Background:Bone marrow involvement(BMI)is a key determinant of treatment strategies and clinical prognosis in classic follicular lymphoma(cFL).The diagnostic performance of baseline positron emission tomography/comput...Background:Bone marrow involvement(BMI)is a key determinant of treatment strategies and clinical prognosis in classic follicular lymphoma(cFL).The diagnostic performance of baseline positron emission tomography/computed tomography(PET/CT)for identifying BMI remains unclear,and its consistency with bone marrow biopsy(BMB)results varies greatly across studies.Such diagnostic uncertainty hinders accurate clinical risk stratification.This study aimed to assess the diagnostic performance of PET/CT and explore associated diagnostic risk factors.Methods:A total of 176 patients with newly diagnosed classic follicular lymphoma were included.Clinical data,hematological parameters,and PET imaging findings were recorded for all patients and compared with the BMB report.Univariate and multivariate analyses were performed to identify risk factors associated with BMB-confirmed BMI,and corresponding receiver operating characteristic(ROC)curves were plotted to evaluate their diagnostic efficacy.Results:Taking BMB as the reference standard for detecting BMI,sensitivity and specificity of PET/CT were 57.9%and 70.0%.PET/CT-derived BMI(p=0.023),bulky mass(>6 cm)(p<0.001),and spleen involvement(p<0.001)were identified as independent risk factors for BMB-confirmed BMI with the diagnostic performance(AUC)as 0.639(p=0.002),0.635(p=0.002),and 0.702(p<0.001),respectively.Furthermore,the combined diagnostic performance of these three factors reached 0.788(p<0.001).Conclusions:Baseline PET/CT provides critical insights into the assessment of BMI in cFL.For patients with PET/CT images of focal with or without diffuse components,BMB may yield false-negative results due to sampling limitations.For patients with a diffuse component,the presence of bulky disease and spleen involvement is strongly predictive of BMB-confirmed BMI.Conversely,the absence of PET/CT-detected BMI,bulky disease,and splenic involvement suggests a low likelihood of BMBconfirmed BMI.展开更多
This study investigated the effects of parental involvement,parenting stress,and social support on the social skills of school-aged children(6-18 years old)with intellectual disabilities(ID).Data were collected from 2...This study investigated the effects of parental involvement,parenting stress,and social support on the social skills of school-aged children(6-18 years old)with intellectual disabilities(ID).Data were collected from 280 Chinese parents(mothers=70.0%,fathers=30.0%)of children with ID through purposive sampling and analyzed using partial least squares structural equation modeling(PLS-SEM).The results indicated that parental involvement not only directly enhanced children’s social skills but also indirectly improved them by alleviating parenting stress,which acted as a partial mediator.Contrary to the stress-buffering hypothesis,social support did not moderate the negative impact of parenting stress on social skills.Theoretically,this study contributes by validating ecological systems theory through a shift in focus from individual deficits to family systems,while also challenging the conventional view of stress-buffering theory.Accordingly,parent-support programs should integrate practical involvement training with systematic stress reduction and provide tailored assistance such as behavior-management training and respite care.展开更多
AIM:To report and analyze cases of sterile intraocular inflammation(IOI)following intravitreal faricimab injections in patients treated for neovascular age-related macular degeneration(nAMD)and diabetic macular edema(...AIM:To report and analyze cases of sterile intraocular inflammation(IOI)following intravitreal faricimab injections in patients treated for neovascular age-related macular degeneration(nAMD)and diabetic macular edema(DME).METHODS:This double-center case series included nine eyes of six patients who developed uveitis after faricimab therapy.Comprehensive clinical evaluation was performed,including slit-lamp examination,intraocular pressure(IOP)measurement,fluorescein and indocyanine green angiography(ICGA),and laboratory tests.Inflammatory responses were treated with topical or systemic corticosteroids,and patients were monitored for visual acuity and inflammatory activity.RESULTS:The incidence of IOI was 0.8%per patient(Innsbruck)and 0.23%(Czechia),with inflammation typically occurring between the third and sixth injection(mean interval:10d post-injection).Inflammator y presentations ranged from anterior uveitis to posterior segment involvement.One notable case demonstrated novel choroidal hypofluorescent lesions on angiography,suggesting deeper ocular involvement.The mean patient age was 76y;five of six affected patients were female.All cases responded to local and systemic corticosteroids,with full recovery of initial visual acuity.CONCLUSION:Sterile IOI after faricimab appears to be a rare but relevant adverse event.Although the incidence falls within expected ranges for anti-vascular endothelial growth factor(anti-VEGF)agents,the observed choroidal involvement represents a potentially new safety signal.Prompt diagnosis and corticosteroid therapy are effective in all cases.Our findings support the need for vigilant post-marketing surveillance and further studies to better understand the underlying mechanisms and risk factors of faricimab-associated inflammation.展开更多
BACKGROUND Understanding the type and extent of coronary artery involvement in patients with acute type A aortic dissection(ATAAD)is vital for surgical planning.The Neri classification has been proposed as a guide for...BACKGROUND Understanding the type and extent of coronary artery involvement in patients with acute type A aortic dissection(ATAAD)is vital for surgical planning.The Neri classification has been proposed as a guide for surgical strategies,however,its prognostic impact on postoperative mortality rates remains understudied in large-scale cohorts.METHODS We reviewed 600 ATAAD patients who underwent surgery and coronary computed tomography angiography from 2016 to 2020 at Fuwai Hospital,National Center for Cardiovascular Diseases,Chinese Academy of Medical Sciences and Peking Union Medical College,Beijing,China.Patients were classified based on the Neri classification system:no coronary artery involvement,type A(ostial involvement),type B(dissection in coronary body),and type C(circumferential detachment or complete avulsion).The primary endpoint was 30-day mortality.RESULTS Overall,28.3%of the patients had coronary artery involvement,with Neri type A,Neri type B,and Neri type C accounting for 13.3%,11.2%,and 3.8%,respectively.The right coronary artery was more frequently involved(25.3%)than the left coronary artery(8.0%).In the unadjusted analysis,patients with coronary artery involvement exhibited a numerically higher 30-day mortality compared to those without(5.3%vs.2.3%)(OR=2.35,95%CI:0.94–5.88,P=0.07),though this difference did not reach statistical significance.However,multivariable adjustment revealed significant association(adjusted OR=3.71,95%CI:1.05–13.13,P=0.04).Interestingly,after additional adjustment for coronary artery bypass grafting,the impact of coronary artery involvement on 30-day mortality no longer remained statistically significant(adjusted OR=3.13,95%CI:0.85–11.58,P=0.09).The 1-year mortality was higher in those with coronary artery involvement,but this significant association disappeared after adjusting for potential confounding variables.Furthermore,no significant difference in 30-day and 1-year mortality were observed among patients with different Neri classifications.CONCLUSIONS In patients with ATAAD who undergo surgery,the presence of coronary artery involvement is significantly associated with an increased risk of 30-day mortality.Proactive coronary artery bypass grafting may potentially mitigate the adverse impact of coronary artery involvement on 30-day mortality.展开更多
Background Patient involvement in health technology assessment(HTA)has been extensively explored and implemented in high-income countries,but little is known about it in low-and middle-income countries(LMICs).This stu...Background Patient involvement in health technology assessment(HTA)has been extensively explored and implemented in high-income countries,but little is known about it in low-and middle-income countries(LMICs).This study aimed to provide a comprehensive picture of the current state and trends of patient involvement in HTA research,which can inform future research in the LMICs.Methods Publications on patient involvement in HTA from January 1,1900,to December 31,2023,were retrieved from the core databases of the Web of Science.We applied a bibliometric analysis to reveal the collaboration patterns,hot topics,and evolution of the research field.Co-occurrence,clustering,citation,and burst analyses were performed using VOSviewer and CiteSpace,with results visualized for interpretation.Results A total of 175 articles were eligible for inclusion.The first valid article was published in 2000.The number of publications has increased since 2011.The most productive countries and institutions were Canada and McMaster University.The studies focused on five hot topics:patient preferences,priority setting,qualitative research,drug development,and hospital-based HTA.The burst analysis revealed that priority setting and cost effectiveness were the research frontiers.Conclusions While patient involvement in HTA research has gained increasing attention,the research conducted in the LMICs remain limited.It is recommended that LMICs participate in international research collaborations,and focus on the five hot topics and emerging frontiers to advance both their research capacity and practical implementations.展开更多

