Background:The sequela of pelvic inflammatory disease(SPID)impair physical and psychosocial health.Integrated traditional Chinese medicine(TCM)nursing,combining herbal enema with moxibustion,may offer benefits,yet evi...Background:The sequela of pelvic inflammatory disease(SPID)impair physical and psychosocial health.Integrated traditional Chinese medicine(TCM)nursing,combining herbal enema with moxibustion,may offer benefits,yet evidence is limited.This study aims to assess the clinical outcomes associated with a defined TCM protocol administered to inpatients,and to contextualize findings using an outpatient cohort receiving standard care.Methods:A prospective,observational study was conducted from February to October 2025,in Beijing,China,and involved 140 women from a Grade A TCM hospital.The intervention group(n=70)comprised hospitalized patients receiving the standard care combined with TCM nursing protocol(TCM enema combined with Moxibustion).The reference group(n=70)comprised outpatients receiving standard care.The primary outcome was the symptom and the sign conditions(assessed with the TCM symptom and the sign rating scales).The secondary outcomes were pelvic pain intensity,sleep quality,and the anxious condition,which were assessed using the corresponding measuring tools,respectively.Data were assessed at baseline and reassessed 14 days after intervention.Results:Baseline analysis confirmed the inpatient cohort represented a higher-severity subset of the disease spectrum.Both the therapeutic pathways were effective.The intervention group demonstrated significant,large-magnitude improvements in all symptom scores postprotocol(al P<0.001;r=0.630.82).The reference group showed moderate improvements(all P<0.001;r=0.480.58).Regarding the between-pathway difference,the superior improvement observed in the inpatient pathway was statistically robust(all P<0.001;r=0.470.79).Conclusion:The integrated TCM nursing protocol is strongly associated with symptom improvement in hospitalized SPID patients,providing real-world benchmarks for future research.展开更多
Traditional villages represent a concentrated expression of the preservation and transmission of traditional culture within the context of rural revitalization,conveying important social,historical,and cultural values...Traditional villages represent a concentrated expression of the preservation and transmission of traditional culture within the context of rural revitalization,conveying important social,historical,and cultural values.Based on the perspective of spatial genes,this study selected 67 typical traditional villages in the Wuling Mountain Area in Southwest China as the case study.Between 2021 and 2024,by identifying and extracting spatial genes through the semi-structured interviews,Laddering Technique,and Landscape Pattern Index,and applying the geo-detector,the study explored the morphological characteristics and influencing mechanisms of traditional Tujia villages from the perspective of spatial genes of ecological-production-living.The findings are as follows:1)the spatial genes of traditional Tujia villages in the Wuling Mountain Area exhibit distinct patterns.Ecological genes,categorized by natural environment and layout,demonstrate a transition from clustered to dispersed patterns from the southern to northern regions.Production genes,categorized by location and cultivation patterns,show concentrated agricultural lands in the south and fragmented in the north.Living genes,divided into house plans,facades,and public buildings,reveal a higher prevalence of courtyards,stilted houses,and public buildings in northern areas.2)The spatial genes have evolved through the combined influence of four key factors:natural environment,socioeconomic development,policy systems,and ethnic culture.3)The integration of multi-source data and the application of both qualitative and quantitative approaches provide a comprehensive framework for analyzing traditional village form and their influencing mechanisms.This methodology offers valuable insights for developing sustainable strategies for the conservation of traditional Tujia traditional villages in this region.展开更多

OBJECTIVE:To evaluate the efficacy of Traditional Chinese Medicine(TCM)pattern-based comprehensive therapy on stable chronic obstructive pulmonary disease(COPD)patients and its influence on metabolic profiles.METHODS:...OBJECTIVE:To evaluate the efficacy of Traditional Chinese Medicine(TCM)pattern-based comprehensive therapy on stable chronic obstructive pulmonary disease(COPD)patients and its influence on metabolic profiles.METHODS:In this multicenter trial,270 participants from nine hospitals were randomly divided into a trial group and a control group at a 1∶2 ratio.The trial group received Tiotropium Bromide Powder for Inhalation(TBPI)plus one of three TCM granules[Shenqi Wenfei formula(参芪温肺方),Shenqi Buzhong formula(参芪补中方),or Shenqi Tiaoshen formula(参芪调肾方)],while the control group got only TBPI.The study had a 24-week treatment and 52-week follow-up.The main outcome was the frequency of acute COPD exacerbations.Secondary outcomes included related rates,scores on COPD assessment test(CAT)and modified medical research council(m MRC)dyspnea scale,6-min walk test(6MWT)distance,and lung function,measured at weeks 0,24,and 52.Serum samples for metabolomics were taken from the trial group before/after treatment and from healthy people.RESULTS:A total of 256 out of 270 patients completed the study.The trial group had fewer acute exacerbations than the control group during 52-week follow-up(P<0.05).At 24 weeks,the trial group improved in CAT,m MRC,and 6MWT compared to the control(P<0.05),with m MRC and 6MWT improvements lasting at week 52.Lung function showed no significant group differences.Metabolomics found 32 different metabolites between healthy and pre-treatment patients,17 reversing posttreatment.Bioinformatics showed pathway changes in pre-treatment patients reversed after treatment.CONCLUSION:TCM pattern-based comprehensive therapy reduces acute exacerbation risk and improves quality of life,respiratory symptoms,and exercise tolerance in stable COPD patients,likely by modulating energy,protein,and amino acid metabolism pathways.展开更多
OBJECTIVE:To identify oxidative stress(OS)-related genes involved in type 2 diabetes mellitus(T2DM)and screen potential Traditional Chinese Medicine(TCM)candidates for therapeutic use.METHODS:Gene expression data from...OBJECTIVE:To identify oxidative stress(OS)-related genes involved in type 2 diabetes mellitus(T2DM)and screen potential Traditional Chinese Medicine(TCM)candidates for therapeutic use.METHODS:Gene expression data from the GSE23343 dataset were obtained from the Gene Expression Omnibus(GEO)database.Differentially expressed genes(DEGs)between healthy and T2DM patients were identified.Weighted gene co-expression network analysis(WGCNA)was performed to select modules highly correlated with clinical traits,and core genes within these modules were identified.OS-related genes were retrieved from the Gene Cards database,and the overlapping DEGs,WGCNA genes,and OS-related genes were considered as hub genes in OS-related T2DM.These hub genes were validated in GSE15653 dataset.Potential TCMs were identified by mapping the hub genes to the Coremine Medical database.In vivo validation was performed using a T2DM rat model established by a high-fat diet and streptozotocin injection.The effects of Wedelolactone were evaluated by assessing gene expression via real-time quantitative reverse transcription PCR(q RT-PCR)and Western blot,alongside metabolic and liver function parameters,including fasting blood glucose,glycated serum protein,insulin resistance,and lipid profiles.RESULTS:A total of 394 DEGs(136 up-regulated and 258 down-regulated DEGs.)were identified in the GSE23343 cohort.WGCNA results showed that the turquoise module(cor=-0.56,P=0.02)and the brown module(cor=0.66,P=0.004)were the most correlated with T2DM.Six hub genes[interleukin 33(IL33),S100 calcium binding protein A8(S100A8),Golgi membrane protein 1(GOLM1),small nuclear ribonucleoprotein U1 subunit 70(SNRNP70),hepatocyte growth factor activator(HGFAC),and oxidative stress induced growth inhibitor 1(OSGIN1)]were identified,with IL33,S100A8,and GOLM1 being up-regulated,and SNRNP70,HGFAC,and GOLM1 being down-regulated.These genes distinguished T2DM from healthy controls with AUC values greater than 0.8.Experimental verification using a T2DM rat model confirmed the expression patterns of the hub genes.In vivo data demonstrated that Wedelolactone significantly reduced oxidative stress,inflammation hepatic lipid accumulation,and improved metabolic parameters such as fasting blood glucose,glycated serum protein,and insulin resistance.CONCLUSION:These findings highlight the critical roles of IL33,S100A8,GOLM1,SNRNP70,HGFAC,and OSGIN1 as biomarkers in OS-related T2DM and suggest that Wedelolactone may be a promising TCM-based therapeutic candidate for T2DM.展开更多

