The term“worm medicines”(Chong Yao)in Traditional Chinese Medicine encompasses a diverse category of small animal-derived therapeutics,including arthropods,amphibians,and reptiles,rather than strictly vermiform organisms.Many of these animals,such as scorpions,centipedes,toads,and horseflies,are toxic,and have been employed in clinical practice for centuries,despite limited understanding of their active compounds and underlying modes of action.Among the bioactive constituents extracted from these taxa,peptides represent a particularly promising class,with over 2000 identified to date.These peptides are primarily secreted from specialized exocrine glands,including venom,salivary,and cutaneous glands,and are characterized by high structural diversity and target specificity.Their potent modulatory effects on the nervous,cardiovascular,and immune systems make them excellent candidates for drug discovery and development.This review examines peptide-based compounds derived from the exocrine secretions of venomous and poisonous taxa historically employed in traditional Chinese“worm”medicines.Emphasis is placed on their biological origins,structural features,and pharmacological activities,highlighting their significant potential as a rich and largely untapped resource for modern therapeutic discovery. 展开更多

Objective:To elucidate the quantification and standardization of cold and hot patterns,the fundamental concepts in traditional Chinese medicine(TCM).Methods:This randomized cross-controlled trial recruited 30 healthy volunteers.Participants in Group 1 underwent 14 days of Coptis chinensis Franch.(C.chinensis,Huang Lian)treatment to shift their body toward the cold pattern,followed by 7 days of washout and 14 days of Cinnamomum cassia Presl(C.cassia,Rou Gui)treatment to shift their body toward the hot pattern.Participants in Group 2 underwent the opposite treatment.Blood and stool samples were collected for routine blood testing and full-length 16S rRNA metagenomic sequencing.Results:Red blood cells(RBC),hemoglobin(HGB),hematocrit(HCT),and blood platelet count(BPC)were increased by C.chinensis,whereas the mean corpuscular HGB concentration(MCHC)was increased by C.cassia.RBC,HGB,HCT,and BPC were positively associated with the cold pattern but negatively correlated with the hot pattern,whereas MCHC showed opposite relationships.C.chinensis-increased Blautia stercoris was positively correlated with RBC,HGB,and HCT under C.chinensis treatment,whereas C.cassia-enhanced Parabacteroides distasonis_A was positively associated with MCHC under C.cassia treatment.Interestingly,five indicators(RBC,HGB,HCT,Blautia stercoris,and Prevotella copri)had an area under the curve of 0.795 for predicting the cold pattern.Although three indicators(MCHC,Akkermansia sp004167605,and Parabacteroides distasonis_A)showed poor predictive ability for the hot pattern,when the individual’s gut microbiota experienced disturbance by cold Chinese materia medica stimulation,the predictive ability increased to 0.703.Conclusion:This study suggests that blood parameters and gut microbes may be potential indicators of hot and cold TCM patterns,which could benefit the objectivity and scientific rigor of TCM. 展开更多

The etiopathogenesis of gastrointestinal diseases is varied in nature.Various etiogenic factors described are infective,inflammatory,viral,bacterial,parasitic,dietary and lifestyle change.Rare causative agents are immunological,and others associated as idiopathic,are undiagnosed by all possible means.Some of the rare diseases are congenital in nature,passing from the parent to the child.Many of the undiagnosed diseases are now being diagnosed as genetic and the genes have been implicated as a causative agent.There is a search for newer treatments for such diseases,which is called genomic medicine.Genomic medicine is an emerging medical discipline that involves the use of genomic information about an individual.This is used both for diagnostic as well as therapeutic decisions to improve the current health domain and pave the way for policymakers for its clinical use.In the developing era of precision medicine,genomics,epigenomics,environmental exposure,and other data would be used to more accurately guide individual diagnosis and treatment.Genomic medicine is already making an impact in the fields of oncology,pharmacology,rare,infectious and many undiagnosed diseases.It is beginning to fuel new approaches in certain medical specialties.Oncology is at the leading edge of incorporating genomics,as diagnostics for genetic and genomic markers are increasingly included in cancer screening,and to guide tailored treatment strategies.Genetics and genetic medicine have been reported to play a role in gastroenterology in several ways,including genetic testing(hereditary pancreatitis and hereditary gastrointestinal cancer syndromes).Genetic testing can also help subtype diseases,such as classifying pancreatitis as idiopathic or hereditary.Gene therapy is a promising approach for treating gastrointestinal diseases that are not effectively treated by conventional pharmaceuticals and surgeries.Gene therapy strategies include gene addition,gene editing,messenger RNA therapy,and gene silencing.Understanding genetic determinants,advances in genetics,have led to a better understanding of the genetic factors that contribute to human disease.Family-member risk stratification and genetic diagnosis can help identify family members who are at risk,which can lead to preventive treatments,lifestyle recommendations,and routine follow ups.Selecting target genes helps identify the gene targets associated with each gastrointestinal disease.Common gastrointestinal diseases associated with genetic abnormalities include-inflammatory bowel disease,gastroesophageal reflux disease,non-alcoholic fatty liver disease,and irritable bowel syndrome.With advancing tools and technology,research in the search of newer and individualized treatment,genes and genetic medicines are expected to play a significant role in human health and gastroenterology. 展开更多

