Gut microbiota:a novel target for traditional Chinese medicine in the treatment of HIV/AIDS immunological non-responders 认领 引用

Despite effective antiretroviral therapy(ART),many individuals with human immunodeficiency virus(HIV)/acquired immune deficiency syndrome(AIDS)achieve viral suppression but fail to fully restore cluster of differentiation 4(CD4)+T lymphocyte(CD4 cell)counts—a condition known as immunological non-response(INRs).INRs are associated with elevated health risks,including increased susceptibility to AIDS-related and non-AIDS-related complications.The pathogenesis of INRs remains incompletely understood,and no established therapeutic interventions exist,posing a major challenge in contemporary HIV/AIDS management.Emerging evidence indicates that INRs exhibit significant alterations in gut microbiota composition.Dysbiosis of the gut microbiota may contribute to persistent immune activation,cytokine imbalance,and cellular pyroptosis,all of which could impair immune reconstitution in people living with HIV/AIDS.Traditional Chinese medicine(TCM)has demonstrated potential immunomodulatory effects and is increasingly utilized in the management of INRs.Targeting the gut microbiota and elucidating the mechanisms by which TCM modulates this microbial ecosystem may offer new avenues for preventing and treating INRs.This review explores the interplay between gut microbiota and TCM,examines the association between gut dysbiosis and INRs,discusses the mechanistic pathways through which microbiota imbalance contributes to INRs development,and highlights how TCM interventions regulate gut microbiota to promote immune recovery.By focusing on the gut microbiota as a therapeutic interface,this article provides novel insights into TCM-based strategies for improving outcomes in INRs and supports the development of innovative treatment approaches. 展开更多

Agave americana L.is potentially a source of functional and nutritive compounds.However,its yield has been compromised by vascular wilt,which is associated with the presence of the Fusarium oxysporum.In response to this phytosanitary problem,the implementation of efficient methods to mitigate the damage.Biotechnological techniques offer a viable alternative to improve and increase the production of species of interest via genetic improvement.By use of mutagenic chemical agents,these techniques have been consolidated as a powerful tool to induce genetic variability and select genotypes with greater tolerance to pathogens.In this study,we evaluated the activity of three pathogenesis-related(PR)enzymes,namelyβ-1,3-glucanase(PR2),chitinase(PR3),and peroxidase(PR9),that were present in second generation(F2)plants belonging to two resistant clonal lines and to one A.americana line that was susceptible to F.oxysporum.This was used to generate information that can be applied to biotechnology strategies for genetic preservation of the species and agro-industrial use.Plants that were six months old were selected,which were susceptible(Control)and resistant(Clone 1,Clone 4).These were inoculated with F.oxysporum and kept under controlled greenhouse conditions.Enzyme activity was assessed at 0 and 72 hpi and then 7,15,and 30 dpi.The results showed a significant decrease inβ-1,3-glucanase enzyme activity in susceptible plants(control)and in clone 4,as compared to clone 1 plants.Regarding chitinase activity,a progressive increase was observed in control plants and clone 1 over time,while a steadier regulation was detected in clone 4.Lastly,peroxidase activity decreased over time in infected control plants,while dynamic variations were observed in both clonal lines,suggesting a regulated response.These findings indicate that enzyme activity plays a key role in defense against pathogens and that each clonal line of A.americana could activate different defense mechanisms against infection by F.oxysporum. 展开更多

BACKGROUND Duodenal papillary adenocarcinoma is exceptionally rare in patients with acquired immunodeficiency syndrome(AIDS),and while this would normally be treated by radical resection,such treatment has historically been considered highrisk in AIDS patients due to their immune deficiencies.CASE SUMMARY This report presents the case of a 63-year-old female with AIDS who underwent pancreaticoduodenectomy for duodenal papillary adenocarcinoma(pT2N1M0,stage IIIA)and achieved 5-year survival.Through standardized antiretroviral therapy,her CD4+levels were maintained above 200 cells/μL.Perioperative infection prophylaxis included piperacillin-tazobactam and ornidazole.Postoperative adjuvant chemotherapy was initiated based on her absolute neutrophil count being≥1.50×109/L but was reduced due to the occurrence of recurrent grade 3 diarrhea;ultimately,the patient could tolerate only 70%of the standard XELOX regimen over four cycles.The surgery was completed without complications.During over 5 years of follow-up,surveillance imaging showed no recurrence or metastasis,the tumor markers remained normal,and her performance status was fully recovered.CONCLUSION This case demonstrates that with immune optimization(CD4+>200 cells/μL),tailored infection prophylaxis,and neutrophil-guided chemotherapy,radical surgery can be feasible and can enable long-term survival in AIDS patients with duodenal papillary adenocarcinoma,suggesting that the CD4+level is a potential threshold for assessing patient eligibility for surgery in comparable cases. 展开更多