OBJECTIVE:To evaluate the efficacy of Traditional Chinese Medicine(TCM)psychosomatic integration therapy in treating subthreshold depression(SD).METHODS:A multicenter randomized controlled trial was conducted.Eligible...OBJECTIVE:To evaluate the efficacy of Traditional Chinese Medicine(TCM)psychosomatic integration therapy in treating subthreshold depression(SD).METHODS:A multicenter randomized controlled trial was conducted.Eligible participants were evaluated by physicians and randomly assigned to either the intervention or control group.The intervention group received group psychotherapy,Jue tune music therapy,and Yuleyin oral formula(郁乐饮).The control group received only group psychotherapy.The intervention period lasted 12 weeks,followed by a 12-week follow-up.The primary outcome was the Center for Epidemiologic Studies Depression Scale(CES-D)score at week 12.Secondary outcomes included dropout rate,scores of the Hamilton Anxiety Scale(HAMA)and the Hamilton Depression Rating Scale(HAMD-17)at week 12,and CES-D scores at weeks 4,8,16,20,and 24.RESULTS:A total of 505 patients were randomly allocated to the two groups,and 496 participants[77%female;(38±16)years]were included in the final analysis.The primary outcome showed no statistically significant difference in CES-D scores between the intervention and control groups at week 12(P>0.05).However,the intervention group exhibited significant reductions in CES-D scores at weeks 4,8,12,16,20,and 24 compared to baseline,and at week 24 compared to week 12(P<0.0001).The control group also showed significant reductions in CES-D scores at weeks 4 and 8 during the intervention period(P<0.0001).Although no significant differences were observed between groups at each specific time point,the intervention group showed a more consistent downward trend.Additionally,both HAMD and HAMA scores significantly decreased from baseline to week 12 in both groups(P<0.0001).CONCLUSIONS:TCM psychosomatic integration therapy,which includes group psychotherapy,Jue tune music therapy,and Yuleyin oral formula,may be effective in improving symptoms of SD,with good safety and feasibility.The complete TCM intervention demonstrated better long-term effectiveness than group psychotherapy alone.Further high-quality studies are needed to validate these findings.展开更多

Objective To develop a depression recognition model by integrating the spirit-expression diagnostic framework of traditional Chinese medicine(TCM)with machine learning algorithms.The proposed model seeks to establish ...Objective To develop a depression recognition model by integrating the spirit-expression diagnostic framework of traditional Chinese medicine(TCM)with machine learning algorithms.The proposed model seeks to establish a TCM-informed tool for early depression screening,thereby bridging traditional diagnostic principles with modern computational approaches.Methods The study included patients with depression who visited the Shanghai Pudong New Area Mental Health Center from October 1,2022 to October 1,2023,as well as students and teachers from Shanghai University of Traditional Chinese Medicine during the same period as the healthy control group.Videos of 3–10 s were captured using a Xiaomi Pad 5,and the TCM spirit and expressions were determined by TCM experts(at least 3 out of 5 experts agreed to determine the category of TCM spirit and expressions).Basic information,facial images,and interview information were collected through a portable TCM intelligent analysis and diagnosis device,and facial diagnosis features were extracted using the Open CV computer vision library technology.Statistical analysis methods such as parametric and non-parametric tests were used to analyze the baseline data,TCM spirit and expression features,and facial diagnosis feature parameters of the two groups,to compare the differences in TCM spirit and expression and facial features.Five machine learning algorithms,including extreme gradient boosting(XGBoost),decision tree(DT),Bernoulli naive Bayes(BernoulliNB),support vector machine(SVM),and k-nearest neighbor(KNN)classification,were used to construct a depression recognition model based on the fusion of TCM spirit and expression features.The performance of the model was evaluated using metrics such as accuracy,precision,and the area under the receiver operating characteristic(ROC)curve(AUC).The model results were explained using the Shapley Additive exPlanations(SHAP).Results A total of 93 depression patients and 87 healthy individuals were ultimately included in this study.There was no statistically significant difference in the baseline characteristics between the two groups(P>0.05).The differences in the characteristics of the spirit and expressions in TCM and facial features between the two groups were shown as follows.(i)Quantispirit facial analysis revealed that depression patients exhibited significantly reduced facial spirit and luminance compared with healthy controls(P<0.05),with characteristic features such as sad expressions,facial erythema,and changes in the lip color ranging from erythematous to cyanotic.(ii)Depressed patients exhibited significantly lower values in facial complexion L,lip L,and a values,and gloss index,but higher values in facial complexion a and b,lip b,low gloss index,and matte index(all P<0.05).(iii)The results of multiple models show that the XGBoost-based depression recognition model,integrating the TCM“spirit-expression”diagnostic framework,achieved an accuracy of 98.61%and significantly outperformed four benchmark algorithms—DT,BernoulliNB,SVM,and KNN(P<0.01).(iv)The SHAP visualization results show that in the recognition model constructed by the XGBoost algorithm,the complexion b value,categories of facial spirit,high gloss index,low gloss index,categories of facial expression and texture features have significant contribution to the model.Conclusion This study demonstrates that integrating TCM spirit-expression diagnostic features with machine learning enables the construction of a high-precision depression detection model,offering a novel paradigm for objective depression diagnosis.展开更多

