Wound dressing management is critical in healthcare,and frequent dressing changes for full-thickness skin wounds can hinder healing.Nanofiber dressings that resemble the extracellular matrix,have gained popularity in ...Wound dressing management is critical in healthcare,and frequent dressing changes for full-thickness skin wounds can hinder healing.Nanofiber dressings that resemble the extracellular matrix,have gained popularity in wound repair,however,it is challenging to explore how to frequently change it without affecting healing processing and avoiding secondary damage.Here,we developed a self-adhesive and detachable nanofiber dressing inspired by Andrias davidianus.Our asymmetric nanofiber dressing exhibits strong adhesion(26 kPa),to the wound at high temperature(approximately 25°C)to the wound surface and can be easily detached(4 kPa)at low temperature(below 8°C),enabling painless dressing changes that minimize secondary injuries.The dressing comprises an outer layer of polylactic acid which provides mechanical property,support,and pollution resistance,with an inner layer of nanofibrous membrane,composed of gelatin and Andrias davidianus skin secretions,which promotes cellular migration,enhances wound healing and possesses inherent antimicrobial properties.Furthermore,the all-natural nanofiber dressings can be prepared on a large scale and offer favorable biocompatibility to meet the basic requirements of wound dressings.These findings demonstrate the potential applicability of our multilayer nanofiber dressing for advancing wound healing practices.展开更多
The unique shock pattern evolution and its corresponding thrust variation of a recently proposed rocket permeable nozzle during an ascent-descent operation are investigated using numerical methods.In the present perme...The unique shock pattern evolution and its corresponding thrust variation of a recently proposed rocket permeable nozzle during an ascent-descent operation are investigated using numerical methods.In the present permeable nozzle flow,the flow pattern and shock system evolution are discussed to demonstrate the flow and hysteresis phenomenon.The formation mechanisms of two featured shocks,permeable shock(PS)and detached shock(DS)are interpreted in two typical scenarios.Accordingly,the interconnection between the nozzle thrust coefficient variation and the shock evolution is clarified.New flow characteristics emerge because of the nozzle permeable section.First,the hysteresis phenomenon manifests through the existence-absence of both the Mach disk and PS/DS.Second,the formation mechanisms of PS and DS differ in low nozzle pressure ratio(NPR,38.49-134.19)and high NPR(134.19-322.00)scenarios.In the low NPR scenario,PS is formed by the core flow deflection due to ambient air inflow,and the DS detachment is formed by a mild pressure gradient between inflow and backflow.In the high NPR scenario,the PS is formed by the boundary layer re-development after the outflow-induced expansion wave,and the DS detachment is formed by the outflow-induced pressure gradient reduction.Third,the hysteresis of thrust coefficient variation is essentially manifested in a low NPR range.Notably,the hysteresis of PS/DS existence resides well in that of the thrust coefficient,suggesting that their existence plays a vital role in varying the nozzle thrust coefficient.展开更多
Shale deformation is crucial in various geological processes.To investigate the effect of deformation on pore characteristics for highly compacted shale,the macro tectonic strain and micro pore structure were analyzed...Shale deformation is crucial in various geological processes.To investigate the effect of deformation on pore characteristics for highly compacted shale,the macro tectonic strain and micro pore structure were analyzed in the deeply buried Wufeng-Longmaxi shale of the Eastern Fold Belt of the Sichuan Basin.Despite similarities in their X-ray diffraction mineral compositions,significant differences in pore properties were observed between the shale samples from weak and strong curved synclines.Pore shapes identified from scanning electron microscopy and the covariation between total organic carbon(TOC)content and porosity suggest that organic pores predominantly contribute to total porosity.However,samples from weak-curved synclines exhibit porosities~2%higher than samples from strongcurved synclines,even with similar TOC contents.Compared to the weak-curved syncline samples,the strong-curved syncline samples show a transformation in organic pore morphology from spherical to elliptical with an increasing aspect ratio and reduced dominant pore size from 1-1000 nm to 1-100 nm due to pore collapse in the organic matrix.The porosities simulated using a syncline bending ductile strain model align well with the measured porosities,indicating that the porosity changes were induced by heterogeneous ductile strain during detachment folding.Strain heterogeneity also triggered tectonic stress heterogeneity,causing clockwise rotation of the principal stress orientation from weak to strong curved synclines.This study improves our understanding of the variations in pore system with syncline deformation strain,providing a theoretical basis for subsequent shale hydrocarbon exploration and resource assessment in complex tectonic zones.展开更多
The movement of three-phase contact line(TPL)has an important effect on the process involving the gas-liquid-solid three-phase system,such as heat and mass transfer,reaction rate and separation efficiency.In this work...The movement of three-phase contact line(TPL)has an important effect on the process involving the gas-liquid-solid three-phase system,such as heat and mass transfer,reaction rate and separation efficiency.In this work,bubble detachment on a flat solid surface was visualized,and the asymmetric retraction behavior of TPL was investigated.Due to the imperfection of solid surfaces,a stick-slip motion of TPL was observed during the retraction process,leading to the asymmetric behavior of TPL.It was found that the lower the detachment speed,the rougher the solid surface and the longer the initial length of the TPL,the more pronounced the TPL asymmetric motion.The dynamic force during bubble detachment was calculated,and the behind mechanism of the asymmetric motion of TPL was revealed.In addition,both the detachment velocity and solid wettability will affect the pinch-off of gas-liquid interface,as well as the volume of trapped bubbles.展开更多
