Coronavirus disease 2019(COVID-19)is a disease that caused a global pandemic and is caused by infection of severe acute respiratory syndrome coronavirus 2 virus.It has affected over 768 million people worldwide,result...Coronavirus disease 2019(COVID-19)is a disease that caused a global pandemic and is caused by infection of severe acute respiratory syndrome coronavirus 2 virus.It has affected over 768 million people worldwide,resulting in approx-imately 6900000 deaths.High-risk groups,identified by the Centers for Disease Control and Prevention,include individuals with conditions like type 2 diabetes mellitus(T2DM),obesity,chronic lung disease,serious heart conditions,and chronic kidney disease.Research indicates that those with T2DM face a hei-ghtened susceptibility to COVID-19 and increased mortality compared to non-diabetic individuals.Examining the renin-angiotensin system(RAS),a vital regulator of blood pressure and pulmonary stability,reveals the significance of the angiotensin-converting enzyme(ACE)and ACE2 enzymes.ACE converts angiotensin-I to the vasoconstrictor angiotensin-II,while ACE2 counters this by converting angiotensin-II to angiotensin 1-7,a vasodilator.Reduced ACE2 exp-ression,common in diabetes,intensifies RAS activity,contributing to conditions like inflammation and fibrosis.Although ACE inhibitors and angiotensin receptor blockers can be therapeutically beneficial by increasing ACE2 levels,concerns arise regarding the potential elevation of ACE2 receptors on cell membranes,potentially facilitating COVID-19 entry.This review explored the role of the RAS/ACE2 mechanism in amplifying severe acute respiratory syndrome cor-onavirus 2 infection and associated complications in T2DM.Potential treatment strategies,including recombinant human ACE2 therapy,broad-spectrum antiviral drugs,and epigenetic signature detection,are discussed as promising avenues in the battle against this pandemic.展开更多

Limited studies investigated the relationship of the ABO blood group and susceptibility to COVID-19 infection. Objective: To investigate the association between the ABO blood group and the COVID-19 susceptibility. Material & Methods: The study is a retrospective cohort study. It included all hospitalized confirmed COVID-19 patients in Qatif central hospital, Eastern province, Saudi Arabia. We included both genders and people who are older than 14 years of age. The study was conducted in the period between 15th March, and 15th April, 2020. Results: Our sample consists of 72 patients;we found a percentage distribution of 23.62%, 33.33%, 9.72%, and 33.33% for blood groups A, B, AB and O, respectively. Our results showed significantly increased risk for AB blood groups (P Conclusion: Our findings suggest that among confirmed COVID-19 patients, patients with AB blood group have higher susceptibility while patients with O blood group have lower susceptibility to COVID-19 infection. Symptoms of coronavirus disease of 2019(COVID-19),caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),range from asymptomatic to life-threatening,affecting the respiratory system and many other organ...Symptoms of coronavirus disease of 2019(COVID-19),caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),range from asymptomatic to life-threatening,affecting the respiratory system and many other organs,and involved in SARS-CoV-2 pathogenesis,particularly its viral entry,is the enzyme transmembrane protease serine 2(TMPRSS2).In response,in this study,polymorphisms in the TMPRSS2 gene(rs2070788 and rs12329760)were examined for their association with COVID-19 susceptibility and severity in Jordanians.The study participants(n=861)included 427 individuals who had never contracted COVID-19(control group)and 434 who had experienced the disease(case group).The case group was subsequently divided into two subgroups:severe(n=166)and non-severe(n=268)COVID-19.The results show that rs2070788 is associated with disease susceptibility in all inheritance models(P<0.05),as well as an association between rs12329760 and COVID-19 disease severity in both the dominant and over-dominant models(P<0.05).Further,a higher body mass index(BMI)and older age were associated with disease severity(p<0.05),though females were less likely to develop severe COVID-19 and to be hospitalized.In conclusion,polymorphisms in the TMPRSS2 gene may be considered a potential predictor of COVID-19 outcomes in the Jordanian population.The study findings could be used to manage similar viral infections in Jordan in the future.展开更多
Objective:To describe the clinical features and effectiveness of Xuanbai Shengmai Decoction(XB,a compound Chinese herbal medicine)on disease progression,clinical effect and viral RNA shedding in COVID-19 cases.Methods...Objective:To describe the clinical features and effectiveness of Xuanbai Shengmai Decoction(XB,a compound Chinese herbal medicine)on disease progression,clinical effect and viral RNA shedding in COVID-19 cases.Methods:We collected demographic and clinical information of 212 COVID-19 cases from December2022 to January 2023,and divided these cases into two groups based on whether XB was used or not.Survival curves in the two groups were estimated by the Kaplan-Meier method.A smoothing approximation of time-to-event distributions was created based on a log-rank test and a restricted mean survival time model,and the Gaussian density estimation method.In addition,the effect of XB treatment on survival time was assessed using a series of Cox proportional hazards models.Results:Of all 212 COVID-19 cases,there were respectively 63 and 149 cases reported in XB group and non-XB group.Of those,128(60.4%)were male,with median age of 75.00 years,and 72.6%had one or more underlying medical conditions.The most common symptoms on admission were cough(64.6%),fever(39.6%),wheezing(27.8%)and sputum(13.2%).In addition,the gender,age group distribution,smoking,and drinking between two groups were of no significant difference(P>0.05).However,the clinical characteristics and CT findings in XB group were more serious than those in non-XB group.Nevertheless,the hazard of death in XB group was lower than that in non-XB group,especially on severe cases.Treatment with XB significantly protected against adverse outcomes,shorten the time of viral RNA shedding.Moreover,interventional therapy of XB within 1–2 days after hospital admission is confirmed with particularly effectiveness.Conclusion:Treatment of XB is effective for COVID-19 cases,particularly in reducing the mortality rate of severe cases,suggesting that XB may be used as an alternative treatment of SARS-COV-2 infection.展开更多
Objective To investigate the impact of the coronavirus disease 2019(COVID-19)pandemic on the bacterial profile of lower respiratory tract infections(LRTIs)and the prevalence of major drug-resistant bacteria in Hubei P...Objective To investigate the impact of the coronavirus disease 2019(COVID-19)pandemic on the bacterial profile of lower respiratory tract infections(LRTIs)and the prevalence of major drug-resistant bacteria in Hubei Province,China,by comparing five-year periods before(2015-2019)and after(2020-2024)the pandemic.Methods A retrospective analysis was conducted on microbial culture and antimicrobial susceptibility test results from sputum and bronchoalveolar lavage fluid(BALF)samples obtained from patients with LRTIs.Pathogen distribution and the prevalence of key drug-resistant bacteria,including methicillin-resistant Staphylococcus aureus(MRSA),cefotaxime/ceftriaxone-resistant Enterobacterales,carbapenem-resistant Enterobacterales(CRE),carbapenem-resistant Pseudomonas aeruginosa(CRPAE),carbapenem-resistant Acinetobacter baumannii(CRABA),ampicillin-resistant Haemophilus influ-enzae(ARHI),and penicillin/erythromycin-resistant Streptococcus pneumoniae(PRSP/ERSP),were