Controls were randomly selected from the Danish Civil Registration System(CRS) and matched for sex, age, and municipality. We used ICD 10 codes to identify the diagnoses of the included patients. The IBD population was divided into three subgroups: Ulcerative colitis(UC),Crohn's disease(CD) and Both the latter referring to those registered with both diagnoses. Subsequently,odds-ratios(OR) and 95%CI were obtained separately for each group and their respective controls. The use of Bonferoni post-test correction adjusted the significance level to P < 0.00125. P-values were estimated using Fisher's exact test.RESULTS There were significantly more women than men in the registry, and a greater percentage of comorbidity in the IBD groups(P < 0.05). Twenty different IMDs were all significantly more frequent in the IBD group. Sixteen were associated with UC versus twelve with CD. In both UC and CD ORs were significantly increased(P < 0.00125) for primary sclerosing cholangitis(PSC),celiac disease, type 1 diabetes(T1D),sarcoidosis,asthma,iridocyclitis,psoriasis,pyoderma gangrenosum, rheumatoid arthritis, and ankylosing spondylitis. Restricted to UC(P < 0.00125) were autoimmune hepatitis, primary biliary cholangitis,Grave's disease, polymyalgia rheumatica, temporal arteritis, and atrophic gastritis. Restricted to CD(P < 0.00125) were psoriatic arthritis and episcleritis. Restricted to women with UC(P < 0.00125) were atrophic gastritis, rheumatoid arthritis, temporal arteritis, and polymyalgia rheumatica. Restricted to women with CD were episcleritis, rheumatoid arthritis, and psoriatic arthritis. The only disease restricted to men(P < 0.00125) was sarcoidosis. CONCLUSION Immune mediated diseases were significantly more frequent in patients with IBD. Our results strengthen the hypothesis that some IMDs and IBD may have overlapping pathogenic pathways.展开更多

The proposed pathogenesis of TNF-α associated neuropathies involves immune-mediated attack to blood-nerve barrier, aggravated production of pro-inflammatory cytokines, and the induction of Schwann cells apoptosis. TNF-α may play a regulatory role by increasing production of interleukin-1 in macrophages, attenuating T cell receptor signaling and regulating apoptosis of potentially autoreactive T cells in EAN. The data suggest that antagonizing TNF-α functions or suppressing TNF-α production may be useful in the acute phase of EAN treatment, but further studies are required.展开更多

Methods: 120 subjects were included in this study, all of whom belonged to rheumatic immune diseases. The patients were randomly divided into two groups by computer. One group was the reference group, which was divided into 60 patients, and the other group was the experimental group, which was equally divided into 60 patients. The reference group adopted the conventional nursing mode, while the experimental group adopted the refined management mode, and compared the application effects obtained under different nursing modes. Results: the improvement of psychological state and nursing satisfaction of the experimental group were significantly better than those of the reference group (P < 0.05). Conclusion: refined management intervention on rheumatic immune diseases can promote the improvement of patients' psychological state, actively cooperate with doctors for treatment, improve patients' satisfaction with nursing care, and build a harmonious nurse-patient relationship, which is worthy of recommendation.
摘要Autoimmune hepatitis(AIH)is an inflammatory liver disease influenced by genetic, environmental, and immunologic factors.Individuals diagnosed with AIH may exhibit concurrent autoimmune manifestations affecting multiple organ systems. The prevalence of AIH associated with other autoimmune diseases has been reported to range from 20%to 40%.This review indicates that the associations between AIH and autoimmune thyroiditis, type 1 diabetes mellitus, ulcerative colitis,Crohn disease, and celiac disease appear to be significant. However, the associations between AIH and primary sclerosing cholangitis, primary biliary cholangitis, rheumatoid arthritis, systemic lupus erythematosus,Sjögren syndrome, and vitiligo are not well-supported.The aim of this review is to evaluate the strength of the reported associations between AIH and other autoimmune diseases, and to update and present the available evidence on their prevalence, proposed underlying pathogenic mechanisms, diagnostic considerations, and treatment approaches.
摘要AIM To investigate whether immune mediated diseases(IMD) are more frequent in patients with inflammatory bowel disease(IBD).METHODS In this population based registry study,a total of 47325 patients with IBD were alive and registered in the Danish National Patient Registry on December 16,2013. Our results strengthen the hypothesis that some IMDs and IBD may have overlapping pathogenic pathways.

