Background:The contribution of gut microbiota to the early stage of type 2 diabetes(T2D)remains incompletely understood.This study established a germ-free mouse model colonized with gut microbiota from donors with T2D to determine whether diabetes-associated microbiota could induce early diabetes-like phenotypes.Methods:Human fecal samples were collected from 18 healthy donors and 14 donors with T2D for microbiota profiling.Based on these features,samples from 12 healthy donors and 3 donors with T2D were selected,pooled within each group,and transplanted into germ-free mice,generating a healthy microbiota recipient group(HM,n=14)and a diabetes-associated microbiota recipient group(DM,n=13).Glucose homeostasis was assessed over 10 weeks using fasting blood glucose and intraperitoneal glucose tolerance tests.Gut microbial succession was analyzed by 16S rRNA sequencing.Fecal and plasma metabolomics were performed to identify metabolic and their associations with microbial changes.Results:DM mice developed an early diabetes-like phenotype characterized by progressive impairment of glucose tolerance,reduced insulin levels,and mild renal alterations,without sustained overt fasting hyperglycemia.Microbial divergence emerged before stable metabolic dysfunction and was accompanied by persistent dysbiosis in DM mice.Integrated metabolomic analysis identified coordinated alterations in fecal and plasma metabolites,with cholic acid and L-Dopa decreased in both compartments.L-Dopa showed consistent associations with altered genera,particularly Ruminococcus and Sellimonas.Conclusions:Transplanting diabetes-associated human gut microbiota effectively induced early glucose dysregulation in germ-free recipient mice.This model provides a framework for studying microbiota-associated events during early T2D-related metabolic deterioration. 展开更多

Multiple myeloma(MM)is the second most common hematological malignancy and still remains incurable.Selinexor(SEL)is the first-in-class exportin 1(XPO1)inhibitor,and has demonstrated survival benefits in relapsedefractory MM patients.Despite its therapeutic potential,dose-limiting toxicities frequently compromise SEL's clinical utility and patients'quality of life.In this work,to improve therapeutic efficacy and minimize side effects of SEL,we designed the monomethyl poly(ethylene glycol)-poly(ε-caprolactone)(mPEG-PCL)micelle-encapsulated nanoformulation for MM therapy.The m PEG-PCL-SEL micelle exhibited controlled release behavior and showed remarkable cytotoxicity in MM cells.Most importantly,in the orthotopic MM model,this micelle system exhibited impressive therapeutic efficacy at low dosages and significantly prolonged the survival of MM-bearing mice.Moreover,the mPEG-PCL-SEL micelle demonstrated a favorable safety profile with fewer gastrointestinal and constitutional symptoms.And we found that compared with the free-SEL,the mPEG-PCL-SEL micelle maintained the integrity of the intestinal barrier,and was more beneficial to the diversity of the intestinal microbiota,which may be related to the improved gastrointestinal tolerability,making it an effective and safe choice for MM treatment. 展开更多

The trade-off between quality and difficulty is a challenge when quantifying ambient antibiotics at trace levels.Compared with the precise yet complicate methods such as mass spectrometry(MS)techniques,the enzyme-linked immunosorbent assay(ELISA)offer a simple alternative.While some studies applied it on quantifying environmental pollutants,diverse optimization procedures were employed and matrix effects were not well-addressed.Here,the quantification capability of solid-phase extraction(SPE)coupled with ELISA on ambient antibiotics was evaluated using a newly developed standardized procedure.SPE-ELISA first underwent more rigorous optimization using an overall performance index and three-dimensional recovery response surface.A series of quantitative indicators including precision(relative standard deviation reached 0.3%),sensitivity(a minimal of 3.8 ng/L variation can be distinguished),limit of detection(0.3µg/L without pretreatment),and recoveries(>90%)of SPE-ELISA were achieved and the corresponding conditions were revealed.To eliminate matrix effects,the standard addition method was adopted.This approach,coupled with the linearization of the nonlinear calibration curve,yielded highly accurate(errors of 9%and 5.2%)and reliable(standard deviation of 0.49 and 0.61)results on measuring simulated surface and wastewaters with 5 ng/L and 10 ng/L sulfamethoxazole,which were highly comparable to those of MS methods(P>0.05).Overall,with more rigorous optimization and matrix effect eliminated,the standardized procedure in this study enabled SPE-ELISA to achieve high-quality quantification results.Considering the high throughputs,simple procedure,and low installation costs of SPE-ELISA,it could be a promising alternative for quantifying ambient antibiotics. 展开更多