AIM:To investigate the effects of shortening the duration of silicone oil tamponade on retinal structure and function in patients undergoing silicone oil removal(SOR)after surgery for primary rhegmatogenous retinal de...AIM:To investigate the effects of shortening the duration of silicone oil tamponade on retinal structure and function in patients undergoing silicone oil removal(SOR)after surgery for primary rhegmatogenous retinal detachment(RRD).METHODS:A total of 58 eligible patients were enrolled and randomly assigned to two groups based on tamponade duration:the short-term group(30-45d)and the conventional group(≥90d).Comprehensive evaluations were performed before and after SOR,including slitlamp examination,best-corrected visual acuity(BCVA)measurement,intraocular pressure(IOP)testing,optical coherence tomography(OCT),optical coherence tomography angiography(OCTA),microperimetry,electroretinography(ERG),and visual evoked potential(VEP)assessment.RESULTS:A total of 33 patients(23 males and 10 females;33 eyes)were enrolled in the short-term SO tamponade group with mean age of 52.45±9.35y,and 25 patients(15 males and 10 females;25 eyes)were enrolled in the conventional SO tamponade group with mean age of 50.80±12.06y.Compared with the conventional group,the short-term silicone oil tamponade group had a significantly lower incidence of silicone oil emulsification and cataract progression,with no significant difference in retinal reattachment success rate.Structurally,short-term tamponade was associated with increased thickness of the retinal ganglion cell layer(RGCL)in the nasal and superior macular regions and improved recovery of superficial retinal vascular density in these areas.Functionally,the shortterm group showed better BCVA and retinal sensitivity both before and 1mo after SOR;additionally,the P100 amplitude in VEP tests was significantly increased in this group.CONCLUSION:Shortening the duration of silicone oil tamponade effectively reduces damage to retinal structure and function without compromising the success rate of retinal reattachment in patients with primary RRD.展开更多
The South Tibetan detachment system(STDS),a dominant extensional fault system in the Himalayan orogen,separates the Greater Himalayan Sequence from the overlying Tethyan Himalayan Sequence in the north.Along the Dhaul...The South Tibetan detachment system(STDS),a dominant extensional fault system in the Himalayan orogen,separates the Greater Himalayan Sequence from the overlying Tethyan Himalayan Sequence in the north.Along the Dhauliganga Valley,NW Himalaya,folded psammitic gneisses in the immediate footwall of the STDS yield a Neoproterozoic~1075 Ma U–Pb detrital zircon population and are intruded by Cambro-Ordovician granitoids.This is followed by protracted magmatism during the Late Eocene–Middle Miocene.In addition to structural analysis and deformation fabric studies,the timing of activity along the South Tibetan Detachment System can be constrained by U–Pb zircon geochronology ages of pre-,syn-and posttectonically deformed and undeformed leucogranite bodies.The concordant leucogranites inherit the Neoproterozoic zircons from the footwall high-grade gneisses during their partial melting.Late Eocene migmatization and melt intrusion(~38–34 Ma)were coeval with Barrovian metamorphic,followed by Late Oligocene extension-induced exhumation.The U–Pb zircon geochronology of leucogranites within the uppermost Greater Himalayan Sequence show Late Eocene–Early Oligocene(35–23 Ma)magmatism.Evidence of Late Eocene(35.3±1.8 Ma)SW-directed contraction,crustal thickening and associated metamorphism is observed in gneisses exhibiting tight isoclinal folds with NE-dipping axial surfaces.The transition from contractional to extensional tectonics might have likely occurred in the Late Oligocene–Early Miocene(24–21 Ma),followed by extension-induced exhumation.Leucogranite emplacement ceased along the South Tibetan Detachment System during the Middle Miocene(17.77±0.61 Ma)and tectonic activity subsequently migrated southwards.展开更多
AIM:To investigate the potential impact of lipidaemic and clinical factors on the development of proliferative vitreoretinopathy(PVR)following uncomplicated primary rhegmatogenous retinal detachment(RRD)surgery in non...AIM:To investigate the potential impact of lipidaemic and clinical factors on the development of proliferative vitreoretinopathy(PVR)following uncomplicated primary rhegmatogenous retinal detachment(RRD)surgery in nondiabetic individuals.METHODS:This was a retrospective,single-center,case-control study of consecutive patients who underwent primary RRD surgery.The study group comprised 145 patients who developed PVR within 3y of follow-up,while the control group comprised 161 patients with RRD who did not develop PVR.Cox regression analysis was utilized to identify independent associations between various risk markers and the occurrence of PVR.RESULTS:The mean age of patients was 52.31y(SD=13.29),and 54.25%(n=166)were male.The median time to PVR formation after surgery was 150d.Multivariate Cox regression indicated that cigarette smoking status[hazard ratio(HR):0.43,95%confidence interval(CI):0.31-0.60,P<0.001],retinal detachment(RD)not involving the macula(HR:0.52,95%CI:0.37-0.73,P<0.001),apolipoprotein A1(ApoA1;HR:1.01,95%CI:1.01-1.02,P<0.001)and apolipoprotein E(ApoE;HR:3.81,95%CI:1.64-8.85,P=0.002)were independent predictors of PVR.CONCLUSION:Apart from macular involvement and smoking,the lipidaemic factors ApoA1 and ApoE are risk factors of PVR after primary RRD surgery.展开更多
Descemet’s membrane detachment(DMD)is defined as the detachment of Descemet’s membrane that occurs spontaneously or secondary to intraocular surgery.This review focuses on the characteristics and incidence of DMD fo...Descemet’s membrane detachment(DMD)is defined as the detachment of Descemet’s membrane that occurs spontaneously or secondary to intraocular surgery.This review focuses on the characteristics and incidence of DMD following phacoemulsification and glaucoma surgery,and aims to compare DMD cases after phacoemulsification(PCE)and different types of glaucoma surgery in terms of incidence,risk factors,clinical manifestations,diagnosis and management strategies.The reported incidence of DMD after PCE ranges from 0 to 5%,and the complication is less frequently observed following glaucoma surgery.Patients with DMD may be asymptomatic or present with severe visual impairment caused by corneal edema,which is associated with the size and location of the detachment.The management of DMD varies according to the primary surgical procedure(PCE or glaucoma surgery),as well as case-specific factors including visual acuity,corneal clarity,and the size and location of the detachment.Longitudinal observational studies are warranted to investigate the underlying cellular mechanisms of DMD.Retrospective studies can be conducted to clarify the incidence and identify all potential risk factors for DMD following glaucoma surgery.In addition,it is crucial to explore all possible risk factors to reduce and prevent this complication.展开更多
AIM:To investigate the underlying causes of surgical failure and reoperation management in patients with rhegmatogenous retinal detachment(RRD)who underwent scleral buckle surgery at our institution.METHODS:This was a...AIM:To investigate the underlying causes of surgical failure and reoperation management in patients with rhegmatogenous retinal detachment(RRD)who underwent scleral buckle surgery at our institution.METHODS:This was a single-center,retrospective,descriptive study.The clinical data of 368 patients(387 eyes)with RRD who underwent scleral buckling(SB)surgery between August 2013 and July 2023 at our institution were collected.The aim was to analyze the causes of recurrence and the rationale for selecting reoperation methods.RESULTS:Totally 368 patients(387 eyes)were included in the analysis,comprising 222 males and 146 females.The average age was 30.26±14.18 years,and the mean follow-up duration was(48.33±20.39)mo.The success rate of SB surgery was 90.2%.Recurrent retinal detachment occurred in 38 eyes.Based on surgical records,the causes of SB failure were analyzed.The recurrence causes included abnormal compression ridge position(position,height,or width)in 14 eyes(36.8%,14/38),hole omission in 11 eyes(29.0%,11/38),proliferative vitreoretinopathy(PVR)in 10 eyes(26.3%,10/38),and new holes in 3 eyes(7.9%,3/38).Among these,8 eyes(21.1%,8/38)underwent repeat SB surgery,while the remaining 30 eyes(78.9%,30/38)underwent pars plana vitrectomy(PPV).Regarding tamponade agents,silicone oil was used in 11 eyes(36.7%,11/30),C3F8 gas in 12 eyes(40.0%,12/30),and sterile air in 7 eyes(23.3%,7/30).CONCLUSION:SB surgery demonstrates a high success rate in the treatment of RRD.However,abnormal compression ridge position,missed holes during surgery,and PVR are the primary causes of SB failure.After addressing the reasons for failure,re-SB surgery or PPV can be effective alternatives.展开更多