AIM:To assess the influence of preoperative lacrimal passage probing and dilation on the therapeutic effectiveness of nasolacrimal silicone intubation in cases of incomplete primary acquired nasolacrimal duct obstruction(PANDO).METHODS:A retrospective case-control clinical study was conducted.The study included patients with incomplete PANDO who underwent nasolacrimal silicone intubation at the hospital between October 2022 and October 2024.Participants were categorized into two groups based on their receipt of nasolacrimal duct probing and dilation prior to intubation.Group A consisted of patients who had undergone one or more sessions of nasolacrimal duct probing and dilation before nasolacrimal silicone intubation,while Group B included patients who did not receive nasolacrimal duct probing and dilation before intubation.All patients had their nasolacrimal silicone tubes removed 3 mo postoperatively and were monitored for 6 mo following tube removal to evaluate surgical success rates.RESULTS:A total of 136 patients(152 eyes)completed the six-month follow-up period.The overall surgical success rate for both groups was 72.4%(110/152 eyes).Group A included 22 males and 36 females,mean age 58.09±11.07 y,and achieved a success rate of 82.8%(53/64 eyes),while Group B included 27 males and 51 females,mean age 59.35±8.84 y and attained a success rate of 64.8%(57/88 eyes).The difference in surgical success rates between the two groups was statistically significant(P=0.014).Furthermore,Group A achieved a complete success rate of 67.2%(43/64 eyes),compared to 47.7%(42/88 eyes)in Group B,with this difference also reaching statistical significance(P=0.017).No serious postoperative complications were observed in either group.CONCLUSION:Nasolacrimal silicone intubation is a safe and effective intervention for incomplete PANDO,with preoperative nasolacrimal duct probing and dilation potentially enhancing therapeutic outcomes. 展开更多

Background:Introduction Human Immunodeficiency Virus(HIV)continues to pose a significant public health challenge worldwide.Depressive and anxiety disorders are common in People Living with HIV(PLHIV)and negatively impact adherence,quality of life,and disease outcomes(World Health Organization[WHO],2025b;UNAIDS,2024).Social stigma,discrimination,unemployment,and poor family support are common predictors of depression and anxiety among PLHIV(Ali et al.,2024).Objective:To find out how common depression and anxiety are among People Living with HIV(PLHIV)at rural area of country in Sindh at the HIV Treatment Centre in Nawabshah Sindh Pakistan,we looked at data from January 2025 to December 2025.Methodology:We did a study with 254 PLHIV who visited the HIV Treatment Centre in Nawabshah from January to December 2025.We collected information using questionnaires that asked about their background,health,depression,and anxiety.We used a computer program called SPSS to analyze the data.We looked for things that might be connected to depression and anxiety.If the results showed a connection,we considered it important if the p-value was than 0.05.Results:Out of 254 people 109(42.9%)had depression and 96(37.8%)had anxiety.We found that depression was more common in people who were unemployed felt like they were being treated unfairly did not have family support had a low income and did not take their HIV medicine as prescribed.Anxiety was more common in women,people who used substances,those who had been sick for a time and people who faced social discrimination.Depression and anxiety are very common among PLHIV at the HIV Treatment Centre,in Nawabshah.We think that checking for health issues and providing support should be a regular part of HIV treatment. 展开更多

Objective:To explore the risk factors of acquired weakness in ICU patients and construct a prediction model.Methods:Using convenience sampling method,245 intensive care patients admitted to ICUs of three tertiary hospitals in Shiyan City from March 2022 to April 2023 were selected,of which 172 cases from March to December 2022 were used as the modeling group and 73 cases from January to April 2023 were used as the validation group.The predictive effect of the model was examined using the area under the curve(AUC)of the receiver operating characteristic curve(ROC)and Hosmer-Lemeshow goodness-of-fit,and the model column line graph was plotted.Results:The incidence of ICU-acquired weakness was 36.63%(63/172)and 38.37%(28/73)in the modeling and validation groups,respectively.Patient’s history of alcohol consumption,mode of admission to the ICU,treatment with CRRT,mechanical ventilation,restraint braking,use of analgesics,use of norepinephrine,use of glucocorticoids,and length of stay in the ICU were the independent influencing factors of acquired weakness in ICU patients(p<0.05).The AUCs of the modeling and validation groups were 0.979 and 0.887,respectively.Conclusion:The risk prediction model constructed in this study can predict the risk of acquired weakness in ICU,which can provide a reference for healthcare professionals to formulate interventions at an early stage. 展开更多