Amyotrophic lateral sclerosis is a rare neurodegenerative disease characterized by the involvement of both upper and lower motor neurons.Early bilateral limb involvement significantly affects patients'daily lives ...Amyotrophic lateral sclerosis is a rare neurodegenerative disease characterized by the involvement of both upper and lower motor neurons.Early bilateral limb involvement significantly affects patients'daily lives and may lead them to be confined to bed.However,the effect of upper and lower motor neuron impairment and other risk factors on bilateral limb involvement is unclear.To address this issue,we retrospectively collected data from 586 amyotrophic lateral sclerosis patients with limb onset diagnosed at Peking University Third Hospital between January 2020 and May 2022.A univariate analysis revealed no significant differences in the time intervals of spread in different directions between individuals with upper motor neuron-dominant amyotrophic lateral sclerosis and those with classic amyotrophic lateral sclerosis.We used causal directed acyclic graphs for risk factor determination and Cox proportional hazards models to investigate the association between the duration of bilateral limb involvement and clinical baseline characteristics in amyotrophic lateral sclerosis patients.Multiple factor analyses revealed that higher upper motor neuron scores(hazard ratio[HR]=1.05,95%confidence interval[CI]=1.01–1.09,P=0.018),onset in the left limb(HR=0.72,95%CI=0.58–0.89,P=0.002),and a horizontal pattern of progression(HR=0.46,95%CI=0.37–0.58,P<0.001)were risk factors for a shorter interval until bilateral limb involvement.The results demonstrated that a greater degree of upper motor neuron involvement might cause contralateral limb involvement to progress more quickly in limb-onset amyotrophic lateral sclerosis patients.These findings may improve the management of amyotrophic lateral sclerosis patients with limb onset and the prediction of patient prognosis.展开更多