BACKGROUND Inducing posterior vitreous detachment(PVD)is a crucial step in vitreoretinal surgery.However,compared with older patients,induction of PVD in pediatric and young adult populations is considerably more diff...BACKGROUND Inducing posterior vitreous detachment(PVD)is a crucial step in vitreoretinal surgery.However,compared with older patients,induction of PVD in pediatric and young adult populations is considerably more difficult.Recent studies have shown the utility of autologous plasmin enzyme in inducing PVD in pediatric patients.AIM To evaluate the anatomical and functional outcomes of patients with retinal detachment,treated with in vivo generated autologous plasmin,as an adjunct to vitreoretinal surgery.METHODS This is a retrospective observational case series including ten eyes of nine patients with retinal detachment,secondary to various pathologies.All eyes that received simultaneous intravitreal injection of 0.1 mL of autologous blood and 25μg of tissue plasminogen activator(t-PA)3 days before surgery were analyzed.RESULTS The mean age of the cohort was 9.3+/-4.16 years,ranging from 5 years to 16 years.Four out of nine patients were female.Five of the ten eyes had previously undergone vitreoretinal surgery.In the study cohort,five patients had high myopia,two patients had Stickler’s syndrome,one had juvenile X-lined retinoschisis,and one had familial exudative vitreoretinopathy associated with retinal detachment.No eye required repeat vitreoretinal surgery for recurrent retinal detachment or membrane peeling following the primary procedure,with adjunctive intravitreal autologous plasmin the median baseline visual acuity was 2.30 logarithm of the minimum angle of resolution(LogMAR)[interquartile range(IQR):1.85-2.60],and the median final visual acuity was 1.54 LogMAR(IQR:1.08-1.78),showing a statistically significant improvement(P=0.01),with five eyes regaining functional visual acuity.CONCLUSION Adjunctive intravitreal autologous plasmin improves the anatomical outcome of vitreoretinal surgery in pediatric patients.However these findings require validation in a large sample,prospective study.展开更多

●AIM:To identify independent risk factors for postoperative proliferative vitreoretinopathy(PVR)in patients with primary rhegmatogenous retinal detachment(RRD)treated with scleral buckling surgery and to develop a no...●AIM:To identify independent risk factors for postoperative proliferative vitreoretinopathy(PVR)in patients with primary rhegmatogenous retinal detachment(RRD)treated with scleral buckling surgery and to develop a nomogram for predicting postoperative PVR.●METHODS:Patients who underwent scleral buckling surgery for primary RRD were retrospectively enrolled.Patients were randomly assigned to a training cohort(n=515)and a validation cohort(n=55).Candidate variables included demographic characteristics,systemic comorbidities,preoperative ocular status,and retinal break features.Independent predictors of postoperative PVR were identified using univariate and multivariate logistic regression analyses.A nomogram was constructed to predict the risk of PVR at 1,3,and 6mo after surgery.Model performance was evaluated using the concordance index(C-index),receiver operating characteristic(ROC)curves,calibration plots,and decision curve analysis(DCA).●RESULTS:A total of 570 eyes were included,with mean age of 52.65±15.70y,and 51.4%of patients were male.Postoperative PVR occurred in 28.8%of eyes overall,including 28.5%in the training cohort and 30.9%in the validation cohort.Postoperative PVR developed in a subset of patients during follow-up.Preoperative PVR,a history of diabetes mellitus,and more than four retinal breaks were identified as independent risk factors for postoperative PVR.In the training cohort,the C-indices of the nomogram at 1,3,and 6mo were 0.888,0.931,and 0.948,respectively,and 0.885,0.885,and 0.909 in the validation cohort.ROC and calibration analyses demonstrated good discrimination and agreement,while DCA showed favorable net clinical benefit across a wide range of threshold probabilities.●CONCLUSION:Preoperative PVR,diabetes mellitus,and more than four retinal breaks are independent predictors of postoperative PVR after scleral buckling for RRD.The proposed nomogram provides accurate individualized risk prediction at 1,3,and 6mo postoperatively,and may assist clinicians in postoperative surveillance and decision-making.展开更多
This study examined the moderating effects of psychological detachment and conscientiousness on the relationship between compulsory citizenship behaviour(CCB)and work-family conflict(WFC)among nurses.Data were gathere...This study examined the moderating effects of psychological detachment and conscientiousness on the relationship between compulsory citizenship behaviour(CCB)and work-family conflict(WFC)among nurses.Data were gathered from 289 nurses(104 males and 185 females)working in public hospitals in the southwestern part of Nigeria.They completed self-report measures on CCB,WFC,psychological detachment,and conscientiousness.Hayes PROCESS Macro results showed that CCB was associated with higher WFC among nurses.Psychological detachment(β=−0.05,p<0.006)and conscientiousness(β=−0.02,p<0.003)significantly moderated the relationship between CCB and WFC,such that higher levels of these factors mitigated the negative impact of CCB on WFC.These findings are consistent with the conservation of resources theory,as they indicate that the negative effects of resource-depleting demands(CCB)are attenuated by the availability of additional resources,namely,psychological detachment and conscientiousness.Interventions aimed at fostering psychological detachment and conscientious work habits,such as time management training or goal-setting workshops,could be strategically implemented to enhance this protective trait among nurses.展开更多