Mitochondrial dysfunction is a key pathological feature, and traditional Chinese medicine(TCM) shows unique potential in t...Chronic heart failure(CHF) remains a global health challenge with limited therapeutic options. Mitochondrial dysfunction is a key pathological feature, and traditional Chinese medicine(TCM) shows unique potential in targeting this mechanism. Evidence from human and animal models of heart failure indicates that TCM can restore mitochondrial function by regulating mitochondrial Ca2+ homeostasis, oxidative stress, energy metabolism, mitochondrial dynamics, and mitophagy. TCM-based treatment of CHF offers notable clinical advantages, including improved therapeutic efficacy, enhanced cardiac function, and reduced incidence of major cardiovascular events. Experimental studies demonstrate that TCM decoctions and monomers modulate signaling pathways such as PPAR–RXRα, NF-κB, and PI3K/AKT to alleviate oxidative stress. TCM also increases AMPK activity via phosphorylation of PGC-1α, indirectly promoting mitochondrial biogenesis;attenuates calcium influx and enhances Ca2+ reuptake, thereby ameliorating myocardial mitochondrial dysfunction in CHF;and improves CHF by rebalancing mitochondrial dynamics and autophagy.展开更多
Objective To address the challenges of systematically identifying herb pairs in traditional Chinese medicine(TCM),we proposed HerbGL,a framework for predicting potential herb pairs that integrates network propagation ...Objective To address the challenges of systematically identifying herb pairs in traditional Chinese medicine(TCM),we proposed HerbGL,a framework for predicting potential herb pairs that integrates network propagation and graph regularization.Methods Based on the assumption that herbal actions induce subtle perturbations in biological systems,a framework named HerbGL was proposed.Random walk with restart(RWR)was first applied to the protein-protein interaction(PPI)network to reconstruct herb-specific perturbation effects and generate weighted subnetworks.Then,to quantify affinity between herb pairs,two network-proximity metrics,Closeness and PageRank,were computed from the weighted subnetworks to construct herb-pair affinity matrices.Finally,these matrices,together with known herb pairs(derived from co-occurrence analysis of TCM formulas with a threshold determined from the inflection point of the frequency distribution),were incorporated into a graph regularization model to predict potential herb pairs.Model performance was assessed through baseline comparison,ablation and robustness experiment under different ratios of positive and negative samples,using the area under the receiver operating characteristic curve(AUROC),the area under the precision-recall curve(AUPRC),accuracy,and precision as evaluation metrics.Furthermore,the predicted herb pairs were validated through both literature evidence and Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analyses.Results The weighted subnetworks constructed by RWR provided a refined simulation of herb-specific perturbation effects,which formed the basis for subsequent affinity modeling and prediction.Analysis of herb pair co-occurrence frequencies revealed a marked change around 150,which was selected as the threshold to distinguish herb pairs from non-herb pairs.HerbGL exhibited superior predictive performance compared with baseline models(AUROC=0.9705,AUPRC=0.9555,accuracy=0.7266,precision=0.9706).Ablation results showed that removing the Closeness and PageRank metrics substantially degraded performance(AUROC=0.8191,AUPRC=0.8768),confirming their necessity.Robustness evaluation under an imbalanced positive-to-negative sample ratio of 1:5 yielded AUROC=0.9696 and AUPRC=0.8404,indicating stable predictive ability.Moreover,multiple case studies further validated the rationality of the predicted herb pairs,such as Fangfeng(Saposhnikoviae Radix)and Qingpi(Citri Reticulatae Pericarpium Viride)which are recorded in Liangpeng Huiji(《良朋汇集》,Collection of Excellent Recipes)Vol.3:Fangfeng Shengma Tang(防风升麻汤).Additionally,pathway enrichment analysis of the Renshen(Ginseng Radix et Rhizoma)and Lianqiao(Forsythiae Fructus)pair further supported the biological plausibility of their compatibility.Conclusion HerbGL offers an effective and biologically informed framework for identifying herb pairs in TCM.Beyond improving herb pair prediction,the framework also provides data support for research on herb compounds and mechanisms,thereby supporting data-driven exploration of TCM compatibility.展开更多
Traditional villages are regarded as living fossils of human civilization,and their sustainable development has attracted widespread global attention.However,balancing the conservation of cultural heritage in traditio...Traditional villages are regarded as living fossils of human civilization,and their sustainable development has attracted widespread global attention.However,balancing the conservation of cultural heritage in traditional villages with residents’modern living needs remains a critical challenge.Despite extensive research,no viable pathway has been identified to resolve this contradiction.The current study innovatively proposes a tripartite framework based on a neo-endogenous development framework to revitalize traditional villages into a livable,workable,beautiful,and harmonious countryside(LWBHC).The framework is a multifaceted concept that includes cultural living inheritance,space adaptation renewal,and industrial value transformation.To deeply understand the application and effectiveness of this framework,this study employs a mixed-methods approach,combining qualitative and quantitative analyses while focusing on two traditional villages in southwestern China for empirical validation.The results indicate that the proposed framework demonstrates strong explanatory power and practical applicability in guiding the construction of LWBHC in traditional villages,effectively revealing the development logic and evolutionary characteristics of the case villages.Furthermore,specific implementation strategies based on the three-dimensional framework are proposed to advance the transformation of traditional villages into LWBHC.This research provides a replicable model for conserving and developing traditional villages in other regions of China and similar global contexts,enabling more traditional villages to achieve harmonious development while perpetuating human civilization.展开更多
The philosophy of“treating disease before its onset”is a fundamental concept of traditional Chinese medicine(TCM),permeating its diagnostic and therapeutic framework,and is central to clinical practice.However,curre...The philosophy of“treating disease before its onset”is a fundamental concept of traditional Chinese medicine(TCM),permeating its diagnostic and therapeutic framework,and is central to clinical practice.However,current TCM diagnostic and treatment models for the“predisease to disease”window period face several limitations,including the lack of comprehensive clinical parameters,difficulties in characterizing and integrating heterogeneous multimodal data,and insufficient dynamic precision in interventions and efficacy evaluations.To address these issues,guided by Professor Candong Li’s theory of TCM stateology,this study focuses on integrating objective multimodal data.It proposes a new model for personalized TCM diagnosis and treatment targeting the“pre-disease to disease”window period.This approach first proposes the idea of restructuring the conceptual framework of“symptom”and integrating multi-source heterogeneous data at macroscopic,mesoscopic,and microscopic levels to form a three-dimensional assessment indicator system.By integrating graph neural networks,convolutional neural networks,attention mechanisms,and knowledge graph-guided weight allocation,this approach enables collaborative representation,alignment,and fusion of multi-source data.Subsequently,it plans to construct a multimodal fusion model at both feature and decision levels,in order to establish mappings between indicators and TCM state elements,and to screen key indicators characterizing pathological evolution during the window period.Furthermore,it proposes a technical path for enhancing model interpretability using methods such as SHapley Additive exPlanations(SHAP)and Ablation-CAM++.Finally,with state assessment as the core,it proposes the concept of constructing a dynamic evaluation method for individualized diagnosis and treatment based on time-series data analysis using algorithms such as long short-term memory(LSTM)networks and gated recurrent units(GRUs).Moreover,a causal inference framework and semi-supervised learning strategies are introduced to enable quantitative evaluation of individual intervention effects and to provide interpretable therapeutic feedback,forming a complete technical path from data representation and fusion,weight adjustment,and interpretability analysis,to dynamic diagnosis feedback.This study aims to address deficiencies in the current TCM diagnosis and treatment model during the“pre-disease to disease”window period and to provide an operational framework for the clinical practice of TCM’s“treating disease before its onset”.展开更多

