BACKGROUND Cognitive-behavioral therapy(CBT)and habit reversal training(HRT)have shown application potential in addressing tic symptoms and comorbid psychiatric conditions.Despite their theoretical potential,empirical...BACKGROUND Cognitive-behavioral therapy(CBT)and habit reversal training(HRT)have shown application potential in addressing tic symptoms and comorbid psychiatric conditions.Despite their theoretical potential,empirical evidence on their combined efficacy remains limited.AIM To evaluate the efficacy of CBT combined with HRT on anxiety disorders in children with Tourette’s syndrome(TS).METHODS Clinical data of children with TS admitted to our hospital from January 2022 to June 2023 were collected,and the patients were grouped into the conventional therapy(control)group and the CBT combined with HRT group.Baseline charac-teristics,anxiety scores,tic severity scores,treatment adherence,and parental satisfaction were assessed.Statistical analysis was performed using t-tests,chi-square tests,and correlation analysis.RESULTS A total of 136 patients,including 65 patients in the control group and 71 patients in the CBT combined with HRT group,were included.The CBT combined with HRT group showed remarkable improvements compared with the control group.Post-intervention assessment revealed a decrease in anxiety scores from 63.52±1.81 to 40.53±1.64(t=2.022,P=0.045),and the Yale Global Tic Severity Scale total score decreased from 22.14±5.67 to 16.28±4.91(t=2.288,P=0.024).Treatment adherence was significantly higher in the CBT combined with HRT group(85.47±7.62%)compared with the control group(82.32±6.54%;t=2.596,P=0.010).Parental satisfaction scores were also higher in the CBT combined with HRT group(8.69±1.77)compared with the control group(7.87±1.92;t=2.592,P=0.011).CONCLUSION This study demonstrates that CBT combined with HRT significantly reduces anxiety symptoms and tic severity in children with TS,with higher treatment adherence and parental satisfaction.These findings support the potential application of this comprehensive therapeutic approach for TS treatment.展开更多

BACKGROUND Public employees worldwide are increasingly concerned about work anxiety and depression.Cognitive-behavioral career coaching has emerged as a promising strategy for addressing these mental health disorders,...BACKGROUND Public employees worldwide are increasingly concerned about work anxiety and depression.Cognitive-behavioral career coaching has emerged as a promising strategy for addressing these mental health disorders,which can negatively impact on a person's overall well-being and performance.AIM To examine whether cognitive-behavioral career coaching reduces work anxiety and depression among Nigerian public employees.METHODS A total of 120 public employees(n=60)suffering from severe anxiety and depression were randomly assigned to the treatment or control groups in this study.Cognitive behavioral coaching was provided twice a week to those in the treatment group,whereas no treatment was given to those in the control group.As part of the study,the Hamilton Anxiety Rating Scales and Beck Depression Inventory were used to collect data.RESULTS Analysis of covariance of the data from participants indicates a significant effect of cognitive-behavioral career coaching on work anxiety and depression.CONCLUSION Insights into the underlying mechanisms by which cognitive behavior career coaching exerts its effects have been gained from this study.Also,the study has gathered valuable data that can inform future practice and guide the development of strategies for supporting mental health at work.展开更多

Methods: One hundred fourteen SAD patients completed manualized group CBT. We examined associations between the personality dimensions of NEO Five Factor Index (NEO-FFI) and the subscales of Symptom Checklist-90 Revised (SCL-90-R) in SAD patients after CBT using multiple regression analysis. Results: High levels of conscientiousness at baseline predicted symptom reduction on 4 SCL-90-R scales, including somatization, obsessive-compulsive, anxiety and global severity index in patients with SAD after CBT. And high levels of agreeableness predicted symptom reduction on 2 SCL-90-R scales, including Hostility and Paranoid Ideation. High levels of openness predicted psychoticism. Conclusion: The present study suggested that high levels of three NEO-FFI dimensions (openness, agreeableness, conscientiousness) might predict comorbid psychological symptoms reduction in SAD patients after CBT. For the purpose of improving comorbid psychological symptoms with SAD patients, it might be useful to pay more attention to these dimensions of NEO-FFI at baseline.展开更多
Pregnancy,being a unique life event with conspicuous physiological and psychological changes,can induce perinatal depression in many expectant mothers.The objective of this paper was to review published articles on th...Pregnancy,being a unique life event with conspicuous physiological and psychological changes,can induce perinatal depression in many expectant mothers.The objective of this paper was to review published articles on the efficacy of the cognitive-behavioral therapy(CBT)to prevent perinatal depression.MEDLINE,Science Direct,the British Nursing Index and Archive and the Cumulative Index of Nursing and Allied Health Literature(CINAHL)were searched for English language articles from 2011 through 2013 inclusive.Ten quality primary quantitative studies were selected according to a critical framework checklist.The results of this review highlight the effectiveness of the CBT to prevent perinatal depression.The various formats were included modified version,in-home setting,tailor-made design or internet-based approach either in group or individually.Duration of the CBT was designed from 5 to 16 sessions and each session was lasting for 45 minute to 2 hours.All of them were focused on correcting the negative distorted cognitions and dysfunctional underlying beliefs that maintain depressive symptoms.A brief CBT with group approach or internet format may be considering in further recommendation.A brief is a time-limited,cost-effective,efficacious and an internet CBT could provide women with the flexibility of receiving treatment at a time and place convenient for them.展开更多