●AIM:To investigate the genetic mutation profiles of generelated retinal detachment(RD)and evaluate the utility of The American College of Medical Genetics and Genomics/Association for Molecular Pathology(ACMG/AMP)pa...●AIM:To investigate the genetic mutation profiles of generelated retinal detachment(RD)and evaluate the utility of The American College of Medical Genetics and Genomics/Association for Molecular Pathology(ACMG/AMP)pathogenicity classification system with emphasis on variants of uncertain significance(VUS)in a young Chinese cohort.●METHODS:A consecutive cohort of 28 patients under 30y with RD and clinical features suggesting genetic etiology was enrolled between September 2024 and August 2025 at Zhongshan Ophthalmic Center.All patients underwent comprehensive ophthalmic examinations,genetic testing,and surgical repair.Genetic variants were interpreted via the ACMG/AMP criteria.●RESULTS:The cohort consisted of 21 males and 7 females,with a mean age of 15.1±6.71y(ranged:4–29).The predominant etiologies were Stickler syndrome(10/28,35.7%),familial exudative vitreoretinopathy(FEVR;6/28,21.4%),and Marfan syndrome(4/28,14.3%).A total of 30 disease-associated variants were identified,among which 60.0%(18/30)were classified as pathogenic/likely pathogenic(P/LP)and 40.0%(12/30)as VUS per ACMG/AMP criteria.The primary factors contributing to VUS classification included high population allele frequency(33.3%),variant novelty(33.3%),and discrepant in silico predictions(25.0%).Patients with P/LP variants exhibited a significantly higher prevalence of high myopia(<-6 D;93.8%vs 50.0%,P=0.027)and tessellated fundus(87.5%vs 50.0%,P=0.044)compared to those with VUSot available(NA)variants.●CONCLUSION:Stickler syndrome,FEVR,and Marfan syndrome are the leading causes of gene-related RD in the Chinese Han cohort.A high VUS rate(40.0%)poses diagnostic challenges,primarily due to population-specific frequency differences,novel variants,and insufficient functional evidence.By integrating clinical history,phenotypic manifestations,and family history,a clear diagnosis can be established in 66.7%of VUS cases.Ethnically tailored genomic databases and expanded multicenter cohorts are needed to improve VUS resolution and enhance the clinical utility of genetic testing in young RD individuals.展开更多
AIM:To evaluate the surgical outcomes of the perfluorocarbon liquid(PFCL)-assisted inverted multilayer internal limiting membrane(ILM)flaps covering technique in macular hole retinal detachment(MHRD)in high myopia wit...AIM:To evaluate the surgical outcomes of the perfluorocarbon liquid(PFCL)-assisted inverted multilayer internal limiting membrane(ILM)flaps covering technique in macular hole retinal detachment(MHRD)in high myopia with axial length(AL)≥30 mm.METHODS:In this retrospective,interventional,consecutive comparative study,44 MHRD eyes were divided into two groups:the PFCL-assisted inverted multilayer ILM flaps covering technique group(Group 1,21 eyes)and the ILM peeling group(Group 2,23 eyes).The follow-up period was>12mo.Postoperative outcomes,including retinal reattachment,macular hole(MH)closure,and bestcorrected visual acuity(BCVA),were assessed.Statistical analysis using the Mann–Whitney U test and Fisher’s exact test was conducted to compare differences between groups.RESULTS:There were no statistically significant differences in baseline preoperative clinical characteristics,including age,sex,AL,diopters,duration of symptom,lens status,posterior staphyloma presence and extent of RD.Retinal reattachment rates were higher in Group 1(90.5%)than in Group 2(82.6%),without statistical significance(P=0.667).MH closure rates were significantly higher in Group 1(85.7%)than in Group 2(17.4%;P<0.001).The Group-1 BCVA(logMAR)improved significantly from 2.13±0.91 preoperatively to 1.21±0.66 postoperatively(P=0.026).The Group 2 BCVA improved significantly from 1.91±0.53 preoperatively to 1.19±0.41 postoperatively(P=0.032).However,there were no significant differences in visual-acuity improvement between groups(P=0.460).CONCLUSION:This technique offers a more effective approach for improving MH closure rates and postoperative visual function in MHRD with AL≥30 mm in high myopia.展开更多
For deep prospects in the foreland thrust belt,southern Junggar Basin,NW China,there are uncertainties in factors controlling the structural deformation,distribution of paleo-structures and detachment layers,and distr...For deep prospects in the foreland thrust belt,southern Junggar Basin,NW China,there are uncertainties in factors controlling the structural deformation,distribution of paleo-structures and detachment layers,and distribution of major hydrocarbon source rocks.Based on the latest 3D seismic,gravity-magnetic,and drilling data,together with the results of previous structural physical simulation and discrete element numerical simulation experiments,the spatial distribution of pre-existing paleo-structures and detachment layers in deep strata of southern Junggar Basin were systematically characterized,the structural deformation characteristics and formation mechanisms were analyzed,the distribution patterns of multiple hydrocarbon source rock suites were clarified,and hydrocarbon accumulation features in key zones were reassessed.The exploration targets in deep lower assemblages with possibility of breakthrough were expected.Key results are obtained in three aspects.First,structural deformation is controlled by two-stage paleo-structures and three detachment layers with distinct lateral variations:the Jurassic layer(moderate thickness,wide distribution),the Cretaceous layer(thickest but weak detachment),and the Paleogene layer(thin but long-distance lateral thrusting).Accordingly,a four-layer composite deformation sequence was identified,and the structural genetic model with paleo-bulge controlling zonation by segments laterally and multiple detachment layers controlling sequence vertically.Second,the Permian source rocks show a distribution pattern with narrow trough(west),multiple sags(central),and broad basin(east),which is depicted by combining high-precision gravity-magnetic data and time-frequency electromagnetic data for the first time,and the Jurassic source rocks feature thicker mudstones in the west and rich coals in the east according to the reassessment.Third,two petroleum systems and a four-layer composite hydrocarbon accumulation model are established depending on the structural deformation strength,trap effectiveness and source-trap configuration.The southern Junggar Basin is divided into three segments with ten zones,and a hierarchical exploration strategy is proposed for deep lower assemblages in this region,that is,focusing on five priority zones,expanding to three potential areas,and challenging two high-risk targets.展开更多

摘要Wound dressing management is critical in healthcare,and frequent dressing changes for full-thickness skin wounds can hinder healing.Nanofiber dressings that resemble the extracellular matrix,have gained popularity in wound repair,however,it is challenging to explore how to frequently change it without affecting healing processing and avoiding secondary damage.Here,we developed a self-adhesive and detachable nanofiber dressing inspired by Andrias davidianus.Our asymmetric nanofiber dressing exhibits strong adhesion(26 kPa),to the wound at high temperature(approximately 25°C)to the wound surface and can be easily detached(4 kPa)at low temperature(below 8°C),enabling painless dressing changes that minimize secondary injuries.The dressing comprises an outer layer of polylactic acid which provides mechanical property,support,and pollution resistance,with an inner layer of nanofibrous membrane,composed of gelatin and Andrias davidianus skin secretions,which promotes cellular migration,enhances wound healing and possesses inherent antimicrobial properties.Furthermore,the all-natural nanofiber dressings can be prepared on a large scale and offer favorable biocompatibility to meet the basic requirements of wound dressings.These findings demonstrate the potential applicability of our multilayer nanofiber dressing for advancing wound healing practices.