compared between the two periods.Results The overall number of bacterial isolates significantly increased during the post-pandemic period.Gram-negative bacteria remained dominant,although their relative composition shifted.The detection rates of common community-acquired pathogens(Haemophilus influenzae,Streptococcus pneumoniae)decreased sharply during the strict control phase(2020-2022)but rebounded from 2023 to 2024.The overall prevalence of most key drug-resistant bacteria followed a decreasing trend.Notably,the detection rates of MRSA and cefotaxime/ceftriaxone-resistant Enterobacterales decreased most mark-edly(≥15%).The prevalence of CRE and CRABA followed a"decrease-then-increase"trend,while carbapenem-resistant Klebsiella pneumoniae detection rates remained higher than the 2015 baseline,and carbapenem-resistant Escherichia coli prevalence was on par with the 2015 level in 2024.Although the detection rate of CRABA tended to decrease,it remained above 60%.In contrast,the detection rate of the ERSP was consistently high(≥90%),whereas that of the ARHI exhibited a continuous upward trend(increasing by more than 30%).Conclusion The COVID-19 pandemic significantly altered the bacterial ecology and resistance patterns of LRTIs.While stringent public health measures initially suppressed the transmission of some resistant pathogens,they may have facilitated the subsequent emergence and spread of more formidable drug-resistant bacteria.Continuous surveillance and reinforced infection control measures are crucial in the post-pandemic era.展开更多
BACKGROUND Some scholars believe that there is a certain correlation between coronavirus disease 2019(COVID-19)infection and the occurrence of osteoporotic vertebral compression fractures(OVCF),but no relevant domesti...BACKGROUND Some scholars believe that there is a certain correlation between coronavirus disease 2019(COVID-19)infection and the occurrence of osteoporotic vertebral compression fractures(OVCF),but no relevant domestic research reports have been published yet.This study retrospectively analyzed the clinical data of patients admitted for OVCF and treated with percutaneous vertebroplasty during the normalized prevention and control period of COVID-19 in Shanghai(after June 1,2022),aiming to explore the individual differences of OVCF patients during this special period and analyze the risk factors for OVCF.AIM To explore risk factors for senile OVCF during COVID-19 normalization.METHODS Retrospective analysis of 230 OVCF patients(June 2022 to June 2023,percutaneous vertebroplasty,observation group)and 236 controls(June 2020 to June 2021,OVCF surgery).Observation group was split into nucleic acidpositive(n=85)and negative(n=145)subgroups.Fracture location/cause,visual analogue scale,activity scores were compared;risk factors identified.RESULTS Observation group had 42.2%thoracic fractures and 38.2%cough-induced fractures(both higher than controls,P0.05).Nucleic acid results,fracture cause/location correlated(P<0.05;cause-location r=0.827).Logistic regression showed fracture cause,nucleic acid result,body mass index,bone mineral density as high-risk factors.CONCLUSION During COVID-19 normalization,cough is the top OVCF cause;thoracic fractures rise.The four factors above are independent risks.For elderly COVID-positive patients with recurrent cough,lung disease treatment,antiosteoporosis therapy,and thoracic braces prevent thoracic OVCF.展开更多
BACKGROUND Myocarditis is an inflammatory disease of the myocardium with a highly variable clinical presentation and is often triggered by viral infections.While corticosteroids have historically been used with cautio...BACKGROUND Myocarditis is an inflammatory disease of the myocardium with a highly variable clinical presentation and is often triggered by viral infections.While corticosteroids have historically been used with caution in viral myocarditis due to concerns over impaired viral clearance,recent insights from the coronavirus disease 2019 pandemic suggest that their use may offer clinical benefits in selected cases.Whether such benefits extend to other viral etiologies,including influenza,remains uncertain.We report a case of recurrent viral myocarditis in a young woman,initially following severe acute respiratory syndrome coronavirus 2(SARSCoV-2)infection and later associated with influenza A(H1N1),illustrating diagnostic and therapeutic challenges.CASE SUMMARY A previously healthy 27-year-old woman presented with fulminant myocarditis after a paucisymptomatic SARSCoV-2 infection.The initial episode was characterized by pericardial effusion,rapidly progressing to cardiogenic shock and severe left ventricular dysfunction,requiring veno-arterial extracorporeal membrane oxygenation.Empirical high-dose corticosteroid therapy,remdesivir,and monoclonal anti-SARS-CoV-2 antibodies led to complete recovery of cardiac function.Two years later,the patient developed recurrent myocarditis triggered by H1N1,presenting with chest pain,elevated cardiac biomarkers,and severe reduction in ejection fraction.Cardiac magnetic resonance confirmed acute myocarditis with severe left ventricular dysfunction.High-dose corticosteroids and levosimendan were administered,resulting in progressive improvement without the need for mechanical support.At discharge,following both episodes,the patient was asymptomatic and had a complete recovery of the left ventricular ejection fraction.Genetic testing was performed to investigate possible predisposition.CONCLUSION This case highlights the diagnostic and therapeutic challenges of recurrent viral myocarditis,as well as the potential role of immunosuppression.展开更多
BACKGROUND The human leukocyte antigen(HLA)system represents one of the most genetically diverse and densely packed genomic regions,playing a fundamental role in orchestrating immune responses.Among molecular markers ...BACKGROUND The human leukocyte antigen(HLA)system represents one of the most genetically diverse and densely packed genomic regions,playing a fundamental role in orchestrating immune responses.Among molecular markers linked to viral pathogenesis and immune modulation are insulin-like growth factor 2(IGF-2)and intestinal fattyacid-binding protein(I-FABP).AIM To investigate the influence of the HLA variant rs1131500,together with IGF-2 and I-FABP,on susceptibility to hepatitis C virus(HCV),coronavirus disease 2019(COVID-19),and their co-occurrence,to identify predictive biomarkers and potential therapeutic targets.METHODS The study involved quantifying circulating levels of IGF-2,I-FABP,and interferon-gamma(IFN-γ)and genotyping HLA rs1131500 using real-time polymerase chain reaction.Participants provided nasopharyngeal swabs for detection of severe acute respiratory syndrome coronavirus 2 RNA and blood samples for HCV RNA analysis and biomarker assessment.RESULTS Levels of IGF-2 and I-FABP were notably higher in all patient categories,with the highest values observed in coinfected individuals(P<0.0001).A moderate positive correlation was observed between I-FABP and IFN-γin COVID-19 cases(r=0.261,P=0.05).IGF-2 had the most substantial predictive value(odds ratio[OR]:4.5-5.5),while IFN-γshowed a protective trend(OR<1)when combined with IGF-2 and I-FABP.CONCLUSION IGF-2 emerged as the most consistent and reliable biomarker across all patient groups,particularly in COVID-19 and co-infections.I-FABP was a strong marker for co-infection,less so for COVID-19,and ineffective for HCV alone.Genetically,the CC genotype was more common in the HCV and Control groups,whereas the TT genotype was associated with COVID-19 and co-infection,suggesting potential diagnostic value.展开更多