Objective:To compare the diagnostic value of 99mTc-MDP SPECT/CT and MSCT in detecting thoracic wall and rib involvement in lung adenocarcinoma.Methods:A retrospective analysis was conducted on the imaging data of 78 t...Objective:To compare the diagnostic value of 99mTc-MDP SPECT/CT and MSCT in detecting thoracic wall and rib involvement in lung adenocarcinoma.Methods:A retrospective analysis was conducted on the imaging data of 78 thoracic wall and rib lesions from 66 patients,a total of 32 males and 34 females,aged(53.2±5.6)years old with pathologically confirmed lung adenocarcinoma who underwent both 99mTc-MDP SPECT/CT and MSCT examinations from March 2017 to September 2023.The diagnostic efficacy of the two imaging modalities was compared using pathological confirmation or clinical follow-up as the gold standard.Results:Pathological confirmation or clinical follow-up revealed 74 lesions with thoracic wall bone involvement in lung adenocarcinoma(20 lesions confirmed by surgical pathology and 54 lesions confirmed by clinical follow-up)and 4 lesions without thoracic wall or rib involvement(2 lesions confirmed by surgical pathology and 2 lesions confirmed by clinical follow-up).The diagnostic sensitivity,specificity,and accuracy of 99mTc-MDP SPECT/CT were 97.3%,50.0%,and 94.9%,respectively.Its diagnostic sensitivity and accuracy were higher than those of MSCT(72.3%and 74.4%,respectively),with statistically significant differences(P0.05).99mTc-MDP SPECT/CT examination revised the MSCT tumor staging in 14 patients[21.2%(14/66)]with lung adenocarcinoma.Conclusion:99mTc-MDP SPECT/CT imaging demonstrates superior diagnostic efficacy compared to MSCT in detecting thoracic wall and rib involvement in lung adenocarcinoma.It offers more accurate tumor staging than MSCT,and an accurate diagnosis aids in clinical treatment decision-making.展开更多

The purpose of this study was to determine gender, father involvement, and mother involvement differences in the psychological profiles of collegiate freshmen athletes as measured by perfectionism, physical self-concept, and psychological well-being. Methods: Eighty-two male and 73 female collegiate freshmen responded to self-reported measures of family involvement, perfectionism, physical self-concept, and psychological well-being. Three separate 2 x 2 x 2 (gender x mother involvement x father involvement) factorial multivariate analysis of variance (MANOVAs) were carried out on the respective subscales from the reported measures. Results: There were significant differences for gender as well as father involvement in perfectionism (p 〈 0.01). Freshmen females had lower concern over mistakes, and greater levels of organization as well as planfulness than males. Furthermore, freshmen athletes with high involved fathers showed greater levels of high standards for others as well as organization. We have recently shown...Bone morphogenetic proteins (Bmp) are well-known to induce bone formation following chondrogenesis, but the direct role of Bmp signaling in the osteoblast lineage is not completely understood. We have recently shown that deletion of the receptor Bmprla in the osteoblast lineage with Dmpl-Cre reduces osteoblast activity in general but stimulates proliferation of preosteoblasts specifically in the cancellous bone region, resulting in diminished periosteal bone growth juxtaposed with excessive cancellous bone formation. Because expression of sclerostin (SOST), a secreted Wnt antagonist, is notably reduced in the Bmprla- deficient osteocytes, we have genetically tested the hypothesis that increased Wnt signaling might mediate the increase in cancellous bone formation in response to Bmprla deletion. Forced expression of human SOST from a Dmpl promoter fragment partially rescues preosteoblast hyperproliferation and cancellous bone overgrowth in the Bmprla mutant mice, demonstrating functional interaction between Bmp and Wnt signaling in the cancellous bone compat^a-tent. To test whether increased Wnt signaling can compensate for the defect in periosteal growth caused by Bmprla deletion, we have generated compound mutants harboring a hyperactive mutation (A214V) in the Wnt receptor Lrp5. However, the mutant Lrp5 does not restore periosteal bone growth in the Bmprla-deficient mice. Thus, Bmp signaling restricts cancellous bone accrual partly through induction of SOST that limits preosteoblast proliferation, but promotes periosteal bone growth apparently independently of Wnt activation.展开更多
Senescence leads to declines in fruit quality and shortening of shelf life.It is known that low temperatures(LTs)efficiently delay fruit senescence and that high temperatures(HTs)accelerate senescence.However,the mole...Senescence leads to declines in fruit quality and shortening of shelf life.It is known that low temperatures(LTs)efficiently delay fruit senescence and that high temperatures(HTs)accelerate senescence.However,the molecular mechanism by which temperature affects senescence is unclear.Herein,through multiomics analyses of fruits subjected to postharvest HT,LT,and room temperature treatments,a total of 56 metabolic compounds and 700 mRNAs were identified to be associated with fruit senescence under HT or LT conditions.These compounds could be divided into antisenescent(I→III)and prosenescent(IV→VI)types.HT affected the expression of 202 mRNAs to enhance the biosynthesis of prosenescent compounds of types V and VI and to inhibit the accumulation of antisenescent compounds of types II and III.LT affected the expression of 530 mRNAs to promote the accumulation of antisenescent compounds of types I and II and to impede the biosynthesis of prosenescent compounds of types IV and V.Moreover,16 microRNAs were isolated in response to HT or LT conditions and interacted with the mRNAs associated with fruit senescence under HT or LT conditions.Transient transformation of pear fruit showed that one of these microRNAs,Novel_188,can mediate fruit senescence by interacting with its target Pbr027651.1.Thus,both HT and LT conditions can affect fruit senescence by affecting microRNA–mRNA interactions,but the molecular networks are different in pear fruit.展开更多