摘要The unique shock pattern evolution and its corresponding thrust variation of a recently proposed rocket permeable nozzle during an ascent-descent operation are investigated using numerical methods.In the present permeable nozzle flow,the flow pattern and shock system evolution are discussed to demonstrate the flow and hysteresis phenomenon.The formation mechanisms of two featured shocks,permeable shock(PS)and detached shock(DS)are interpreted in two typical scenarios.Accordingly,the interconnection between the nozzle thrust coefficient variation and the shock evolution is clarified.New flow characteristics emerge because of the nozzle permeable section.First,the hysteresis phenomenon manifests through the existence-absence of both the Mach disk and PS/DS.Second,the formation mechanisms of PS and DS differ in low nozzle pressure ratio(NPR,38.49-134.19)and high NPR(134.19-322.00)scenarios.In the low NPR scenario,PS is formed by the core flow deflection due to ambient air inflow,and the DS detachment is formed by a mild pressure gradient between inflow and backflow.In the high NPR scenario,the PS is formed by the boundary layer re-development after the outflow-induced expansion wave,and the DS detachment is formed by the outflow-induced pressure gradient reduction.Third,the hysteresis of thrust coefficient variation is essentially manifested in a low NPR range.Notably,the hysteresis of PS/DS existence resides well in that of the thrust coefficient,suggesting that their existence plays a vital role in varying the nozzle thrust coefficient.

摘要Shale deformation is crucial in various geological processes.To investigate the effect of deformation on pore characteristics for highly compacted shale,the macro tectonic strain and micro pore structure were analyzed in the deeply buried Wufeng-Longmaxi shale of the Eastern Fold Belt of the Sichuan Basin.Despite similarities in their X-ray diffraction mineral compositions,significant differences in pore properties were observed between the shale samples from weak and strong curved synclines.Pore shapes identified from scanning electron microscopy and the covariation between total organic carbon(TOC)content and porosity suggest that organic pores predominantly contribute to total porosity.However,samples from weak-curved synclines exhibit porosities~2%higher than samples from strongcurved synclines,even with similar TOC contents.Compared to the weak-curved syncline samples,the strong-curved syncline samples show a transformation in organic pore morphology from spherical to elliptical with an increasing aspect ratio and reduced dominant pore size from 1-1000 nm to 1-100 nm due to pore collapse in the organic matrix.The porosities simulated using a syncline bending ductile strain model align well with the measured porosities,indicating that the porosity changes were induced by heterogeneous ductile strain during detachment folding.Strain heterogeneity also triggered tectonic stress heterogeneity,causing clockwise rotation of the principal stress orientation from weak to strong curved synclines.This study improves our understanding of the variations in pore system with syncline deformation strain,providing a theoretical basis for subsequent shale hydrocarbon exploration and resource assessment in complex tectonic zones.

摘要The movement of three-phase contact line(TPL)has an important effect on the process involving the gas-liquid-solid three-phase system,such as heat and mass transfer,reaction rate and separation efficiency.In this work,bubble detachment on a flat solid surface was visualized,and the asymmetric retraction behavior of TPL was investigated.Due to the imperfection of solid surfaces,a stick-slip motion of TPL was observed during the retraction process,leading to the asymmetric behavior of TPL.It was found that the lower the detachment speed,the rougher the solid surface and the longer the initial length of the TPL,the more pronounced the TPL asymmetric motion.The dynamic force during bubble detachment was calculated,and the behind mechanism of the asymmetric motion of TPL was revealed.In addition,both the detachment velocity and solid wettability will affect the pinch-off of gas-liquid interface,as well as the volume of trapped bubbles.

摘要AIM:To investigate the effects of shortening the duration of silicone oil tamponade on retinal structure and function in patients undergoing silicone oil removal(SOR)after surgery for primary rhegmatogenous retinal detachment(RRD).METHODS:A total of 58 eligible patients were enrolled and randomly assigned to two groups based on tamponade duration:the short-term group(30-45d)and the conventional group(≥90d).Comprehensive evaluations were performed before and after SOR,including slitlamp examination,best-corrected visual acuity(BCVA)measurement,intraocular pressure(IOP)testing,optical coherence tomography(OCT),optical coherence tomography angiography(OCTA),microperimetry,electroretinography(ERG),and visual evoked potential(VEP)assessment.RESULTS:A total of 33 patients(23 males and 10 females;33 eyes)were enrolled in the short-term SO tamponade group with mean age of 52.45±9.35y,and 25 patients(15 males and 10 females;25 eyes)were enrolled in the conventional SO tamponade group with mean age of 50.80±12.06y.Compared with the conventional group,the short-term silicone oil tamponade group had a significantly lower incidence of silicone oil emulsification and cataract progression,with no significant difference in retinal reattachment success rate.Structurally,short-term tamponade was associated with increased thickness of the retinal ganglion cell layer(RGCL)in the nasal and superior macular regions and improved recovery of superficial retinal vascular density in these areas.Functionally,the shortterm group showed better BCVA and retinal sensitivity both before and 1mo after SOR;additionally,the P100 amplitude in VEP tests was significantly increased in this group.CONCLUSION:Shortening the duration of silicone oil tamponade effectively reduces damage to retinal structure and function without compromising the success rate of retinal reattachment in patients with primary RRD.