Objective To elucidate the biological basis of traditional Chinese medicine(TCM)syndromes from the perspective of“same syndrome,different diseases”in patients with chronic hepatitis B(CHB),liver cirrhosis(LC),and he...Objective To elucidate the biological basis of traditional Chinese medicine(TCM)syndromes from the perspective of“same syndrome,different diseases”in patients with chronic hepatitis B(CHB),liver cirrhosis(LC),and hepatocellular carcinoma(HCC),thereby providing a complementary approach for the diagnosis and treatment of chronic liver diseases(CLD).Methods To investigate the dynamic characteristics of TCM syndromes in CLD,transcriptomic profiling of peripheral blood mononuclear cells(PBMCs)was performed from patients with CHB,LC,or HCC presenting with three TCM syndromes:liver gallbladder dampness heat syndrome(LGDHS),liver depression spleen deficiency syndrome(LDSDS),and liver kidney Yin deficiency syndrome(LKYDS).These participants were recruited at Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine between August 1,2018 and December 31,2021.Differentially expressed genes(DEGs)were identified using the random variance model(RVM)F test with false discovery rate(FDR)correction.Principal component analysis(PCA)and unsupervised hierarchical clustering were applied to visualize sample grouping.Dynamic network biomarkers(DNB)analysis was employed to detect critical transition stages during syndrome evolution,followed by Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analyses to characterize the functional roles and pathway involvement of the DNB members.Random forest(RF)analysis and the area under the receiver operating characteristic(ROC)curves(AUC)were used to rank the importance of candidate genes.External validation was performed using microarray data from an independent CLD cohort(GSE89377)and RNA-seq data from The Cancer Genome Atlas Liver Hepatocellular Carcinoma(TCGA-LIHC)dataset.Additionally,reverse transcription quantitative polymerase chain reaction(RT-qPCR)was performed on an independent cohort of LC patients to validate the expression levels of the candidate genes.Results The study included a total of 132 participants.DNB analysis identified LDSDS stage as a critical tipping point during TCM syndrome evolution across CHB,LC,and HCC.The phosphoinositide 3-kinase/protein kinase B(PI3K-AKT)signaling pathway was consistently enriched in the DNB analysis across all three types of CLD,suggesting its potential involvement in the critical transition of TCM syndromes.Among the 24 core DNB members of the PI3K-AKT pathway,four genes—integrin subunit beta 1(ITGB1),collagen type IV alpha 1 chain(COL4A1),collagen type IV alpha 2 chain(COL4A2),and DNA damage inducible transcript 3(DDIT3)—were identified by RF analysis(Gini score>1)and ROC analysis.ROC analysis demonstrated high discriminative ability for distinguishing LGDHS from LKYDS in CHB patients,with AUC of 0.7891 for ITGB1,0.7070 for COL4A1,0.7148 for COL4A2,and 0.8945 for DDIT3.In the independent CLD cohort(GSE89377),all four genes showed significant stepwise upregulation from normal to CHB,LC,and HCC(all P<0.05).In the TCGA-LIHC dataset,their expression progressively increased with tumor stage.RT-qPCR validation in an independent LC cohort(30 LGDHS vs.30 LKYDS)confirmed that ITGB1,COL4A2,and DDIT3 were significantly upregulated in LKYDS compared with LGDHS(P=0.0152,0.0186,and 0.0247,respectively),whereas COL4A1 showed a non-significant upward trend(P=0.1201).Conclusion This study introduces a novel approach to understanding the molecular features underlying TCM syndrome evolution in CLD.The PI3K-AKT pathway and four identified genes(ITGB1,COL4A1,COL4A2,and DDIT3)play crucial roles in the transition from excess(LGDHS)to deficiency(LKYDS)via the critical LDSDS stage.These findings offer potential quantitative biomarkers and therapeutic targets for TCM syndrome differentiation and may help arrest syndrome progression in CLD.展开更多
Traditional Chinese medicine(TCM)is a holistic medical system that classifies and treats diseases based on the concept of patterns.These patterns describe the pathophysiological process of a disease at a specific stag...Traditional Chinese medicine(TCM)is a holistic medical system that classifies and treats diseases based on the concept of patterns.These patterns describe the pathophysiological process of a disease at a specific stage,reflecting both external signs and internal features.TCM patterns are central to pattern differentiation,treatment,clinical practice,and theoretical development in TCM.However,scientific explanation of TCM patterns remains limited because of subjective diagnostic criteria,the absence of a standardized experimental medical system,and unclear biological mechanisms,which restrict the modernization and globalization of TCM.Addressing challenges such as strong subjectivity in diagnosis,lack of standardized experimental systems,and unclear biological mechanisms is necessary to clarify the scientific meaning of TCM patterns and to provide technical approaches for the modernization and globalization of TCM.A strategy focused on“pathogenic factors,genetic predisposition,and disease progression stages”was adopted.This approach included the following:(Ⅰ)constructing disease-pattern integrated biological models with animal models,organoids,and multi-organ chips;(Ⅱ)applying multi-omics technologies,such as spatial omics,single-cell omics,and dynamic metabolic flux omics;(Ⅲ)using artificial intelligence(AI)and big data for data integration and prediction of pattern evolution;and(Ⅳ)validating formula–pattern associations through the“pattern differentiation through formula efficacy”approach.These strategies directly address the main obstacles in TCM pattern research by providing objective,quantifiable,and reproducible methodologies.Constructing disease-pattern integrated models enabled cross-scale research platforms.Applying multi-omics technologies allowed analysis of complex biological bases.AI and big data approaches addressed challenges related to heterogeneous data.The“formula-based pattern differentiation”approach supported precise interventions and the development of new drugs.This interdisciplinary framework advances TCM pattern research by moving from empirical description to objective quantification.By integrating innovative approaches,the study establishes a foundation for systematic,evidence-based TCM diagnosis and treatment,supporting accuracy and promoting international recognition and modernization of TCM.The study shows that combining multi-omics technologies,AI-driven data analysis,and disease-pattern models enables objective quantification of TCM patterns and clarifies their biological mechanisms.展开更多
Traditional Chinese medicine(TCM)has garnered increasing attention globally,with its modernization becoming a prominent research focus both within China and internationally.However,the lack of a precise definition for...Traditional Chinese medicine(TCM)has garnered increasing attention globally,with its modernization becoming a prominent research focus both within China and internationally.However,the lack of a precise definition for TCM modernization has hindered clear guidance for its development.Additionally,cancer remains a significant global public health challenge,largely untreatable with current methods.Therefore,a comprehensive understanding of TCM modernization is crucial for its evolution,revolution,drug discovery,and cancer therapy.This study provides an overview of the history,theory,characteristics,and evolution of TCM,highlighting its potential in cancer prevention and treatment.We propose a definition for TCM modernization,innovative Chinese medicine(ICM),and elucidate strategies to elevate TCM from a supporting role to a leading one.Electronic databases such as PubMed,Web of Science,ScienceDirect,and Clinical Trials were utilized to retrieve relevant literature spanning from 1979 to 2024,with most publications being from the last five years,using keywords like“Traditional Chinese medicine”,“Cancer”,“Mechanism”,and“Clinical trial”.In this study,we introduce the theory of TCM modernization following target identification and initial compound screening:ICM,defined by“3 D”elements:definite active ingredient composition and content,determined functional mechanism,and detection through evidence-based medicine.Overall,the“3 D”definition of ICM will establish a standard for ICM,accelerate TCM modernization,enhance drug discovery targeting cancer and various human diseases,and benefit patients worldwide.展开更多

摘要Background:The sequela of pelvic inflammatory disease(SPID)impair physical and psychosocial health.Integrated traditional Chinese medicine(TCM)nursing,combining herbal enema with moxibustion,may offer benefits,yet evidence is limited.This study aims to assess the clinical outcomes associated with a defined TCM protocol administered to inpatients,and to contextualize findings using an outpatient cohort receiving standard care.Methods:A prospective,observational study was conducted from February to October 2025,in Beijing,China,and involved 140 women from a Grade A TCM hospital.The intervention group(n=70)comprised hospitalized patients receiving the standard care combined with TCM nursing protocol(TCM enema combined with Moxibustion).The reference group(n=70)comprised outpatients receiving standard care.The primary outcome was the symptom and the sign conditions(assessed with the TCM symptom and the sign rating scales).The secondary outcomes were pelvic pain intensity,sleep quality,and the anxious condition,which were assessed using the corresponding measuring tools,respectively.Data were assessed at baseline and reassessed 14 days after intervention.Results:Baseline analysis confirmed the inpatient cohort represented a higher-severity subset of the disease spectrum.Both the therapeutic pathways were effective.The intervention group demonstrated significant,large-magnitude improvements in all symptom scores postprotocol(al P<0.001;r=0.630.82).The reference group showed moderate improvements(all P<0.001;r=0.480.58).Regarding the between-pathway difference,the superior improvement observed in the inpatient pathway was statistically robust(all P<0.001;r=0.470.79).Conclusion:The integrated TCM nursing protocol is strongly associated with symptom improvement in hospitalized SPID patients,providing real-world benchmarks for future research.