Objective:Although relatively rare,adult immunoglobulin A vasculitis(IgAV)can lead to severe complications and longer hospitalization,and result in poor prognosis,when compared to childhood IgAV.Hence,early identifica...Objective:Although relatively rare,adult immunoglobulin A vasculitis(IgAV)can lead to severe complications and longer hospitalization,and result in poor prognosis,when compared to childhood IgAV.Hence,early identification and prevention for patients prone to develop systemic involvement are essential.The purpose of this study was to explore the correlations of common serological markers with the development of systemic involvement in adult IgAV.Methods:A retrospective analysis was performed for adult IgAV patients,who were hospitalized in Wuhan Union Hospital between January 2016 and December 2019.A total of 259 patients were enrolled,and the pre-treatment serological markers were comprehensively assessed.Results:In the present study,49.0% and 33.2% of patients developed renal and gastrointestinal(GI)involvement,respectively.Furthermore,the elevated levels of white blood cells count,D-Dimer(D-D),C-reactive protein(CRP)and neutrophil granulocyte ratio(NE%)>60% were significantly associated with GI involvement in the univariate analysis,while the decrease in high density lipoprotein level,and the elevated D-D and CRP levels were significantly associated with renal involvement(P<0.05).Moreover,a prediction model that combined multiple markers was established by performing a logistic regression analysis,and this presented a more favorable value of prediction than the individual serological markers.Conclusion:The present study suggests that common serological markers have close correlations with systemic involvement in adult IgAV,and that the establishment of a prediction model for systemic involvement may be helpful in facilitating personalized therapeutic strategies and clinical management for IgAV patients.展开更多
AIM:To assess the effects of neoadjuvant chemoradiotherapy(CRT) on the presence of extracapsular lymph node involvement(LNI) and its prognostic value in patients with resected esophageal cancer.METHODS:Two hundred and...AIM:To assess the effects of neoadjuvant chemoradiotherapy(CRT) on the presence of extracapsular lymph node involvement(LNI) and its prognostic value in patients with resected esophageal cancer.METHODS:Two hundred and ninety-eight patients with advanced esophageal cancer underwent esophagectomy between 1997 and 2006.One hundred and ninety patients(63.8%) were treated with neoadjuvant CRT prior to resection.A total of 986 metastatic LNs were examined.Survival of the patients was analyzed according to intra-and extra-capsular LNI.RESULTS:Five-year survival rate was 22.5% for the entire patient population.Patients with extracapsular LNI had a 5-year survival rate of 16.7%,which was comparable to the 15.8% in patients with infiltrated nodes of the celiac trunk(pM1lymph).In contrast to patients treated with surgery alone,neoadjuvant therapy resulted in signif icantly(P = 0.001) more patients with pN0/M0(51.6% vs 25.0%).In 17.6% of the patients with surgery alone vs 16.8% with neoadjuvant CRT,extracapsular LNI was detected.Neoadjuvant therapy does not reduce the occurrence of extracapsular LNI.CONCLUSION:Extracapsular LNI is an independent negative prognostic factor not influenced by neoadjuvant CRT.In a revised staging system for esophageal cancer,extracapsular LNI should be considered.展开更多
In order to evaluate the effects of La-involvement on biomass pyrolysis behaviors and properties of produced biochar, oak sawdust(OS) and corn straw(CS) were employed for thermogravimetric-differential thermogravi...In order to evaluate the effects of La-involvement on biomass pyrolysis behaviors and properties of produced biochar, oak sawdust(OS) and corn straw(CS) were employed for thermogravimetric-differential thermogravimetric(TG-DTG) analysis and producing biochar with/without La-involvement. Results indicated the initial and final temperatures were shifted toward lower temperature as LaCl3 was involved in pyrolysis. Mass loss and average mass loss rate during pyrolysis decreased with La-involvement. The kinetics indicated that the first-order reaction kinetic model well matched the pyrolysis process. As La-involved OS and CS were employed, their apparent activation energies(Ea) were reduced, and their pyrolysis characteristic index(I) were higher comparing with the OS and CS without La-involvement. Based on the produced biochar, the yield and ash content were increased by La-involvement, and the O/C ratio and iodine sorption value(ISV) were also enhanced. Obviously, the loaded LaCl3 could facilitate pyrolysis process, and the produced biochar exhibited a great adsorption potential in aqueous solution. According to the results from FT-IR(Fourier transform infrared spectroscopy) analysis, La in pyrolysis functioned as accelerating lignin decomposition via condensing –OH, breaking aliphatic C–H and aromatic rings on lignin, cutting the links of C-O-C among the monomers in lignocellulose. LaCl3 was finally converted to La2O3 in biochar after pyrolysis.展开更多