摘要The South Tibetan detachment system(STDS),a dominant extensional fault system in the Himalayan orogen,separates the Greater Himalayan Sequence from the overlying Tethyan Himalayan Sequence in the north.Along the Dhauliganga Valley,NW Himalaya,folded psammitic gneisses in the immediate footwall of the STDS yield a Neoproterozoic~1075 Ma U–Pb detrital zircon population and are intruded by Cambro-Ordovician granitoids.This is followed by protracted magmatism during the Late Eocene–Middle Miocene.In addition to structural analysis and deformation fabric studies,the timing of activity along the South Tibetan Detachment System can be constrained by U–Pb zircon geochronology ages of pre-,syn-and posttectonically deformed and undeformed leucogranite bodies.The concordant leucogranites inherit the Neoproterozoic zircons from the footwall high-grade gneisses during their partial melting.Late Eocene migmatization and melt intrusion(~38–34 Ma)were coeval with Barrovian metamorphic,followed by Late Oligocene extension-induced exhumation.The U–Pb zircon geochronology of leucogranites within the uppermost Greater Himalayan Sequence show Late Eocene–Early Oligocene(35–23 Ma)magmatism.Evidence of Late Eocene(35.3±1.8 Ma)SW-directed contraction,crustal thickening and associated metamorphism is observed in gneisses exhibiting tight isoclinal folds with NE-dipping axial surfaces.The transition from contractional to extensional tectonics might have likely occurred in the Late Oligocene–Early Miocene(24–21 Ma),followed by extension-induced exhumation.Leucogranite emplacement ceased along the South Tibetan Detachment System during the Middle Miocene(17.77±0.61 Ma)and tectonic activity subsequently migrated southwards.

摘要AIM:To investigate the potential impact of lipidaemic and clinical factors on the development of proliferative vitreoretinopathy(PVR)following uncomplicated primary rhegmatogenous retinal detachment(RRD)surgery in nondiabetic individuals.METHODS:This was a retrospective,single-center,case-control study of consecutive patients who underwent primary RRD surgery.The study group comprised 145 patients who developed PVR within 3y of follow-up,while the control group comprised 161 patients with RRD who did not develop PVR.Cox regression analysis was utilized to identify independent associations between various risk markers and the occurrence of PVR.RESULTS:The mean age of patients was 52.31y(SD=13.29),and 54.25%(n=166)were male.The median time to PVR formation after surgery was 150d.Multivariate Cox regression indicated that cigarette smoking status[hazard ratio(HR):0.43,95%confidence interval(CI):0.31-0.60,P<0.001],retinal detachment(RD)not involving the macula(HR:0.52,95%CI:0.37-0.73,P<0.001),apolipoprotein A1(ApoA1;HR:1.01,95%CI:1.01-1.02,P<0.001)and apolipoprotein E(ApoE;HR:3.81,95%CI:1.64-8.85,P=0.002)were independent predictors of PVR.CONCLUSION:Apart from macular involvement and smoking,the lipidaemic factors ApoA1 and ApoE are risk factors of PVR after primary RRD surgery.
摘要Descemet’s membrane detachment(DMD)is defined as the detachment of Descemet’s membrane that occurs spontaneously or secondary to intraocular surgery.This review focuses on the characteristics and incidence of DMD following phacoemulsification and glaucoma surgery,and aims to compare DMD cases after phacoemulsification(PCE)and different types of glaucoma surgery in terms of incidence,risk factors,clinical manifestations,diagnosis and management strategies.The reported incidence of DMD after PCE ranges from 0 to 5%,and the complication is less frequently observed following glaucoma surgery.Patients with DMD may be asymptomatic or present with severe visual impairment caused by corneal edema,which is associated with the size and location of the detachment.The management of DMD varies according to the primary surgical procedure(PCE or glaucoma surgery),as well as case-specific factors including visual acuity,corneal clarity,and the size and location of the detachment.Longitudinal observational studies are warranted to investigate the underlying cellular mechanisms of DMD.Retrospective studies can be conducted to clarify the incidence and identify all potential risk factors for DMD following glaucoma surgery.In addition,it is crucial to explore all possible risk factors to reduce and prevent this complication.
摘要AIM:To investigate the underlying causes of surgical failure and reoperation management in patients with rhegmatogenous retinal detachment(RRD)who underwent scleral buckle surgery at our institution.METHODS:This was a single-center,retrospective,descriptive study.The clinical data of 368 patients(387 eyes)with RRD who underwent scleral buckling(SB)surgery between August 2013 and July 2023 at our institution were collected.The aim was to analyze the causes of recurrence and the rationale for selecting reoperation methods.RESULTS:Totally 368 patients(387 eyes)were included in the analysis,comprising 222 males and 146 females.The average age was 30.26±14.18 years,and the mean follow-up duration was(48.33±20.39)mo.The success rate of SB surgery was 90.2%.Recurrent retinal detachment occurred in 38 eyes.Based on surgical records,the causes of SB failure were analyzed.The recurrence causes included abnormal compression ridge position(position,height,or width)in 14 eyes(36.8%,14/38),hole omission in 11 eyes(29.0%,11/38),proliferative vitreoretinopathy(PVR)in 10 eyes(26.3%,10/38),and new holes in 3 eyes(7.9%,3/38).Among these,8 eyes(21.1%,8/38)underwent repeat SB surgery,while the remaining 30 eyes(78.9%,30/38)underwent pars plana vitrectomy(PPV).Regarding tamponade agents,silicone oil was used in 11 eyes(36.7%,11/30),C3F8 gas in 12 eyes(40.0%,12/30),and sterile air in 7 eyes(23.3%,7/30).CONCLUSION:SB surgery demonstrates a high success rate in the treatment of RRD.However,abnormal compression ridge position,missed holes during surgery,and PVR are the primary causes of SB failure.After addressing the reasons for failure,re-SB surgery or PPV can be effective alternatives.