Coronavirus disease 2019(COVID-19)vaccination has reduced the morbidity and severity of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infections and has resulted in many benefits for humans.Conversely,alt...Coronavirus disease 2019(COVID-19)vaccination has reduced the morbidity and severity of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infections and has resulted in many benefits for humans.Conversely,although not frequent,hepatic injury,such as autoimmune hepatitis and drug-induced liver injury due to COVID-19 vaccination,has been observed in scattered cases.Furthermore,recent increases in the number of case series publications have progressively contributed to the understanding of the pathophysiological mechanisms,clinical management strategies,and prognostic implications associated with post-COVID-19 liver injury.Nonetheless,several critical aspects remain to be elucidated,including the optimal timing for initiating therapeutic interventions,the criteria for their discontinuation,and the clinical appropriateness of vaccine rechallenge.Here,I comprehensively review the literature concerning liver injury after COVID-19 vaccination and discuss possible measures to help prevent unexpected liver injury after COVID-19 vaccination considering the expected continued future occurrence of SARS-CoV-2 infection.展开更多
Background:Resident physicians may experience persistent occupational and psychological strain during the early post-COVID-19 period.Burnout and depressive symptoms can overlap,but they represent diff erent aspects of...Background:Resident physicians may experience persistent occupational and psychological strain during the early post-COVID-19 period.Burnout and depressive symptoms can overlap,but they represent diff erent aspects of psychological burden and should be assessed using standardized instruments.Objective:To estimate the prevalence of occupational burnout and SDS-defined depressive symptoms among resident physicians undergoing standardized residency training at Shanghai Tenth People’s Hospital and to examine the association between burnout scores and depressive-symptom severity.Methods:This single-center cross-sectional anonymous questionnaire study was conducted at Shanghai Tenth People’s Hospital,Shanghai,China,from January to June 2023.A total of 132 eligible resident physicians were invited through hospital residency-training coordination,and all 132 returned questionnaires(response rate,100.0%).Occupational burnout was assessed using a Chinese burnout questionnaire structured around the MBI-GS dimensions,and depressive symptoms were assessed using the validated Chinese Self-Rating Depression Scale(SDS).Internal consistency was assessed using Cronbach’s alpha.Prevalence estimates were reported with Wilson 95%confi dence intervals(CIs).Pearson correlation and multivariable linear regression were used to examine the association between burnout score and SDS standard score.Results:Overall,102/132(77.3%;95%CI,69.4 to 83.6)residents met the predefined burnout criterion.In complete valid SDS responses,81/124(65.3%;95%CI,56.6 to 73.1)residents had SDS-defi ned depressive symptoms.Cronbach’s alpha was 0.887 for the burnout questionnaire and 0.861 for the SDS.Burnout score was positively correlated with SDS standard score(n=124;r=0.64,95%CI,0.52 to 0.73;p<0.001).In multivariable linear regression,burnout score remained positively associated with SDS standard score after adjustment for sex,age group,training duration,and grouped specialty(adjusted beta=0.35,95%CI,0.27 to 0.44;p<0.001).Conclusions:Occupational burnout and SDS-defined depressive symptoms were common among resident physicians at Shanghai Tenth People’s Hospital in the early post-COVID-19 period.The fi ndings suggest that routine screening,workload management,mentorship,and accessible psychological support may be useful for residents at risk.Because this was a cross-sectional study,causality cannot be inferred.展开更多
BACKGROUND Dynamic alterations in lymphocyte subsets demonstrate significant correlations with clinical disease severity in patients with coronavirus disease 2019(COVID-19).As the most prevalent chronic liver disease ...BACKGROUND Dynamic alterations in lymphocyte subsets demonstrate significant correlations with clinical disease severity in patients with coronavirus disease 2019(COVID-19).As the most prevalent chronic liver disease globally,non-alcoholic fatty liver disease(NAFLD)exhibits distinct chronic inflammatory and immunometabolic disturbances that may substantially affect immune response patterns in COVID-19 patients.Nevertheless,the characteristics of lymphocyte subset dynamics and their clinical implications in COVID-19-NAFLD remain to be fully elucidated.AIM To characterize the dynamic changes in lymphocyte subsets among COVID-19 patients with NAFLD,in order to delineate their immunological profiles and inform clinical management strategies.METHODS The cohort study compared lymphocyte subpopulations in 858 COVID-19 patients and 670 COVID-19-NAFLD patients at admission,discharge,and 2-week/4-week post-discharge follow-ups.RESULTS Compared to COVID-19 patients without NAFLD,NAFLD-comorbid patients demonstrated persistently elevated CD3+CD4+counts as well as lymphocyte counts and percentages at admission and at the 2-week and 4-week follow-ups post-discharge(all P<0.05).Among COVID-19-NAFLD patients,those aged≥60 years had significantly lower CD3+counts,CD3+CD4+counts,CD3+CD8+counts,lymphocyte counts and percentages,and CD19+counts and percentages at all assessed time points(all P<0.05);significant liver fibrosis correlated with reduced CD3+CD4+counts,CD3+CD8+counts,and lymphocyte counts and percentages across all time points(all P<0.05);multimorbidity(≥3 comorbidities)exacerbated immune imbalance,marked by elevated CD3+CD4+percentages and CD56+counts at admission,increased CD3+CD4+counts,lymphocyte counts,and CD19+counts and percentages at discharge,as well as sustained increases in CD3+CD4+counts at the 2-week follow-up and higher CD3+CD4+percentages at the 4-week post-discharge follow-up(all P<0.05);and obesity and elevated liver enzymes were independently linked to higher CD3+CD4+counts,CD19+counts,and lymphocyte counts at all post-admission evaluations(from discharge through the 4-week follow-up)(all P<0.05).CONCLUSION Age,liver fibrosis,comorbidities,obesity,liver enzyme abnormalities,vaccination status,low-density lipoprotein cholesterol,and hemoglobin A1c significantly modulate immune responses in COVID-19-NAFLD patients,warranting targeted clinical attention.Furthermore,patients with uncomplicated NAFLD(including lean NAFLD)also require particular clinical attention to mitigate risks of immune imbalance.展开更多
Objective:To assess the prevalence of post COVID-19 conditions,functional disability impacting daily living,and associated socio-demographic determinants.Methods:In this cross-sectional door-to-door survey,post COVID-...Objective:To assess the prevalence of post COVID-19 conditions,functional disability impacting daily living,and associated socio-demographic determinants.Methods:In this cross-sectional door-to-door survey,post COVID-19 conditions were identified.Functional impairment was assessed using EuroQoL five-dimension five-level(EQ-5D-5L)questionnaire.Relative risk of symptoms in the COVID-19 positive group as compared to the symptoms in the COVID-19 negative group was calculated.Chi-square test with subsequent Bonferroni's correction was performed to analyze the association of symptoms with age categories,sex and severity of infection.P value<0.05 was considered to be statistically significant.Results:Out of 57610 persons surveyed,COVID-19 test reports were available for 34602(1799 positive and 32803 negative)respondents.Post COVID-19 complaints were significantly more prevalent in infected individuals,including loss of taste(RR 8.82,95%CI 4.77-16.31,P<0.01),loss of smell(RR 7.29,95%CI 3.74-14.22,P<0.01),fatigue(RR 2.97,95%CI 2.56-3.44,P<0.01),muscle pains(RR 1.86,95%CI 1.59-2.18,P<0.01).EQ-5D-5L scores reflected greater difficulty in mobility,self-care,daily activities,and mental health among the infected group,especially in moderate to severe cases.Conclusions:The survey highlights a significantly higher burden of post COVID-19 complaints and functional limitations among COVID-19 positive individuals.These findings underscore the necessity for long-term management strategies to mitigate the burden of post COVID-19 conditions and poor functional status.展开更多