Methods: The 91 PTSD patients in Wenchuan hit by a strong earthquake were randomly divided into a control group of 24 cases treated by the cognitive-behavior therapy, and a treatment group of 67 cases treated by both cognitive-behavior therapy and acupoint stimulation. The scores were evaluated according to Chinese version of the incident effect scale revised (IES-R) and the self-compiled questionnaire for the major post-traumatic psychological condition, and the curative effect was compared between the two groups. Results: The total scores of IES-R, the scores of all factors and the total scores of the questionnaire in the two groups after treatment were much lower than those before treatment (P〈0.01). The comparison of reduction in the factor scores between the two groups showed that the curative effect inthe treatment group was better that of in the control group. Conclusion: The acupoint stimulation is effective for the PTSD patients, with better results than that of cognitive-behavior therapy used alone展开更多

BACKGROUND Functional neurological disorder(FND)in children is a complex and multifaceted condition characterized by neurological symptoms that cannot be explained by organic pathology.Despite its prevalence,FND in pe...BACKGROUND Functional neurological disorder(FND)in children is a complex and multifaceted condition characterized by neurological symptoms that cannot be explained by organic pathology.Despite its prevalence,FND in pediatric populations remains under-researched,with challenges in diagnosis and management AIM To synthesize the current literature on FND in children,focusing on clinical presentation,diagnostic approaches,treatment strategies,and outcomes.METHODS A comprehensive literature search was conducted across multiple databases,including PubMed,Scopus,and Web of Science,for articles published up to August 2024.Studies were included if they addressed FND in pediatric populations,specifically focusing on review articles,research articles,systematic reviews,meta-analyses,case reports,guidelines,expert opinions,and editorials.Data extraction and quality assessment were performed according to PRISMA guidelines.A total of 308 articles were included in the final analysis.RESULTS The analysis included 189 review articles,57 research articles,3 systematic reviews and meta-analyses,5 case reports,2 guidelines,5 expert opinions,and 2 editorials.Key findings revealed a broad spectrum of symptoms,including motor and sensory disturbances and psychological factors contributing to the onset and persistence of FND.Diagnostic challenges were frequently highlighted,emphasizing the need for interdisciplinary approaches.Treatment strategies varied,with cognitive-behavioral therapy(CBT)and multidisciplinary care emerging as the most effective approaches.The outcomes varied,with early intervention being critical for a better prognosis.CONCLUSION Early diagnosis and multidisciplinary care,including CBT,are critical for improving outcomes in pediatric FND.Standardized diagnostic criteria and treatment protocols are needed to enhance clinical management.展开更多

The case study aimed to decrease the agression and acting out behavior as result of being victimized at school by the peers. The aim was to assess and manage the child’s aggressive behavior and academic underperformance which played a significant role in the child’s low self-esteem and emotional regulation. A comprehensive assessment was conducted to rule out the difficulties and a multi-faceted intervention strategy was utilized including anger management and structured activity scheduling that helped that child to improve his academic performance as well as to learn to manage his emotional expression. Throughout 16 sessions, the intervention targeted key behavioural indicators such as emotional expression, and aggression;post-assessment results demonstrated a 22% improvement in the child’s behavioral and academic challenges. The findings suggest that a multi-faceted therapeutic approach can be effective in addressing complex issues of aggression and academic underperformance in children, highlighting the importance of integrated psychological and educational interventions.展开更多

BACKGROUND Patients with chronic hepatitis B(CHB)experience various problems,including low psychological flexibility,negative emotions,and poor sleep quality.Therefore,effective nursing interventions are required to r...BACKGROUND Patients with chronic hepatitis B(CHB)experience various problems,including low psychological flexibility,negative emotions,and poor sleep quality.Therefore,effective nursing interventions are required to reduce adverse events.Acceptance and commitment therapy(ACT)combined with enabling cognitivebehavioral education(ECBE)can improve patients'psychological and sleep.Therefore,we speculate that this may also be effective in patients with CHB.AIM To investigate the effects of different intervention methods on psychological flexibility,negative emotions,and sleep quality in patients with CHB.METHODS This retrospective study examined clinical and evaluation data of 129 patients with CHB.Intervention methods were divided into a conventional group(routine nursing,n=69)and a combination group(ACT combined with ECBE,n=60).We observed changes in psychological flexibility,negative emotions,sleep quality,and self-care ability in both groups.Observation items were evaluated using the Acceptance and Action Questionnaire-2nd Edition(AAQ-II),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),Pittsburgh Sleep Quality Index(PSQI),and Exercise of Self-Care Agency Scale(ESCA).RESULTS Compared with the conventional group,the AAQ-II score of the combined group was lower(F_(between-group effect)=8.548;F_(time effects)=25.020;F_(interaction effects)=52.930;all P<0.001),the SAS score(t=5.445)and SDS score(t=7.076)were lower(all P<0.001),as were the PSQI dimensions(tsleep quality=4.581,tfall sleep time=2.826,tsleep time=2.436,tsleep efficiency=5.787,tsleep disorder=5.008,thypnotic drugs=3.786,tdaytime dysfunction=4.812);all P<0.05).The ESCA scores for all dimensions were higher(thealth knowledge level=6.994,t self-concept=5.902,tself-responsibility=19.820,tself-care skills=8.470;all P<0.001).CONCLUSION ACT combined with ECBE in patients with CHB can improve psychological flexibility and sleep quality,alleviate negative emotions,and improve self-care.展开更多

