Streptococcus suis has garnered increasing attention due to its implication in severe infections in both swine and humans,as well as its development of multidrug resistance.The phenomenon of collateral sensitivity,whereby resistance to one antibiotic leads to increased sensitivity to another,provides new opportunities for mitigating the evolution of resistance.In this study,we evolved resistance in S.suis to 11 clinically used antibiotics and characterized the resulting collateral sensitivity profiles,revealing a complex network of interactions.Based on our findings,we identified dozens of such drug pairs and demonstrated collateral sensitivity to gamithromycin in ciprofloxacin-resistant S.suis both in vitro and in vivo.Gamithromycin effectively limits the evolution of resistance and reduces the mutant selection window for ciprofloxacinresistant S.suis strains.Mechanistic studies indicated that the heightened sensitivity of ciprofloxacin-resistant S.suis to gamithromycin was associated with increased intracellular gamithromycin accumulation due to membrane potential alterations and reduced functions of proton motive force(PMF)-dependent efflux pumps.Furthermore,collateral sensitivitybased treatments significantly resensitized ciprofloxacin-resistant S.suis strains to gamithromycin,resulting in superior efficacy,lower pharmacodynamic targets,and higher treatment success rates in a murine thigh infection model.Our results indicate that gamithromycin sensitivity in S.suis is a collateral consequence of resistance to ciprofloxacin,providing valuable insight for the strategic design of collate ral sensitivity-based antibiotic therapies for S.suis infections. 展开更多

In this study, a cerebral infarction model of ischemiaeperfusion injury received electroacupuncture at GV26(15 Hz and 1 m A, continuous wave [biphasic pulses], for 5 minutes). Electroacupuncture effectively promoted regional cerebral blood flow on the infarct and non-infarct sides, increased infarct lesions, lectin, and number of blood vessels, upregulated von Willebrand factor and cell proliferation marker Ki67 expression, and diminished neurological severity score. These findings confirm that electroacupuncture at GV26 promotes establishment of collateral circulation and angiogenesis, and improves neurological function. 展开更多

Hemiplegia caused by stroke indicates dysfunction of the network between the brain and limbs, namely collateral shock in the brain. Contralateral needling is the insertion of needles into acupoints on the relative healthy side of the body to treat diseases such as apoplexy. However, there is little well-designed and controlled clinical evidence for this practice. This study investigated whether contralateral needling could treat hemiplegia after acute ischemic stroke in 106 randomly selected patients with acute ischemic stroke. These patients were randomly assigned to three groups: 45 in the contralateral needling group, receiving acupuncture on the unaffected limbs; 45 in the conven- tional acupuncture group, receiving acupuncture on the hemiplegic limbs; and 16 in the control group, receiving routine treatments without acupuncture. Acupuncture at acupoints Chize (LU5) in the upper limb and Jianliao (TEl4) in the lower limb was performed for 45 minutes daily for 30 consecutive days. The therapeutic effective rate, Neurological Deficit Score, Modified Barthel Index and FugI-Meyer Assessment were evaluated. The therapeutic effective rate of contralateral nee- dling was higher than that of conventional acupuncture (46.67% vs. 31.11%, P 〈 0.05). The neuro- logical deficit score of contralateral needling was significantly decreased compared with conven- tional acupuncture (P 〈 0.01). The Modified Barthel Index and FugI-Meyer Assessment score of contralateral needling increased more significantly than those of conventional acupuncture (both P 〈 0.01). The present findings suggest that contralateral needling unblocks collaterals and might be more effective than conventional acupuncture in the treatment of hemiplegia following acute ischemic stroke. 展开更多

Background To investigate the effects of collateral coronary circulation on the outcome of the patients with anterior myocardial infarction(MI)with left anterior desending artery occlusion abruptly.Methods Data of 189 patients with acute anterior MI who had a primary percutaneous coronary intervention(PCI)in the first 12 h from the onset of symptoms between January 2004 and December 2008 were retrospective analyzed.Left anterior descending arteries(LAD)of all patients were occluded.LADs were reopened with primary PCI.According to the collateral circulation,all patients were classified to two groups:no collateral group(n=111),patients without angiographic collateral filling of LAD or side branches(collateral index 0)and collateral group(n=78),and patients with angiographic collateral filling of LAD or side branches(collateral index 1,2 or 3).At one year's follow-up,the occurrence of death,reinfarctlon,stent thrombosis(ST),target vessel revascularization and readmission because of heart failure were observed.Results At one year,the mortality was lower in patients with collateral circulation compared with those without collateral circulation(1%vs.8%,P=0.049),whereas there Were no differences in the occurrence of reinfarction,ST,target vessel revascularization and readmission because of heart failure.The occurrence of composite of endpoint was lower in patients with collateral circulation compared with those without collateral circulation(12%vs.26%;P=0.014).Conclusions Pre-exist collateral circulation may prefigure the satisfactory prognosis to the patients with acute anterior MI after primary PCI in the first 12 h of MI onset. 展开更多