摘要We reported a female patient with unrecognized celiac disease and multiple extra intestinal manifestations, mainly related to a deranged immune function, including macroamilasemia, macrolipasemia, IgA nephropathy, thyroiditis, and anti-b2-glicoprotein-1 antibodies, that disappeared or improved after the implementation of a gluten-free diet.
摘要Tumor necrosis factor-α (TNF-α) plays a key role in the pathogenesis of experimental autoimmune neuritis (EAN) as well as Guillain-Barre syndrome. The data suggest that antagonizing TNF-α functions or suppressing TNF-α production may be useful in the acute phase of EAN treatment, but further studies are required.

摘要The intestinal tract is an important digestive organ and detoxification organ of the human body, and its circling structure is vividly called the“second brain”of the human body. There are hundreds of millions of bacterium in the intestinal tract. These bacteria live in mutual benefit with the body, provide energy and nutrients for the host and themselves through fermented food, participate in the metabolism of the body, and form a metabolic mode of cometabolism between the host and the symbiotic flora. In addition, intestinal flora can also help the body resist the invasion of pathogens, promote human health, and resist diseases.More and more studies have shown that when the body is subjected to exogenous or endogenous stimuli, the microbial flora in the intestinal will change, and the disturbance of intestinal flora is closely related to the occurrence and development of inflammatory bowel diseases, metabolic diseases, immune system diseases, mental system diseases, and tumors. This article reviews the research progress of the intestinal flora affecting the pathogenesis of various diseases, aiming to provide new references and ideas for the clinical treatment of diseases.

摘要Background:Emerging evidence suggests that autoantibodies targeting podocytes are potential contributors to idiopathic nephrotic syndrome(INS);however, the specific mechanisms remain unclear. This study aims to explore the pathogenic role and underlying mechanisms of anti-vinculin autoantibodies in INS.Methods:Serum anti-vinculin autoantibody levels detected by protein microarray and clinical data were compared among INS patients(n=147),healthy individuals(n=84),and patients with other kidney or immune diseases(n=100 of each disease).Immune-mediated mouse models were established to verify the pathogenicity of anti-vinculin autoantibodies.Mouse urine was monitored for urine protein levels, while immunofluorescence, pathological staining, and electron microscopy assessed kidney pathological and ultrastructural changes.Transcriptome sequencing of mouse kidney tissues was performed to investigate the key molecular mechanisms and signaling pathways involved in kidney injury postimmunization.Results:Anti-vinculin autoantibody levels were specifically elevated in INS patients, with a 54.42%positivity rate, correlating with urinary albumin, serum albumin, cholesterol, and CD19 levels. The average anti-vinculin autoantibody levels dropped markedly in pediatric INS patients during remission.Mouse experiments revealed that injecting anti-vinculin antibodies or recombinant vinculin protein induced proteinuria and podocyte injury in the immunized mice, and the renal phenotype closely resembled the pathological characteristics of minimal change disease.Transcriptome sequencing of renal tissues revealed up-regulation of inflammation, immune responses, cytokine activities, and B cell activation pathways in the immunized mice, while cytoskeleton-related functions were down-regulated.Conclusions:Autoantibodies targeting vinculin act as pathogenic autoantibodies in INS and hold potential value for diagnosing and monitoring INS progression.
摘要in this study serum complement mediated immune complex solubilizing capacity (CMSC) and immune complement precipitation capacity (ICPIC) of 32 children with acute glomerulonephritis (AGN) were measured. The data showed that the level of serum CMSC and ICPIC was markedly decreased in acute phase and returned to normal in the 7th week after onset of disease.Correlation analysis revealed that there were positive correlation between the level of serum CMSC and ICPIC and the serum concentration of CH50, C3, C4, but no linear correlation between the level of serum CMSC and ICPIC and the amount of CIC. These results suggest that the declined serum CMSC and ICPIC in AGN may be associ ated with the pathogenesis of AGN.

摘要Objective: to explore and analyze the clinical characteristics of rheumatic immune disease complicated with thrombocytopenia. Methods: from October 2019 to September 2021, 108 cases of rheumatic immune patients admitted to our hospital were selected as the investigation objects, among which 58 cases with thrombocytopenia were selected. The clinical status of rheumatic immune diseases with thrombocytopenia was summarized by comparing the thrombocytopenia, hemorrhage degree and the clinical response of platelets under various treatments of these patients. Results: sjogren's syndrome was the most common type of patients. The clinical effect of general immunosuppressive agents was poor after hormone addition. Leflunomide was improved, platelet was increased and spontaneous hemorrhage was stopped. Conclusion: systemic lupus erythematosus, Sjogren's syndrome and other diseases have a high incidence rate in rheumatic diseases complicated with thrombocytopenia, among which the Sjogren's complications have the greatest impact on thrombocytopenia. The clinical effect is not obvious under the treatment of hormone dosage or general immunosuppressive agents, but flunomide can be selected with good effect.