Objectives:Prevalence rates of overweight and obesity are dramatically ever-increasing across the world.Therefore,this study was to evaluate the effect of mindfulness-based cognitive therapy(MBCT)on weight loss,hypert...Objectives:Prevalence rates of overweight and obesity are dramatically ever-increasing across the world.Therefore,this study was to evaluate the effect of mindfulness-based cognitive therapy(MBCT)on weight loss,hypertension,and attentional bias towards food cues in a group of women affected with this condition.Methods:A total of 45 participants were selected out of women referring to the Nutrition and Diet Therapy Clinic affiliated to Shahid Beheshti University of Medical Sciences,Iran,and then randomized into three groups of 15.The first experimental group was subjected to an energy-restricted diet therapy together with MBCT during 8 sessions,the second group took the diet therapy alone,and the third group received no intervention.Body mass index(BMI),hypertension,and attentional bias towards food cues were correspondingly evaluated before,at the end,and four weeks after the completion of the interventions.Results:The results of this study revealed that MBCT,along with diet therapy,had been significantly more effective in weight loss,decrease in BMI,lower systolic blood pressure(SBP),and attentional bias towards food cues compared with the diet therapy alone(P≤0.01).MBCT had no significant impact on the decline in diastolic blood pressure(DBP)in participants in the follow-up phase.Conclusion:This study demonstrated that MBCT along with the conventional diet therapy was more effective in weight loss,decrease in BMI,hypertension control,as well as attentional bias towards food cues than the diet therapy alone.展开更多
The Persian Gulf War of 1990 to 1991 involved the deployment of nearly 700,000 American troops to the Middle East.Deployment-related exposures to toxic substances such as pesticides,nerve agents,pyridostigmine bromide...The Persian Gulf War of 1990 to 1991 involved the deployment of nearly 700,000 American troops to the Middle East.Deployment-related exposures to toxic substances such as pesticides,nerve agents,pyridostigmine bromide(PB),smoke from burning oil wells,and petrochemicals may have contributed to medical illness in as many as 250,000 of those American troops.The cluster of chronic symptoms,now referred to as Gulf War Illness(GWI),has been studied by many researchers over the past two decades.Although over$500 million has been spent on GWI research,to date,no cures or condition-specific treatments have been discovered,and the exact pathophysiology remains elusive.Using the 2007 National Institute of Health(NIH)Roadmap for Medical Research model as a reference framework,we reviewed studies of interventions involving GWI patients to assess the progress of treatment-related GWI research.All GWI clinical trial studies reviewed involved investigations of existing interventions that have shown efficacy in other diseases with analogous symptoms.After reviewing the published and ongoing registered clinical trials for cognitivebehavioral therapy,exercise therapy,acupuncture,coenzyme Q10(CoQ10),mifepristone,and carnosine in GWI patients,we identified only four treatments(cognitive-behavioral therapy,exercise therapy,CoQ10,and mifepristone)that have progressed beyond a phase II trial.We conclude that progress in the scientific study of therapies for GWI has not followed the NIH Roadmap for Medical Research model.Establishment of a standard case definition,prioritized GWI research funding for the characterization of the pathophysiology of the condition,and rapid replication and adaptation of early phase,single site clinical trials could substantially advance research progress and treatment discovery for this condition.展开更多
摘要High-altitude respiratory diseases,characterized by hypoxia-induced complications such as pulmonary edema and chronic bronchitis,present critical challenges to both physical and mental health in vulnerable populations.The recent study by Meng et al demonstrates that integrating cognitive-behavioral therapy(CBT)with standard care significantly improves anxiety levels and sleep quality in affected patients.These key findings hold direct implications for advancing respiratory management in hypoxic environments.Meng et al’s observation of reduced serum hypoxia biomarkers(hypoxia-inducible factor-1α and erythropoietin)further suggests potential enhancements in physiological adaptation.However,the interpretation of these biomarker changes as evidence of improved hypoxia tolerance requires cautious examination.The study’s reliance on indirect indicators fails to establish whether CBT’s benefits stem primarily from psychological modulation or genuine cellular adaptation.Furthermore,the 5-day intervention window coincides with the half-life of erythropoietin,raising questions about the durability of these effects in the context of hypoxic acclimatization,which typically requires weeks to months.To resolve these ambiguities,future studies should prioritize longitudinal designs tracking both respiratory function and molecular biomarkers over clinically relevant timeframes.Integrating CBT with objective pulmonary assessments would clarify its role in optimizing oxygen utilization pathways.Such advances could refine targeted interventions for high-altitude respiratory rehabilitation,ultimately strengthening evidence-based care for this unique patient population.