摘要Rhegmatogenous retinal detachment(RRD)is a serious ocular condition marked by the separation of the neuroretina from the retinal pigment epithelium(RPE).The pathogenesis of RRD involves intricate molecular and cellular mechanisms,including inflammation,cell migration,and the activation of proliferative signaling pathways.One of the most challenging complications of RRD is proliferative vitreoretinopathy(PVR),which refers to the proliferation and contraction of fibrocellular membranes on the retinal surface and in the vitreous cavity.PVR is a major cause of surgical failure in RRD,as it can lead to recurrent retinal detachment and severe vision loss.However,the pathogenesis of PVR is not yet fully understood,and the treatment options are quite limited.Recent advances in analytical techniques have offered valuable insights into the molecular alterations present in the subretinal fluid(SRF)of patients with RRD.This review seeks to consolidate the current knowledge regarding the SRF profile in RRD and PVR,emphasizing potential biomarkers and therapeutic targets.
摘要Dear Editor,We report a case of a patient with isolated left abducens nerve palsy and bilateral resolving serous retinal detachment(SRD)following a recent pregnancy complicated by severe pre-eclampsia.Extraocular motility testing revealed an abduction deficit in the left eye and 25 prism diopters of esotropia.Patching of the left eye was done to relieve the diplopia and the patient was observed.Over subsequent months,the patient showed gradual improvement.
摘要BACKGROUND Inducing posterior vitreous detachment(PVD)is a crucial step in vitreoretinal surgery.However,compared with older patients,induction of PVD in pediatric and young adult populations is considerably more difficult.Recent studies have shown the utility of autologous plasmin enzyme in inducing PVD in pediatric patients.AIM To evaluate the anatomical and functional outcomes of patients with retinal detachment,treated with in vivo generated autologous plasmin,as an adjunct to vitreoretinal surgery.METHODS This is a retrospective observational case series including ten eyes of nine patients with retinal detachment,secondary to various pathologies.All eyes that received simultaneous intravitreal injection of 0.1 mL of autologous blood and 25μg of tissue plasminogen activator(t-PA)3 days before surgery were analyzed.RESULTS The mean age of the cohort was 9.3+/-4.16 years,ranging from 5 years to 16 years.Four out of nine patients were female.Five of the ten eyes had previously undergone vitreoretinal surgery.In the study cohort,five patients had high myopia,two patients had Stickler’s syndrome,one had juvenile X-lined retinoschisis,and one had familial exudative vitreoretinopathy associated with retinal detachment.No eye required repeat vitreoretinal surgery for recurrent retinal detachment or membrane peeling following the primary procedure,with adjunctive intravitreal autologous plasmin the median baseline visual acuity was 2.30 logarithm of the minimum angle of resolution(LogMAR)[interquartile range(IQR):1.85-2.60],and the median final visual acuity was 1.54 LogMAR(IQR:1.08-1.78),showing a statistically significant improvement(P=0.01),with five eyes regaining functional visual acuity.CONCLUSION Adjunctive intravitreal autologous plasmin improves the anatomical outcome of vitreoretinal surgery in pediatric patients.However these findings require validation in a large sample,prospective study.
摘要Dear Editor,Paediatric bilateral panuveitis is an uncommon but sightthreatening condition that requires prompt recognition and aggressive management.Bacillary layer detachment(BLD),increasingly[1]recognized on optical coherence tomography(OCT),reflects disruption within the photoreceptor layer.Although frequently reported in acute Vogt-Koyanagi-Harada(VKH)disease[2],BLD is not pathognomonic and may occur in other inflammatory or infectious disorders.We present a pediatric case of bilateral panuveitis with BLD,illustrating the diagnostic challenges and highlighting the importance of early systemic immunosuppressive therapy[3].

摘要0 INTRODUCTION Due to the sudden and highly destructive nature of slope rock collapse,developing effective early warning systems has become an urgent challenge in geotechnical engineering(Cai and Detournay,2024;Loew et al.,2017).Traditional monitoring methods primarily target the acceleration stage preceding disasters(such as displacement monitoring for landslides and debris flows),which is effective for early warning of plastic collapse disasters but often inadequate for brittle failure modes(Walter et al.,2019;Chao et al.,2018;Crosta et al.,2017).
摘要●AIM:To identify independent risk factors for postoperative proliferative vitreoretinopathy(PVR)in patients with primary rhegmatogenous retinal detachment(RRD)treated with scleral buckling surgery and to develop a nomogram for predicting postoperative PVR.●METHODS:Patients who underwent scleral buckling surgery for primary RRD were retrospectively enrolled.Patients were randomly assigned to a training cohort(n=515)and a validation cohort(n=55).Candidate variables included demographic characteristics,systemic comorbidities,preoperative ocular status,and retinal break features.Independent predictors of postoperative PVR were identified using univariate and multivariate logistic regression analyses.A nomogram was constructed to predict the risk of PVR at 1,3,and 6mo after surgery.Model performance was evaluated using the concordance index(C-index),receiver operating characteristic(ROC)curves,calibration plots,and decision curve analysis(DCA).●RESULTS:A total of 570 eyes were included,with mean age of 52.65±15.70y,and 51.4%of patients were male.Postoperative PVR occurred in 28.8%of eyes overall,including 28.5%in the training cohort and 30.9%in the validation cohort.Postoperative PVR developed in a subset of patients during follow-up.Preoperative PVR,a history of diabetes mellitus,and more than four retinal breaks were identified as independent risk factors for postoperative PVR.In the training cohort,the C-indices of the nomogram at 1,3,and 6mo were 0.888,0.931,and 0.948,respectively,and 0.885,0.885,and 0.909 in the validation cohort.ROC and calibration analyses demonstrated good discrimination and agreement,while DCA showed favorable net clinical benefit across a wide range of threshold probabilities.●CONCLUSION:Preoperative PVR,diabetes mellitus,and more than four retinal breaks are independent predictors of postoperative PVR after scleral buckling for RRD.The proposed nomogram provides accurate individualized risk prediction at 1,3,and 6mo postoperatively,and may assist clinicians in postoperative surveillance and decision-making.
