Objective The lower trapezius musculocutaneous flap(LTMF)is a workhorse flap for the reconstruction of soft-tissue defects of the head and neck.However,when harvesting a large LTMF to repair massive defects,primary closure of the donor site may be challenging.This study aimed to evaluate the feasibility and safety of using a perforator propeller flap(PPF)for primary closure of LTMF donor sites and to analyze the buddy flap donor-site closure strategies through a comprehensive literature review.Methods We retrospectively reviewed data from patients who underwent LTMF donor-site defect reconstruction using PPF between January 2015 and October 2025.Data on patient demographics,defect features,flap characteristics,postoperative complications,and outcomes were collected and analyzed.In addition,a comprehensive literature search was conducted in PubMed,Embase,Scopus,and Web of Science to identify studies published before October 9,2025,describing the use of an additional flap to close the donor site of a primary flap.Results Seventeen patients underwent donor-site reconstruction of LTMFs using PPFs.The harvested LTMFs measured an average of 29.8×11.5 cm.The PPFs measured an average of 15.7 cm in length and 5.9 cm in width.All donor sites were closed primarily,and all PPFs survived completely.Complications occurred in three cases,including two cases of distal partial necrosis of the LTMF and one wound dehiscence at the LTMF donor site.The distal necroses were managed with debridement followed by secondary local flap reconstruction,whereas the wound dehiscence healed after conservative treatment.In the literature review,43 studies comprising 547 cases were identified.Three reconstructive categories were established:Type 1(locoregional flap),Type 2(distant pedicled flap),and Type 3(free flap)donor-site closures.Primary closure was achieved in 97.1%of patients,with an overall complication rate of 15.4%.Commonly adopted donor-site closure techniques for harvesting large flaps from different anatomical regions were summarized,providing a comprehensive overview of region-specific reconstructive strategies and their clinical outcomes.Conclusions The PPF provides a safe and effective option for achieving primary closure of large LTMF donor sites.Using one flap to reconstruct the donor site of another large flap represents a reliable and versatile approach that facilitates primary closure while minimizing donor-site morbidity and adhering to the"like-with-like reconstruction"principle. 展开更多

Cemented paste backfill(CPB)is a key material in underground mining,providing essential ground support while aiding in tailings management.However,current research has overlooked the combined effects of horizontal rockwall closure stress and vertical self-loading stress,referred to as multiaxial stress,on the CPB's consolidation behavior and its mechanical properties development.Understanding and assessing these effects is critical because they directly affect the stability and performance of CPB structures.In this study,a novel multiaxial compressive stress curing and monitoring apparatus was used to simulate two horizontal rockwall closure scenarios with a consistent backfilling rate,under both drained and undrained conditions.Key parameters assessed included unconfined compressive strength(UCS),deformation during curing,stress-strain behavior,and modulus of elasticity.The results highlight that rockwall closure,combined with vertical stress,plays a pivotal role in the consolidation behavior of CPB,significantly affecting key mechanical properties.Higher horizontal stress from faster rockwall closure intensified compression during curing,leading to reduced porosity,enhanced particle rearrangement,and accelerated consolidation.This intensified consolidation leads to notable improvements in mechanical properties,including increased UCS,enhanced stiffness,and a higher modulus of elasticity,indicating improved load-bearing capacity.Moreover,the interaction between multiaxial stress and drainage conditions influenced stress-strain behavior and deformation,with drained conditions promoting earlier plasticity and higher peak stresses.These findings underscore the critical influence of multiaxial stress,combined with drainage conditions,on CPB performance,offering valuable insights for optimizing CPB design in underground mining applications. 展开更多