摘要Irritable bowel syndrome(IBS)is a common disorder,reported to be found in 5%-20%of the general population.Its management accounts for up to 25%of a gastroenterologist’s workload in the outpatient department,and the main symptoms are abdominal pain,bloating,and altered bowel habits.Despite a great amount of available pharmacological treatments aimed at a wide variety of gastrointestinal and brain targets,many patients have not shown adequate symptom relief.In recent years,there has been increasing evidence to suggest that psychological treatments,in particular cognitive-behavioral therapy(CBT),are effective for the management of IBS.This review discusses CBT for the management of IBS.CBT has proved to be effective in alleviating the physical and psychological symptoms of IBS and has thus been recommended as a treatment option for the syndrome.
摘要BACKGROUND Cognitive-behavioral therapy(CBT)and habit reversal training(HRT)have shown application potential in addressing tic symptoms and comorbid psychiatric conditions.Despite their theoretical potential,empirical evidence on their combined efficacy remains limited.AIM To evaluate the efficacy of CBT combined with HRT on anxiety disorders in children with Tourette’s syndrome(TS).METHODS Clinical data of children with TS admitted to our hospital from January 2022 to June 2023 were collected,and the patients were grouped into the conventional therapy(control)group and the CBT combined with HRT group.Baseline charac-teristics,anxiety scores,tic severity scores,treatment adherence,and parental satisfaction were assessed.Statistical analysis was performed using t-tests,chi-square tests,and correlation analysis.RESULTS A total of 136 patients,including 65 patients in the control group and 71 patients in the CBT combined with HRT group,were included.The CBT combined with HRT group showed remarkable improvements compared with the control group.Post-intervention assessment revealed a decrease in anxiety scores from 63.52±1.81 to 40.53±1.64(t=2.022,P=0.045),and the Yale Global Tic Severity Scale total score decreased from 22.14±5.67 to 16.28±4.91(t=2.288,P=0.024).Treatment adherence was significantly higher in the CBT combined with HRT group(85.47±7.62%)compared with the control group(82.32±6.54%;t=2.596,P=0.010).Parental satisfaction scores were also higher in the CBT combined with HRT group(8.69±1.77)compared with the control group(7.87±1.92;t=2.592,P=0.011).CONCLUSION This study demonstrates that CBT combined with HRT significantly reduces anxiety symptoms and tic severity in children with TS,with higher treatment adherence and parental satisfaction.These findings support the potential application of this comprehensive therapeutic approach for TS treatment.

摘要This editorial explores the study by Mkpoikanke Sunday Otu and Maximus Monaheng Sefotho on the use of cognitive-behavioral career coaching(CBCC)to reduce work anxiety and depression among public employees.Public sector workers often face significant psychological stressors,leading to mental health issues that impair well-being and job performance.The study employed a grouprandomized trial design,involving 120 public employees diagnosed with severe anxiety and depression.Participants were divided into a treatment group,receiving CBCC,and a control group with no intervention.Results showed a significant reduction in anxiety and depression levels in the treatment group,sustained through follow-up assessments.The findings underscore the clinical relevance of CBCC as an effective intervention for improving mental health among public employees.Future research should explore the scalability of CBCC across different sectors and cultural contexts,as well as its integration with other mental health interventions.This editorial advocates for the broader implementation of CBCC practices in public service to enhance employee productivity and psychological well-being.By addressing cognitive distortions and fostering adaptive coping mechanisms,CBCC can help public employees navigate complex professional environments,ultimately contributing to a healthier and more efficient workforce.