Objective:Olfactory dysfunction(OD)is a common and persistent symptom of coronavirus disease 2019(COVID-19)for which effective treatments remain limited.Acupuncture,widely used in Chinese medicine for olfactory disord...Objective:Olfactory dysfunction(OD)is a common and persistent symptom of coronavirus disease 2019(COVID-19)for which effective treatments remain limited.Acupuncture,widely used in Chinese medicine for olfactory disorders has uncertain efficacy in post-COVID-19 OD.Therefore,in this study,we aimed to explore the feasibility and preliminary efficacy of acupuncture in patients with post-COVID-19 OD.Methods:This study was a dual-phased study.Phase I involved a systematic review conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)guidelines,encompassing eight databases.Phase II consisted of a pilot randomized controlled trial(RCT)enrolling 25 patients with moderate to severe post-COVID-19 OD.Participants were randomly assigned to receive either real(group A)or sham(group B)acupuncture.The intervention included eight sessions over 4 weeks,with assessments at baseline,post-cycle 1,2-week washout,post-cycle-2 treatment,and 2-week follow-up.The primary outcome was the change in scores on the 12-item Assessment of Self-Reported Olfactory Functioning and Quality of Life(ASOF).Secondary outcomes included the short version of the Questionnaire of Olfactory Disorders-Negative Statements(sQOD-NS)and the traditional Chinese version of the University of Pennsylvania Smell Identification Test(UPSIT-TC).Results:The systematic review identified 22 studies(15 case reports,seven RCTs)suggesting that acupuncture may positively influence olfactory function.In the pilot RCT,no significant baseline differences were observed between groups.Group A showed steady improvement across all ASOF domains during Cycle 1(SOC,mean=5.86;SRP,mean=3.26;ORQ,mean=3.98),with slight declines during cycle 2 and 2-week follow-up,though scores remained above baseline.Post-cycle 1,group A showed higher mean SOC and ORQ scores compared with group B(7.00 vs.4.67;3.98 vs.3.39).Improvements in secondary outcomes were also more pronounced in group A,whereas group B showed minimal changes.Conclusion:Acupuncture appears to be a safe and potentially effective treatment for post-COVID-19 OD,warranting validation through larger clinical trials.展开更多
The COVID-19 pandemic led to three years(2020-2022)of human activity restrictions in China,significantly impacting urban air quality,yet long-term health risks from PM2.5-bound heavy metals remain unclear.This stud...The COVID-19 pandemic led to three years(2020-2022)of human activity restrictions in China,significantly impacting urban air quality,yet long-term health risks from PM2.5-bound heavy metals remain unclear.This study observed PM2.5 and 21 associated metals in Shenzhen over six years(2017-2022),aiming to identify sources,assess their health impacts,and explore COVID-19 effects on health risks.The total carcinogenic risk(CR)of 5 metals decreased by 23%during the pandemic(2020-2022)compared to pre-pandemic levels(2017-2019),similar to the trends of PM2.5 and total metals concentrations.In contrast,the non-carcinogenic risk(NCR)of 11 metals increased by 20%,primarily due to the rise in manganese(Mn)concentration caused by vehicle emissions,but NCR still below the risk threshold.Vehicle emissions were the main contributor to NCR(68%)and CR(55%).Machine learning revealed that the source emissions reduction is the primary factor contributing to the decline in the CR of metals bound to PM2.5,except for the increased health risk associated with vehicles.During the pandemic,the slowed growth rate of CR of vehicle emissions and the significant decline in CR of fugitive dust demonstrated the effectiveness of pandemic-induced traffic and production restrictions in reducing anthropogenic emissions and mitigating PM2.5-bound heavy metal health risks in Shenzhen.However,total CR of heavy metals in 2022 exceeded the acceptable risk threshold(1×10-6),emphasizing the ongoing challenge of regulating PM2.5 and the need to prioritize vehicle emission management.展开更多
Accurate and quick diagnosis of COVID-19 remains a global challenge,as conventional RT-PCR testing suffers from high false-negative rates,delays,and limited availability.Chest X-ray imaging,while more accessible,requi...Accurate and quick diagnosis of COVID-19 remains a global challenge,as conventional RT-PCR testing suffers from high false-negative rates,delays,and limited availability.Chest X-ray imaging,while more accessible,requires expert interpretation and is prone to human error.Although several deep learning methods have been applied,many face issues of overfitting,insufficient feature selection,and reduced reliability.In order to overcome these constraints,we suggest a hybrid framework,COVID-19-DSCNN-XISSP,which integrates a Deep Spatial Convolutional Neural Network(DSCNN)for robust spatial excision of features with the Exponential Incremental Social Ski Driver with Sine Cosine Algorithm(XISSP)for optimal feature selection.This strategy enhances classification accuracy while reducing computational overhead.Extensive experiments on benchmark COVID-19 chest X-ray datasets demonstrate that the proposed model outperforms existing approaches in terms of accuracy,precision,and robustness.The findings highlight the potential of COVID-19-DSCNN-XISSP as an economical,automated,and expandable tool for assisting radiologists in early and reliable detection of COVID-19,thereby supporting improved clinical decision-making and pandemic control.展开更多

摘要Coronavirus disease 2019(COVID-19)is a disease that caused a global pandemic and is caused by infection of severe acute respiratory syndrome coronavirus 2 virus.It has affected over 768 million people worldwide,resulting in approx-imately 6900000 deaths.High-risk groups,identified by the Centers for Disease Control and Prevention,include individuals with conditions like type 2 diabetes mellitus(T2DM),obesity,chronic lung disease,serious heart conditions,and chronic kidney disease.Research indicates that those with T2DM face a hei-ghtened susceptibility to COVID-19 and increased mortality compared to non-diabetic individuals.Examining the renin-angiotensin system(RAS),a vital regulator of blood pressure and pulmonary stability,reveals the significance of the angiotensin-converting enzyme(ACE)and ACE2 enzymes.ACE converts angiotensin-I to the vasoconstrictor angiotensin-II,while ACE2 counters this by converting angiotensin-II to angiotensin 1-7,a vasodilator.Reduced ACE2 exp-ression,common in diabetes,intensifies RAS activity,contributing to conditions like inflammation and fibrosis.Although ACE inhibitors and angiotensin receptor blockers can be therapeutically beneficial by increasing ACE2 levels,concerns arise regarding the potential elevation of ACE2 receptors on cell membranes,potentially facilitating COVID-19 entry.This review explored the role of the RAS/ACE2 mechanism in amplifying severe acute respiratory syndrome cor-onavirus 2 infection and associated complications in T2DM.Potential treatment strategies,including recombinant human ACE2 therapy,broad-spectrum antiviral drugs,and epigenetic signature detection,are discussed as promising avenues in the battle against this pandemic.