Research on the Resistance Mechanism of Third-generation EGFR-TKIs in Non-Small Cell Lung Cancer 认领 引用

Third-generation epidermal growth factor receptor tyrosine kinase inhibitors(EGFR-Tkis)represented by osimertinib have become the first-line standard treatment option for EGFR-mutated non-small cell lung cancer(NSCLC),significantly improving progression-free survival and overall survival in patients.However,almost all patients eventually develop acquired resistance,which severely limits the long-term efficacy.This article systematically reviews the main molecular mechanisms of resistance to third-generation EGFR-Tkis and the latest research progress.Existing evidence suggests that resistance mechanisms can be roughly divided into two major categories:EGFR-dependent(on-target)and EGFR-independent(off-target).EGFR-dependent resistance mainly includes secondary mutations such as C797S,mutations in other kinase domains,and EGFR gene amplification,which directly affect drug binding to the target.Egfr-independent resistance is more complex,involving bypass signaling pathway activation(such as MET amplification,HER2 or AXL abnormalities),histological transformation(such as adenocarcinoma to small cell lung cancer transformation),downstream signaling pathway abnormalities(RAS/MAPK,PI3K/AKT pathways),and cell cycle regulation imbalance.In addition,recent studies have further revealed the important roles of metabolic reprogramming,epigenetic regulation,tumor-microenvironment interactions,and drug-resistant persistent cells in the formation of resistance.In terms of therapeutic strategies,novel approaches such as combined targeted therapy for resistance mechanisms,next-generation EGFR inhibitors,bispecific antibodies,antibody-drug conjugations,immunotherapy and cell therapy are advancing and showing good clinical prospects in some patients.Overall,third-generation EGFR-TKI resistance shows high heterogeneity and dynamic evolution characteristics,and in the future,multi-omics detection,dynamic molecular monitoring,and individualized combination therapy strategies will be needed to achieve more precise and durable disease control. 展开更多

AIM:To investigate the etiology of ocular pathogens and to establish the various pathogens present in human immunodeficiency virus(HIV)patients with cytomegalovirus retinitis(CMVR).METHODS:A total of 17 HIV-infected patients with concomitant eye disorders were enrolled.Patients were divided into CMVR group(10 patients,18 eyes)and non-CMVR group(7 patients,9 eyes)based on clinical manifestations and the presence of cytomegalovirus(CMV)-DNA in ocular specimens.The viral load of CMV was assessed using polymerase chain reaction in aqueous humor,vitreous fluid,and peripheral blood samples of patients in the CMVR group.Additionally,peripheral blood CD4+T cell counts were measured in both groups.RESULTS:In the CMVR group,the CMV-DNA load in the vitreous and aqueous humor samples was substantially higher than in the peripheral blood samples(P<0.01).CMVDNA load in the aqueous humor and vitreous samples of the two eyes in the CMVR group was determined to be statistically significant(10 patients,16 eyes,P=0.018,0.012).Peripheral blood CD4+T cell counts in the CMVR group were adversely linked with the CMV-DNA load in both the aqueous humor and peripheral blood(P=0.005,0.048).Compared with the non-CMVR group,the peripheral blood CD4+T cell count in the CMVR group decreased significantly(P=0.014).The peripheral blood CD4+T cell count exceeded 300 cells/μL in 85.71%of non-CMVR patients,whereas it was below 100 cells/μL in 90.00%of the CMVR group.The intraocular specimens of the patients who underwent CMVR testing did not include any additional infections.CONCLUSION:In HIV-associated CMVR patients,there may exist alternative,yet unidentified,infection pathways for intraocular CMV in addition to the conventional route.The substantial difference in CMV-DNA load between the eyes of most CMVR patients suggests that CMV may originate from different sources in each eye.The proportion of peripheral blood CD4+T cells in HIV patients is negatively correlated with the quantity of CMV viruses in their eyes.The peripheral blood count of<100 cells/μL indicates a considerable increase in the risk of concurrent CMVR.Multi-ocular pathogen presentations are uncommon in HIV individuals with CMVR. 展开更多