Objective To establish selection criteria for essential medicines list,and to provide reference for the scientific and reasonable selection of essential medicines in China.Methods The selection criteria for essential medicines in WHO,India,Kenya and South Africa were compared and analyzed through literature research.Results and Conclusion It is found that the selection criteria for essential medicines in the above countries and organizations are consistent in such aspects as safety,efficacy,economic efficiency,and drug quality.However,there are different criteria in other aspects as well.The selection criteria for China’s essential medicines are preliminarily constructed by drawing on the joint criteria of WHO and the above-mentioned countries combined with our national conditions.Finally,fishbone diagram analysis method is used to refine and improve the selection criteria. 展开更多

Safety of herbal medicines in Korean medicine clinics in Republic of Korea:A nationwide retrospective longitudinal cohort study 认领 引用

Objective:Laboratory tests are commonly used in Korean medicine(KM)clinics to monitor the safety of herbal medicine(HM).Although the incidence of herb-induced liver injury(HILI)in Republic of Korea has been reported,there is a lack of data from KM clinics,which account for over 90%of KM institutions.This absence is due to the lack of a systematic pharmacovigilance system for HMs.To partially understand this issue,this study investigated changes in liver function test(LFT)and renal function test(RFT)after HM prescriptions in primary settings.Methods:This retrospective analysis utilized laboratory test results,including complete blood count,LFT and RFT,collected from 238 KM clinics across the Republic of Korea.The study population comprised patients who underwent laboratory testing before and after HM treatment between March 2020 and November 2021.We compared laboratory test results using paired t-tests or Wilcoxon signed-rank tests.Subgroup analyses were conducted by sex and elapsed time between tests.The Mc Nemar test was used to compare the proportions of cases with abnormal LFT and RFT levels according to lifestyle habits and comorbidities.Additionally,the incidence of liver injury was estimated by identifying cases with aspartate aminotransferase(AST)or alanine aminotransferase(ALT)levels exceeding five times the upper limit of normal after HM prescription.Results:A total of 2791 cases were included in the analysis.The levels of AST and ALT decreased significantly after HM prescription(P<0.0001).This result was consistent in subgroups stratified by sex and for test intervals of within 30 and 60 days(P<0.05).The proportion of cases with abnormal AST and ALT levels significantly decreased after HM prescription(P<0.001).Out of 2791 cases,only 2(0.07%)were identified as having liver injury after HM prescription,but causality was not confirmed.Conclusion:The findings suggest that the use of HM in KM clinics in Republic of Korea is generally safe.While a small number of HMs may be associated with liver injury,causality remains uncertain.Establishing a national pharmacovigilance system is crucial for accurately monitoring the incidence and risk factors of HILI. 展开更多