摘要Introduction: It has been found that the viral infection is linked to the ABO blood group. Limited studies investigated the relationship of the ABO blood group and susceptibility to COVID-19 infection. Objective: To investigate the association between the ABO blood group and the COVID-19 susceptibility. Material & Methods: The study is a retrospective cohort study. It included all hospitalized confirmed COVID-19 patients in Qatif central hospital, Eastern province, Saudi Arabia. We included both genders and people who are older than 14 years of age. The study was conducted in the period between 15th March, and 15th April, 2020. Results: Our sample consists of 72 patients;we found a percentage distribution of 23.62%, 33.33%, 9.72%, and 33.33% for blood groups A, B, AB and O, respectively. Our results showed significantly increased risk for AB blood groups (P Conclusion: Our findings suggest that among confirmed COVID-19 patients, patients with AB blood group have higher susceptibility while patients with O blood group have lower susceptibility to COVID-19 infection. Our study has limitations and further studies are needed.

摘要Symptoms of coronavirus disease of 2019(COVID-19),caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),range from asymptomatic to life-threatening,affecting the respiratory system and many other organs,and involved in SARS-CoV-2 pathogenesis,particularly its viral entry,is the enzyme transmembrane protease serine 2(TMPRSS2).In response,in this study,polymorphisms in the TMPRSS2 gene(rs2070788 and rs12329760)were examined for their association with COVID-19 susceptibility and severity in Jordanians.The study participants(n=861)included 427 individuals who had never contracted COVID-19(control group)and 434 who had experienced the disease(case group).The case group was subsequently divided into two subgroups:severe(n=166)and non-severe(n=268)COVID-19.The results show that rs2070788 is associated with disease susceptibility in all inheritance models(P<0.05),as well as an association between rs12329760 and COVID-19 disease severity in both the dominant and over-dominant models(P<0.05).Further,a higher body mass index(BMI)and older age were associated with disease severity(p<0.05),though females were less likely to develop severe COVID-19 and to be hospitalized.In conclusion,polymorphisms in the TMPRSS2 gene may be considered a potential predictor of COVID-19 outcomes in the Jordanian population.The study findings could be used to manage similar viral infections in Jordan in the future.

摘要Objective:To describe the clinical features and effectiveness of Xuanbai Shengmai Decoction(XB,a compound Chinese herbal medicine)on disease progression,clinical effect and viral RNA shedding in COVID-19 cases.Methods:We collected demographic and clinical information of 212 COVID-19 cases from December2022 to January 2023,and divided these cases into two groups based on whether XB was used or not.Survival curves in the two groups were estimated by the Kaplan-Meier method.A smoothing approximation of time-to-event distributions was created based on a log-rank test and a restricted mean survival time model,and the Gaussian density estimation method.In addition,the effect of XB treatment on survival time was assessed using a series of Cox proportional hazards models.Results:Of all 212 COVID-19 cases,there were respectively 63 and 149 cases reported in XB group and non-XB group.Of those,128(60.4%)were male,with median age of 75.00 years,and 72.6%had one or more underlying medical conditions.The most common symptoms on admission were cough(64.6%),fever(39.6%),wheezing(27.8%)and sputum(13.2%).In addition,the gender,age group distribution,smoking,and drinking between two groups were of no significant difference(P>0.05).However,the clinical characteristics and CT findings in XB group were more serious than those in non-XB group.Nevertheless,the hazard of death in XB group was lower than that in non-XB group,especially on severe cases.Treatment with XB significantly protected against adverse outcomes,shorten the time of viral RNA shedding.Moreover,interventional therapy of XB within 1–2 days after hospital admission is confirmed with particularly effectiveness.Conclusion:Treatment of XB is effective for COVID-19 cases,particularly in reducing the mortality rate of severe cases,suggesting that XB may be used as an alternative treatment of SARS-COV-2 infection.
摘要Objective To investigate the impact of the coronavirus disease 2019(COVID-19)pandemic on the bacterial profile of lower respiratory tract infections(LRTIs)and the prevalence of major drug-resistant bacteria in Hubei Province,China,by comparing five-year periods before(2015-2019)and after(2020-2024)the pandemic.Methods A retrospective analysis was conducted on microbial culture and antimicrobial susceptibility test results from sputum and bronchoalveolar lavage fluid(BALF)samples obtained from patients with LRTIs.Pathogen distribution and the prevalence of key drug-resistant bacteria,including methicillin-resistant Staphylococcus aureus(MRSA),cefotaxime/ceftriaxone-resistant Enterobacterales,carbapenem-resistant Enterobacterales(CRE),carbapenem-resistant Pseudomonas aeruginosa(CRPAE),carbapenem-resistant Acinetobacter baumannii(CRABA),ampicillin-resistant Haemophilus influ-enzae(ARHI),and penicillin/erythromycin-resistant Streptococcus pneumoniae(PRSP/ERSP),were compared between the two periods.Results The overall number of bacterial isolates significantly increased during the post-pandemic period.Gram-negative bacteria remained dominant,although their relative composition shifted.The detection rates of common community-acquired pathogens(Haemophilus influenzae,Streptococcus pneumoniae)decreased sharply during the strict control phase(2020-2022)but rebounded from 2023 to 2024.The overall prevalence of most key drug-resistant bacteria followed a decreasing trend.Notably,the detection rates of MRSA and cefotaxime/ceftriaxone-resistant Enterobacterales decreased most mark-edly(≥15%).The prevalence of CRE and CRABA followed a"decrease-then-increase"trend,while carbapenem-resistant Klebsiella pneumoniae detection rates remained higher than the 2015 baseline,and carbapenem-resistant Escherichia coli prevalence was on par with the 2015 level in 2024.Although the detection rate of CRABA tended to decrease,it remained above 60%.In contrast,the detection rate of the ERSP was consistently high(≥90%),whereas that of the ARHI exhibited a continuous upward trend(increasing by more than 30%).Conclusion The COVID-19 pandemic significantly altered the bacterial ecology and resistance patterns of LRTIs.While stringent public health measures initially suppressed the transmission of some resistant pathogens,they may have facilitated the subsequent emergence and spread of more formidable drug-resistant bacteria.Continuous surveillance and reinforced infection control measures are crucial in the post-pandemic era.

