Background:Benign prostatic hyperplasia(BPH)is prevalent among aging men.Whether exposure to internet-derived health information is associated with disease awareness and symptom burden at initial diagnosis remains unclear. Methods:We conducted a cross-sectional study of 400 newly diagnosed BPH outpatients.Participants were classified by prior exposure to internet-derived BPH content.BPH awareness and International Prostate Symptom Scores(IPSS)were compared using the Mann-Whitney U test and multivariate linear regression adjusted for age, education, and disposable income.Stratified analyses were performed according to educational attainment, disposable income, and content format. Results:Exposure to internet-derived BPH content was independently associated with higher awareness and lower IPSS.Compared with unexposed participants, exposed participants had higher awareness(median 1.5 vs.0)and lower IPSS(median 7.0 vs.10.5)(both p<0.001).In multivariable models, being unexposed was associated with a 1.39-point lower awareness score and a 2.20-point higher IPSS(both p<0.001).In stratified analyses, the associations remained significant across education and income strata.Among exposed participants, those accessing text-based or mixed-format content had higher awareness and lower IPSS than those exposed to videos only(both p<0.001).Conclusions:This study shows that exposure to internet-derived health content is significantly associated with higher BPH awareness and a lower symptom burden at initial presentation.Text-based or mixed-format content may be more effective than video-only formats. 展开更多

OBJECTIVE:To investigate the association between Traditional Chinese Medicine(TCM)constitution and fear of cancer recurrence(FCR)in breast cancer survivors.METHODS:A cross-sectional study was conducted among breast cancer patients at Guang'anmen Hospital of the Chinese Academy of Traditional Chinese Medicine,China, during the period from April 9,2023,to July 2,2024.Data were collected on participants'common sociodemographic characteristics, medical history, scores on the Fear of Cancer Recurrence Inventory(FCRI),and TCM constitution classification.Statistical analyses employed independent samples t-test,χ2test,Fisher's exact test,one-way analysis of variance,Spearman's correlation coefficient, multiple linear regression analysis, and multivariate logistic regression analysis.RESULTS:A total of 279 eligible breast cancer patients enrolled in this study. The most prevalent TCM constitutions were‘Qi-depression',‘Peaceful'and‘Yangdeficiency'.All nine TCM constitutions of breast cancer patients exhibited significant correlations with FCR scores. The strongest positive correlation was observed between‘Qi-stagnation'and FCR(r=0.5576,P<0.0001),followed by‘Qi-deficiency'(r=0.4465,P<0.0001).By contrast, the Peaceful correlation showed a significant negative correlation with FCR(r=-0.363,P<0.0001).Multiple linear regression analysis showed that TCM constitution, lymph node metastasis, and endocrine therapy were influencing factors for FCR in breast cancer patients(P<0.05).Additionally,multivariate logistic regression analysis was performed to identify factors associated with TCM constitution in breast cancer patients. After adjusting for potential confounding factors including age, disease stage, pathological type, immunohistochemical status, lymph node metastasis, disease duration, and treatment modality,FCR,disease stage, age, and surgical modality were found to be significantly associated with TCM constitution(all P<0.05).CONCLUSION:This study demonstrates a significant association between TCM constitution and FCR in breast cancer patients, suggesting that FCR can potentially be interpreted within the theoretical framework of TCM.These findings thus provide a theoretical foundation for TCMC-informed management strategies targeting FCR in breast cancer patients. 展开更多