摘要Background:Bone marrow involvement(BMI)is a key determinant of treatment strategies and clinical prognosis in classic follicular lymphoma(cFL).The diagnostic performance of baseline positron emission tomography/computed tomography(PET/CT)for identifying BMI remains unclear,and its consistency with bone marrow biopsy(BMB)results varies greatly across studies.Such diagnostic uncertainty hinders accurate clinical risk stratification.This study aimed to assess the diagnostic performance of PET/CT and explore associated diagnostic risk factors.Methods:A total of 176 patients with newly diagnosed classic follicular lymphoma were included.Clinical data,hematological parameters,and PET imaging findings were recorded for all patients and compared with the BMB report.Univariate and multivariate analyses were performed to identify risk factors associated with BMB-confirmed BMI,and corresponding receiver operating characteristic(ROC)curves were plotted to evaluate their diagnostic efficacy.Results:Taking BMB as the reference standard for detecting BMI,sensitivity and specificity of PET/CT were 57.9%and 70.0%.PET/CT-derived BMI(p=0.023),bulky mass(>6 cm)(p<0.001),and spleen involvement(p<0.001)were identified as independent risk factors for BMB-confirmed BMI with the diagnostic performance(AUC)as 0.639(p=0.002),0.635(p=0.002),and 0.702(p<0.001),respectively.Furthermore,the combined diagnostic performance of these three factors reached 0.788(p<0.001).Conclusions:Baseline PET/CT provides critical insights into the assessment of BMI in cFL.For patients with PET/CT images of focal with or without diffuse components,BMB may yield false-negative results due to sampling limitations.For patients with a diffuse component,the presence of bulky disease and spleen involvement is strongly predictive of BMB-confirmed BMI.Conversely,the absence of PET/CT-detected BMI,bulky disease,and splenic involvement suggests a low likelihood of BMBconfirmed BMI.
摘要This study investigated the effects of parental involvement,parenting stress,and social support on the social skills of school-aged children(6-18 years old)with intellectual disabilities(ID).Data were collected from 280 Chinese parents(mothers=70.0%,fathers=30.0%)of children with ID through purposive sampling and analyzed using partial least squares structural equation modeling(PLS-SEM).The results indicated that parental involvement not only directly enhanced children’s social skills but also indirectly improved them by alleviating parenting stress,which acted as a partial mediator.Contrary to the stress-buffering hypothesis,social support did not moderate the negative impact of parenting stress on social skills.Theoretically,this study contributes by validating ecological systems theory through a shift in focus from individual deficits to family systems,while also challenging the conventional view of stress-buffering theory.Accordingly,parent-support programs should integrate practical involvement training with systematic stress reduction and provide tailored assistance such as behavior-management training and respite care.
摘要AIM:To report and analyze cases of sterile intraocular inflammation(IOI)following intravitreal faricimab injections in patients treated for neovascular age-related macular degeneration(nAMD)and diabetic macular edema(DME).METHODS:This double-center case series included nine eyes of six patients who developed uveitis after faricimab therapy.Comprehensive clinical evaluation was performed,including slit-lamp examination,intraocular pressure(IOP)measurement,fluorescein and indocyanine green angiography(ICGA),and laboratory tests.Inflammatory responses were treated with topical or systemic corticosteroids,and patients were monitored for visual acuity and inflammatory activity.RESULTS:The incidence of IOI was 0.8%per patient(Innsbruck)and 0.23%(Czechia),with inflammation typically occurring between the third and sixth injection(mean interval:10d post-injection).Inflammator y presentations ranged from anterior uveitis to posterior segment involvement.One notable case demonstrated novel choroidal hypofluorescent lesions on angiography,suggesting deeper ocular involvement.The mean patient age was 76y;five of six affected patients were female.All cases responded to local and systemic corticosteroids,with full recovery of initial visual acuity.CONCLUSION:Sterile IOI after faricimab appears to be a rare but relevant adverse event.Although the incidence falls within expected ranges for anti-vascular endothelial growth factor(anti-VEGF)agents,the observed choroidal involvement represents a potentially new safety signal.Prompt diagnosis and corticosteroid therapy are effective in all cases.Our findings support the need for vigilant post-marketing surveillance and further studies to better understand the underlying mechanisms and risk factors of faricimab-associated inflammation.

摘要BACKGROUND Understanding the type and extent of coronary artery involvement in patients with acute type A aortic dissection(ATAAD)is vital for surgical planning.The Neri classification has been proposed as a guide for surgical strategies,however,its prognostic impact on postoperative mortality rates remains understudied in large-scale cohorts.METHODS We reviewed 600 ATAAD patients who underwent surgery and coronary computed tomography angiography from 2016 to 2020 at Fuwai Hospital,National Center for Cardiovascular Diseases,Chinese Academy of Medical Sciences and Peking Union Medical College,Beijing,China.Patients were classified based on the Neri classification system:no coronary artery involvement,type A(ostial involvement),type B(dissection in coronary body),and type C(circumferential detachment or complete avulsion).The primary endpoint was 30-day mortality.RESULTS Overall,28.3%of the patients had coronary artery involvement,with Neri type A,Neri type B,and Neri type C accounting for 13.3%,11.2%,and 3.8%,respectively.The right coronary artery was more frequently involved(25.3%)than the left coronary artery(8.0%).In the unadjusted analysis,patients with coronary artery involvement exhibited a numerically higher 30-day mortality compared to those without(5.3%vs.2.3%)(OR=2.35,95%CI:0.94–5.88,P=0.07),though this difference did not reach statistical significance.However,multivariable adjustment revealed significant association(adjusted OR=3.71,95%CI:1.05–13.13,P=0.04).Interestingly,after additional adjustment for coronary artery bypass grafting,the impact of coronary artery involvement on 30-day mortality no longer remained statistically significant(adjusted OR=3.13,95%CI:0.85–11.58,P=0.09).The 1-year mortality was higher in those with coronary artery involvement,but this significant association disappeared after adjusting for potential confounding variables.Furthermore,no significant difference in 30-day and 1-year mortality were observed among patients with different Neri classifications.CONCLUSIONS In patients with ATAAD who undergo surgery,the presence of coronary artery involvement is significantly associated with an increased risk of 30-day mortality.Proactive coronary artery bypass grafting may potentially mitigate the adverse impact of coronary artery involvement on 30-day mortality.