摘要BACKGROUND Some scholars believe that there is a certain correlation between coronavirus disease 2019(COVID-19)infection and the occurrence of osteoporotic vertebral compression fractures(OVCF),but no relevant domestic research reports have been published yet.This study retrospectively analyzed the clinical data of patients admitted for OVCF and treated with percutaneous vertebroplasty during the normalized prevention and control period of COVID-19 in Shanghai(after June 1,2022),aiming to explore the individual differences of OVCF patients during this special period and analyze the risk factors for OVCF.AIM To explore risk factors for senile OVCF during COVID-19 normalization.METHODS Retrospective analysis of 230 OVCF patients(June 2022 to June 2023,percutaneous vertebroplasty,observation group)and 236 controls(June 2020 to June 2021,OVCF surgery).Observation group was split into nucleic acidpositive(n=85)and negative(n=145)subgroups.Fracture location/cause,visual analogue scale,activity scores were compared;risk factors identified.RESULTS Observation group had 42.2%thoracic fractures and 38.2%cough-induced fractures(both higher than controls,P0.05).Nucleic acid results,fracture cause/location correlated(P<0.05;cause-location r=0.827).Logistic regression showed fracture cause,nucleic acid result,body mass index,bone mineral density as high-risk factors.CONCLUSION During COVID-19 normalization,cough is the top OVCF cause;thoracic fractures rise.The four factors above are independent risks.For elderly COVID-positive patients with recurrent cough,lung disease treatment,antiosteoporosis therapy,and thoracic braces prevent thoracic OVCF.
摘要BACKGROUND Myocarditis is an inflammatory disease of the myocardium with a highly variable clinical presentation and is often triggered by viral infections.While corticosteroids have historically been used with caution in viral myocarditis due to concerns over impaired viral clearance,recent insights from the coronavirus disease 2019 pandemic suggest that their use may offer clinical benefits in selected cases.Whether such benefits extend to other viral etiologies,including influenza,remains uncertain.We report a case of recurrent viral myocarditis in a young woman,initially following severe acute respiratory syndrome coronavirus 2(SARSCoV-2)infection and later associated with influenza A(H1N1),illustrating diagnostic and therapeutic challenges.CASE SUMMARY A previously healthy 27-year-old woman presented with fulminant myocarditis after a paucisymptomatic SARSCoV-2 infection.The initial episode was characterized by pericardial effusion,rapidly progressing to cardiogenic shock and severe left ventricular dysfunction,requiring veno-arterial extracorporeal membrane oxygenation.Empirical high-dose corticosteroid therapy,remdesivir,and monoclonal anti-SARS-CoV-2 antibodies led to complete recovery of cardiac function.Two years later,the patient developed recurrent myocarditis triggered by H1N1,presenting with chest pain,elevated cardiac biomarkers,and severe reduction in ejection fraction.Cardiac magnetic resonance confirmed acute myocarditis with severe left ventricular dysfunction.High-dose corticosteroids and levosimendan were administered,resulting in progressive improvement without the need for mechanical support.At discharge,following both episodes,the patient was asymptomatic and had a complete recovery of the left ventricular ejection fraction.Genetic testing was performed to investigate possible predisposition.CONCLUSION This case highlights the diagnostic and therapeutic challenges of recurrent viral myocarditis,as well as the potential role of immunosuppression.
摘要BACKGROUND The human leukocyte antigen(HLA)system represents one of the most genetically diverse and densely packed genomic regions,playing a fundamental role in orchestrating immune responses.Among molecular markers linked to viral pathogenesis and immune modulation are insulin-like growth factor 2(IGF-2)and intestinal fattyacid-binding protein(I-FABP).AIM To investigate the influence of the HLA variant rs1131500,together with IGF-2 and I-FABP,on susceptibility to hepatitis C virus(HCV),coronavirus disease 2019(COVID-19),and their co-occurrence,to identify predictive biomarkers and potential therapeutic targets.METHODS The study involved quantifying circulating levels of IGF-2,I-FABP,and interferon-gamma(IFN-γ)and genotyping HLA rs1131500 using real-time polymerase chain reaction.Participants provided nasopharyngeal swabs for detection of severe acute respiratory syndrome coronavirus 2 RNA and blood samples for HCV RNA analysis and biomarker assessment.RESULTS Levels of IGF-2 and I-FABP were notably higher in all patient categories,with the highest values observed in coinfected individuals(P<0.0001).A moderate positive correlation was observed between I-FABP and IFN-γin COVID-19 cases(r=0.261,P=0.05).IGF-2 had the most substantial predictive value(odds ratio[OR]:4.5-5.5),while IFN-γshowed a protective trend(OR<1)when combined with IGF-2 and I-FABP.CONCLUSION IGF-2 emerged as the most consistent and reliable biomarker across all patient groups,particularly in COVID-19 and co-infections.I-FABP was a strong marker for co-infection,less so for COVID-19,and ineffective for HCV alone.Genetically,the CC genotype was more common in the HCV and Control groups,whereas the TT genotype was associated with COVID-19 and co-infection,suggesting potential diagnostic value.
摘要Coronavirus disease 2019(COVID-19)vaccination has reduced the morbidity and severity of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infections and has resulted in many benefits for humans.Conversely,although not frequent,hepatic injury,such as autoimmune hepatitis and drug-induced liver injury due to COVID-19 vaccination,has been observed in scattered cases.Furthermore,recent increases in the number of case series publications have progressively contributed to the understanding of the pathophysiological mechanisms,clinical management strategies,and prognostic implications associated with post-COVID-19 liver injury.Nonetheless,several critical aspects remain to be elucidated,including the optimal timing for initiating therapeutic interventions,the criteria for their discontinuation,and the clinical appropriateness of vaccine rechallenge.Here,I comprehensively review the literature concerning liver injury after COVID-19 vaccination and discuss possible measures to help prevent unexpected liver injury after COVID-19 vaccination considering the expected continued future occurrence of SARS-CoV-2 infection.