摘要Traditional villages represent a concentrated expression of the preservation and transmission of traditional culture within the context of rural revitalization,conveying important social,historical,and cultural values.Based on the perspective of spatial genes,this study selected 67 typical traditional villages in the Wuling Mountain Area in Southwest China as the case study.Between 2021 and 2024,by identifying and extracting spatial genes through the semi-structured interviews,Laddering Technique,and Landscape Pattern Index,and applying the geo-detector,the study explored the morphological characteristics and influencing mechanisms of traditional Tujia villages from the perspective of spatial genes of ecological-production-living.The findings are as follows:1)the spatial genes of traditional Tujia villages in the Wuling Mountain Area exhibit distinct patterns.Ecological genes,categorized by natural environment and layout,demonstrate a transition from clustered to dispersed patterns from the southern to northern regions.Production genes,categorized by location and cultivation patterns,show concentrated agricultural lands in the south and fragmented in the north.Living genes,divided into house plans,facades,and public buildings,reveal a higher prevalence of courtyards,stilted houses,and public buildings in northern areas.2)The spatial genes have evolved through the combined influence of four key factors:natural environment,socioeconomic development,policy systems,and ethnic culture.3)The integration of multi-source data and the application of both qualitative and quantitative approaches provide a comprehensive framework for analyzing traditional village form and their influencing mechanisms.This methodology offers valuable insights for developing sustainable strategies for the conservation of traditional Tujia traditional villages in this region.

摘要Traditional knowledge, biological genetic resources and folk literature and art are intellectual property rights of cultural heritage that can be shared by regional groups without time limit. This paper studies the traditional knowledge and cultural heritage of Xinjiang agriculture from the aspects of traditional knowledge, important agricultural heritage system, intangible cultural heritage, biological genetic resources, tangible cultural heritage, frontier development and defense culture, and cultural tourism resources. It analyzes the main problems existing in the protection and inheritance of them, and puts forward suggestions such as inheriting and sharing intellectual property rights of cultural heritage, improving the protection system of biological germplasm resources, establishing national-level cultural ecological protection (experimental) zones, promoting agricultural science and technology cultural exchanges, creating Xinjiang's characteristic Great Wall culture, deeply integrating "agriculture+culture+tourism", building national and autonomous region cultural parks, and dynamically inheriting agricultural cultural heritage.

摘要OBJECTIVE:To evaluate the efficacy of Traditional Chinese Medicine(TCM)pattern-based comprehensive therapy on stable chronic obstructive pulmonary disease(COPD)patients and its influence on metabolic profiles.METHODS:In this multicenter trial,270 participants from nine hospitals were randomly divided into a trial group and a control group at a 1∶2 ratio.The trial group received Tiotropium Bromide Powder for Inhalation(TBPI)plus one of three TCM granules[Shenqi Wenfei formula(参芪温肺方),Shenqi Buzhong formula(参芪补中方),or Shenqi Tiaoshen formula(参芪调肾方)],while the control group got only TBPI.The study had a 24-week treatment and 52-week follow-up.The main outcome was the frequency of acute COPD exacerbations.Secondary outcomes included related rates,scores on COPD assessment test(CAT)and modified medical research council(m MRC)dyspnea scale,6-min walk test(6MWT)distance,and lung function,measured at weeks 0,24,and 52.Serum samples for metabolomics were taken from the trial group before/after treatment and from healthy people.RESULTS:A total of 256 out of 270 patients completed the study.The trial group had fewer acute exacerbations than the control group during 52-week follow-up(P<0.05).At 24 weeks,the trial group improved in CAT,m MRC,and 6MWT compared to the control(P<0.05),with m MRC and 6MWT improvements lasting at week 52.Lung function showed no significant group differences.Metabolomics found 32 different metabolites between healthy and pre-treatment patients,17 reversing posttreatment.Bioinformatics showed pathway changes in pre-treatment patients reversed after treatment.CONCLUSION:TCM pattern-based comprehensive therapy reduces acute exacerbation risk and improves quality of life,respiratory symptoms,and exercise tolerance in stable COPD patients,likely by modulating energy,protein,and amino acid metabolism pathways.

摘要OBJECTIVE:To identify oxidative stress(OS)-related genes involved in type 2 diabetes mellitus(T2DM)and screen potential Traditional Chinese Medicine(TCM)candidates for therapeutic use.METHODS:Gene expression data from the GSE23343 dataset were obtained from the Gene Expression Omnibus(GEO)database.Differentially expressed genes(DEGs)between healthy and T2DM patients were identified.Weighted gene co-expression network analysis(WGCNA)was performed to select modules highly correlated with clinical traits,and core genes within these modules were identified.OS-related genes were retrieved from the Gene Cards database,and the overlapping DEGs,WGCNA genes,and OS-related genes were considered as hub genes in OS-related T2DM.These hub genes were validated in GSE15653 dataset.Potential TCMs were identified by mapping the hub genes to the Coremine Medical database.In vivo validation was performed using a T2DM rat model established by a high-fat diet and streptozotocin injection.The effects of Wedelolactone were evaluated by assessing gene expression via real-time quantitative reverse transcription PCR(q RT-PCR)and Western blot,alongside metabolic and liver function parameters,including fasting blood glucose,glycated serum protein,insulin resistance,and lipid profiles.RESULTS:A total of 394 DEGs(136 up-regulated and 258 down-regulated DEGs.)were identified in the GSE23343 cohort.WGCNA results showed that the turquoise module(cor=-0.56,P=0.02)and the brown module(cor=0.66,P=0.004)were the most correlated with T2DM.Six hub genes[interleukin 33(IL33),S100 calcium binding protein A8(S100A8),Golgi membrane protein 1(GOLM1),small nuclear ribonucleoprotein U1 subunit 70(SNRNP70),hepatocyte growth factor activator(HGFAC),and oxidative stress induced growth inhibitor 1(OSGIN1)]were identified,with IL33,S100A8,and GOLM1 being up-regulated,and SNRNP70,HGFAC,and GOLM1 being down-regulated.These genes distinguished T2DM from healthy controls with AUC values greater than 0.8.Experimental verification using a T2DM rat model confirmed the expression patterns of the hub genes.In vivo data demonstrated that Wedelolactone significantly reduced oxidative stress,inflammation hepatic lipid accumulation,and improved metabolic parameters such as fasting blood glucose,glycated serum protein,and insulin resistance.CONCLUSION:These findings highlight the critical roles of IL33,S100A8,GOLM1,SNRNP70,HGFAC,and OSGIN1 as biomarkers in OS-related T2DM and suggest that Wedelolactone may be a promising TCM-based therapeutic candidate for T2DM.