BACKGROUND: Hepatobiliary cystadenocarcinoma represents a rare epithelial malignant tumor derived from the intrahepatic bile duct. METHODS: A 71-year-old woman, who had undergone laparoscopic drainage of a cystic lesion of the right hepatic lobe, was misdiagnosed as having hepatic echinococcal disease, and received intracystic infusion of 95% ethanol four years ago. She was admitted to our hospital for further treatment. RESULTS: Physical examination revealed dilated superficial veins across the right abdominal wall. After mapping the direction of blood flow in these vessels, we assumed that this was a sign of inferior vena cava obstruction. Abdominal ultrasound, computed tomography, magnetic resonance imaging combined with magnetic resonance angiography showed a large cystic mass in the right upper quadrant and epigastrium, displacing the adjacent structures, adherent to the inferior vena cava, which was not patent, resulting in dilation of superficial epigastric veins. The patient underwent an exploratory laparotomy. Total excision of the huge mass measuring 16×15 cm was possible under selective vascular exclusion of the liver. Removal of the tumor resulted in immediate restoration of flow in the inferior vena cava. On the basis of the pathology and findings of immunohistochemical analysis, a hepatobiliary cystadenocarcinoma was diagnosed.CONCLUSIONS: In the present case, hepatobiliary cystadenocar-cinoma was accompanied by dilated superficial venous collaterals due to inferior vena cava obstruction. Selective vascular exclusion of the liver allowed a safe oncological resection of the tumor. 展开更多

In acute ischemic stroke,collateral circulation plays an important role in maintaining blood flow to the tissue that is at risk of progressing into ischemia,and in increasing the successful recanalization rate without hemorrhagic transformation.We have reported that well-developed collateral circulation is associated with smaller infarct volume and better long-term neurological outcome,and it disappears promptly once the effective recanalization is achieved.Contrary to the belief that collateral vessels develop over time in chronic stenotic condition,there exists a phenomenon that collateral circulation develops immediately in acute stenosis or occlusion of the arteries and it seems to be triggered by fluid shear stress,which occurs between the territories of stenotic/occluded arteries and those fed by surrounding intact arteries.We believe that this acute development of collateral circulation is a target of novel therapeutics in ischemic stroke and refer our recent attempt in enhancing collateral circulation by modulating sphingosine-1-phosphate receptor 1,which is a known shear-stress mechanosensing protein. 展开更多

BACKGROUND: Early hepatic artery thrombosis (eHAT) has been recognized as an important cause of graft loss and mortality. However, the incidence, etiology and outcome are not dear, especially for children. The present study was to investigate the formation of collateral artery flow after irreversible eHAT and its impact on patient's prognosis. METHODS: We analyzed eHAT after liver transplantation in children from October 2006 to April 2015 in our center, illustrated the formation of collateral hepatic artery flow after irreversible eHAT and explored the diagnosis, complications, treatment and prognosis. The basic and follow-up ultrasonographic images were also compared. RESULTS: Of the 330 pediatric liver recipients, 22 (6.67%) de- veloped eHAT within 1 month. Revascularization attempts in- duding surgical thrombectomy, interventional radiology and conservational treatment (thrombolysis) were successful in 5 patients. Among the 17 patients who had irreversible eHAT, follow-up ultrasonography revealed that collateral artery flow was developed as early as 2 weeks after eHAT. Liver abscess and bile duct complication occurred secondary to eHAT in variable time. CONCLUSIONS: Collateral arterial formation is a compensatory adaptation to eHAT to supply blood to liver grafts. However, the severe bile duct damage secondary to eHAT is irreversible and retransplantation is unavoidable. 展开更多