AIM:To investigate the long-term outcomes in acute primary angle closure(APAC)patients treated with lens extraction(LE)surgery and to identify risk factors for glaucomatous optic neuropathy(GON).METHODS:In this longitudinal observational study,detailed medical histories of APAC patients and comprehensive ophthalmic examinations at final followup were collected.Logistic regression analysis was performed to identify predictors of blindness.Univariate and multivariate linear regression analyses were conducted to determine risk factors associated with visual outcomes.RESULTS:This study included 39 affected eyes of 31 subjects(26 females)with an average age of 74.1±8.0y.At 6.7±4.2y after APAC attack,2(5.7%)eyes had bestcorrected visual acuity(VA)worse than 3/60.Advanced glaucomatous visual field loss was observed in 15(39.5%)affected eyes and 5(25.0%)fellow eyes.Nine affected eyes(23.7%)had GON,and 11(28.9%)were blind.Six(15.4%)affected eyes and 2(9.1%)fellow eyes had suspicious progression.A significantly higher blindness rate in factory workers compared to office workers.Logistic regression identified that worse VA at attack(OR 10.568,95%CI 1.288-86.695;P=0.028)and worse early postoperative VA(OR 13.214,95%CI 1.157-150.881;P=0.038)were risk factors for blindness.Multivariate regression showed that longer duration of elevated intraocular pressure(P=0.004)and worse early postoperative VA(P=0.009)were associated with worse visual outcomes.CONCLUSION:Despite LE surgery,some APAC patients experience continued visual function deterioration.Lifelong monitoring is necessary.Target pressure and progression rates should be re-evaluated during follow-up. 展开更多

Geochronology of detrital zircons from Ordovician-Devonian sedimentary rocks of the South Qinling Belt:Implications for the Shangdan Ocean closure 认领 引用

The Shangdan suture zone preserves critical information about the Paleozoic collision between the North and South Qinling Belts.Although this collision event significantly influenced the tectonic evolution of the Qinling Orogenic Belt,the timing remains a subject of debate.In this study,we comprehensively analyzed the Th/U ratios,elongation ratios,and U-Pb ages of detrital zircons to investigate provenance signatures and tectonic-sedimentary settings.Detailed U-Pb dating of detrital zircons from Ordovician to Devonian strata in the Shanyang and Xunyang area of South Qinling Belt revealed age clusters of 430-450,710-840,and910-960 Ma for Devonian and Silurian samples,with peaks at~440,~800,and~940 Ma,respectively.Ordovician samples predominantly exhibited ages of 710-850 Ma,peaking at~735 Ma,but lacked the 910-960Ma age group.Comparison with magmatic events in adjacent terranes and detrital zircon mixing provenance modeling identified 430-450 and910-960 Ma peaks associated with granitoids in the North Qinling Belt,while the 710-840 Ma age population corresponded to sources from the South China Block and South Qinling Belt.Provenance analysis indicated that Devonian and Silurian samples originated from the northern and southern sides of the Shangdan suture zone,whereas Ordovician samples were derived from the southern side.This shift in provenance suggests a significant change in sedimentary sources in the South Qinling Belt from the Ordovician to the Silurian.The closure of the Shangdan Ocean likely occurred during the Silurian-Devonian period,resulting in a northern provenance for the Silurian and Devonian strata. 展开更多

The combined oblique and vertical everting running suturing(COVER)technique,which uses a combination of oblique and vertical running everting closures,has been proposed as an alternative approach to close the wound in order to offer biomechanical stability with an appropriate biological healing environment.By employing an angled suture orientation with a vertical everting pattern,the COVER technique redistributes tensile forces throughout wound margins,providing a more even stress field,thereby enabling precise apposition of tissues at several anatomic levels.Biologically,this arrangement would induce dilution of local stress while maintaining continual microcirculatory flow,preventing inflammation,and allowing controlled deposition of collagen and epithelialization.Recent studies have reported clinical results that suggest a potential benefit from the COVER technique in terms of healing rates,cosmetic outcome,and complication rates compared with conventional suture techniques.Mechanistic and concept-driven analyses of biomechanical implications,biological outcomes of the COVER configuration,and its applications in wound management under high-tension or reconstructive situations are exemplified. 展开更多