Ultra-high-strength aluminumalloy profile is an ideal choice for aerospace structuralmaterials due to its excellent specific strength and corrosion resistance. However, issues such as uneven metal flow, stress concentration, and forming defects are prone to occur during their extrusion. This study focuses on an Al-Zn-Mg-Cu ultra-high-strength aluminum alloy profile with a double-U,multi-cavity thin-walled structure.Firstly, hot compression experiments were conducted at temperatures of 350○C,400○C,and 450○C,with strain rates of 0.01 and 1.0 s−1,to investigate the plastic deformation behavior of the material.Subsequently,a 3D coupled thermo-mechanical extrusion simulation model was established using Deform-3D to systematically analyze the influence of die structure and process parameters on metal flow velocity, effective stress/strain,and temperature distribution. The simulation revealed significant velocity differences, stress concentration, and uneven temperature distribution.Key parameters, including mesh density, extrusion ratio, die fillet, and bearing length, were optimized through full-factorial experiments. This optimization, combined with a stepped flow-guiding die design, effectively improved the metal flow pattern during extrusion.Trial production based on both the initial and optimized parameters were carried out.A comparative analysis demonstrates that the optimized scheme results in a final profile whose cross-section matches the target design closely, with complete filling of complex features and no obvious forming defects. This research provides a valuable reference for the extrusion process optimization and die design of complex-section profiles made from ultra-high-strength aluminum alloys. 展开更多

BACKGROUND Cognitive impairment is a well-recognized complication of liver diseases, primarily associated with cirrhotic portal hypertension. However, its spectrum and prevalence in patients with non-cirrhotic portal hypertension due to hepatosplenic schistosomiasis(HSS)remain poorly characterized, particularly regarding domains beyond minimal hepatic encephalopathy(MHE).AIM To characterize cognitive performance and determine the prevalence of deficits across multiple domains in a large cohort of patients with HSS.METHODS This cross-sectional study enrolled 200 adult patients with confirmed HSS at a Brazilian reference center.Participants underwent a comprehensive psychometric battery, including Mini-Mental State Examination for global cognition, the animal verbal fluency test for MHE and semantic fluency, the clock drawing test for visuospatial and executive functions, the digit span test(DST)for attention and working memory, and the Goo-Go test for inhibitory control.Impairment was defined using established,education-adjusted cut-offs.RESULTS The cohort had a mean age of 56.4 years and limited formal education(mean 4.9 years).Global cognitive impairment, defined as a Mini-Mental Examination Score<25,was identified in 36.5%of patients.Domain-specific assessment revealed a high prevalence of deficits in visuospatial and planning abilities, affecting 67%of patients according to the clock drawing test.Working memory impairments were detected in 53.5%(forward DST)and 83%(backward DST)of participants.Abnormal performance on the Go/No-Go test, indicating deficits in inhibitory control, was observed in 66.5%of the sample. The animal verbal fluency test identified MHE in 24.5%of participants,a finding significantly associated with lower educational attainment(P<0.01).Notably,while MHE was strongly linked to the presence of portosystemic shunts(P=0.0018),deficits in other cognitive domains were highly prevalent regardless of portosystemic shunts status.CONCLUSION Cognitive impairment in HSS is highly prevalent and encompasses a broad range of deficits, extending well beyond MHE.These findings support the implementation of routine, multidimensional cognitive screening in standard management of HSS patients. 展开更多

BACKGROUND Accurate estimation of urinary bladder volume(UBV)is important for diagnosing and managing bladder disorders.Computed tomography(CT)is often used as a reference standard;however, existing measurement methods vary in complexity and efficiency.AIM To determine whether UBV can be accurately estimated on CT using a single sagittal measurement compared with multidimensional measurements and reference volumetric analysis.METHODS CT abdomen-pelvis studies of 80 individuals without urinary tract pathology were retrospectively analyzed.Bladder volume was determined using manual volumetric tracing as the reference standard.Sagittal long axis[sagittal length(SL)],sagittal short axis[sagittal short(SS)],and transverse diameter[right-to-left(RL)]were measured, and single-and multidimensional products(SL,SL×SS,SL×SS×RL)were correlated with reference volume using Pearson correlation coefficients.Linear regression models were derived, and agreement was assessed using Bland-Altman analysis.RESULTS SL demonstrated the strongest single-dimension correlation with bladder volume(r=0.92),compared with SS(r=0.87)and RL(r=0.68).Multidimensional measurements showed slightly higher correlations(SL×SS:r=0.98;SL×SS×RL:r=0.99).A simplified linear formula was derived:Bladder volume(mL)=5×SL(mm)-222.Bland-Altman analysis showed negligible bias and all values within the limits of agreement for the single-dimension model.CONCLUSION UBV can be accurately estimated using a single sagittal CT measurement in healthy individuals. This simplified approach provides a rapid and reproducible alternative to multidimensional methods without meaningful loss of accuracy and may serve as a reference standard for validating ultrasound-based bladder volume estimation. 展开更多