摘要Background Patient involvement in health technology assessment(HTA)has been extensively explored and implemented in high-income countries,but little is known about it in low-and middle-income countries(LMICs).This study aimed to provide a comprehensive picture of the current state and trends of patient involvement in HTA research,which can inform future research in the LMICs.Methods Publications on patient involvement in HTA from January 1,1900,to December 31,2023,were retrieved from the core databases of the Web of Science.We applied a bibliometric analysis to reveal the collaboration patterns,hot topics,and evolution of the research field.Co-occurrence,clustering,citation,and burst analyses were performed using VOSviewer and CiteSpace,with results visualized for interpretation.Results A total of 175 articles were eligible for inclusion.The first valid article was published in 2000.The number of publications has increased since 2011.The most productive countries and institutions were Canada and McMaster University.The studies focused on five hot topics:patient preferences,priority setting,qualitative research,drug development,and hospital-based HTA.The burst analysis revealed that priority setting and cost effectiveness were the research frontiers.Conclusions While patient involvement in HTA research has gained increasing attention,the research conducted in the LMICs remain limited.It is recommended that LMICs participate in international research collaborations,and focus on the five hot topics and emerging frontiers to advance both their research capacity and practical implementations.

摘要The immune system is involved in many age-related pathological changes,also plays an important role in tissue regeneration after injury.But no immune involvement has been discussed regarding cataract since it is presumed that lens has no source of immune cells as an avascular zone.Latest research has challenged the longstanding view of the lens as an immune-privileged tissue,revealing the presence of resident immune cells and active immune responses within the lens.Thus,we summarized the immune involvement in maintaining lens homeostasis,which may be a deleterious role in the induction of lens opacification if inappropriately activated.Furthermore,bioengineer-based immunomodulatory therapies to fine-tune the micro immune environment within lens may be future strategies for in situ lens regeneration,as a novel treatment for cataract.

摘要Amyotrophic lateral sclerosis is a rare neurodegenerative disease characterized by the involvement of both upper and lower motor neurons.Early bilateral limb involvement significantly affects patients'daily lives and may lead them to be confined to bed.However,the effect of upper and lower motor neuron impairment and other risk factors on bilateral limb involvement is unclear.To address this issue,we retrospectively collected data from 586 amyotrophic lateral sclerosis patients with limb onset diagnosed at Peking University Third Hospital between January 2020 and May 2022.A univariate analysis revealed no significant differences in the time intervals of spread in different directions between individuals with upper motor neuron-dominant amyotrophic lateral sclerosis and those with classic amyotrophic lateral sclerosis.We used causal directed acyclic graphs for risk factor determination and Cox proportional hazards models to investigate the association between the duration of bilateral limb involvement and clinical baseline characteristics in amyotrophic lateral sclerosis patients.Multiple factor analyses revealed that higher upper motor neuron scores(hazard ratio[HR]=1.05,95%confidence interval[CI]=1.01–1.09,P=0.018),onset in the left limb(HR=0.72,95%CI=0.58–0.89,P=0.002),and a horizontal pattern of progression(HR=0.46,95%CI=0.37–0.58,P<0.001)were risk factors for a shorter interval until bilateral limb involvement.The results demonstrated that a greater degree of upper motor neuron involvement might cause contralateral limb involvement to progress more quickly in limb-onset amyotrophic lateral sclerosis patients.These findings may improve the management of amyotrophic lateral sclerosis patients with limb onset and the prediction of patient prognosis.