The final closure time of the Paleo-Asian Ocean(PAO)is key to understanding the evolution of the Central Asian Orogenic Belt,and it has remained highly controversial for a long time.Here,we conduct structural investigations and zircon U-Pb and 40Ar/39Ar dating on the Hulan Metamorphic Complex(HMC)in central Jilin Province to determine the final closure time and direction of the eastern PAO.The metamorphic complexes are composed mainly of schists,marbles,gneisses,quartzites,metamorphosed plutons and dikes,which were involved in a nearly NEE-directed ductile thrusting and deformed to mylonites at a temperature of 550-450℃.Protolith ages of 262-252 Ma from three metamorphosed igneous rocks,emplacement ages of 229-221 Ma from two unmetamorphosed igneous intrusions and metamorphic ages of 243-238 Ma from three gneisses are yielded based on the zircon U-Pb dating.Three muscovite samples and one biotite sample yield 40Ar/39Ar plateau ages of 230-228 and 222 Ma,respectively.By integrating the dating results from this study and previous studies,the timing of metamorphism of the HMC is constrained to an interval between 248 and 229 Ma,and its peak metamorphism is reached at 243-238 Ma(Middle Triassic).During the period of 230-220 Ma,the HMC has already been uplifted to a geothermal level of 350-300℃.We consider that the metamorphism and shortening ductile deformation of the HMC is related to the final closure of the eastern PAO along the eastern Solonker Suture in an NEE-SWW closure direction.The Middle Triassic closure time of the eastern segment suggests a west-to-east scissor-style closure model for the PAO. 展开更多

BACKGROUND Loop stoma is usually used to divert the fecal stream,with the aim of protecting the downstream anastomosis or allowing sufficient time for the repaired perforation site to heal.It is usually reversed once the purpose of its creation is served.Spontaneous stoma closure is a rare phenomenon,and its mechanism remains unclear.The factors contributing to this phenomenon encompass stoma retraction and the healing process of enterocutaneous fistula.CASE SUMMARY We present a case of mesh hernioplasty after the spontaneous closure of a transverse loop colostomy.This is the third case of spontaneous closure of loop colostomy and the first reported case of mesh hernioplasty following the spontaneous stoma closure in the English literature.CONCLUSION Spontaneous stoma closure is rare.Long-term follow-up is recommended to detect the development of incisional hernia at an early stage. 展开更多

Stomatal movement is a crucial response of plants to drought stress.Several NAC transcription factors have been implicated in drought tolerance in maize seedlings,whereas the molecular mechanism by which they directly regulate ABA signaling components within the context of the four-cell stomatal complex remains unknown.In this study,we characterized ZmNAC16 in regulating drought resistance and stomatal closure in maize.Through phenotypic analysis of transgenic overexpression and gene-edited mutant lines,we assessed ZmNAC16 function in stomatal dynamics and drought tolerance.Overexpression of ZmNAC16 in transgenic maize resulted in increased leaf temperature,reduced stomatal conductance,decreased water loss,and enhanced drought tolerance,whereas gene-edited mutants exhibited opposite phenotypes.ZmNAC16 localized to the nucleus and acted as a transcriptional activator.Disruption of ZmNAC16 significantly downregulated key ABA signaling components,including ZmCIPK3 and ZmPP2C81,whose promoters were directly bound by ZmNAC16.Consistently,ZmNAC16 knockout mutants showed impaired ABA-induced stomatal closure,while overexpression lines displayed heightened stomatal sensitivity to ABA.Collectively,our findings demonstrate that ZmNAC16 integrates ABA signaling to regulate stomatal closure and drought tolerance in maize,positioning it as a promising target for enhancing crop resilience. 展开更多