Objective To explore effective therapeutic method for the treatment of ocular myasthenia gravis.Methods Sixty cases of ocular myasthenia gravis were selected.The acupuncture was applied at Dàzhuī(大椎GV 14),Bǎihuì(百会GV 20),Shàngxīng(上星GV 23)of Governor Vessel,and its crossing points with Yinqiao and Yangqiao Meridian,such as Jīngmíng(睛明BL 1),shēnmài(申脉BL 62),Dìcāng(地仓ST 4),Jūliáo(巨髎ST 3),Chéngqì(承泣ST 1),etc.Besides,ginger-partition moxibustion was applied at Yángbái(阳白GB 14)and Sìbái(四白ST 2).The plum-blossom needle was applied at upper limb distribution of lung channel of hand taiyin,heart channel of hand shaoyin and pericardium channel of hand jueyin.The patients’clinical symptoms,potential degradation percentages of repetitive nerve stimulation(RNS)in low frequency were observed.Results 8 cases were clinically cured,20 cases were basically cured,12 cases were remarkably effective,15 cases were effective and 5 cases were failed.The total effective rate was 91.7%.Compared with before the treatment,the potential degradation percentages of repetitive nerve stimulation in low frequency were lowered[(19.44±6.28)%vs(8.78±3.12)%,(P0.05)].Conclusion Acupuncture and moxibustion at extra-meridian and collateral for ocular myasthenia gravis could achieve positive efficacy. 展开更多

Objective:To observe the clinical efficacy of knee joint pain treated with scraping combined with collateral bloodletting therapy in comparison with the routine acupuncture techniques.Methods:From March 2017 through to September 2017,60 outpatients of knee joint pain were collected from the Acupuncture-Moxibustion Department.They were randomly divided into the group A(treated with scraping combined with collateral bloodletting therapy)and the group B(treated with the simple needling therapy of acupuncture),30 cases in each one.Once treatment was given every day and10 times consist of one course,two courses of treatment were required.At the end of treatment,the visual analogue scale(VAS)was adopted to evaluate the pain index,tenderness index and the overall effect of the patients in the two groups.Based on the Lysholm knee scale,the scores were compared in the patients of the two groups before and after treatment.Results:The total effective rate was 96.7%in the group A and was 86.7%in the group B,indicating the significant difference(P〈0.05).VAS value was 3.03±0.62 in the group A and was 3.67±0.74 in the group B after treatment,indicating that difference is statistically significant(P〈0.05).The Lysholm knee score was 89.2±4.8 in the group A and was 82.4±2.6 in the group B after treatment,indicating that difference is statistically significant(P〈0.05).Conclusion:The scraping combined with collateral bloodletting therapy is significant in the efficacy on knee joint pain and feasible in the clinical guidance. 展开更多

In this study, the theoretical knowledge, application method and clinical experience of professor JIA Chun-sheng in blooddraining therapy on the back were mainly introduced. Professor JIA Chun-sheng focuses on two key viewpoints: "Attacking pathogenic qi is to supplement deficiency," and "Eliminating stagnant pathogen can clean the intestines and stomach, and removing concretions and conglomerations can harmonize Fing and wei", which have been considered as the important theoretical bases of direction of clinical practice. Generally, professor JIA Chun-sheng conducts blood-draining therapy at Dazhui (大椎 GV 14), Feishu (肺俞 BL 13), Xinshu (心俞 BL 15), Geshu (膈俞 BL 17), Ganshu (肝俞 BL 18), Pishu (脾俞 BE 20) and Shenshu (肾俞 BL 23) on the back in order to improve the blood circulation in local and corresponding zong-fu organs, thus being beneficial to harmonize qi and blood, or eliminate dampness, heat, wind, stasis and toxins. BACKGROUND Recently,the use of ligament advanced reinforcement system(LARS)artificial ligament,a new graft which has several unique advantages such as no donor-site morbidity,early recovery and no risk of disease transmission which has been a significant breakthrough for anatomical ligament reconstruction.Growing studies suggested that the special design of the LARS ligament with open fibers in its intra-articular part was believed to be more resistant to torsional fatigue and wearing.However,the safety and efficacy of LARS artificial ligament for ankle joint lateral collateral ankle ligament reconstruction has not been defined to date.AIM To evaluate the clinical results of all-arthroscopic anatomical reconstruction of ankle joint lateral collateral ligaments with the LARS artificial ligament for chronic ankle instability.METHODS Twenty-two patients with chronic lateral instability underwent anatomical reconstruction of the lateral collateral ligaments of ankle with LARS artificial ligament.The visual analogue score(VAS),American Orthopaedic Foot and Ankle Society score(AOFAS score)and Karlsson score were used to evaluate the clinical results before and after surgery.RESULTS A total of 22 patients(22 ankles)were followed up for a mean of 12 mo.All patients reported significant improvement compared to their preoperative status.The mean AOFAS score improved from 42.3±4.9 preoperatively to 90.4±6.7 postoperatively.The mean Karlsson score improved from 38.5±3.2 preoperatively to 90.1±7.8 postoperatively.The mean VAS score improved from 1.9±2.5 preoperatively to 0.8±1.7 postoperatively.CONCLUSION All-arthroscopic anatomical reconstruction of the lateral collateral ligaments with LARS artificial ligament achieved a satisfactory surgical outcome for chronic ankle instability. 展开更多

