkinson’s disease(PD)undergoing long-term levodopa therapy are prone to develop levodopa-induced dyskinesia(LID).Amantadine is the main drug recommended for the treatment of LID by current guidelines,but it is far from meeting clinical needs.Tianqi Pingchan Granule(TPG),a compound Chinese herbal medicine,has been developed to relieve symptom of LID.Objective This randomized controlled trial evaluated the efficacy and safety of the combination of TPG and amantadine for LID.Design,setting,participants and interventions This is a randomized double-blind placebo-controlled trial,conducted from January 2020 to August 2021 at 6 sites in Jiangsu,Zhejiang and Shanghai,China.One hundred PD patients with≥0.5 h of LID were randomly assigned to either the TPG plus amantadine group(TPG group)or the placebo plus amantadine group(placebo group),and treated for a period of 12 weeks.To ensure unbiased results,all study participants,investigators and sponsors were unaware of group allocations.Additionally,the data analysts remained blinded until the analysis was finalized.Main outcome measures The primary outcome was assessed using the Unified Dyskinesia Rating Scale(UDysRS)(Range 0–104).The key secondary end point was improvement of motor and non-motor symptoms.Safety analyses included all enrolled patients.Results One hundred patients were enrolled and randomized into the two treatment groups.The changes in UDysRS at week 12 were–11.02 for the TPG group and–4.19 for the placebo group(treatment difference–6.83[–10.53 to–3.12];P=0.0004).Adverse events were reported for 2 of 50 patients(4.0%)in each of the groups.Conclusion This study indicated that a 12-week treatment of amantadine plus TPG effectively reduced UDysRS scores and was well tolerated,demonstrating the efficacy and safety of TPG for the treatment of LID in PD.
摘要AIM:To explore the effect of miR-184 and miR-205 on the proliferation and metastasis of conjunctival mucosa associated lymphoid tissue(MALT)lymphoma.METHODS:Tissue of tumor and adjacent normal control from 5 patients with conjunctival MALT was included.RPMI8226 cell line was selected to verify the effect of mi RNAs in B cells.The function of micro RNA on the RPMI8226 cell apoptosis,migration and invasion was evaluated by apoptosis assay and Transwell assay.The m RNA and protein expression were examined by quantitative RT-PCR and Western blotting.The effect of micro RNA on regulation of downstream gene expression was evaluated by luciferase report assay.RESULTS:A decreased level of miR-184 and miR-205 was observed in MALT lymphoma tissue.Exogenous miR-184 and miR-205 analogues promoted apoptosis,and inhibited the survival,migration,and invasion of RPMI8226 cells.miR-184 and miR-205 inhibitor reversed the process.The RNA and protein level of Ras L10 B and TNFAIP8 were downregulated in MALT lymphoma tissue.The exogenous of miR-184 and miR-205 promoted the expression of Ras L10 B and TNFAIP8.Meanwhile,inhibition of miR-184 and miR-205 repressed the expression of target gene,Ras L10 B and TNFAIP8.CONCLUSION:miR-184 and miR-205 suppresses the tumorigenesis of conjunctival MALT lymphoma through regulating Ras L10 B and TNFAIP8.
摘要11β-hydroxysteroid dehydrogenase type 1 (11β-HSD1) and type 2 (11β-HSD2) are expressed in rat testis, where they regulate the local concentrations of glucocorticoids. Here, we investigated the expression and localization of 11β-HSD in rat testis during postnatal development, and the regulation of these genes by luteinizing hormone (LH) and androgens, mRNA and protein levels were analyzed by quantitative real-time-polymerase chain reaction and western blotting, respectively, in testes collected from rats at postnatal day (PND) 7, 14, 21, 35, and 90, and from rats treated with LH, 7α.methyl-19-nortestosterone (MENT) and testosterone at PND 21 and PND 90. Immunohistochemical staining was used to identify the localization of the 11β-HSD in rat testis at PND 7, 14, and 90. We found that 11β-HSD1 expression was restricted to the interstitial areas, and that its levels increased during rat testis development. In contrast, whereas 11β-HSD2 was expressed in both the interstitial areas and seminiferous tubules at PND 7, it was present only in the interstitial areas at PND 90, and its levels declined during testicular development. Moreover, 11β-HSD1 mRNA was induced by LH in both the PND 21 and 90 testes and by MENT at PND 21, whereas 11β-HSD2 mRNA was induced by testosterone and MENT in the PND 21 testis and by LH in the PND 90 testis. In conclusion, our study indicates that the 11β-HSD1 and 11β-HSD2 genes have distinct patterns of spatiotemporal expression and hormonal regulation during postnatal development of the rat testis.