BACKGROUND Gallbladder stones combined with common bile duct stones are frequently encountered.Laparoscopic choledocholithotomy is an effective treatment,but the optimal bile duct closure method primary closure vs T-tube drainage remains controversial.Primary closure may offer faster recovery and fewer tube-related complications,yet its safety and advantages over T-tube drainage require further validation.This study compared the effectiveness and safety of the two methods.AIM To assess the comparative effectiveness and safety of primary ductal closure vs conventional T-tube drainage in patients undergoing laparoscopic choledocholithotomy for gallbladder and common bile duct stones.METHODS Clinical data of 102 patients who underwent surgery in the First Department of General Surgery,Peking University Shougang Hospital,from October 2023 to October 2025,were retrospectively analyzed.The primary closure group consisted of 52 patients,and the T-tube drainage group had 50 patients.Factors for comparison were stone number and size,common bile duct diameter,stone clearance rate,liver function variables,operation time,drainage volume,indwelling time,postoperative hospital stay duration,total costs,and postoperative complications.Multivariate logistic regression was used to adjust for baseline differences in common bile duct diameter.RESULTS The two groups did not differ significantly in stone characteristics or clearance rates(P>0.05),although common bile duct diameter distribution differed between groups(P<0.05).After adjusting for common bile duct diameter and other covariates,primary closure remained associated with significantly shorter operative time,less drainage from the Winslow foramen,faster tube removal,shorter hospital stay,and lower costs(all P<0.05).Postoperative day 3 liver function parameters(total bilirubin,direct bilirubin,alanine aminotransferase,aspartate aminotransferase,alkaline phosphatase)were significantly lower in the primary closure group,but these differences should be interpreted with caution due to potential confounding by intraoperative factors.While bile leakage did not differ significantly between the two methods,the primary closure group suffered from significantly fewer electrolyte imbalances(P<0.05);the majority of electrolyte disturbances in the T-tube group were hypokalemia and hyponatremia,possibly related to biliary fluid loss.CONCLUSION Primary closure appears to be a safe and effective alternative to T-tube drainage during laparoscopic choledocholithotomy,with potential benefits in recovery and resource utilization.However,these findings are limited by the retrospective,single-center design and residual confounding;prospective studies are needed. 展开更多

Endoscopic clips are widely used for tissue approximation and hemostasis in gastrointestinal endoscopy.Since their introduction,significant technological advancements have expanded their functionality and versatility.Modern through-the-scope clips are now available in a range of designs with differences in open width,stem length,rotatability,opening/closing capabilities,and tensile strength-allowing tailored application depending on lesion size,location,and indication.Over-the-scope clips represent a more recent innovation,enabling robust,full-thickness closure of larger defects,ulcers,leaks,and fistulas.Mounted externally on the distal tip of the endoscope,Over-the-scope clips accommodate a fundamentally different and larger clip design compared to through-the-scope systems,allowing for deeper tissue capture and stronger compression.This review summarizes recent advances in endoscopic clipping,discusses device selection based on lesion characteristics and anatomical location,and provides practical technical tips.Emphasis is placed on the"anatomy"of clips,decision-making in clip choice,and optimal positioning techniques to maximize procedural success and safety in gastrointestinal defect closure and hemostasis. 展开更多

Interventional therapy has emerged as a transformative alternative to open surgery owing to its minimal invasiveness and fast recovery.However,it still presents risks of iatrogenic injury from vascular access,necessitating prompt and reliable vascular closure.Although various closure systems have been developed,they often suffer from complicated deployment procedures,potential for loosening,and risk of device migration.Herein,we develop a smart self-deployable vascular closure device enabled by a semi-interpenetrating network(sIPN)that synergistically integrates a programmable shape memory effect and robust tissue adhesion.The sIPN was designed by interpenetrating flexible,dopamine-functionalized poly(tetrahydrofuran)(PTD)chains into a photo-crosslinkable poly(ε-caprolactone)-based copolymer(PCC)network.The vascular closure model was fabricated via a UVassisted fused deposition modeling printing strategy,significantly reducing mechanical anisotropy while facilitating structural customization.The resulting device exhibits autonomous self-deployment at 37℃,along with reliable tissue adhesion under physiological conditions(maximum shear strength of 150.5 kPa).In vitro,the material demonstrates exceptional hemocompatibility(below 3%)and excellently enhanced cell migration(up to 38.9%).In vivo,immunofluorescence analysis reveals a promotion for CD31(162.18%)and aSMA(154.93%)compared to the control group.These results highlight the PTD/PCC sIPN as a bioadaptive,multifunctional material platform for intelligent vascular closure,offering great promise for clinical translation in interventional therapies. 展开更多