摘要The term“worm medicines”(Chong Yao)in Traditional Chinese Medicine encompasses a diverse category of small animal-derived therapeutics,including arthropods,amphibians,and reptiles,rather than strictly vermiform organisms.Many of these animals,such as scorpions,centipedes,toads,and horseflies,are toxic,and have been employed in clinical practice for centuries,despite limited understanding of their active compounds and underlying modes of action.Among the bioactive constituents extracted from these taxa,peptides represent a particularly promising class,with over 2000 identified to date.These peptides are primarily secreted from specialized exocrine glands,including venom,salivary,and cutaneous glands,and are characterized by high structural diversity and target specificity.Their potent modulatory effects on the nervous,cardiovascular,and immune systems make them excellent candidates for drug discovery and development.This review examines peptide-based compounds derived from the exocrine secretions of venomous and poisonous taxa historically employed in traditional Chinese“worm”medicines.Emphasis is placed on their biological origins,structural features,and pharmacological activities,highlighting their significant potential as a rich and largely untapped resource for modern therapeutic discovery.

摘要OBJECTIVE:To evaluate the efficacy of Traditional Chinese Medicine(TCM)psychosomatic integration therapy in treating subthreshold depression(SD).METHODS:A multicenter randomized controlled trial was conducted.Eligible participants were evaluated by physicians and randomly assigned to either the intervention or control group.The intervention group received group psychotherapy,Jue tune music therapy,and Yuleyin oral formula(郁乐饮).The control group received only group psychotherapy.The intervention period lasted 12 weeks,followed by a 12-week follow-up.The primary outcome was the Center for Epidemiologic Studies Depression Scale(CES-D)score at week 12.Secondary outcomes included dropout rate,scores of the Hamilton Anxiety Scale(HAMA)and the Hamilton Depression Rating Scale(HAMD-17)at week 12,and CES-D scores at weeks 4,8,16,20,and 24.RESULTS:A total of 505 patients were randomly allocated to the two groups,and 496 participants[77%female;(38±16)years]were included in the final analysis.The primary outcome showed no statistically significant difference in CES-D scores between the intervention and control groups at week 12(P>0.05).However,the intervention group exhibited significant reductions in CES-D scores at weeks 4,8,12,16,20,and 24 compared to baseline,and at week 24 compared to week 12(P<0.0001).The control group also showed significant reductions in CES-D scores at weeks 4 and 8 during the intervention period(P<0.0001).Although no significant differences were observed between groups at each specific time point,the intervention group showed a more consistent downward trend.Additionally,both HAMD and HAMA scores significantly decreased from baseline to week 12 in both groups(P<0.0001).CONCLUSIONS:TCM psychosomatic integration therapy,which includes group psychotherapy,Jue tune music therapy,and Yuleyin oral formula,may be effective in improving symptoms of SD,with good safety and feasibility.The complete TCM intervention demonstrated better long-term effectiveness than group psychotherapy alone.Further high-quality studies are needed to validate these findings.

摘要Against the backdrop of China's aging society and under the macro policy of building a Healthy China proposed at the Fifth Plenary Session of the 18th Central Committee of the Communist Party of China,the dissemination of Traditional Chinese Medicine(TCM)health knowledge has emerged as particularly crucial and imperative.This paper systematically examines the current status of TCM health knowledge dissemination in China.It identifies multiple challenges in the dissemination process,including multidimensional constraints related to content dissemination,media channels,and audience characteristics.Drawing on these findings,this paper proposes a strategic framework centered on high-quality content,with standardized media platforms as key hubs and audience empowerment as the ultimate goal.

摘要Objective To develop a depression recognition model by integrating the spirit-expression diagnostic framework of traditional Chinese medicine(TCM)with machine learning algorithms.The proposed model seeks to establish a TCM-informed tool for early depression screening,thereby bridging traditional diagnostic principles with modern computational approaches.Methods The study included patients with depression who visited the Shanghai Pudong New Area Mental Health Center from October 1,2022 to October 1,2023,as well as students and teachers from Shanghai University of Traditional Chinese Medicine during the same period as the healthy control group.Videos of 3–10 s were captured using a Xiaomi Pad 5,and the TCM spirit and expressions were determined by TCM experts(at least 3 out of 5 experts agreed to determine the category of TCM spirit and expressions).Basic information,facial images,and interview information were collected through a portable TCM intelligent analysis and diagnosis device,and facial diagnosis features were extracted using the Open CV computer vision library technology.Statistical analysis methods such as parametric and non-parametric tests were used to analyze the baseline data,TCM spirit and expression features,and facial diagnosis feature parameters of the two groups,to compare the differences in TCM spirit and expression and facial features.Five machine learning algorithms,including extreme gradient boosting(XGBoost),decision tree(DT),Bernoulli naive Bayes(BernoulliNB),support vector machine(SVM),and k-nearest neighbor(KNN)classification,were used to construct a depression recognition model based on the fusion of TCM spirit and expression features.The performance of the model was evaluated using metrics such as accuracy,precision,and the area under the receiver operating characteristic(ROC)curve(AUC).The model results were explained using the Shapley Additive exPlanations(SHAP).Results A total of 93 depression patients and 87 healthy individuals were ultimately included in this study.There was no statistically significant difference in the baseline characteristics between the two groups(P>0.05).The differences in the characteristics of the spirit and expressions in TCM and facial features between the two groups were shown as follows.(i)Quantispirit facial analysis revealed that depression patients exhibited significantly reduced facial spirit and luminance compared with healthy controls(P<0.05),with characteristic features such as sad expressions,facial erythema,and changes in the lip color ranging from erythematous to cyanotic.(ii)Depressed patients exhibited significantly lower values in facial complexion L,lip L,and a values,and gloss index,but higher values in facial complexion a and b,lip b,low gloss index,and matte index(all P<0.05).(iii)The results of multiple models show that the XGBoost-based depression recognition model,integrating the TCM“spirit-expression”diagnostic framework,achieved an accuracy of 98.61%and significantly outperformed four benchmark algorithms—DT,BernoulliNB,SVM,and KNN(P<0.01).(iv)The SHAP visualization results show that in the recognition model constructed by the XGBoost algorithm,the complexion b value,categories of facial spirit,high gloss index,low gloss index,categories of facial expression and texture features have significant contribution to the model.Conclusion This study demonstrates that integrating TCM spirit-expression diagnostic features with machine learning enables the construction of a high-precision depression detection model,offering a novel paradigm for objective depression diagnosis.

摘要Chronic heart failure(CHF) remains a global health challenge with limited therapeutic options. Mitochondrial dysfunction is a key pathological feature, and traditional Chinese medicine(TCM) shows unique potential in targeting this mechanism. Evidence from human and animal models of heart failure indicates that TCM can restore mitochondrial function by regulating mitochondrial Ca2+ homeostasis, oxidative stress, energy metabolism, mitochondrial dynamics, and mitophagy. TCM-based treatment of CHF offers notable clinical advantages, including improved therapeutic efficacy, enhanced cardiac function, and reduced incidence of major cardiovascular events. Experimental studies demonstrate that TCM decoctions and monomers modulate signaling pathways such as PPAR–RXRα, NF-κB, and PI3K/AKT to alleviate oxidative stress. TCM also increases AMPK activity via phosphorylation of PGC-1α, indirectly promoting mitochondrial biogenesis;attenuates calcium influx and enhances Ca2+ reuptake, thereby ameliorating myocardial mitochondrial dysfunction in CHF;and improves CHF by rebalancing mitochondrial dynamics and autophagy.