Acquired brain injury(ABI)is an injury that affects the brain structure and function.Traditional ABI treatment strategies,including medications and rehabilitation therapy,exhibit their ability to improve its impairments in cognition,emotion,and physical activity.Recently,near-infrared(NIR)photobiomodulation(PBM)has emerged as a promising physical intervention method for ABI,demonstrating that low-level light therapy can modulate cellular metabolic processes,reduce the in flammation and reactive oxygen species of ABI microenvironments,and promote neural repair and regeneration.Preclinical studies using ABI models have been carried out,revealing the potential of PBM in promoting brain injury recovery although its clinical application is still in its early stages.In this review,we first inspected the possible physical and biological mechanisms of NIR-PBM,and then reported the pathophysiology and physiology of ABI underlying NIR-PBM intervention.Therefore,the potential of NIR-PBM as a therapeutic intervention in ABI was demonstrated and it is also expected that further work can facilitate its clinical applications. 展开更多

Intensive care unit(ICU)acquired sarcopenia and myosteatosis are increasingly recognized complications of critical illness,characterized by a rapid loss of ske-letal muscle mass,quality,and function.These conditions result from a complex interplay of systemic inflammation,immobilization,catabolic stress,mitochon-drial dysfunction,and immune dysregulation,often culminating in impaired recovery,prolonged hospitalization,and increased long-term mortality.First identified in survivors of sepsis and prolonged mechanical ventilation,these muscle abnormalities were initially described using computed tomography-based assessments of muscle area and density.Subsequent advances in imaging,biomarker discovery,and functional testing have enabled earlier detection and risk stratification across diverse ICU populations.While nutritional optimization and early mobilization form the cornerstone of current prevention and treatment strategies,the emergence of novel approaches,including automated artificial intelligence-based screening,neuromuscular electrical stimulation,and targeted pharmacologic therapies,has broadened the clinical scope of interventions.Despite their significant prognostic implications,ICU-acquired sarcopenia and myosteatosis remain under-recognized in routine critical care practice.This mini-review aims to synthesize current knowledge regarding their pathophysiology,available diagnostic modalities,prognostic relevance,and the evolving landscape of therapeutic strategies for long-term functional recovery in critically ill patients. 展开更多

Objectives:This study aimed to describe the development process of the peer precision-matching and health management mobile platform"Aspark"for people living with human immunodeficiency virus(HIV)(PLWH)and to evaluate its usability.Methods:The study assembled a multidisciplinary research and development team comprising researchers,software engineers,art designers,clinical nursing specialists,HIV peer volunteers,and psychological counselors.The team employed an agile development model to iteratively build the platform.Using the social determinants of health framework and the interpersonal circumplex,we identified 14 matching variables for the mobile platform.A video production team was formed to create peer training videos on health management for PLWH.We integrated relevant tools to provide PLWH with various types of support,including informational,instrumental,emotional,affiliational,and appraisal assistance.From March to May 2024,the research team used convenience sampling to select 130 platform users.It administered an electronic questionnaire based on the System Usability Scale to evaluate the platform's overall usability.Results:The platform incorporates multiple functional modules,including"Precision Matching,""Drug Interaction Query,""Medication Reminder,""Health Management,""Mood Diary,""Community Discussion,"and"Activity Center,"achieving comprehensive coverage across informational,emotional,instrumental,social,and appraisal support.The precision matching module facilitates intelligent pairing between patients and peer volunteers;the health management module allows for the entry of test results and trend analysis;medication reminders and check-in features enhance medication adherence;the mood diary supports emotional recording and expressive writing;while the community module offers functions such as topic discussions,appointment scheduling,and organization of offline activities.Among the 130 valid questionnaires,more than 83%of users scored above 68 on the System Usability Scale,indicating that the platform demonstrates favourable usability.Conclusions:The"Aspark"platform offers a feasible and promising digital solution for precision matching in HIV peer support and health management,enabling healthcare providers to deliver personalized and continuous health support to patients through its functions.Well-designed clinical trials should be conducted in the future to evaluate the effectiveness of this tool. 展开更多