●AIM:To investigate the monthly distribution of acute primary angle-closure(APAC)admissions in a specialized hospital in Shenzhen and its association with climatic factors.●METHODS:A 5-year retrospective study of AP... ●AIM:To investigate the monthly distribution of acute primary angle-closure(APAC)admissions in a specialized hospital in Shenzhen and its association with climatic factors.●METHODS:A 5-year retrospective study of APAC inpatients from 2017 to 2021 at Shenzhen Eye Hospital were conducted.The medical records were retrieved and climate information was obtained from the government website.The monthly APAC admissions and evaluated the correlation with climatic factors were compared,including precipitation,air pressure,temperature,sunshine duration,humidity,and evaporation.●RESULTS:The overall mean age of 997 APAC inpatients was 64.63±9.89y,with females predominating(72.12%)and exhibiting significantly shorter axial lengths than males(22.26±0.89 vs 22.87±0.87 mm,P<0.001).APAC admissions peaked in May,especially for males≤65y.Significant positive correlations were identified between admissions and humidity(r=0.498,P<0.01)as well as precipitation(r=0.306,P<0.05),while evaporation correlated negatively(r=-0.333,P<0.05).Males exhibited elevated APAC risk under high humidity[odds ratio(OR)=2.76,P=0.038],while increased air pressure(OR=0.12;95%CI:0.07-0.19),higher temperature(OR=0.22;95%CI:0.15-0.31),and greater evaporation(OR=0.92;95%CI:0.90-0.94)each exerted protective effects against such humidity-associated risk(all P<0.001).●CONCLUSION:Elevated relative humidity in May is the primary climatic driver,significantly increasing the risk of APAC attack in males≤65y.However,these correlations are not significant in female,suggesting the possibility of an associated confounding factor. 展开更多

BACKGROUND Intestinal stoma closure can lead to severe gut microbiome dysbiosis,adversely affecting postoperative recovery and quality of life.An integrative approach combining traditional Chinese medicine(TCM)and Western medicine within a multi-disciplinary team(MDT)framework presents a promising strategy to enhance gut health.However,its efficacy has not been systematically evaluated.AIM To investigate the efficacy of an MDT approach combining TCM and Western medicine in promoting gut microbiome recovery after intestinal stoma closure.METHODS A retrospective study design was employed,analyzing data from patients who underwent intestinal stoma closure between January 2020 and December 2023.Patients were divided into two groups:The control group,which received standard Western medical care,and the MDT group,which received an integrative approach combining TCM and Western medicine.Medical records were reviewed to collect data on patient demographics,clinical characteristics,pre-and postoperative gut microbiome analysis,gastrointestinal symptoms,hospital stay duration,psychological assessments,immunological indicators,and nutritional status.RESULTS The study included 251 patients,with 122 in the control group and 129 in the MDT group.Baseline characteristics were similar between the groups.The MDT group showed significantly higher alpha diversity(Shannon index)postoper-atively compared with the control group.The MDT group had higher operational taxonomic unit counts for Bacteroidetes and Proteobacteria than the control group,whereas the control group had higher operational taxonomic unit counts for Firmicutes than the MDT group.The MDT group demonstrated lower prevalence rates of abdominal pain,bloating,and diarrhea postoperatively and shorter hospital stays than the control group.Patients in the MDT group reported improvements in chronic condition management,psychological well-being,immunological indicators,and nutritional status.The MDT group exhibited enhanced immune function,improved intestinal permeability,and better quality of life than the control group.CONCLUSION This retrospective study suggests that the MDT approach,combining TCM and Western medicine,has the potential to promote gut microbiome recovery and improve overall outcomes after intestinal stoma closure. 展开更多