Electrocatalytic CO2reduction to multi‑carbon(C2+)products offers a promising route for sustainable fuel and chemical production,yet achieving high selectivity and activity under industrial current densities remains a challenge.In this study,we first employed density functional theory(DFT)calculations to screen candidate Cu‑based catalysts and predicted that an Ag‑Cu2O interface would optimize the electronic structure,improve CO intermediate coverage,and lower the energy barrier for C‑C coupling.Subsequently,we engineered an Ag/Cu2O heterojunction catalyst via surface reconstruction and interfacial integration to enhance the activity and C2+selectivity of copper‑based catalysts in CO2RR.The preferred Ag/Cu2O‑2 heterojunction catalyst achieved a C2+Faradaic efficiency(FE)of 77.8%at‑300 mA cm‑2in a flow cell.Comprehensive in situ characterization and additional DFT calculations reveal that Ag‑Cu2O interfaces enhance CO adsorption,thereby promoting C‑C coupling and accelerating its subsequent proton‑coupled electron transfer(PCET).This synergy ultimately boosts C2+products formation over the Ag/Cu2O catalyst. 展开更多

BACKGROUND Superficial basal cell carcinoma(BCC)is a common skin cancer for which surgical treatment is currently the primary clinical approach.However,due to the large surgical incision,noticeable scarring can easily occur postoperatively,affecting the patient’s facial aesthetics.As such,providing appropriate nursing support during the perioperative period is essential.AIM To investigate the impact of psychological nursing on anxiety,depression,wound healing rates,and satisfaction in patients with superficial BCC.METHODS Clinical data from 87 patients with superficial facial BCC,who were treated at the Central Hospital of Enshi Prefecture Tujia and Miao Autonomous Prefecture between February 2023 and February 2025,were retrospectively analyzed.Patients were divided into 2 groups according to nursing method:Control(routine nursing,n=42);and intervention(psychological in addition to routine nursing,n=45).Anxiety and depression symptoms,wound healing rate,pain level,postoperative complications,quality of life,and satisfaction with nursing were compared between the groups.RESULTS After the nursing intervention,the study group exhibited significantly lower Hamilton Anxiety and Depression Scale scores than the control group(P<0.05).The wound healing rate in the intervention group(91.11%)was significantly higher than that in the control group(80.95%)(P<0.05),and the visual analog scale score was lower in the intervention group(P<0.05).The total incidence of complications in the intervention group(6.67%)was lower than that in the control group(19.05%)(P<0.05).Postoperative quality of life and satisfaction with nursing were higher in the intervention group(P<0.05).CONCLUSION Psychological nursing intervention(s)effectively alleviated negative emotions,promoted wound healing,reduced pain,improved satisfaction,and reduced postoperative complications in patients with superficial facial BCC. 展开更多

Melon is a globally cultivated horticultural crop with a predominantly hybrid commercial seed market in China.Seedling morphology,particularly hypocotyl color,is a valuable trait for rapid F1 hybrid seed purity assessment.While green hypocotyls are common,white hypocotyls are rare in melon germplasm.This study identified a mutant with white hypocotyl but green leaves from the heavy ion beam mutant library.Genetic analysis revealed that a single recessive gene controlled the white hypocotyl,designated CmGhc1.A single-base deletion in the fifth exon of CmGhc1 led to a truncated CmGhc1 lacking the HTH-MYB DNA binding domain,likely affecting its transcriptional activity.CmGhc1 was localized in the nucleus,and yeast two-hybrid analysis and a dual-LUC assay demonstrated it as a transcription repressor.Furthermore,a KASP marker(hc1)was developed and verified as a functional marker for breeding white hypocotyl germplasms in melon.RNA-seq data revealed that CmGhc1 significantly affected the transcription of genes related to chlorophyll metabolism and photosynthesis in hypocotyl.In summary,these findings contribute to our understanding of chloroplast biogenesis and provide a valuable tool for melon breeding. 展开更多

Rail profile optimization is a critical strategy for mitigating wear and extending service life.However,damage at the wheel-rail contact surface goes beyond simple rail wear,as it also involves fatigue phenomena.Focusing solely on wear and not addressing fatigue in profile optimization can lead to the propagation of rail cracks,the peeling of material off the rail,and even rail fractures.Therefore,we propose an optimization approach that balances rail wear and fatigue for heavy-haul railway rails to mitigate rail fatigue damage.Initially,we performed a field investigation to acquire essential data and understand the characteristics of track damage.Based on theory and measured data,a simulation model for wear and fatigue was then established.Subsequently,the control points of the rail profile according to cubic non-uniform rational B-spline(NURBS)theory were set as the research variables.The rail’s wear rate and fatigue crack propagation rate were adopted as the objective functions.A multi-objective,multi-variable,and multi-constraint nonlinear optimization model was then constructed,specifically using a Levenberg Marquardt-back propagation neural network as optimized by the particle swarm optimization algorithm(PSO-LM-BP neural network).Ultimately,optimal solutions from the model were identified using a chaos microvariation adaptive genetic algorithm,and the effectiveness of the optimization was validated using a dynamics model and a rail damage model. 展开更多