摘要Background:Resident physicians may experience persistent occupational and psychological strain during the early post-COVID-19 period.Burnout and depressive symptoms can overlap,but they represent diff erent aspects of psychological burden and should be assessed using standardized instruments.Objective:To estimate the prevalence of occupational burnout and SDS-defined depressive symptoms among resident physicians undergoing standardized residency training at Shanghai Tenth People’s Hospital and to examine the association between burnout scores and depressive-symptom severity.Methods:This single-center cross-sectional anonymous questionnaire study was conducted at Shanghai Tenth People’s Hospital,Shanghai,China,from January to June 2023.A total of 132 eligible resident physicians were invited through hospital residency-training coordination,and all 132 returned questionnaires(response rate,100.0%).Occupational burnout was assessed using a Chinese burnout questionnaire structured around the MBI-GS dimensions,and depressive symptoms were assessed using the validated Chinese Self-Rating Depression Scale(SDS).Internal consistency was assessed using Cronbach’s alpha.Prevalence estimates were reported with Wilson 95%confi dence intervals(CIs).Pearson correlation and multivariable linear regression were used to examine the association between burnout score and SDS standard score.Results:Overall,102/132(77.3%;95%CI,69.4 to 83.6)residents met the predefined burnout criterion.In complete valid SDS responses,81/124(65.3%;95%CI,56.6 to 73.1)residents had SDS-defi ned depressive symptoms.Cronbach’s alpha was 0.887 for the burnout questionnaire and 0.861 for the SDS.Burnout score was positively correlated with SDS standard score(n=124;r=0.64,95%CI,0.52 to 0.73;p<0.001).In multivariable linear regression,burnout score remained positively associated with SDS standard score after adjustment for sex,age group,training duration,and grouped specialty(adjusted beta=0.35,95%CI,0.27 to 0.44;p<0.001).Conclusions:Occupational burnout and SDS-defined depressive symptoms were common among resident physicians at Shanghai Tenth People’s Hospital in the early post-COVID-19 period.The fi ndings suggest that routine screening,workload management,mentorship,and accessible psychological support may be useful for residents at risk.Because this was a cross-sectional study,causality cannot be inferred.

摘要BACKGROUND Dynamic alterations in lymphocyte subsets demonstrate significant correlations with clinical disease severity in patients with coronavirus disease 2019(COVID-19).As the most prevalent chronic liver disease globally,non-alcoholic fatty liver disease(NAFLD)exhibits distinct chronic inflammatory and immunometabolic disturbances that may substantially affect immune response patterns in COVID-19 patients.Nevertheless,the characteristics of lymphocyte subset dynamics and their clinical implications in COVID-19-NAFLD remain to be fully elucidated.AIM To characterize the dynamic changes in lymphocyte subsets among COVID-19 patients with NAFLD,in order to delineate their immunological profiles and inform clinical management strategies.METHODS The cohort study compared lymphocyte subpopulations in 858 COVID-19 patients and 670 COVID-19-NAFLD patients at admission,discharge,and 2-week/4-week post-discharge follow-ups.RESULTS Compared to COVID-19 patients without NAFLD,NAFLD-comorbid patients demonstrated persistently elevated CD3+CD4+counts as well as lymphocyte counts and percentages at admission and at the 2-week and 4-week follow-ups post-discharge(all P<0.05).Among COVID-19-NAFLD patients,those aged≥60 years had significantly lower CD3+counts,CD3+CD4+counts,CD3+CD8+counts,lymphocyte counts and percentages,and CD19+counts and percentages at all assessed time points(all P<0.05);significant liver fibrosis correlated with reduced CD3+CD4+counts,CD3+CD8+counts,and lymphocyte counts and percentages across all time points(all P<0.05);multimorbidity(≥3 comorbidities)exacerbated immune imbalance,marked by elevated CD3+CD4+percentages and CD56+counts at admission,increased CD3+CD4+counts,lymphocyte counts,and CD19+counts and percentages at discharge,as well as sustained increases in CD3+CD4+counts at the 2-week follow-up and higher CD3+CD4+percentages at the 4-week post-discharge follow-up(all P<0.05);and obesity and elevated liver enzymes were independently linked to higher CD3+CD4+counts,CD19+counts,and lymphocyte counts at all post-admission evaluations(from discharge through the 4-week follow-up)(all P<0.05).CONCLUSION Age,liver fibrosis,comorbidities,obesity,liver enzyme abnormalities,vaccination status,low-density lipoprotein cholesterol,and hemoglobin A1c significantly modulate immune responses in COVID-19-NAFLD patients,warranting targeted clinical attention.Furthermore,patients with uncomplicated NAFLD(including lean NAFLD)also require particular clinical attention to mitigate risks of immune imbalance.

摘要Objective:To assess the prevalence of post COVID-19 conditions,functional disability impacting daily living,and associated socio-demographic determinants.Methods:In this cross-sectional door-to-door survey,post COVID-19 conditions were identified.Functional impairment was assessed using EuroQoL five-dimension five-level(EQ-5D-5L)questionnaire.Relative risk of symptoms in the COVID-19 positive group as compared to the symptoms in the COVID-19 negative group was calculated.Chi-square test with subsequent Bonferroni's correction was performed to analyze the association of symptoms with age categories,sex and severity of infection.P value<0.05 was considered to be statistically significant.Results:Out of 57610 persons surveyed,COVID-19 test reports were available for 34602(1799 positive and 32803 negative)respondents.Post COVID-19 complaints were significantly more prevalent in infected individuals,including loss of taste(RR 8.82,95%CI 4.77-16.31,P<0.01),loss of smell(RR 7.29,95%CI 3.74-14.22,P<0.01),fatigue(RR 2.97,95%CI 2.56-3.44,P<0.01),muscle pains(RR 1.86,95%CI 1.59-2.18,P<0.01).EQ-5D-5L scores reflected greater difficulty in mobility,self-care,daily activities,and mental health among the infected group,especially in moderate to severe cases.Conclusions:The survey highlights a significantly higher burden of post COVID-19 complaints and functional limitations among COVID-19 positive individuals.These findings underscore the necessity for long-term management strategies to mitigate the burden of post COVID-19 conditions and poor functional status.