摘要BACKGROUND Public employees worldwide are increasingly concerned about work anxiety and depression.Cognitive-behavioral career coaching has emerged as a promising strategy for addressing these mental health disorders,which can negatively impact on a person's overall well-being and performance.AIM To examine whether cognitive-behavioral career coaching reduces work anxiety and depression among Nigerian public employees.METHODS A total of 120 public employees(n=60)suffering from severe anxiety and depression were randomly assigned to the treatment or control groups in this study.Cognitive behavioral coaching was provided twice a week to those in the treatment group,whereas no treatment was given to those in the control group.As part of the study,the Hamilton Anxiety Rating Scales and Beck Depression Inventory were used to collect data.RESULTS Analysis of covariance of the data from participants indicates a significant effect of cognitive-behavioral career coaching on work anxiety and depression.CONCLUSION Insights into the underlying mechanisms by which cognitive behavior career coaching exerts its effects have been gained from this study.Also,the study has gathered valuable data that can inform future practice and guide the development of strategies for supporting mental health at work.
摘要Aim: The present study aimed to examine the predictors of comorbid psychological symptoms in social anxiety disorder (SAD) after cognitive-behavioral therapy (CBT). Methods: One hundred fourteen SAD patients completed manualized group CBT. We examined associations between the personality dimensions of NEO Five Factor Index (NEO-FFI) and the subscales of Symptom Checklist-90 Revised (SCL-90-R) in SAD patients after CBT using multiple regression analysis. Results: High levels of conscientiousness at baseline predicted symptom reduction on 4 SCL-90-R scales, including somatization, obsessive-compulsive, anxiety and global severity index in patients with SAD after CBT. And high levels of agreeableness predicted symptom reduction on 2 SCL-90-R scales, including Hostility and Paranoid Ideation. High levels of openness predicted psychoticism. Conclusion: The present study suggested that high levels of three NEO-FFI dimensions (openness, agreeableness, conscientiousness) might predict comorbid psychological symptoms reduction in SAD patients after CBT. For the purpose of improving comorbid psychological symptoms with SAD patients, it might be useful to pay more attention to these dimensions of NEO-FFI at baseline.
摘要Pregnancy,being a unique life event with conspicuous physiological and psychological changes,can induce perinatal depression in many expectant mothers.The objective of this paper was to review published articles on the efficacy of the cognitive-behavioral therapy(CBT)to prevent perinatal depression.MEDLINE,Science Direct,the British Nursing Index and Archive and the Cumulative Index of Nursing and Allied Health Literature(CINAHL)were searched for English language articles from 2011 through 2013 inclusive.Ten quality primary quantitative studies were selected according to a critical framework checklist.The results of this review highlight the effectiveness of the CBT to prevent perinatal depression.The various formats were included modified version,in-home setting,tailor-made design or internet-based approach either in group or individually.Duration of the CBT was designed from 5 to 16 sessions and each session was lasting for 45 minute to 2 hours.All of them were focused on correcting the negative distorted cognitions and dysfunctional underlying beliefs that maintain depressive symptoms.A brief CBT with group approach or internet format may be considering in further recommendation.A brief is a time-limited,cost-effective,efficacious and an internet CBT could provide women with the flexibility of receiving treatment at a time and place convenient for them.
摘要Objective To investigate the efficacy of cognitivebehavioraltherapy for insomnia (CBT-i) or combinationwith tapered hypnotic agents. Methods Seventy-five patientswere randomized into either CBT-i group (n = 37)or combination group ( n = 38). The duration of treatmentlasted for 8 weeks. The efficacy was evaluated byPittsburgh sleep quality index (PSQI),Beck depressionindex (BDI),Beck anxiety inventory (BAI) and sleepdiary variables at baseline,in the process and end oftreatment. Results ( 1 ) Compared with the results atbaseline,the total scores of PSQI,BDI and BAI in bothgroups significantly decreased at the end of treatment:CBT-i group,PSQI (4. 7 ± 2. 5) vs. (12. 9 ± 3. 5);BDI (3. 2 ± 4. 4) vs. (9. 7 ± 6. 4);BAI (4. 2 ± 5. 6)vs. ( 10. 7 ± 8. 1 );and combination group, PSQI(5. 8 ± 2. 8) vs. (13. 9 ± 3. 1);BDI (4. 5 ± 4. 8) vs.(13. 8 ± 8. 7);BAI (4. 4 ± 4. 0) vs. (14. 1 ± 6. 3) (allP < 0. 01). (2) Compared with the results at baseline,subjective sleep quality ( SQ ), sleep onset latency(SOL),sleep efficiency (SE),sleep disturbance (SD)and used sleep medication (USM) in PSQI in combinationgroup significantly decreased at week 4 and 8 ( allP < 0. 05). The total sleep time (TST) and daytime dysfunction(DF) in PSQI significantly decreased at week 8( both P < 0. 05 ). ( 3 ) Compared with combinationgroup,improvement of SOL and SE in CBT-i group wassuperior (both P = 0. 01). Conclusion CBT-i for chronicinsomnia is effective in both CBT-i alone and combinationwith tapered hypnotic agents. CBT-i group is superiorin improving SOL and SE. Combination regimen in ourstudy can significantly reduce the doses of medication.