摘要BACKGROUND In addition to nodal lesions,over 30%of mantle cell lymphomas(MCLs)also have gastrointestinal involvement,characteristically presenting as multiple lymphomatous polyposis(MLP),which rarely involve the esophagus.Most related papers have been case reports,and no comprehensive studies have been conducted;thus,the actual clinical situation has remained unknown for a long time.AIM To elucidate the actual clinical situation of esophageal involvement of MCL presenting with MLP,including its prognosis.METHODS From January 2001 to December 2021,among MCL patients whose gastrointestinal lesions were histopathologically confirmed by endoscopic biopsy at our center,6 patients with MLP in the esophagus were selected.We retro-spectively examined the clinical features of these patients,including their prognosis.RESULTS In all patients,multiple lesions were present in the gastrointestinal tract other than the esophagus and in the lymph nodes throughout the body,and most patients also had lesions involving the bone marrow or spleen.Most of the treatments include chemotherapy,with a 50%survival period of less than 2 years and a 5-year survival rate of approximately 30%,indicating a poor prognosis.CONCLUSION Patients with esophageal involvement of the MCL who presented with MLP had a large tumor burden and poor survival.
摘要A 31-year-old woman with untreated hypertension presented with a progressively worsening occipital headache for 3 months.In the last 4 days,she developed a pulsating holocranial headache that awakened her nocturnally and worsened with Valsalva maneuver.Neurological examination revealed no focal deficits.The patient reported decreased visual acuity,and fundoscopic examination demonstrated grade III hypertensive retinopathy,but no optic disc edema.Blood pressure was 230/160 mmHg and laboratory analysis showed elevated serum creatinine(3.42 mg/dL).Autoimmune and infectious markers were negative.
摘要Objective:To compare the diagnostic value of 99mTc-MDP SPECT/CT and MSCT in detecting thoracic wall and rib involvement in lung adenocarcinoma.Methods:A retrospective analysis was conducted on the imaging data of 78 thoracic wall and rib lesions from 66 patients,a total of 32 males and 34 females,aged(53.2±5.6)years old with pathologically confirmed lung adenocarcinoma who underwent both 99mTc-MDP SPECT/CT and MSCT examinations from March 2017 to September 2023.The diagnostic efficacy of the two imaging modalities was compared using pathological confirmation or clinical follow-up as the gold standard.Results:Pathological confirmation or clinical follow-up revealed 74 lesions with thoracic wall bone involvement in lung adenocarcinoma(20 lesions confirmed by surgical pathology and 54 lesions confirmed by clinical follow-up)and 4 lesions without thoracic wall or rib involvement(2 lesions confirmed by surgical pathology and 2 lesions confirmed by clinical follow-up).The diagnostic sensitivity,specificity,and accuracy of 99mTc-MDP SPECT/CT were 97.3%,50.0%,and 94.9%,respectively.Its diagnostic sensitivity and accuracy were higher than those of MSCT(72.3%and 74.4%,respectively),with statistically significant differences(P0.05).99mTc-MDP SPECT/CT examination revised the MSCT tumor staging in 14 patients[21.2%(14/66)]with lung adenocarcinoma.Conclusion:99mTc-MDP SPECT/CT imaging demonstrates superior diagnostic efficacy compared to MSCT in detecting thoracic wall and rib involvement in lung adenocarcinoma.It offers more accurate tumor staging than MSCT,and an accurate diagnosis aids in clinical treatment decision-making.
摘要Background: The parent-to-child influences can be adaptive and contribute to the optimal psychological well-being and positive perception of the athlete. Contrary to the healthy parental involvement, a family can also have negative effects on an athlete development. The purpose of this study was to determine gender, father involvement, and mother involvement differences in the psychological profiles of collegiate freshmen athletes as measured by perfectionism, physical self-concept, and psychological well-being. Methods: Eighty-two male and 73 female collegiate freshmen responded to self-reported measures of family involvement, perfectionism, physical self-concept, and psychological well-being. Three separate 2 x 2 x 2 (gender x mother involvement x father involvement) factorial multivariate analysis of variance (MANOVAs) were carried out on the respective subscales from the reported measures. Results: There were significant differences for gender as well as father involvement in perfectionism (p 〈 0.01). Freshmen females had lower concern over mistakes, and greater levels of organization as well as planfulness than males. Furthermore, freshmen athletes with high involved fathers showed greater levels of high standards for others as well as organization. 摘要Multiple myeloma(MM)is a common malignant hematological tumor in adults,which is characterized by clonal malignant proliferation of plasma cells in the bone marrow and secretion of a large number of abnormal monoclonal immunoglobulins(M protein),leading to bone destruction,hypercalcemia,anemia,and renal insufficiency(Alexandrakis et al.,2015;Yang et al.,2018).Since a large number of new drugs,represented by proteasome inhibitors and immunomodulators,have been successfullyused to treat MM.

摘要Tea plants are subjected to multiple stresses during growth,development,and postharvest processing,which affects levels of secondary metabolites in leaves and influences tea functional properties and quality.Most studies on secondary metabolism in tea have focused on gene,protein,and metabolite levels,whereas upstream regulatory mechanisms remain unclear.In this review,we exemplify DNA methylation and histone acetylation,summarize the important regulatory effects that epigenetic modifications have on plant secondary metabolism,and discuss feasible research strategies to elucidate the underlying specific epigenetic mechanisms of secondary metabolism regulation in tea.This information will help researchers investigate the epigenetic regulation of secondary metabolism in tea,providing key epigenetic data that can be used for future tea genetic breeding.

摘要Bone morphogenetic proteins (Bmp) are well-known to induce bone formation following chondrogenesis, but the direct role of Bmp signaling in the osteoblast lineage is not completely understood. We have recently shown that deletion of the receptor Bmprla in the osteoblast lineage with Dmpl-Cre reduces osteoblast activity in general but stimulates proliferation of preosteoblasts specifically in the cancellous bone region, resulting in diminished periosteal bone growth juxtaposed with excessive cancellous bone formation. Because expression of sclerostin (SOST), a secreted Wnt antagonist, is notably reduced in the Bmprla- deficient osteocytes, we have genetically tested the hypothesis that increased Wnt signaling might mediate the increase in cancellous bone formation in response to Bmprla deletion. Forced expression of human SOST from a Dmpl promoter fragment partially rescues preosteoblast hyperproliferation and cancellous bone overgrowth in the Bmprla mutant mice, demonstrating functional interaction between Bmp and Wnt signaling in the cancellous bone compat^a-tent. To test whether increased Wnt signaling can compensate for the defect in periosteal growth caused by Bmprla deletion, we have generated compound mutants harboring a hyperactive mutation (A214V) in the Wnt receptor Lrp5. However, the mutant Lrp5 does not restore periosteal bone growth in the Bmprla-deficient mice. Thus, Bmp signaling restricts cancellous bone accrual partly through induction of SOST that limits preosteoblast proliferation, but promotes periosteal bone growth apparently independently of Wnt activation.