Stroke can be categorized as ischemic and hemorrhagic on the basis of its origin.The pathophysiology following a stroke is complex,and is characterized by ongoing inflammation,neuronal injury,and the accumulation of reactive oxygen species in the brain,all of which reflect a dynamic process of change.This complexity hinders achievement of significant therapeutic outcomes with standard stroke treatment procedures,limiting post-stroke recovery.This review presents an innovative post-stroke therapeutic approach that utilizes nanomedicines to modify the cerebral microenvironment.It highlights the primary roles of chronic inflammation and nerve repair issues in causing prolonged impairment in stroke patients.Traditional therapies show limited effectiveness in achieving neuroprotection,immunoregulation,and neural regeneration during the subacute and chronic phases of stroke.Therefore,effective stroke management requires the use of specific therapeutic strategies tailored to the pathological characteristics of each phase.Various types of nanomedicines possess distinct physicochemical properties and can be selected on the basis of the specific therapeutic needs.Surface-modification technologies have significantly enhanced the ability of nanomedicines to penetrate the blood-brain barrier and improve their targeting capabilities in drug administration.However,the stability,biocompatibility,and long-term safety of nanomedicines require further optimization for clinical application.Nanomedicines represent a novel approach to stroke treatment through targeted delivery and multifaceted regulatory mechanisms.These medicines provide distinct advantages,particularly in addressing chronic inflammation and promoting nerve regeneration.As a result,nanomedicines are expected to significantly improve rehabilitation outcomes and quality of life for stroke patients in the future,emerging as a crucial modality for stroke treatment. 展开更多

Objective To better understand the quality risks in non-clinical tests of traditional Chinese medicines(TCMs)and natural medicines(NMs),and to provide suggestions and reference for improving the quality of TCMs and NMs in both non-clinical tests and successful marketing.Methods According to the“Good Laboratory Practice(GLP)”“Provisions of Drug Registration”“Requirements for the Classification and Application of Traditional Chinese Medicine Registration”“Technical Requirements for the Research of New Natural Drugs”and the“Quality Management Standards to Produce Chinese Medicinal Materials”,etc.,the quality risks of TCMs and NMs in non-clinical tests were analyzed.Results and Conclusion In the non-clinical trial stage of traditional Chinese medicines and NMs,the quality risks for pharmaceutical companies came from the sources of herbs and simple quality identification method of active ingredients.Clinical research organization(CRO)had many experimental personnel that caused the uneven experimental level.Besides,due to the poor standardization of pharmacodynamic experiments in research institutes of universities,the original records were imperfect,and the experience in exploring drug safety experiments was insufficient.Therefore,quality risks in non-clinical tests of TCMs and NMs should be controlled.Pharmaceutical companies should be the main body of research and development of TCMs and NMs.However,research institutes of universities and CRO companies can provide corresponding technical support to jointly promote the development of non-clinical tests of TCMs and NMs. 展开更多

Evaluation of in vitro antibacterial effects of seven single traditional Chinese medicines and their combined formulations 认领 引用