OBJECTIVE:To comprehensively evaluate randomized controlled trials(RCTs)investigating the effects of moxibustion on people with human immunodeficiency virus(HIV)disease.METHODS:A systematic search was conducted across eight electronic databases up to August 20,2024.The primary outcome were all-cause mortality and acquired immunodeficiency syndrome(AIDS)-related mortality.Two authors independently screened titles and abstracts,and extracted data onto a pre-designed datasheet.Discrepancies were resolved through consensus.The Cochrane risk-of-bias tool 2.0 was used to assess methodological quality,Meta-analysis was performed when appropriate,and the quality of evidence was assessed through Grading of Recommendations,Assessment,Development and Evaluations approach.RESULTS:Eleven RCTs(n=834)on moxibustion for HIV/AIDS were included,focusing on individuals with HIV infection(5 RCTs,n=426),AIDS patients(3 RCTs,n=223),or both(3 RCTs,n=185).Complications identified included diarrhea(5 RCTs),pulmonary infection(1 RCT),anxiety and depression(1 RCT)and peripheral neuropathy(1 RCT).The risk of bias in the included RCTs was assessed as either high or uncertain.No trial reported mortality or the incidence of AIDS-related complications following treatment.Wheat-grain sized cone moxibustion was associated with increased CD4+counts in patients with lung infections[1 RCT,n=36,mean difference(MD)=78.83 cells/μL].Individual studies reported improvements of quality of life,as measured by the World Health Organization Quality of Life-Brief Version(WHOQOL-BREF)and WHOQOL HIV instrument,with various moxibustion types,but clinical heterogeneity prevented data pooling.Additionally,some studies reported symptom improvement,each using different criteria for symptom improvement.Moxa stick moxibustion plus Western Medicine compared with Western Medicine alone showed a non-significant trend towards improved symptom resolution[2 RCTs,n=125,risk ratio=1.19,95%confidence interval(0.99,1.43)].Moxibustion plus antiretroviral therapy(ART)may reduce gastrointestinal adverse events compared to ART alone[1 RCT,n=10014%vs 32%,P<0.05].The quality of evidence was low to very low.CONCLUSION:This systematic review suggests that moxibustion as an adjunct therapy may have potential benefits in improving immune function and quality of life for HIV/AIDS patients.Limited quality of evidence precludes definitive conclusions,and further high-quality research is needed. 展开更多

Debate regarding the premature aging of knee joints in acquired immune deficiency syndrome(AIDS)patients has remained contentious,with conjectures pointing towards its correlation with distinct antiviral regimes.Protease inhibitors(PIs)stand as a prominent class of antiviral agents frequently utilized in AIDS management and have been significantly linked to premature senescence.This study aimed to investigate whether PI-containing regimens would accelerate osteoarthritis(OA)development and explore the molecular mechanisms underlying this association.A retrospective cohort of 151 HIV-infected individuals,categorized into PI and non-PI groups,was established.Patients in PI group exhibited lower KOOS and a higher prevalence of radiological knee OA than those in non-PI group.Additionally,25 anti-HIV drugs were screened and among all antiviral drugs,lopinavir had the most detrimental impact on cartilage anabolism,accelerating cartilage senescence and promoting mouse OA development.Mechanistically,lopinavir accelerated cellular senescence by inhibiting Zmpste24 and interfering nuclear membrane stability,which leads to decreased binding between nuclear membrane-binding protein Usp7 and Mdm2 and activates Usp7/Mdm2/p53 pathway.Zmpste24 overexpression reduces OA severity in mice.These findings suggest that PI-containing regimens accelerate cartilage senescence and OA development through Zmpste24 inhibition,which provides new insights into the selection of HIV regimens. 展开更多

AIM:To investigate the clinical profile of patients with acquired lacrimal sac mucocele(ALSM)and evaluate the efficacy of endoscopic dacryocystorhinostomy(En-DCR)for this condition.METHODS:En-DCRs were performed on 141 patients with ALSM patients from January 2016 to March 2022.The clinical baseline information and magnetic resonance imaging(MRI)images were recorded and summarized.To assess the effectiveness of En-DCR therapy,both anatomical and functional success rate was assessed during a 12mo follow-up.RESULTS:A total of 141 patients,with a mean age of 57.70±14.11y,were enrolled in this study.Majority of the patients were female(n=91;64.54%)and all had unilateral disease.All patients had a previous history of epiphora and purulent secretion,and the duration from lacrimal duct obstruction to mucocele formation ranged from 6 to 120mo.MRI findings consistently revealed an enlarged sac diameter,fluid accumulation separated by a thin rim from adjacent tissues,which is indicative of lacrimal sac mucocele.En-DCR was performed with an anatomical success rate of 93.62%and a functional success rate of 81.56%.CONCLUSION:ALSM is more commonly seen in females and unilaterally.It is essentially a complication of lacrimal duct obstruction.MRI characteristics can be used for precise clinical diagnosis,while En-DCR emerges as an optimal therapy for this condition.Our results provide a comprehensive reference for the diagnosis and treatment of ALSM. 展开更多