摘要Objective To address the challenges of systematically identifying herb pairs in traditional Chinese medicine(TCM),we proposed HerbGL,a framework for predicting potential herb pairs that integrates network propagation and graph regularization.Methods Based on the assumption that herbal actions induce subtle perturbations in biological systems,a framework named HerbGL was proposed.Random walk with restart(RWR)was first applied to the protein-protein interaction(PPI)network to reconstruct herb-specific perturbation effects and generate weighted subnetworks.Then,to quantify affinity between herb pairs,two network-proximity metrics,Closeness and PageRank,were computed from the weighted subnetworks to construct herb-pair affinity matrices.Finally,these matrices,together with known herb pairs(derived from co-occurrence analysis of TCM formulas with a threshold determined from the inflection point of the frequency distribution),were incorporated into a graph regularization model to predict potential herb pairs.Model performance was assessed through baseline comparison,ablation and robustness experiment under different ratios of positive and negative samples,using the area under the receiver operating characteristic curve(AUROC),the area under the precision-recall curve(AUPRC),accuracy,and precision as evaluation metrics.Furthermore,the predicted herb pairs were validated through both literature evidence and Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analyses.Results The weighted subnetworks constructed by RWR provided a refined simulation of herb-specific perturbation effects,which formed the basis for subsequent affinity modeling and prediction.Analysis of herb pair co-occurrence frequencies revealed a marked change around 150,which was selected as the threshold to distinguish herb pairs from non-herb pairs.HerbGL exhibited superior predictive performance compared with baseline models(AUROC=0.9705,AUPRC=0.9555,accuracy=0.7266,precision=0.9706).Ablation results showed that removing the Closeness and PageRank metrics substantially degraded performance(AUROC=0.8191,AUPRC=0.8768),confirming their necessity.Robustness evaluation under an imbalanced positive-to-negative sample ratio of 1:5 yielded AUROC=0.9696 and AUPRC=0.8404,indicating stable predictive ability.Moreover,multiple case studies further validated the rationality of the predicted herb pairs,such as Fangfeng(Saposhnikoviae Radix)and Qingpi(Citri Reticulatae Pericarpium Viride)which are recorded in Liangpeng Huiji(《良朋汇集》,Collection of Excellent Recipes)Vol.3:Fangfeng Shengma Tang(防风升麻汤).Additionally,pathway enrichment analysis of the Renshen(Ginseng Radix et Rhizoma)and Lianqiao(Forsythiae Fructus)pair further supported the biological plausibility of their compatibility.Conclusion HerbGL offers an effective and biologically informed framework for identifying herb pairs in TCM.Beyond improving herb pair prediction,the framework also provides data support for research on herb compounds and mechanisms,thereby supporting data-driven exploration of TCM compatibility.

摘要Traditional villages are regarded as living fossils of human civilization,and their sustainable development has attracted widespread global attention.However,balancing the conservation of cultural heritage in traditional villages with residents’modern living needs remains a critical challenge.Despite extensive research,no viable pathway has been identified to resolve this contradiction.The current study innovatively proposes a tripartite framework based on a neo-endogenous development framework to revitalize traditional villages into a livable,workable,beautiful,and harmonious countryside(LWBHC).The framework is a multifaceted concept that includes cultural living inheritance,space adaptation renewal,and industrial value transformation.To deeply understand the application and effectiveness of this framework,this study employs a mixed-methods approach,combining qualitative and quantitative analyses while focusing on two traditional villages in southwestern China for empirical validation.The results indicate that the proposed framework demonstrates strong explanatory power and practical applicability in guiding the construction of LWBHC in traditional villages,effectively revealing the development logic and evolutionary characteristics of the case villages.Furthermore,specific implementation strategies based on the three-dimensional framework are proposed to advance the transformation of traditional villages into LWBHC.This research provides a replicable model for conserving and developing traditional villages in other regions of China and similar global contexts,enabling more traditional villages to achieve harmonious development while perpetuating human civilization.

摘要The philosophy of“treating disease before its onset”is a fundamental concept of traditional Chinese medicine(TCM),permeating its diagnostic and therapeutic framework,and is central to clinical practice.However,current TCM diagnostic and treatment models for the“predisease to disease”window period face several limitations,including the lack of comprehensive clinical parameters,difficulties in characterizing and integrating heterogeneous multimodal data,and insufficient dynamic precision in interventions and efficacy evaluations.To address these issues,guided by Professor Candong Li’s theory of TCM stateology,this study focuses on integrating objective multimodal data.It proposes a new model for personalized TCM diagnosis and treatment targeting the“pre-disease to disease”window period.This approach first proposes the idea of restructuring the conceptual framework of“symptom”and integrating multi-source heterogeneous data at macroscopic,mesoscopic,and microscopic levels to form a three-dimensional assessment indicator system.By integrating graph neural networks,convolutional neural networks,attention mechanisms,and knowledge graph-guided weight allocation,this approach enables collaborative representation,alignment,and fusion of multi-source data.Subsequently,it plans to construct a multimodal fusion model at both feature and decision levels,in order to establish mappings between indicators and TCM state elements,and to screen key indicators characterizing pathological evolution during the window period.Furthermore,it proposes a technical path for enhancing model interpretability using methods such as SHapley Additive exPlanations(SHAP)and Ablation-CAM++.Finally,with state assessment as the core,it proposes the concept of constructing a dynamic evaluation method for individualized diagnosis and treatment based on time-series data analysis using algorithms such as long short-term memory(LSTM)networks and gated recurrent units(GRUs).Moreover,a causal inference framework and semi-supervised learning strategies are introduced to enable quantitative evaluation of individual intervention effects and to provide interpretable therapeutic feedback,forming a complete technical path from data representation and fusion,weight adjustment,and interpretability analysis,to dynamic diagnosis feedback.This study aims to address deficiencies in the current TCM diagnosis and treatment model during the“pre-disease to disease”window period and to provide an operational framework for the clinical practice of TCM’s“treating disease before its onset”.

摘要Background:Cyperi Rhizoma,derived from Cyperus rotundus L.,is a widely used medicinal herb in traditional Chinese medicine(TCM),with Shandong Province recognized as its geo-authentic habitat.However,the quality of Cyperi Rhizoma varies significantly across different regions,potentially influencing its therapeutic efficacy.This study investigates the influence of geographic origin on the chemical composition and overall quality of Cyperi Rhizoma.Methods:A comprehensive approach,including traditional quality assessment,GC-MS(g as c hromatography-m ass s pectrometry),RP-HPLC(r everse p hase h igh-p erformance l iquid c hromatography),and FT-IR(f ourier t ransform i nfrared s pectroscopy)techniques,was employed to analyze Cyperi Rhizoma samples from Shandong Province.These methods examined the physical appearance,chemical profile,and content variations,particularly focusing onα-cyperone.Results:Traditional quality assessments revealed noticeable differences in the external characteristics of the samples.GC-MS analysis identified a variety of unique chemical constituents,while RP-HPLC and FT-IR showed significant variations inα-cyperone content,with higher levels found in Shandong samples.Conclusion:These results demonstrate that geographic origin is a critical determinant of Cyperi Rhizoma quality,with Shandong specimens exhibiting superiorα-cyperone levels and characteristic phytochemical profiles.This validates the geo-authenticity concept in TCM and provides actionable data for developing evidence-based quality standards,suggesting that provenance should be prioritized in medicinal material selection and pharmacopeial specifications.