摘要Senescence leads to declines in fruit quality and shortening of shelf life.It is known that low temperatures(LTs)efficiently delay fruit senescence and that high temperatures(HTs)accelerate senescence.However,the molecular mechanism by which temperature affects senescence is unclear.Herein,through multiomics analyses of fruits subjected to postharvest HT,LT,and room temperature treatments,a total of 56 metabolic compounds and 700 mRNAs were identified to be associated with fruit senescence under HT or LT conditions.These compounds could be divided into antisenescent(I→III)and prosenescent(IV→VI)types.HT affected the expression of 202 mRNAs to enhance the biosynthesis of prosenescent compounds of types V and VI and to inhibit the accumulation of antisenescent compounds of types II and III.LT affected the expression of 530 mRNAs to promote the accumulation of antisenescent compounds of types I and II and to impede the biosynthesis of prosenescent compounds of types IV and V.Moreover,16 microRNAs were isolated in response to HT or LT conditions and interacted with the mRNAs associated with fruit senescence under HT or LT conditions.Transient transformation of pear fruit showed that one of these microRNAs,Novel_188,can mediate fruit senescence by interacting with its target Pbr027651.1.Thus,both HT and LT conditions can affect fruit senescence by affecting microRNA–mRNA interactions,but the molecular networks are different in pear fruit.
摘要Inflammatory bowel disease (IBD) has been associated with either clinical or subclinical airway and parenchymal lung involvement and interstitial lung complications. Several studies have reported that atopy has a high prevalence in IBD patients. Overlapping allergic disorders seem to be present in both the respiratory and gastrointestinal systems. The purpose of this review is to update clinicians on recent available literature and to discuss the need for a highly suspicious approach by clinicians.

摘要Objective:Although relatively rare,adult immunoglobulin A vasculitis(IgAV)can lead to severe complications and longer hospitalization,and result in poor prognosis,when compared to childhood IgAV.Hence,early identification and prevention for patients prone to develop systemic involvement are essential.The purpose of this study was to explore the correlations of common serological markers with the development of systemic involvement in adult IgAV.Methods:A retrospective analysis was performed for adult IgAV patients,who were hospitalized in Wuhan Union Hospital between January 2016 and December 2019.A total of 259 patients were enrolled,and the pre-treatment serological markers were comprehensively assessed.Results:In the present study,49.0% and 33.2% of patients developed renal and gastrointestinal(GI)involvement,respectively.Furthermore,the elevated levels of white blood cells count,D-Dimer(D-D),C-reactive protein(CRP)and neutrophil granulocyte ratio(NE%)>60% were significantly associated with GI involvement in the univariate analysis,while the decrease in high density lipoprotein level,and the elevated D-D and CRP levels were significantly associated with renal involvement(P<0.05).Moreover,a prediction model that combined multiple markers was established by performing a logistic regression analysis,and this presented a more favorable value of prediction than the individual serological markers.Conclusion:The present study suggests that common serological markers have close correlations with systemic involvement in adult IgAV,and that the establishment of a prediction model for systemic involvement may be helpful in facilitating personalized therapeutic strategies and clinical management for IgAV patients.

摘要AIM:To assess the effects of neoadjuvant chemoradiotherapy(CRT) on the presence of extracapsular lymph node involvement(LNI) and its prognostic value in patients with resected esophageal cancer.METHODS:Two hundred and ninety-eight patients with advanced esophageal cancer underwent esophagectomy between 1997 and 2006.One hundred and ninety patients(63.8%) were treated with neoadjuvant CRT prior to resection.A total of 986 metastatic LNs were examined.Survival of the patients was analyzed according to intra-and extra-capsular LNI.RESULTS:Five-year survival rate was 22.5% for the entire patient population.Patients with extracapsular LNI had a 5-year survival rate of 16.7%,which was comparable to the 15.8% in patients with infiltrated nodes of the celiac trunk(pM1lymph).In contrast to patients treated with surgery alone,neoadjuvant therapy resulted in signif icantly(P = 0.001) more patients with pN0/M0(51.6% vs 25.0%).In 17.6% of the patients with surgery alone vs 16.8% with neoadjuvant CRT,extracapsular LNI was detected.Neoadjuvant therapy does not reduce the occurrence of extracapsular LNI.CONCLUSION:Extracapsular LNI is an independent negative prognostic factor not influenced by neoadjuvant CRT.In a revised staging system for esophageal cancer,extracapsular LNI should be considered.

摘要In order to evaluate the effects of La-involvement on biomass pyrolysis behaviors and properties of produced biochar, oak sawdust(OS) and corn straw(CS) were employed for thermogravimetric-differential thermogravimetric(TG-DTG) analysis and producing biochar with/without La-involvement. Results indicated the initial and final temperatures were shifted toward lower temperature as LaCl3 was involved in pyrolysis. Mass loss and average mass loss rate during pyrolysis decreased with La-involvement. The kinetics indicated that the first-order reaction kinetic model well matched the pyrolysis process. As La-involved OS and CS were employed, their apparent activation energies(Ea) were reduced, and their pyrolysis characteristic index(I) were higher comparing with the OS and CS without La-involvement. Based on the produced biochar, the yield and ash content were increased by La-involvement, and the O/C ratio and iodine sorption value(ISV) were also enhanced. Obviously, the loaded LaCl3 could facilitate pyrolysis process, and the produced biochar exhibited a great adsorption potential in aqueous solution. According to the results from FT-IR(Fourier transform infrared spectroscopy) analysis, La in pyrolysis functioned as accelerating lignin decomposition via condensing –OH, breaking aliphatic C–H and aromatic rings on lignin, cutting the links of C-O-C among the monomers in lignocellulose. LaCl3 was finally converted to La2O3 in biochar after pyrolysis.