Objective The relationship between non-high-density lipoprotein(NHDL)cholesterol to high-density lipoprotein cholesterol(HDL-C)ratio(NHHR)and stoke remains unknown. This study aimed to evaluate the association between the adult NHHR and stroke occurrence in the United States of America(USA).Methods To clarify the relationship between the NHHR and stroke risk, this study used a multivariable logistic regression model and a restricted cubic spline(RCS)model to investigate the association between the NHHR and stroke, and data from the National Health and Nutrition Examination Survey(NHANES)from 2005 to 2018.Subgroup and sensitivity analyses were conducted to test the robustness of the results. Results This study included 29,928 adult participants,of which 1,165 participants had a history of stroke.Logistic regression analysis of variables demonstrated a positive association between NHHR and stroke(OR 1.24,95%CI:1.03-1.50,P=0.026).Compared with the lowest reference group of NHHR,participants in the second, third, and fourth quartile had a significantly increased risk of stroke after full adjustments(OR:1.35,95%CI:1.08-1.69)(OR:1.83,95%CI:1.42-2.36)(OR:2.04,95%CI:1.50-2.79).In the total population,a nonlinear dose-response relationship was observed between the NHHR and stroke risk(P non-linearity=0.002).This association remained significant in several subgroup analyses.Further investigation of the NHHR may enhance our understanding of stroke prevention and treatment. Conclusion Our findings suggest a positive correlation between the NHHR and an increased prevalence of stroke, potentially serving as a novel predictive factor for stroke.Timely intervention and management of the NHHR may effectively mitigate stroke occurrence.Prospective studies are required to validate this association and further explore the underlying biological mechanisms. 展开更多

The downstream region of the Yangtze River in China experiences tidal influences, with most tributary inlets regulated by hydraulic hubs that control water diversion and drainage. The cross-sectional characteristics of these rivers under intermittent diversion and drainage remain uncertain due to various boundary conditions, including tidal fluctuations from the Yangtze River, hydraulic hub operations, and tributary water levels. This study examines the Yanglintang River as a case study, utilizing a two-dimensional water-sediment numerical model to analyze the response of river cross-sections under varying diversion-to-drainage ratios. The findings demonstrate that river cross-sections transform from a regular trapezoidal shape to a pot-bottom configuration, achieving equilibrium within 2 to 3 years under the influence of water diversion and drainage. The operational duration of water diversion and drainage shows minimal impact on river cross-sectional morphology, with bed scouring reaching approximately 10 cm when the operational duration doubles. However, the flow rate of water diversion-drainage operations emerges as the primary factor controlling river cross-sectional morphology, resulting in bed scouring magnitude of approximately 1 m under doubled flow rate conditions. 展开更多

Objective:To study the relationship between qi stagnation constitution and suboptimal health status(SHS)or lifestyle. Methods:From 2012 to 2013,we conducted a cross-sectional survey of 24159 Chinese individuals aged 12-80 years. The qi stagnation constitution was assessed using the Constitution in Chinese Medicine Questionnaire.Health status was evaluated through medical records and the Subhealth Measurement Scale V1.0(SHMS V1.0).Health-promoting lifestyles were measured using the Health-Promoting Lifestyle Profile Ⅱ(HPLP-Ⅱ).Results:Of the 24159 participants,16.1%and 15.2%were classified as“always”and“sometimes”having the qi stagnation constitution, respectively. Those classified as“rarely”having the qi stagnation constitution scored higher on both the HPLP-Ⅱ and SHMS V1.0.The participants classified as“always”having the qi stagnation constitution showed a significant association with SHS or disease compared to other imbalanced constitutions. Those in the“always”category were approximately 21 times more likely to be classified as having SHS(odds ratio[OR]:21.17,95%confidence interval[CI]:15.74-28.45),whereas those in the“sometimes”category were approximately six times more likely(OR:5.89,95%CI:5.04-6.90).Accordingly,the qi stagnation constitution score was significantly associated with the diagnosis of SHS,with an area under the curve of 0.77(P<.001).A score of 18.75 yielded the highest Youden Index(0.407),with a sensitivity of 60.5%and a specificity of 80.3%.Significant associations were observed between health-promoting lifestyles and qi stagnation constitution severity in an ordinal regression analysis(P<.001).Protective factors included stress management(OR:1.59),self-actualization(OR:1.57),and exercise(OR:1.36).In contrast, poorer interpersonal relationships(OR:0.79),greater health responsibilities(OR:0.86),and poorer nutrition(OR:0.91)were associated with increased severity. Conclusion:Modulating the qi stagnation constitution through lifestyle interventions may help prevent the progression of SHS to disease, which aligns with core preventive principles in traditional Chinese medicine. 展开更多