摘要Dysfunctional sleep beliefs and chronotype together influence student sleep health and academic performance.Altınöz et al,published in the recent issue of the World Journal of Psychiatry,provided evidence that maladaptive sleep beliefs undermine academic achievement indirectly through reduced sleep quality,with evening-type students showing the highest vulnerability.Based on these findings,this commentary highlights a cognitive-circadian interaction model,where negative sleep cognitions intensify the mismatch between biological rhythms and institutional schedules.Morning-type students benefit from better alignment,whereas evening-types face compounded risk from distorted beliefs and circadian conflict.We further discuss the methodological strengths and limitations of the original study and propose future directions focused on longitudinal validation,objective sleep assessments,and chronotype-sensitive interventions.Targeting dysfunctional sleep beliefs through behavioral strategies and more flexible academic scheduling may improve both sleep quality and educational outcomes in high-pressure learning environments.
摘要Ancient Hindu philosophical texts have startled European thinkers and philosophers in the 17th century and have impacted the entire humanity at the global level;various scholars like Arthur Schopenhauer,Ralph Waldo Emerson,and other learned scholars of Indology have given their positive opinion in their respective contemporary researches and translation and suggest to understand these texts at deeper level.Edwin Arnold opines these texts as“par excellence”,“The way to cross over the Death”in order to realize the actual form of blissful existence that is self-effulgent and nothing remains for attainment after realizing the self in the entire cosmos.The Katha Upanishad is one of the most celebrated and studied Vedanta sub-school scriptures.Centred on a dialogue between a young boy named Nachiketa and Yama,the god of death,it defines the nature of the self,the actual meaning of liberation,and the primacy of Self-knowledge over material pleasure.Unlike many abstract Vedic texts,it conveys profound metaphysics through a compelling,dramatic narrative.This story-driven format makes highly complex spiritual concepts accessible to a wider audience.It introduces the vital distinction between“Shreyas”(lasting path to spiritual liberation)and“Preyas”(the pleasant,temporary pursuit of material wealth and worldly desires)and underlines the need to understand the true and false nature of our desires in a more meaningful way to realize the everlasting state of happiness.The text contains the iconic parable of the chariot which explains the life process.It illustrates how the body is a chariot,the intellect is the charioteer,the mind is the reins,and the senses are the horses.In fact,this framework remains a fundamental teaching on righteous form of mindfulness choice and implications which can be included in the process of clinical counselling.
摘要Objective: To study the curative effect of acupoint stimulation on the earthquake-caused post-traumatic stress disorder (PTSD). Methods: The 91 PTSD patients in Wenchuan hit by a strong earthquake were randomly divided into a control group of 24 cases treated by the cognitive-behavior therapy, and a treatment group of 67 cases treated by both cognitive-behavior therapy and acupoint stimulation. The scores were evaluated according to Chinese version of the incident effect scale revised (IES-R) and the self-compiled questionnaire for the major post-traumatic psychological condition, and the curative effect was compared between the two groups. Results: The total scores of IES-R, the scores of all factors and the total scores of the questionnaire in the two groups after treatment were much lower than those before treatment (P〈0.01). The comparison of reduction in the factor scores between the two groups showed that the curative effect inthe treatment group was better that of in the control group. Conclusion: The acupoint stimulation is effective for the PTSD patients, with better results than that of cognitive-behavior therapy used alone

摘要This study addressed the critical need for an integrated,personalized approach to perimenopausal mental health,addressing both biological and psychosocial fac-tors.Current research highlighted the influence of hormonal fluctuations,genetic predispositions,and lifestyle factors in shaping perimenopausal mental health outcomes.This transitional period is marked by significant hormonal fluctuations contributing to heightened anxiety,depression,and sleep disturbances,affecting the women’s quality of life.Traditional pharmacological treatments,including selective serotonin reuptake inhibitors and hormone replacement therapy,have limitations due to variable efficacy and side effects,emphasizing the need for precision medicine.Advancements in pharmacogenomics and metabolomics provide new avenues for individualized treatments,with genetic markers(e.g.,Solute carrier organic anion transporter family member 1B1,estrogen receptor 1/estrogen receptor 2,and tachykinin receptor 3)guiding hormone therapy resp-onses.Emerging digital health technologies,such as artificial intelligence-driven diagnostics,wearable monitoring,and telehealth platforms,offer scalable,real-time mental health support,though regulatory and clinical validation challenges remain.Furthermore,integrative treatment models combining hormone-based therapy with non-pharmacological interventions demonstrate significant efficacy in alleviating perimenopausal symptoms.Future directions should prioritize the clinical validation and ethical implementation of digital health solutions,ensuring safety,efficacy,and user accessibility.A multidisciplinary,patient-centric model,incorporating genetics,endocrinology,digital health,and psychosocial interventions,is essential for optimizing perimenopausal mental health outcomes.