In recent years,with the intensification of research on antimicrobial agents,the adverse effects of antibiotics and the escalating issue of antibiotic resistance have become increasingly prominent.Prolonged antibiotic usage can induce high levels of drug resistance in certain pathogenic microorganisms,thereby complicating clinical treatment.Against this backdrop,traditional Chinese medicine(TCM)has emerged as a promising resource for combating antibiotic-resistant bacterial infections due to its natural composition,relatively low side-effect profile,and minimal potential for inducing resistance.Consequently,research on TCM has attracted widespread attention and has become a focal point in antimicrobial studies.To evaluate the in vitro antibacterial activities of water extracts and their combined formulations from seven traditional Chinese herbs,Pomegranate peel,Scutellaria baicalensis,Fraxini cortex,Eucalyptus leaves,Artemisiae argyi,Scrophularia ningpoensis,and Paeonia lactiflora,against Escherichia coli(E.coli),Staphylococcus aureus(S.aureus),Enterobacter aerogenes,and Micrococcus luteus,the HPLC method was employed to quantify the content of active antibacterial compounds in each herb.Antibacterial efficacy was assessed using the filter paper disc diffusion method to determine inhibition zone diameters,and minimum inhibitory concentrations(MICs)were measured through the two-fold serial dilution technique.Based on these results,optimal antibacterial combinations were identified.Quantitative analysis revealed that Paeoniflorin in P.lactiflora was 16.80 mg/g,Baicalin in S.baicalensis reached 90.20 mg/g,and Aesculin in F.cortex was 9.16 mg/g.Chlorogenic acid,present in both Eucalyptus leaves and A.argyi,was measured at 4.66 and 3.54 mg/g,respectively.Punicalagin in Pomegranate peel was 22.18 mg/g,whereas Picfeltarraenin IA in S.ningpoensis was 1.70 mg/g.Regarding antibacterial activity,P.lactiflora and S.ningpoensis inhibited E.coli,with inhibition zone diameters of 12.2 and 11.8 mm,respectively.Pomegranate peel,Eucalyptus leaves,S.baicalensis,and P.lactiflora suppressed E.aerogenes,producing inhibition zones of 20.8,17.2,10.1 and 10.0 mm,respectively.Pomegranate peel and S.baicalensis exhibited strong inhibitory effects against S.aureus,with inhibition zones of 20.8 and 14.7 mm,sharing an MIC of 250 mg/mL.Against M.luteus,Pomegranate peel and Eucalyptus leaves demonstrated pronounced activity,with inhibition zones of 25.4 and 19.2 mm,and MICs of 125 and 1000 mg/mL,respectively.Notably,the combination of Pomegranate peel and S.baicalensis displayed a synergistic effect against S.aureus,with an optimal formulation ratio of 2:1.In contrast,the Pomegranate peel-Eucalyptus leaves combination synergistically inhibited M.luteus at the same 2:1 ratio.These findings underscored the potential of TCM combinations to enhance the inhibition of antibiotic-resistant bacterial strains.This study provided a robust foundation for developing novel natural antimicrobial agents and offered fresh insights and strategies to combat antibiotic resistance,highlighting both their scientific significance and practical application potential. 展开更多

Molecular glues(MGs)are small molecules that modulate protein–protein interactions by inducing or stabilizing ternary complexes,thereby reprogramming cellular networks rather than inhibiting single targets.Natural products,shaped by evolutionary selection and characterized by exceptional structural diversity,constitute a rich and underexplored source of glue-like compounds.Here,we propose a conceptual framework for rationalizing the multi-target efficacy of traditional medicines through molecular glue mechanisms.Diverse classes of natural products,including polyketides,terpenoids,steroids,lignans,organic acids,and alkaloids,have been shown to promote proximity-driven regulation of signaling pathways and targeted protein degradation.This interaction-based mode of action provides a molecular explanation for the systems-level therapeutic effects of traditional formulations that act through multiple pathways simultaneously.Integration of molecular glue theory with network pharmacology and modern chemical biology approaches offers a strategy to decode the complex pharmacology of herbal medicines and to expand the druggable proteome.Advances in proximity-based screening technologies and machine learning are expected to further accelerate the discovery of natural product-derived molecular glues for next-generation therapeutics. 展开更多

Non-alcoholic fatty liver disease(NAFLD)is a metabolic disease characterized by abnormal deposition of lipid in hepatocytes.If not intervened in time,NAFLD may develop into liver fibrosis or liver cancer,and ultimately threatening life.NAFLD has complicated etiology and pathogenesis,and there are no effective therapeutic means and specific drugs.Currently,insulin sensitizers,lipid-lowering agents and hepatoprotective agents are often used for clinical intervention,but these drugs have obvious side effects,and their effectiveness and safety need to be further confirmed.Adenosine monophosphate(AMP)-activated protein kinase(AMPK)plays a central role in maintaining energy homeostasis.Activated AMPK can enhance lipid degradation,alleviate insulin resistance(IR),suppress oxidative stress and inflammatory response,and regulate autophagy,thereby alleviating NAFLD.Natural herbal medicines have received extensive attention recently because of their regulatory effects on AMPK and low side effects.In this article,we reviewed the biologically active natural herbal medicines(such as natural herbal medicine formulas,extracts,polysaccharides,andmonomers)that reported in recent years to treatNAFLD via regulating AMPK,which can serve as a foundation for subsequent development of candidate drugs for NAFLD. 展开更多