The angina pectoris in coronary heart disease belongs to the category of"chest impediment"in traditional Chinese medicine,and belongs to the syndrome of root deficiency with tip excess.Its main pathogenesis is obstruction of the heart collaterals,and treatment mainly adopts dredging collaterals and relieving pain.Based on the treatment principles of dredging collaterals with pungent,aiming at the etiology,pathogenesis and disease characteristics of angina pectoris in coronary heart disease,this article systematically analyzes the Method of Dredging Collaterals with Pungent including the methods of dredging collaterals with pungent moisture,dredging collaterals with pungent warm,dredging collaterals with pungent aroma,and dredging collaterals with insect type drugs,to provide ideas for the treatment of angina pectoris in coronary heart disease. 展开更多

Objective To investigate therelationships between serum concentration of insulin -like growth factor - I (IGF-I) and left ventricular function as well as coronary collateral circulation in patients with coronary artery disease (CAD) . Methods In 41 patients with CAD and 15 control subjects without CAD, the concentrations of serum IGF - I were measured using radioimmunoassay. The relationships between the concentration of serum IGF - I and Leaman coronary artery score, Rentrop grade of coronary collateral circulation, left ventricular ejection fraction (LVEF) as well as left ventricular wall motion Cortina score were assessed. Results 1. There was no significant difference in the mean level of serum IGF -I between the CAD group and the control group (107. 92±44.74 ng/ml vs 113.05 ±33. 65 ng/ml, P> 0. 05), but the IGF - I concentrations in the subgroup with collateral circulation were significantly greater than that in the control group (147. 33 ±29. 92 ng/ml vs 113. 05±33. 65 ng/ml, P < 0. 01) or in the subgroup without collateral circulation (147. 33 ±29. 92 ng/ml vs 80. 01±29. 75 ng/ml , P < 0. 01). 2. The serum concentration of IGF -I had no significant correlation to the Leaman coronary artery score. 3. The serum level of IGF -I had significantly positive correlation to both LVEF ( r = 0. 45, P < 0. 001) and the Rentrop grade of coronary collateral circulation ( r = 0. 74, P < 0. 001), and was negatively related to the left ventricular wall motion Cortina score (r = -0. 53, P < 0. 001). 4. The Leaman coronary artery score had no significant correlation to the Rentrop grade of coronary collateral circulation. 5. The Leaman coronary artery score was related to neither the LVEF nor the Cortina score in the whole CAD group. In the subgroup without coronary collateral circulation, however, the Leaman score had significantly negative correlation to LVEF ( r = - 0. 46, P < 0. 05) and positive correlation to the Cortina score (r = 0. 47, P < 0. 05) . Conclusions The serum concentration of IGF -I was associated with both left ventricular function and coronary collateral circulation in patients with CAD. IGF -I may play a role in promoting coronary collateral circulation and in protecting left ventricular function in patients with coronary artery disease. 展开更多