摘要BACKGROUND Functional neurological disorder(FND)in children is a complex and multifaceted condition characterized by neurological symptoms that cannot be explained by organic pathology.Despite its prevalence,FND in pediatric populations remains under-researched,with challenges in diagnosis and management AIM To synthesize the current literature on FND in children,focusing on clinical presentation,diagnostic approaches,treatment strategies,and outcomes.METHODS A comprehensive literature search was conducted across multiple databases,including PubMed,Scopus,and Web of Science,for articles published up to August 2024.Studies were included if they addressed FND in pediatric populations,specifically focusing on review articles,research articles,systematic reviews,meta-analyses,case reports,guidelines,expert opinions,and editorials.Data extraction and quality assessment were performed according to PRISMA guidelines.A total of 308 articles were included in the final analysis.RESULTS The analysis included 189 review articles,57 research articles,3 systematic reviews and meta-analyses,5 case reports,2 guidelines,5 expert opinions,and 2 editorials.Key findings revealed a broad spectrum of symptoms,including motor and sensory disturbances and psychological factors contributing to the onset and persistence of FND.Diagnostic challenges were frequently highlighted,emphasizing the need for interdisciplinary approaches.Treatment strategies varied,with cognitive-behavioral therapy(CBT)and multidisciplinary care emerging as the most effective approaches.The outcomes varied,with early intervention being critical for a better prognosis.CONCLUSION Early diagnosis and multidisciplinary care,including CBT,are critical for improving outcomes in pediatric FND.Standardized diagnostic criteria and treatment protocols are needed to enhance clinical management.
摘要This minireview focuses on psychological distress and treatment adherence-issues in patients with chronic hepatitis B(CHB).It begins by discussing the epidemiology and disease burden of CHB,and addresses the relationship between psychological distress and treatment adherence.Next,it delves into the current status and risk factors for psychological distress among patients with CHB,and explores the challenges and risk factors related to treatment adherence.Subsequently,it explores the development and implementation of integrated nursing strategies,including psychological interventions and support,self-efficacy enhancement strategies,social support-system optimization,personalized medical care,and technological innovation.Finally,it highlights the limitations of current interventions and clarifies future research priorities.This minireview aims to provide a comprehensive theoretical basis and practical guidance for improving treatment outcomes and quality of life of patients with CHB.In summary,we reveal that psychological distress significantly impacts treatment adherence in patients with CHB and that it is essential to adopt integrated nursing strategies to address these challenges.These findings highlight the need to consider the psychological states of individuals and develop targeted interventions to improve treatment outcomes.
摘要This case study explores the efficacy of school-based intervention to address psychosocial challenges faced by an 11-year-old adolescent. The case study aimed to decrease the agression and acting out behavior as result of being victimized at school by the peers. The aim was to assess and manage the child’s aggressive behavior and academic underperformance which played a significant role in the child’s low self-esteem and emotional regulation. A comprehensive assessment was conducted to rule out the difficulties and a multi-faceted intervention strategy was utilized including anger management and structured activity scheduling that helped that child to improve his academic performance as well as to learn to manage his emotional expression. Throughout 16 sessions, the intervention targeted key behavioural indicators such as emotional expression, and aggression;post-assessment results demonstrated a 22% improvement in the child’s behavioral and academic challenges. The findings suggest that a multi-faceted therapeutic approach can be effective in addressing complex issues of aggression and academic underperformance in children, highlighting the importance of integrated psychological and educational interventions.