摘要BACKGROUND Visceral obesity is increasingly prevalent among adolescents and young adults and is commonly recognized as a risk factor for type 2 diabetes.Estrogen[17β-estradiol(E2)]is known to offer protection against obesity via diverse me-chanisms,while its specific effects on visceral adipose tissue(VAT)remain to be fully elucidated.AIM To investigate the impact of E2 on the gene expression profile within VAT of a mouse model of prediabetes.METHODS Metabolic parameters were collected,encompassing body weight,weights of visceral and subcutaneous adipose tissues(VAT and SAT),random blood glucose levels,glucose tolerance,insulin tolerance,and overall body composition.The gene expression profiles of VAT were quantified utilizing the Whole Mouse Genome Oligo Microarray and subsequently analyzed through Agilent Feature Extraction software.Functional and pathway analyses were conducted employing Gene Ontology and Kyoto Encyclopedia of Genes and Genomes analyses,respectively.RESULTS Feeding a high-fat diet(HFD)moderately increased the weights of both VAT and SAT,but this increase was mitigated by the protective effect of endogenous E2.Conversely,ovariectomy(OVX)led to a significant increase in VAT weight and the VAT/SAT weight ratio,and this increase was also reversed with E2 treatment.Notably,OVX diminished the expression of genes involved in lipid metabolism compared to HFD feeding alone,signaling a widespread reduction in lipid metabolic activity,which was completely counteracted by E2 adminis-tration.This study provides a comprehensive insight into E2's local and direct protective effects against visceral adiposity in VAT at the gene level.CONCLUSION In conclusion,the present study demonstrated that the HFD-induced over-nutritional challenge disrupted the gene expression profile of visceral fat,leading to a universally decreased lipid metabolic status in E2 deficient mice.E2 treatment effectively reversed this condition,shedding light on the mechanistic role and therapeutic potential of E2 in combating visceral obesity.
摘要Dear Editor, 46,XY disorder of sex development (DSD) is characterized by a female phenotype in an individual with a normal 46,XY karyotype.1 The clinical phenotTpe of 46,XY DSD is characterized by bilateral undescended testes and a normal female appearance, including breasts, female external genitalia, and other secondary sex characteristics, but the absence of a uterus or ovaries.2 Development of the male external genitalia in the fetal period depends on the biosynthesis of testosterone (T), the conversion ofT into dihydrotestosterone (DHT) by steroid 5α-reductase type 2 (SRD5A2), and the response of functionally active androgen receptors in genital tissues) The disruption of any of these stages will block the differentiation of internal and external genitalia and cause 46,XY DSD.

摘要Pseudorabies,also known as Aujeszky's disease,is one of the most economically important viral diseases in pigs and is lethal to other susceptible animals(Ren et al.2020).The causative agent,pseudorabies virus(PRV),is an enveloped virus with a large double-stranded DNA genome encoding at least 70 proteins(Wong et al.2019).PRV belongs to the family Herpesviridae,subfamily Alphaherpesvirinae,genus Varicellovirus and infects multiple animals,such as pigs,dogs,cats,rabbits,cattle,sheep,goats,minks.
摘要Background Pulmonary thromboembolism is rare in children,but can be life-threatening.Timely diagnosis and treatment of pulmonary thromboembolism are crucial for reducing mortality associated with pulmonary thromboembolism in children.While guidelines for pulmonary thromboembolism in adults are available,guidelines for standardized diagnosis and management of pulmonary thromboembolism in children are not.This expert consensus aims to provide recommendations for the management of pulmonary thromboembolism in children based on the current best available evidence.Data sources Following the World Health Organization Handbook for Guideline Development,the expert panel consisted of 30 members from different clinical areas.The panel identified clinical questions through systematic reviews and expert discussions,systematically reviewed evidence on pulmonary thromboembolism in children,and evaluated the quality of the evidence using the Grading of Recommendations Assessment,Development,and Evaluation(GRADE)approach.Using the GRADE Evidence to Decision Framework,the panel made recommendations,considering the effects of interventions,resource use,values and preferences,equity,acceptability,and feasibility.Results The epidemiology,classification,and pathophysiology characteristics are summarized.The expert panel developed33 recommendations addressing 20 questions related to diagnosis steps,treatment approaches such as anticoagulant therapy,thrombolysis therapy,catheter-based interventional therapy,surgical embolectomy,multidisciplinary team,and treatment of patients with comorbidities,prognosis,education,as well as follow-up.Among these,18 are weak recommendations based on very low quality evidence,and 15 are good practice statements.Conclusions The expert panel provided recommendations for pulmonary thromboembolism in children based on available evidence,which was generally low in quality and volume.The panel urges further research on early identification and diagnosis strategies,preventive and therapeutic regimens,and long-term management for pulmonary thromboembolism.
摘要Background and Objectives:To evaluate the potential benefits of Bacteroides fragilis 839(BF839),a nextgeneration probiotics,in reducing myelosuppression and gastrointestinal toxicity associated with chemotherapy in breast cancer patient.Methods and Study Design:40 women with early breast cancer were randomly assigned to the BF839(n=20)or placebo(n=20)during the administration of adjuvant chemotherapy(4 cycles of epirubicin 100mg/m2 and cyclophosphamide 600mg/m2).Myelosuppression and gastrointestinal adverse effects were monitored in both groups.Results:Throughout the four treatment cycles,the percentage of patients experiencing myelosuppression was 42.5%in the BF839 group,significantly lower than the 66.3%observed in the control group(p=0.003).Two patients in the BF839 group and three patients in the placebo group received recombinant human granulocyte colony-stimulating factor(rhG-CSF)due to leukopeniaeutropenia.When considering an ITT analysis,which included all patients regardless of rhG-CSF treatment,the BF839 group exhibited less reduction from baseline in white blood cells(-0.31±1.19 vs-1.15±0.77,p=0.012)and neutrophils(0.06±1.00 vs-0.84±0.85,p=0.004)compared to the placebo group.The difference became even more significant when excluding the patients who received rhG-CSF injections.Throughout the four treatment cycles,compared to the placebo group,the BF839 group had significantly lower rates of 3-4 grade nausea(35.0%vs 71.3%,p=0.001),vomiting(20.0%vs 45.0%,p=0.001),and diarrhea(15.0%vs 30.0%,p=0.023).Conclusions:These findings suggest that BF839 has the potential to effectively mitigate myelosuppression and gastrointestinal toxicity associated with chemotherapy in breast cancer patients.