Acute myocardial infarction model was established in all dogs, Salvia Miltiorrhiza or saline was administered to the 2 groups respectively for 21 days. Coronary angiography was performed before and after ligation of coronary artery. The capillaries and distribution vessel density in tissue sections of myocardia were assessed. Angiography 2 h and 22 days after ligation of coronary artery revealed significant augmentation of collateral vessels, and capillaries and distribution vessel density in ischemic zone on day 22 showed statistically significant augmentation in group D as compared with that of controls ( P <0. 05 and P <0. 01). It is concluded that Salvia Miltiorrhiza might improve the opening and formation of coronary collateral circulation of ischemic myocardia in dogs and thereby protecting myocardia from ischemia. 展开更多

The ulnar collateral ligament(UCL) is a vital structure to the overhead athlete,especially the baseball pitcher.For reasons not completely understood,UCL injurieshave become increasingly more common in major league baseball(MLB) pitchers over the past 10 years.UCL reconstruction(UCLR) is the current gold standard of treatment for these injuries in MLB pitchers who wish to return to sport(RTS) at a high level and who have failed a course of non-operative treatment.Results following UCLR in MLB pitchers have been encouraging,with multiple RTS rates now cited at greater than 80%.Unfortunately,with the rising number of UCLR,there has also been a spike in the number of revision UCLR in MLB pitchers.Similar to primary UCLR,the etiology of the increase in revision UCLR,aside from an increase in the number of pitchers who have undergone a primary UCLR,remains elusive.The current literature has attempted to address several questions including those surrounding surgical technique(method of exposure,graft choice,management of the ulnar nerve,concomitant elbow arthroscopy,etc.),post-operative rehabilitation strategies,and timing of RTS following UCLR.While some questions have been answered,many remain unknown.The literature surrounding UCLR in MLB pitchers will be reviewed,and future directions regarding this injury in these high level athletes will be discussed. 展开更多

BACKGROUND The prognostic value of coronary collateral circulation(CC)in patients undergoing chronic total occlusion(CTO)percutaneous coronary intervention(PCI)is underdetermined.The purpose of the study was to assess the prognostic value of current two CC grading systems and their association with long-term outcomes in patients with CTO underwent PCI.METHODS We consecutively enrolled patients with single-vessel CTO underwent PCI between January 2010 and December2013.All patients were categorized into well-developed or poor-developed collaterals group according to angiographic Werner's CC(grade 2 vs.grade 0–1)or Rentrop(grade 3 vs.grade 0–2)grading system.The primary endpoint was 5-year cardiac death.RESULTS Of 2452 enrolled patients,the overall technical success rate was 74.1%.Well-developed collaterals were present in686 patients(28.0%)defined by Werner's CC grade 2,and in 1145 patients(46.7%)by Rentrop grade 3.According to Werner's CC grading system,patients with well-developed collaterals had a lower rate of 5-year cardiac death compared with those with poordeveloped collaterals(1.6%vs.3.3%,P=0.02),those with suboptimal recanalization was associated with higher rate of 5-year cardiac death compared with optimal recanalization(4.7%vs.0.8%,P=0.01)and failure patients(4.7%vs.1.6%,P=0.12).However,the similar effect was not shown in Rentrop grading system.CONCLUSIONS In patients with the single-vessel CTO underwent PCI,well-developed collaterals by Werner's CC definition were associated with lower rate of 5-year cardiac death.Werner's CC grading system had a greater prognostic value than Rentrop grading system in patients with CTO underwent PCI. 展开更多

Embolization of collateral veins is often treated with rigid coils(Gianturco and interlocking detachable coils type).However,when dealing with tortuous and dilated collateral veins,there is a high risk for technical failure and coil migration due to inflexibility of the coils.To safely and successfully solve this problem,Guglielmi detachable coils(GDC) can be used for embolization.Their flexibility allows for easy navigation in tortuous veins,low risk of unintended coil release or coil migration,and safe deployment.A 12-year-old girl with a single ventricle had severe cyanosis and a low exercise tolerance 5 years after Fontan procedure.The symp-toms were caused by a tortuous and dilated collateral from the left phrenic vein into the left pulmonary vein,forming a right-to-left shunt.The collateral,which had a large diameter and high flow,and therefore a high risk of coil migration,was successfully embolized with 8 GDC.There were no complications such as coil migration or cerebral infarction.Transcatheter embolization increased her systemic oxygen saturation from 81%-84% to 94%-95%,and increased her ability to exercise.The embolization procedure using flexible GDC was low risk compared with other rigid coil embolization techniques when performing embolization of tortuous and dilated collateral veins. 展开更多