Rice is the world's largest food crop,but it often encounters flowering asynchronization problems during hybrid rice seed production.In addition,the slow closure of female florets leads to seed mildew and affects the quality.The hormone abscisic acid(ABA)plays a crucial role in plant responses to abiotic stresses.Previous studies showed that exogenous ABA promotes floret closure,although the molecular mechanisms and effects of endogenous ABA on floret closure remain unknown.In this study,the effect of endogenous ABA on floret closure and the molecular mechanism by which ABA promotes floret closure through sugar transporters were investigated by changing the expression levels of OsNCED3 and OsPYL1 in rice.The results showed that overexpression(OE)-OsNCED3increased the endogenous ABA level of florets.Florets closed 5.91 min earlier and OsNCED3 gene knockout line delayed the closure of florets by 5.08 min compared with the wild type.In addition,OsPYL1 regulated the endogenous ABA content and changed the sensitivity to ABA such that the floret closure times for OE and CRISPR-Cas9(CR)were 9.84 min earlier and 12.78 min later,respectively,resulting in an increase in the split husk rate to 15.4%.The gene expression levels of some sugar transporters(STs)changed.The OsPYL1 and OsSWEET4proteins could interact on the cell membrane.These results indicate that ABA promotes the closure of rice florets and the enhanced sensitivity to ABA promotes this effect even more.The molecular mechanism is mainly related to downstream sugar transporters that respond to the ABA signaling pathway,especially OsSWEET4. 展开更多

Underhand cut-and-fill mining has been widely used in underground mining operations,especially when the rock mass or orebody is of poor quality or prone to rockburst due to high stress.In such cases,mining workers should carry out all production activities under the cemented backfill roof or sill mat instead of a highly fractured and unstable rock roof or a strong rock roof with a high potential of rockburst.Therefore,the stability and required strength of the sill mat are critical issues for mining engineers.In 1991,Mitchell considered that sill mat could fail by caving,sliding,rotation,and flexure.Mitchell also proposed an analytical solution to determine the minimum required strength of the sill mat for each type of failure based on two stiff or immobile rock walls.However,recent publications using numerical modeling and field measurements indicate that the compressive stresses in the sill mat induced by rock wall closure due to a stope excavation beneath the sill mat can be significant.It is thus highly necessary to investigate the required strength of the sill mat by considering rock wall closure.In this study,the crushing failure of sill mat due to rock wall closure generated by underground excavation and a new failure mode called"crushing and caving”is revealed by numerical modeling.An analytical solution corresponding to each failure mode is then developed to estimate the minimum required cohesion(cmin)of the sill mat.A criterion is also proposed to determine if the sill mat fails by crushing or crushing-and-caving failure.The proposed analytical solution does not involve any correction coefficients.The validity of the proposed analytical solution is demonstrated by numerical modeling.The proposed analytical solution can thus be employed to predict the cmin of sill mat subjected to wall closure generated by underlying stope excavation. 展开更多

In the underhand cut-and-fill mining method,a sill mat(i.e.an artificial horizontal pillar)constructed by cemented backfill is essential to prevent mine workers from being directly exposed under problematic rock roofs.A critical issue is to determine the minimum required strength of the sill mat to ensure a safe and cost-effective design.Until now,Mitchell’s analytical solution is the only available option,considering two stiff and immobile rock walls.Unavoidable rock wall closure associated with stope excavation below the sill mat was neglected.This,along with other undefined parameters,explains why Mitchell’s solution is rarely used in sill mat design.A new analytical solution for determining the minimum required strength of the sill mat accounting for wall closure is necessary.In this study,a closed-form analytical solution for estimating rock wall closure generated by stope excavation below a sill mat is developed by using Salamon’s and Flamant’s models.The proposed analytical solution does not contain any coefficients of correction or calibration.Despite several assumptions(or somewhat of oversimplifications)necessary to render a simple analytical solution possible,good agreements are obtained between the rock wall closures predicted by applying the proposed analytical solution and those obtained numerically with FLAC3D for many cases with arbitrarily chosen geometrical and material parameters.The proposed analytical solution is therefore validated and can be used to evaluate the rock wall closure generated by stope excavation below a sill mat. 展开更多