Background:Currently,there is a deficiency in a strong risk prediction framework for precisely evaluating the likelihood of severe postoperative complications in patients undergoing elective hepato-pancreatobiliary surgery subsequent to experiencing breakthrough infection of coronavirus disease 2019(COVID-19).This study aimed to find factors predicting postoperative complications and construct an innovative nomogram to pinpoint patients who were susceptible to developing severe complications following breakthrough infection of COVID-19 after undergoing elective hepato-pancreato-biliary surgery.Methods:This multicenter retrospective cohort study included consecutive patients who underwent elective hepato-pancreato-biliary surgeries between January 3 and April 1,2023 from four hospitals in China.All of these patients had experienced breakthrough infection of COVID-19 prior to their surgeries.Additionally,two groups of patients without preoperative COVID-19 infection were included as comparative controls.Surgical complications were meticulously documented and evaluated using the comprehensive complication index(CCI),which ranged from 0(uneventful course)to 100(death).A CCI value of 20.9 was identified as the threshold for defining severe complications.Results:Among 2636 patients who were included in this study,873 were included in the reference group I,941 in the reference group II,389 in the internal cohort,and 433 in the external validation cohort.Multivariate logistic regression analysis revealed that completing a full course of COVID-19 vaccination>6 months before surgery,undergoing surgery within 4 weeks of diagnosis of COVID-19 breakthrough infection,operation duration of 4 h or longer,cancer-related surgery,and major surgical procedures were significantly linked to a CCI>20.9.A nomogram model was constructed utilizing CCI>20.9 in the training cohort[area under the curve(AUC):0.919,95%confidence interval(CI):0.881–0.957],the internal validation cohort(AUC:0.910,95%CI:0.847–0.973),and the external validation cohort(AUC:0.841,95%CI:0.799–0.883).The calibration curve for the probability of CCI>20.9 demonstrated good agreement between the predictions made by the nomogram and the actual observations.Conclusions:The developed model holds significant potential in aiding clinicians with clinical decisionmaking and risk stratification for patients who have experienced breakthrough infection of COVID-19 prior to undergoing elective hepato-pancreato-biliary surgery. 展开更多

Background:Given the suboptimal quality of end‐of‐life care among patients with cancer in China,promoting living wills is critical in this population.Living wills ensure that individuals can receive the medical care they desire during the terminal phase of an illness,maintain their dignity,and ultimately achieve a good death.However,current awareness and attitudes about living wills among Chinese patients with cancer remain unclear.We administered a questionnaire survey on living wills to patients with malignant tumors to assess their most important needs and increase understanding about living wills.Methods:In this cross‐sectional study using convenience sampling,inpatients with malignant tumors in Wuhan completed our questionnaire between July 2020 and June 2021.We collected patients'sociodemographic characteristics and details regarding their knowledge and attitudes about living wills.Results:Among 213 patients with malignant tumors,114(53.52%)had heard of living wills;125(58.69%)expressed their willingness to sign the“Five Wishes”living will document after learning about it through the questionnaire.Patients with malignant tumors had a high level of desire for the following living will items:the lives of family and friends return to normal as soon as possible after their death,maintaining personal hygiene and dignity,and remaining pain‐free.The knowledge level of patients with malignant tumors was related to their educational level(p<0.05)and self‐care ability(p<0.05).Conclusions:Patients with malignant tumors have a high need for comfort,cleanliness,and pain relief in the terminal stages.Patients with a higher level of education and those with poorer self‐care ability had greater knowledge and acceptance of living wills.Promotion can first be targeted toward more highly educated patients and can then be gradually expanded to other groups. 展开更多

Periodontitis is a chronic inflammatory disease caused by oral pathogens,and the osteogenic potential of human periodontal ligament stem cells(hPDLSCs)is severely impaired under the inflammatory environment.Current clinical periodontitis treatment strategies such as surgical interventions and antibiotic delivery still suffer from poor antibacterial efficacy,difficulty in ameliorating excessive inflammatory responses and slow periodontal tissue regeneration.Here,we have innovatively developed a non-surgical treatment strategy based on a functional composite hydrogel.A composite hydrogel system(Pt@ZIF-8/ALN-ac/Gel)containing bioactive zeolite imidazolate framework-8(ZIF-8)integrated with platinum nanoparticles(Pt@ZIF-8)and alendronate acrylamide(ALN-ac)was constructed on the basis of gelatin methacryloyl(GelMA)to achieve enhanced antibacterial effect and reactive oxygen species(ROS)scavenging ability while promoting the osteogenic potential of hPDLSCs.We confirmed that Pt@ZIF-8/ALN-ac/Gel was able to continuously release Zn2+and exerted an obvious antibacterial effect against Porphyromonas gingivalis.In vitro experiments proved that Pt@ZIF-8/ALN-ac/Gel had good biocompatibility,while efficiently featuring excellent reactive oxygen species(ROS)scavenging capacity,increasing alkaline phosphatase activity,and promoting extracellular matrix mineralization by hPDLSCs.In vivo,Pt@ZIF-8/ALN-ac/Gel significantly inhibited the alveolar bone deterioration and reduced osteoclast activation and inflammation,thereby promoting the regeneration of damaged tissues.These findings demonstrated superior therapeutic efficacy in the reported clinical periodontitis treatment,exhibiting great potential for application. 展开更多

