We introduce to the structure and effect of each functional module in this information system. This information system consists of 7 functional modules:background information of seismology and geology, subsystem for rapid evaluation of damage loss from earthquake, subsystem for the decisive information on earthquake emergence, information query, maintenance and management of system, help, quit. Background: Discharged against medical advice (DAMA) is defined as any instance when a patient wants to leave the hospital against the managing physician’s decision. This study aimed to identify factors that influenc...Background: Discharged against medical advice (DAMA) is defined as any instance when a patient wants to leave the hospital against the managing physician’s decision. This study aimed to identify factors that influence patients to decide to be DAMA. Methods: A descriptive, cross-sectional study. The study was conducted in the emergency department (ED) of King Fahad Medical City (KFMC)-Saudi Arabia-Riyadh city. A questionnaire in both Arabic and English was distributed to all participants to fill in either English or Arabic. Results: Between 1 March and 30 April 2021, 510 responses were collected. Most of the study participants (31.4%) were over the age of 54. Our findings showed that 12.5% of our participants had taken discharge against medical advice in the past. Results Regarding Factors That Influence Patients to Decide on DAMA Showed: Regarding Inappropriate behavior and disrespect of the physician or staff to the patient and his relatives, 262 (51.4%) participants, 85 (16.7%) participants, and 163 (32%) participants agreed, neutral, and disagreed, respectively. Regarding the Lack of physicians’ and nurses’ attention to the patient and his relatives (emotionally), our result showed that 278 (54.5%) participants, 95 (18.6%) participants, and 137 (26.9%) participants agreed, neutral, and disagree, respectively. Regarding failure to inform the patient or his relatives of his condition, it showed that 257 (50.4%) participants, 95 (18.6%) participants, and 158 (31%) participants agreed, neutral, and disagreed, respectively. Regarding feeling better from DAMA, our result showed 226 (44.3%) participants, 119 (23.3%) participants, and 165 (32.4%) participants agreed, neutral, and disagreed, respectively. Regarding patients’ or their relative’s tiredness of hospital stay, the result showed that 166 (32.5%) participants, 104 (20.4%) participants, and 240 (47.1%) participants agreed, neutral, and disagreed, respectively. Conclusion: The long wait time to be seen by a physician was the major factor that forced patients to leave the emergency department against medical advice.展开更多

According to census report 2011, there are 60.3% people belonging to 15 to 59 years of age group and the percentage of people above 60+ age is 8.6%. Our country fails to provide due respect, care, affection, security and health related facilities, which elderly people utmost require. A number of cases of adult abuses are reported in news papers as these people are easy victims of violence and criminal activities. The society comes to know about only those problems, which are reported, the innumerable cases of disrespect and neglect are not given any consideration. The situation becomes all the more worst when perpetrators are very near relatives and in some cases blood relatives also. This growing insensitiveness is a threat to our society. Sincere efforts are needed to tackle this alarming problem. Leaving a health facility to see a bonesetter is common there, but a source of complications. The purpose of this work was to analyze the results obtained by bonesetter after discharge against medical advice. Methodology: Analytical cross-sectional study over a period of one year, including all patients admitted for limb fracture, discharged against medical advice for treatment with a bonesetter. Results: Thirty-nine patients were selected. The average age was 36.9 years, and the sex ratio was 5.50. The fractures were open in 35.9%. At a mean follow-up of 19 months, the course was marked by pain in 34.4% and inequality in limb length in 62.5%. Thirteen fractures (40.6%) were consolidated with axis defect, and 11 were without axis defect (34.4%). There were eight non-union (25%), and eleven patients (34.4%) presented with stiffness in one or more joints. Seven patients had died. Discussion: The problem of discharge against medical advice is found in all hospitals but is particularly marked for fractures in our context. The absence of an X-ray and ignorance of the rules of asepsis lead to complications. Only the fractures of the two bones of the leg had progressed well (5/8 good results). The rate of vicious calluses (40.6%) and non-union (25%) is high. Conclusion: Although a few cases have progressed favorably, the results of patients traditionally treated after discharge against medical advice are generally poor.展开更多