摘要BACKGROUND Patients with chronic hepatitis B(CHB)experience various problems,including low psychological flexibility,negative emotions,and poor sleep quality.Therefore,effective nursing interventions are required to reduce adverse events.Acceptance and commitment therapy(ACT)combined with enabling cognitivebehavioral education(ECBE)can improve patients'psychological and sleep.Therefore,we speculate that this may also be effective in patients with CHB.AIM To investigate the effects of different intervention methods on psychological flexibility,negative emotions,and sleep quality in patients with CHB.METHODS This retrospective study examined clinical and evaluation data of 129 patients with CHB.Intervention methods were divided into a conventional group(routine nursing,n=69)and a combination group(ACT combined with ECBE,n=60).We observed changes in psychological flexibility,negative emotions,sleep quality,and self-care ability in both groups.Observation items were evaluated using the Acceptance and Action Questionnaire-2nd Edition(AAQ-II),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),Pittsburgh Sleep Quality Index(PSQI),and Exercise of Self-Care Agency Scale(ESCA).RESULTS Compared with the conventional group,the AAQ-II score of the combined group was lower(F_(between-group effect)=8.548;F_(time effects)=25.020;F_(interaction effects)=52.930;all P<0.001),the SAS score(t=5.445)and SDS score(t=7.076)were lower(all P<0.001),as were the PSQI dimensions(tsleep quality=4.581,tfall sleep time=2.826,tsleep time=2.436,tsleep efficiency=5.787,tsleep disorder=5.008,thypnotic drugs=3.786,tdaytime dysfunction=4.812);all P<0.05).The ESCA scores for all dimensions were higher(thealth knowledge level=6.994,t self-concept=5.902,tself-responsibility=19.820,tself-care skills=8.470;all P<0.001).CONCLUSION ACT combined with ECBE in patients with CHB can improve psychological flexibility and sleep quality,alleviate negative emotions,and improve self-care.
摘要Depression is a prevalent psychiatric disorder that often leads to poor quality of life and impaired functioning.Treatment during the acute phase of a major depressive episode aims to help the patient reach a remission state and eventually return to their baseline level of functioning.Pharmacotherapy,especially selective serotonin reuptake inhibitors antidepressants,remains the most frequent option for treating depression during the acute phase,while other promising pharmacological options are still competing for the attention of practitioners.Depressionfocused psychotherapy is the second most common option for helping patients overcome the acute phase,maintain remission,and prevent relapses.Electroconvulsive therapy is the most effective somatic therapy for depression in some specific situations;meanwhile,other methods have limits,and their specific indications are still being studied.Combining medications,psychotherapy,and somatic therapies remains the most effective way to manage resistant forms of depression.
摘要Objectives:Prevalence rates of overweight and obesity are dramatically ever-increasing across the world.Therefore,this study was to evaluate the effect of mindfulness-based cognitive therapy(MBCT)on weight loss,hypertension,and attentional bias towards food cues in a group of women affected with this condition.Methods:A total of 45 participants were selected out of women referring to the Nutrition and Diet Therapy Clinic affiliated to Shahid Beheshti University of Medical Sciences,Iran,and then randomized into three groups of 15.The first experimental group was subjected to an energy-restricted diet therapy together with MBCT during 8 sessions,the second group took the diet therapy alone,and the third group received no intervention.Body mass index(BMI),hypertension,and attentional bias towards food cues were correspondingly evaluated before,at the end,and four weeks after the completion of the interventions.Results:The results of this study revealed that MBCT,along with diet therapy,had been significantly more effective in weight loss,decrease in BMI,lower systolic blood pressure(SBP),and attentional bias towards food cues compared with the diet therapy alone(P≤0.01).MBCT had no significant impact on the decline in diastolic blood pressure(DBP)in participants in the follow-up phase.Conclusion:This study demonstrated that MBCT along with the conventional diet therapy was more effective in weight loss,decrease in BMI,hypertension control,as well as attentional bias towards food cues than the diet therapy alone.
摘要The Persian Gulf War of 1990 to 1991 involved the deployment of nearly 700,000 American troops to the Middle East.Deployment-related exposures to toxic substances such as pesticides,nerve agents,pyridostigmine bromide(PB),smoke from burning oil wells,and petrochemicals may have contributed to medical illness in as many as 250,000 of those American troops.The cluster of chronic symptoms,now referred to as Gulf War Illness(GWI),has been studied by many researchers over the past two decades.Although over$500 million has been spent on GWI research,to date,no cures or condition-specific treatments have been discovered,and the exact pathophysiology remains elusive.Using the 2007 National Institute of Health(NIH)Roadmap for Medical Research model as a reference framework,we reviewed studies of interventions involving GWI patients to assess the progress of treatment-related GWI research.All GWI clinical trial studies reviewed involved investigations of existing interventions that have shown efficacy in other diseases with analogous symptoms.After reviewing the published and ongoing registered clinical trials for cognitivebehavioral therapy,exercise therapy,acupuncture,coenzyme Q10(CoQ10),mifepristone,and carnosine in GWI patients,we identified only four treatments(cognitive-behavioral therapy,exercise therapy,CoQ10,and mifepristone)that have progressed beyond a phase II trial.We conclude that progress in the scientific study of therapies for GWI has not followed the NIH Roadmap for Medical Research model.Establishment of a standard case definition,prioritized GWI research funding for the characterization of the pathophysiology of the condition,and rapid replication and adaptation of early phase,single site clinical trials could substantially advance research progress and treatment discovery for this condition.