Constructing impermeable curtains to contain contaminant in aquifers is a costly and complex process that can impact the structure integrity of aquifer systems.Are impermeable curtains necessary for a groundwater contaminant remediation project?This study evaluates the necessity of impermeable curtains for groundwater contaminant remediation projects.Specifically,it considers remediation efforts based on the Pump and Treat(PAT)technique under various hydrogeological conditions and contaminant properties,comparing the total remediation cost and effectiveness.To further investigate,a multi-objective simulation and optimization model,utilizing the Multi-Objective Fast Harmony Search(MOFHS)algorithm,was employed to identify optimal groundwater remediation system designs that without impermeable curtains.Both a two-dimensional(2-D)hypothetical example and a three-dimensional(3-D)field example were used to assess the necessity of constructing impermeable curtains.The 2-D hypothetical example demonstrated that the installation of impermeable curtain is justified only when the dispersivity(αL)of the contaminant reaches 100 meters.In most cases,particularly at sites with porosity(n)under 0.3,alternative,more cost-effective,and efficient remediation strategies may be available,making impermeable barriers unnecessary.The optimization results of the 3-D field example further corroborate the conclusions derived from the 2-D hypothetical example.These findings provide valuable guidance for more scientifically informed,reasonable,and cost-effective groundwater contaminant remediation projects. 展开更多

BACKGROUND Atopic dermatitis is a chronic inflammatory skin disease that poses a substantial burden on patients'physical health,as well as on their psychological health and quality of life.This study specifically examines active disease phases(excluding spontaneous remission periods)to investigate the efficacy of continuous treatments on the psychological state and life quality of patients with atopic dermatitis.AIM To evaluate the effectiveness of continuous treatment strategies,including dermatological care,psychological counseling,education,and long-term follow-up,on the psychological state and quality of life in patients with atopic dermatitis.METHODS A retrospective cohort study was conducted on 120 atopic dermatitis patients treated at our hospital between June 2023 and May 2024.A total of 120 patients were randomly assigned into control and intervention groups(60 patients each).The control group underwent routine treatment,and the intervention group was treated using continuous treatment strategies,including personalized skin care plan,psychological counseling,patient education,and long-term follow-up.Quality of life and psychological status of patients were assessed using the dermatology life quality index and Hospital Anxiety and Depression Scale.RESULTS At 12 months post-intervention,the dermatology life quality index scores in the intervention group were significantly lower than those in the control group(P<0.01),implying improved quality of life.The anxiety and depression symptoms were significantly lower in the intervention group when compared to the control group(P<0.05)as per the Hospital Anxiety and Depression Scale.In the intervention group,the rate of recurrence of skin symptoms was 15%compared to 35%in the control group,with a statistically significant difference between the two groups.CONCLUSION Continuous treatment for atopic dermatitis patients greatly improve their psychological state and quality of life,enhance disease-free survival,and offer new trains of thought for clinical treatment.Personalized integrated management over a prolonged time is important to improve quality of life in such patients. 展开更多

Chen et al demonstrated that regulator of G protein signaling(RGS)4 promotes gastric cancer(GC)progression by activating the focal adhesion kinase/phospha-tidyl-inositol-3-kinase/protein kinase B pathway and inducing epithelial-mesen-chymal transition.Although their multilevel approach integrating clinical data,functional assays,and xenograft models demonstrated a key role for RGS4 in GC pathogenesis,several limitations should be considered.The mechanism of the RGS4-focal adhesion kinase interaction remains unclear,specifically whether it involves direct binding or intermediaries.The clinical analysis of 90 patients lacks stratification by GC subtypes or immune features,potentially limiting generaliz-ability.Furthermore,fully validating RGS4’s oncogenic role requires additional studies,including functional assays in chemotherapy-resistant and metastatic cell lines,metastasis models including orthotopic implantation and tail vein injection,and comparison with other RGS family members.Addressing these via targeted mechanistic studies and expanded clinical validation could strengthen RGS4’s po-tential as a therapeutic target in GC. 展开更多