Anti-HCV was tested with recombinant immunoblot assay (RIBA) by using three antigens (C22, C33c, and C100-3) encoded by different regions of HCV genome. The changes of individual anti-HCV and ALT were compared with the change of HCV RNA. The results showed that persistent disappearance of serum HCV RNA was closely related to the changes of anti-C33c (P<0. 01) and anti-C100-3 (P<0. 005), but there was no relation between persistent ALT normality and HCV viremia clearance (P<0. 05). In conclusion, monitoring anti-C33c and anti-C100-3 could indicate the changes or HCV viremia. The normalization of ALT after interferon treatment did not indicate disappearance of HCV viremia.展开更多

From the point of view of losses in monetar...The question of the impact of war on ecosystems still remains secondary in the internal and external policy of states, society and the agenda of international organizations. From the point of view of losses in monetary terms, the values of ecosystem damages obtained in the work, which are a consequence of the impact of hostilities on the environment, correspond to the annual budgets of the largest countries in the world or exceed them. The presented calculations significantly exceed the known normative methods, the use of which in the conditions of war is limited in space and time. Objective difficulties associated with the uncertainty of many processes of the development of ecological systems and their reaction to the multifactorial impact of war are also significant limitations. Therefore, as part of the study, a method of assessing the impact of war on the environment is proposed, which is based on the patterns of energy flows in ecosystems from the moment it is binding by producers. This made it possible to take into account in the calculations the principle of functional integrity of the ecological system, according to which the destruction or damage of the components of a functionally whole environment will necessarily cause negative phenomena in the development of ecological systems. The results are presented in the form of real values of ecological losses in energy and monetary equivalents, as consequences of the loss of ecosystem services. As the results of the research show, the minimum amount of damage to ecosystems from Russian tanks is 43,500 USD per day. Environmental damage from Russian fighter jets has been estimated at $1.5 billion per week since the start of the war. Noise from military operations causes losses of at least 2.3 billion US dollars per year. The obtained results create prerequisites for improving the system of ensuring environmental safety at the local, state, and international levels and transferring the obtained solutions into safety-shaping practice.展开更多

[ Method ] SPF layers were randomly divided into four groups, namely group A1, A2, B1 and B2. The group A1 was...[ Objective] To study the correlation between HI titer and protection against IBV challenge in SPF layers. [ Method ] SPF layers were randomly divided into four groups, namely group A1, A2, B1 and B2. The group A1 was immunized with H120 live vaccine. The group A2 was first immunized with H120 live vaccine and later boosted with ND-IB-EDS trivalent inactivated vaccine. The group B1 was used as unimmunized chal- lenge control. The group B2 was kept as unimmunized unchallenged control. The blood samples were taken prior and post-vaccination at intervals and HI tests were conducted. At the laying peak, the group A1, A2 and B1 were challenged with IBV M4t virulent strain. The clinical features and egg production of layers were monitored and recorded. [Result] After 30 d post vaccination with H120 live vaccine, the HI titer reached 4.45 log2; after 30 days boosting with ND-IB-EDS trivalent inactivated vaccine, the HI titer reached to 7.35 log2. Before challenge, HI antibody titer in group A1, A2, B1 and B2 were respectively 4.24 log2, 7.40 Iog2, 2.10 log2 and 2.10 log2. After challenge, chickens in unimmunized challenge control group B1 showed respiratory symptoms, egg production dropped by 30.9%, and they produced more soft-shelled, no-shelled or abnormal eggs. In the group A1, some chickens had light respiratory symptoms and egg production dropped by 11.7%. In the group A2, the egg production of all chickens was as normal as the group B2. [ Conclusion] When the HI titer was over 6 log2, challenge by virulent virus had no impact on egg produc- tion; when the HI titer was 5 log2, 4 log2 and less 3 log2, egg production dropped by 6.0%, 11.3% and 29.6%, respectively. Thus, the HI anti- body level in chickens has close correlation with protection against IBV challenge.展开更多