摘要Coronavirus Disease 2019(COVID-19),caused by the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),affects multiple organ systems,with the respiratory system being the primary target.Respiratory organoids,which closely mimic the structure and function of the human respiratory tract,have emerged as essential tools for studying SARS-CoV-2 infection.This review summarizes current methods for generating various respiratory organoids,including nasal,tonsil,airway,bronchial,and alveolar organoids,and highlights their application in investigating the mechanism of SARS-CoV-2 infection and evaluating potential therapeutic agents.Meanwhile,this review also introduces respira-tory organoid-on-a-chip technology,which can precisely regulate culture conditions and incorporate vascularization and immune cells to enhance physiological complexity,thereby providing crucial support for investigating SARS-CoV-2-induced lung injury,immune responses,and conducting high-throughput drug screening.The aim of this review is to provide valuable insights for further research into the pathogenesis and intervention strategies of COvID-19.

摘要Objective:Olfactory dysfunction(OD)is a common and persistent symptom of coronavirus disease 2019(COVID-19)for which effective treatments remain limited.Acupuncture,widely used in Chinese medicine for olfactory disorders has uncertain efficacy in post-COVID-19 OD.Therefore,in this study,we aimed to explore the feasibility and preliminary efficacy of acupuncture in patients with post-COVID-19 OD.Methods:This study was a dual-phased study.Phase I involved a systematic review conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)guidelines,encompassing eight databases.Phase II consisted of a pilot randomized controlled trial(RCT)enrolling 25 patients with moderate to severe post-COVID-19 OD.Participants were randomly assigned to receive either real(group A)or sham(group B)acupuncture.The intervention included eight sessions over 4 weeks,with assessments at baseline,post-cycle 1,2-week washout,post-cycle-2 treatment,and 2-week follow-up.The primary outcome was the change in scores on the 12-item Assessment of Self-Reported Olfactory Functioning and Quality of Life(ASOF).Secondary outcomes included the short version of the Questionnaire of Olfactory Disorders-Negative Statements(sQOD-NS)and the traditional Chinese version of the University of Pennsylvania Smell Identification Test(UPSIT-TC).Results:The systematic review identified 22 studies(15 case reports,seven RCTs)suggesting that acupuncture may positively influence olfactory function.In the pilot RCT,no significant baseline differences were observed between groups.Group A showed steady improvement across all ASOF domains during Cycle 1(SOC,mean=5.86;SRP,mean=3.26;ORQ,mean=3.98),with slight declines during cycle 2 and 2-week follow-up,though scores remained above baseline.Post-cycle 1,group A showed higher mean SOC and ORQ scores compared with group B(7.00 vs.4.67;3.98 vs.3.39).Improvements in secondary outcomes were also more pronounced in group A,whereas group B showed minimal changes.Conclusion:Acupuncture appears to be a safe and potentially effective treatment for post-COVID-19 OD,warranting validation through larger clinical trials.

摘要The COVID-19 pandemic led to three years(2020-2022)of human activity restrictions in China,significantly impacting urban air quality,yet long-term health risks from PM2.5-bound heavy metals remain unclear.This study observed PM2.5 and 21 associated metals in Shenzhen over six years(2017-2022),aiming to identify sources,assess their health impacts,and explore COVID-19 effects on health risks.The total carcinogenic risk(CR)of 5 metals decreased by 23%during the pandemic(2020-2022)compared to pre-pandemic levels(2017-2019),similar to the trends of PM2.5 and total metals concentrations.In contrast,the non-carcinogenic risk(NCR)of 11 metals increased by 20%,primarily due to the rise in manganese(Mn)concentration caused by vehicle emissions,but NCR still below the risk threshold.Vehicle emissions were the main contributor to NCR(68%)and CR(55%).Machine learning revealed that the source emissions reduction is the primary factor contributing to the decline in the CR of metals bound to PM2.5,except for the increased health risk associated with vehicles.During the pandemic,the slowed growth rate of CR of vehicle emissions and the significant decline in CR of fugitive dust demonstrated the effectiveness of pandemic-induced traffic and production restrictions in reducing anthropogenic emissions and mitigating PM2.5-bound heavy metal health risks in Shenzhen.However,total CR of heavy metals in 2022 exceeded the acceptable risk threshold(1×10-6),emphasizing the ongoing challenge of regulating PM2.5 and the need to prioritize vehicle emission management.
摘要Accurate and quick diagnosis of COVID-19 remains a global challenge,as conventional RT-PCR testing suffers from high false-negative rates,delays,and limited availability.Chest X-ray imaging,while more accessible,requires expert interpretation and is prone to human error.Although several deep learning methods have been applied,many face issues of overfitting,insufficient feature selection,and reduced reliability.In order to overcome these constraints,we suggest a hybrid framework,COVID-19-DSCNN-XISSP,which integrates a Deep Spatial Convolutional Neural Network(DSCNN)for robust spatial excision of features with the Exponential Incremental Social Ski Driver with Sine Cosine Algorithm(XISSP)for optimal feature selection.This strategy enhances classification accuracy while reducing computational overhead.Extensive experiments on benchmark COVID-19 chest X-ray datasets demonstrate that the proposed model outperforms existing approaches in terms of accuracy,precision,and robustness.The findings highlight the potential of COVID-19-DSCNN-XISSP as an economical,automated,and expandable tool for assisting radiologists in early and reliable detection of COVID-19,thereby supporting improved clinical decision-making and pandemic control.
摘要目的系统评价重症监护病房(ICU)患者新型冠状病毒感染(COVID-19)相关肺曲霉病(CAPA)感染危险因素。方法计算机检索PubMed、Cochrane Library、Embase、Web of Science、中国知网(CNKI)、万方数据、维普数据库(VIP)中关于ICU患者CAPA感染危险因素的研究,检索时限均为建库至2025年3月31日。由2名研究者独立筛选文献与提取资料,采用纽卡斯尔-渥太华(NOS)量表进行质量评价,使用RevMan 5.4软件进行Meta分析。结果共纳入38篇观察性研究,包括11312例患者。Meta分析结果显示,男性、年龄、吸烟、急性生理学与慢性健康状况评分Ⅱ(APACHEⅡ评分)、简化急性生理学评分Ⅱ(SAPSⅡ评分)、肝硬化、糖尿病、慢性阻塞性肺疾病(COPD)、心血管疾病、EORTC/MSGERC宿主因素、实体器官移植、血液系统恶性肿瘤、长期使用糖皮质激素、免疫缺陷、Charlson合并症指数、使用血管活性药物和/或正性肌力药物、接受机械通气、侵入性机械通气、机械通气时间、肾脏替代治疗、白细胞介素-6(IL-6)抑制剂治疗等因素均为ICU患者CAPA感染危险因素。身体质量指数(BMI)高和肥胖均为ICU患者CAPA感染的保护性因素(均P<0.05)。结论ICU患者CAPA感染危险因素较多,及时识别相关危险因素有助于尽早实施规范的抗真菌治疗,从而改善患者预后。