摘要Objective To elucidate the biological basis of traditional Chinese medicine(TCM)syndromes from the perspective of“same syndrome,different diseases”in patients with chronic hepatitis B(CHB),liver cirrhosis(LC),and hepatocellular carcinoma(HCC),thereby providing a complementary approach for the diagnosis and treatment of chronic liver diseases(CLD).Methods To investigate the dynamic characteristics of TCM syndromes in CLD,transcriptomic profiling of peripheral blood mononuclear cells(PBMCs)was performed from patients with CHB,LC,or HCC presenting with three TCM syndromes:liver gallbladder dampness heat syndrome(LGDHS),liver depression spleen deficiency syndrome(LDSDS),and liver kidney Yin deficiency syndrome(LKYDS).These participants were recruited at Shuguang Hospital Affiliated to Shanghai University of Traditional Chinese Medicine between August 1,2018 and December 31,2021.Differentially expressed genes(DEGs)were identified using the random variance model(RVM)F test with false discovery rate(FDR)correction.Principal component analysis(PCA)and unsupervised hierarchical clustering were applied to visualize sample grouping.Dynamic network biomarkers(DNB)analysis was employed to detect critical transition stages during syndrome evolution,followed by Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway enrichment analyses to characterize the functional roles and pathway involvement of the DNB members.Random forest(RF)analysis and the area under the receiver operating characteristic(ROC)curves(AUC)were used to rank the importance of candidate genes.External validation was performed using microarray data from an independent CLD cohort(GSE89377)and RNA-seq data from The Cancer Genome Atlas Liver Hepatocellular Carcinoma(TCGA-LIHC)dataset.Additionally,reverse transcription quantitative polymerase chain reaction(RT-qPCR)was performed on an independent cohort of LC patients to validate the expression levels of the candidate genes.Results The study included a total of 132 participants.DNB analysis identified LDSDS stage as a critical tipping point during TCM syndrome evolution across CHB,LC,and HCC.The phosphoinositide 3-kinase/protein kinase B(PI3K-AKT)signaling pathway was consistently enriched in the DNB analysis across all three types of CLD,suggesting its potential involvement in the critical transition of TCM syndromes.Among the 24 core DNB members of the PI3K-AKT pathway,four genes—integrin subunit beta 1(ITGB1),collagen type IV alpha 1 chain(COL4A1),collagen type IV alpha 2 chain(COL4A2),and DNA damage inducible transcript 3(DDIT3)—were identified by RF analysis(Gini score>1)and ROC analysis.ROC analysis demonstrated high discriminative ability for distinguishing LGDHS from LKYDS in CHB patients,with AUC of 0.7891 for ITGB1,0.7070 for COL4A1,0.7148 for COL4A2,and 0.8945 for DDIT3.In the independent CLD cohort(GSE89377),all four genes showed significant stepwise upregulation from normal to CHB,LC,and HCC(all P<0.05).In the TCGA-LIHC dataset,their expression progressively increased with tumor stage.RT-qPCR validation in an independent LC cohort(30 LGDHS vs.30 LKYDS)confirmed that ITGB1,COL4A2,and DDIT3 were significantly upregulated in LKYDS compared with LGDHS(P=0.0152,0.0186,and 0.0247,respectively),whereas COL4A1 showed a non-significant upward trend(P=0.1201).Conclusion This study introduces a novel approach to understanding the molecular features underlying TCM syndrome evolution in CLD.The PI3K-AKT pathway and four identified genes(ITGB1,COL4A1,COL4A2,and DDIT3)play crucial roles in the transition from excess(LGDHS)to deficiency(LKYDS)via the critical LDSDS stage.These findings offer potential quantitative biomarkers and therapeutic targets for TCM syndrome differentiation and may help arrest syndrome progression in CLD.

摘要Traditional Chinese medicine(TCM)is a holistic medical system that classifies and treats diseases based on the concept of patterns.These patterns describe the pathophysiological process of a disease at a specific stage,reflecting both external signs and internal features.TCM patterns are central to pattern differentiation,treatment,clinical practice,and theoretical development in TCM.However,scientific explanation of TCM patterns remains limited because of subjective diagnostic criteria,the absence of a standardized experimental medical system,and unclear biological mechanisms,which restrict the modernization and globalization of TCM.Addressing challenges such as strong subjectivity in diagnosis,lack of standardized experimental systems,and unclear biological mechanisms is necessary to clarify the scientific meaning of TCM patterns and to provide technical approaches for the modernization and globalization of TCM.A strategy focused on“pathogenic factors,genetic predisposition,and disease progression stages”was adopted.This approach included the following:(Ⅰ)constructing disease-pattern integrated biological models with animal models,organoids,and multi-organ chips;(Ⅱ)applying multi-omics technologies,such as spatial omics,single-cell omics,and dynamic metabolic flux omics;(Ⅲ)using artificial intelligence(AI)and big data for data integration and prediction of pattern evolution;and(Ⅳ)validating formula–pattern associations through the“pattern differentiation through formula efficacy”approach.These strategies directly address the main obstacles in TCM pattern research by providing objective,quantifiable,and reproducible methodologies.Constructing disease-pattern integrated models enabled cross-scale research platforms.Applying multi-omics technologies allowed analysis of complex biological bases.AI and big data approaches addressed challenges related to heterogeneous data.The“formula-based pattern differentiation”approach supported precise interventions and the development of new drugs.This interdisciplinary framework advances TCM pattern research by moving from empirical description to objective quantification.By integrating innovative approaches,the study establishes a foundation for systematic,evidence-based TCM diagnosis and treatment,supporting accuracy and promoting international recognition and modernization of TCM.The study shows that combining multi-omics technologies,AI-driven data analysis,and disease-pattern models enables objective quantification of TCM patterns and clarifies their biological mechanisms.

摘要Traditional Chinese medicine(TCM)has garnered increasing attention globally,with its modernization becoming a prominent research focus both within China and internationally.However,the lack of a precise definition for TCM modernization has hindered clear guidance for its development.Additionally,cancer remains a significant global public health challenge,largely untreatable with current methods.Therefore,a comprehensive understanding of TCM modernization is crucial for its evolution,revolution,drug discovery,and cancer therapy.This study provides an overview of the history,theory,characteristics,and evolution of TCM,highlighting its potential in cancer prevention and treatment.We propose a definition for TCM modernization,innovative Chinese medicine(ICM),and elucidate strategies to elevate TCM from a supporting role to a leading one.Electronic databases such as PubMed,Web of Science,ScienceDirect,and Clinical Trials were utilized to retrieve relevant literature spanning from 1979 to 2024,with most publications being from the last five years,using keywords like“Traditional Chinese medicine”,“Cancer”,“Mechanism”,and“Clinical trial”.In this study,we introduce the theory of TCM modernization following target identification and initial compound screening:ICM,defined by“3 D”elements:definite active ingredient composition and content,determined functional mechanism,and detection through evidence-based medicine.Overall,the“3 D”definition of ICM will establish a standard for ICM,accelerate TCM modernization,enhance drug discovery targeting cancer and various human diseases,and benefit patients worldwide.