BACKGROUND Mycoplasma pneumoniae(M.pneumoniae)is considered to be one of the causative agents of community acquired pneumonia in children with general or severe course of disease.Severe M.pneumoniae pneumonia(SMPP)has emerged as a crucial global health concern due to high mortality rate in children under 5 years,potentially life-threatening complications,and growing challenges in pediatric treatment associated with rising macrolide resistance.Additionally,MPP can be complicated by other bacterial and/or viral pathogens,which may exacerbate disease severity.After the lifting of strict non-pharmaceutical interventions(NPIs)worldwide,the dramatic rise of incidence of MPP in Asia and Europe was observed.AIM To perform the comprehensive study of community acquired MPP cases registered in 2023 in Baoding Hospital,China.METHODS A total of 1160 children from 1 month to 15 years old with confirmed MPP diagnosis were enrolled in the study.The blood and respiratory samples were collected within the 24 hours after admission.The hematological parameters,biochemical markers,cytokine profiles were assessed.The respiratory samples were tested for the presence of M.pneumoniae and other 23 bacterial/viral pathogens by multiplex polymerase chain reaction(PCR).The macrolide resistance mutations(A2063G,A2064G in the 23S rRNA gene of M.pneumoniae)were determined by PCR.RESULTS Number of MPP cases has dramatically increased starting August with peak in November.SMPP and general MPP(GMPP)were identified in 264 and 896 of 1160 hospitalized children.The binary logistic regression analysis identified six[C-reactive protein(CRP),lactate dehydrogenase,procalcitonin,erythrocyte sedimentation rate,fibrin and fibrinogen degradation products(FDPs),D-dimer]and four(neutrophils,CRP,FDPs,prothrombin time)predictors of SMPP in age groups 2-5 years and 6-15 years,respectively.Children with SMPP showed significantly higher levels of cytokine interleukin(IL)-17F(2-5 years),and cytokines interferon-gamma,tumor necrosis factoralpha,IL-10(6-13 years).Concomitant viral/bacterial pathogens were determined in 24.3%and 28.0%cases of SMPP and GMPP.Among them,Streptococcus pneumoniae(S.pneumoniae)and Haemophilus influenzae(H.influenzae)were predominant.93.2%cases of MPP were associated with macrolide resistant M.pneumoniae.CONCLUSION Specific MPP epidemiological pattern associated with lifting NPIs was revealed:Increase of hospitalized cases,prevalence of S.pneumoniae and H.influenzae among concomitant pathogens,93.2%of macrolide resistant M.pneumonia. 展开更多

Objective:Accurate and rapid detection of disease biomarkers is critical for early diagnosis,timely intervention,and effective disease management.Impact Statement:This strategy is exemplified through the development of 2 biosensors for detecting HIV-1 DNA and HIV-1 p24,biomarkers associated with acquired immunodeficiency syndrome(AIDS).Introduction:We propose a general biosensing strategy that leverages a treble signal amplification cascade,demonstrating its versatility and applicability across diverse biomolecular targets.Methods:The method integrates multiple amplification mechanisms to achieve unparalleled sensitivity.Initially,the hybridization of 2 aided probes with the target triggers isothermal amplification facilitated by polymerase and nicking enzyme,providing a robust preliminary signal enhancement.Repeated cycles of primer extension,nicking,and signal primer dissociation then generate multiple signal primers.These primers are further amplified via rolling circle amplification(RCA),resulting in an important secondary signal boost.Finally,the amplified products activate CRISPRCas12a-mediated trans-cleavage,achieving a tertiary level of signal enhancement.Results:This cascade amplification approach achieves remarkable sensitivity,with detection limits of 62 aM for nucleic acids and 8.48 pg/ml for proteins,positioning it as a broadly applicable framework.Clinical samples were assayed,which indicates its capability in clinical diagnosis.Conclusion:Beyond HIV detection,the modular design of this strategy allows adaptation for various biomarkers,showcasing its potential as a universal platform for molecular diagnostics in healthcare and research. 展开更多