摘要This study examined the moderating effects of psychological detachment and conscientiousness on the relationship between compulsory citizenship behaviour(CCB)and work-family conflict(WFC)among nurses.Data were gathered from 289 nurses(104 males and 185 females)working in public hospitals in the southwestern part of Nigeria.They completed self-report measures on CCB,WFC,psychological detachment,and conscientiousness.Hayes PROCESS Macro results showed that CCB was associated with higher WFC among nurses.Psychological detachment(β=−0.05,p<0.006)and conscientiousness(β=−0.02,p<0.003)significantly moderated the relationship between CCB and WFC,such that higher levels of these factors mitigated the negative impact of CCB on WFC.These findings are consistent with the conservation of resources theory,as they indicate that the negative effects of resource-depleting demands(CCB)are attenuated by the availability of additional resources,namely,psychological detachment and conscientiousness.Interventions aimed at fostering psychological detachment and conscientious work habits,such as time management training or goal-setting workshops,could be strategically implemented to enhance this protective trait among nurses.

摘要●AIM:To investigate the genetic mutation profiles of generelated retinal detachment(RD)and evaluate the utility of The American College of Medical Genetics and Genomics/Association for Molecular Pathology(ACMG/AMP)pathogenicity classification system with emphasis on variants of uncertain significance(VUS)in a young Chinese cohort.●METHODS:A consecutive cohort of 28 patients under 30y with RD and clinical features suggesting genetic etiology was enrolled between September 2024 and August 2025 at Zhongshan Ophthalmic Center.All patients underwent comprehensive ophthalmic examinations,genetic testing,and surgical repair.Genetic variants were interpreted via the ACMG/AMP criteria.●RESULTS:The cohort consisted of 21 males and 7 females,with a mean age of 15.1±6.71y(ranged:4–29).The predominant etiologies were Stickler syndrome(10/28,35.7%),familial exudative vitreoretinopathy(FEVR;6/28,21.4%),and Marfan syndrome(4/28,14.3%).A total of 30 disease-associated variants were identified,among which 60.0%(18/30)were classified as pathogenic/likely pathogenic(P/LP)and 40.0%(12/30)as VUS per ACMG/AMP criteria.The primary factors contributing to VUS classification included high population allele frequency(33.3%),variant novelty(33.3%),and discrepant in silico predictions(25.0%).Patients with P/LP variants exhibited a significantly higher prevalence of high myopia(<-6 D;93.8%vs 50.0%,P=0.027)and tessellated fundus(87.5%vs 50.0%,P=0.044)compared to those with VUSot available(NA)variants.●CONCLUSION:Stickler syndrome,FEVR,and Marfan syndrome are the leading causes of gene-related RD in the Chinese Han cohort.A high VUS rate(40.0%)poses diagnostic challenges,primarily due to population-specific frequency differences,novel variants,and insufficient functional evidence.By integrating clinical history,phenotypic manifestations,and family history,a clear diagnosis can be established in 66.7%of VUS cases.Ethnically tailored genomic databases and expanded multicenter cohorts are needed to improve VUS resolution and enhance the clinical utility of genetic testing in young RD individuals.

摘要AIM:To evaluate the surgical outcomes of the perfluorocarbon liquid(PFCL)-assisted inverted multilayer internal limiting membrane(ILM)flaps covering technique in macular hole retinal detachment(MHRD)in high myopia with axial length(AL)≥30 mm.METHODS:In this retrospective,interventional,consecutive comparative study,44 MHRD eyes were divided into two groups:the PFCL-assisted inverted multilayer ILM flaps covering technique group(Group 1,21 eyes)and the ILM peeling group(Group 2,23 eyes).The follow-up period was>12mo.Postoperative outcomes,including retinal reattachment,macular hole(MH)closure,and bestcorrected visual acuity(BCVA),were assessed.Statistical analysis using the Mann–Whitney U test and Fisher’s exact test was conducted to compare differences between groups.RESULTS:There were no statistically significant differences in baseline preoperative clinical characteristics,including age,sex,AL,diopters,duration of symptom,lens status,posterior staphyloma presence and extent of RD.Retinal reattachment rates were higher in Group 1(90.5%)than in Group 2(82.6%),without statistical significance(P=0.667).MH closure rates were significantly higher in Group 1(85.7%)than in Group 2(17.4%;P<0.001).The Group-1 BCVA(logMAR)improved significantly from 2.13±0.91 preoperatively to 1.21±0.66 postoperatively(P=0.026).The Group 2 BCVA improved significantly from 1.91±0.53 preoperatively to 1.19±0.41 postoperatively(P=0.032).However,there were no significant differences in visual-acuity improvement between groups(P=0.460).CONCLUSION:This technique offers a more effective approach for improving MH closure rates and postoperative visual function in MHRD with AL≥30 mm in high myopia.

摘要For deep prospects in the foreland thrust belt,southern Junggar Basin,NW China,there are uncertainties in factors controlling the structural deformation,distribution of paleo-structures and detachment layers,and distribution of major hydrocarbon source rocks.Based on the latest 3D seismic,gravity-magnetic,and drilling data,together with the results of previous structural physical simulation and discrete element numerical simulation experiments,the spatial distribution of pre-existing paleo-structures and detachment layers in deep strata of southern Junggar Basin were systematically characterized,the structural deformation characteristics and formation mechanisms were analyzed,the distribution patterns of multiple hydrocarbon source rock suites were clarified,and hydrocarbon accumulation features in key zones were reassessed.The exploration targets in deep lower assemblages with possibility of breakthrough were expected.Key results are obtained in three aspects.First,structural deformation is controlled by two-stage paleo-structures and three detachment layers with distinct lateral variations:the Jurassic layer(moderate thickness,wide distribution),the Cretaceous layer(thickest but weak detachment),and the Paleogene layer(thin but long-distance lateral thrusting).Accordingly,a four-layer composite deformation sequence was identified,and the structural genetic model with paleo-bulge controlling zonation by segments laterally and multiple detachment layers controlling sequence vertically.Second,the Permian source rocks show a distribution pattern with narrow trough(west),multiple sags(central),and broad basin(east),which is depicted by combining high-precision gravity-magnetic data and time-frequency electromagnetic data for the first time,and the Jurassic source rocks feature thicker mudstones in the west and rich coals in the east according to the reassessment.Third,two petroleum systems and a four-layer composite hydrocarbon accumulation model are established depending on the structural deformation strength,trap effectiveness and source-trap configuration.The southern Junggar Basin is divided into three segments with ten zones,and a hierarchical exploration strategy is proposed for deep lower assemblages in this region,that is,focusing on five priority zones,expanding to three potential areas,and challenging two high-risk targets.