BACKGROUND The triglyceride glucose-body mass index(TyG-BMI)is a novel indicator of insulin resistance(IR).Obstructive sleep apnea(OSA)is a prevalent disorder characterized by recurrent complete or partial collapse of the pharyngeal airway during sleep;however, the relationship between these two conditions remains unexplored. We hypothesized that a higher TyG-BMI is associated with the occurrence of OSA.AIM To assess the association between TyG-BMI and OSA in adults in the United States.METHODS A cross-sectional study was conducted utilizing data from the National Health and Nutrition Examination Surveys spanning from 2005-2008 to 2015-2018.TyGBMI was calculated as Ln[triglyceride(mg/dL)×fasting blood glucose(mg/dL)/2]×BMI.Restricted cubic splines were used to analyze the risk of TyG-BMI and OSA occurrence. To identify potential nonlinear relationships,we combined Cox proportional hazard regression with smooth curve fitting. We also conducted sensitivity and subgroup analyses to verify the robustness of our findings.RESULTS We included 16794 participants in the final analysis.Multivariate regression analysis showed that participants with a higher TyG-BMI had a higher OSA incidence. After adjusting for all covariates,TyG-BMI was positively correlated with the prevalence of OSA(odds ratio:1.28;95%confidence interval:1.17,1.40;P<0.001);no significant nonlinear relationship was observed.Subgroup analysis showed no strong correlation between TyG-BMI and OSA in patients with diabetes. The correlation between TyG-BMI and OSA was influenced by age, sex, smoking status, marital status, hypertensive stratification, and obesity;these subgroups played a moderating role between TyGBMI and OSA.Even after adjusting for all covariates, there was a positive association between TYG-BMI and OSA prevalence.CONCLUSION A higher TyG-BMI index is linked to higher chances of developing OSA.As TyG-BMI is an indicator of IR,managing IR may help reduce the risk of OSA. 展开更多

AIM:To assess the relationship between dietary inflammatory index(DII)and prevalence of glaucoma among individuals aged 40y and above in the United States.METHODS:Participants were drawn from 2 cycles of the National Health and Nutrition Examination Survey(NHANES,2005-2008)for a cross-sectional study.DII was calculated from 24-hour dietary recall questionnaire conducted by experienced researchers and data analyzed in R according to the NHANES user guide,“Stratified Multi-stage Probability Sampling”.The relationship between DII and glaucoma was evaluated by multi-factor logistic regression analysis and the existence of a non-linear association examined by restricted cubic spline(RCS)analysis.RESULTS:A total of 5359 subjects were included and the cross-sectional analysis weighted to represent the US population of 109 million.DII was elevated in glaucoma patients(P<0.001)and smoking and alcohol use contributed to significant differences(P<0.001).DII correlated negatively with Healthy Eating Index(HEI)-2015(Spearman rank correlation coefficient,r=-0.49).RCS analysis showed a linear relationship between DII and glaucoma risk(P of non-linear relationship=0.575).CONCLUSION:An increased DII is strongly associated with high risk of glaucoma and diet-induced inflammation should be controlled to delay glaucoma progression. 展开更多
