This study investigated the spatial and temporal patterns of ablation,debris-cover characteristics,and crevasse distribution on Baishui River Glacier No.1 based on using multi-year in situ observations(2019-2022).The results reveal a strong elevation-dependent ablation gradient,with annual ablation rates decreasing from 3.8-5.4 m w.e.at 4550-4600 m asl to 1.1-2.1 m w.e.above 4750 m asl,following a gradient of-1.53 m w.e.(100 m)-1.Ablation along the central flowline was 21%-42%higher than along lateral margins.Debris covers approximately 20%of the glacier surface,with 92%concentrated below 4800 m asl.Coverage transitions from near continuous(>80%)below 4600 m asl to a patchy distribution at higher elevations.The debris-ablation relationship is nonlinear:2-5 cm of debris increases melt to 6.7-7.3 cm/day,while thicknesses exceeding 15 cm reduce ablation by 35%-62%.The relationship between melting and debris thickness follows a nonlinear curve(theÖstrem curve),in which thin debris enhances melting relative to bare ice,while thick debris insulates.Crevasses disrupt debris continuity and create localised melt hotspots,complicating traditional mass balance models.By enhancing radiation absorption,hydrothermal coupling,and debris redistribution,they amplify ablation,which is often overlooked.These findings are key to understanding how debris-covered crevasse-rich temperate glaciers respond to climate change. 展开更多

BACKGROUND Management of difficult bile duct stones(DBDS)remains challenging when complete ductal clearance cannot be achieved during the initial endoscopic retrograde cholangiopancreatography(ERCP).Temporary biliary stenting is commonly used in this setting;however,the optimal stent type remains debated.AIM To evaluate outcomes of plastic stents(PS)vs fully covered self-expandable metal stents(cSEMS)after unsuccessful bile duct stone extraction.METHODS This retrospective single-center study included patients who underwent ERCP with PS or cSEMS biliary stent placement between January 2020 and May 2025 due to complex choledocholithiasis following unsuccessful initial ductal clearance.The primary outcome was presence of residual stones at second-look ERCP.Secondary outcomes included reintervention rates and post-ERCP complications.To reduce selection bias,1:1 propensity score matching(PSM)was performed for age,stone size,and stone number.Multivariate logistic regression was used to identify independent predictors of residual stones.RESULTS Seventy-seven patients were included(47 PS and 30 cSEMS).After PSM,48 matched patients(24 per group)were analyzed.Residual stones at second ERCP were more frequent in the PS group than in the cSEMS group before(29.8%vs 10.0%,P=0.036)and after(25.0%vs 8.3%,P=0.041)matching.Re-stenting was required in 17.0%of PS patients,whereas none of the cSEMS patients required repeat stenting(P=0.015 before matching and P=0.020 after matching).Rates of post-ERCP pancreatitis and bleeding were similar between the groups.Multivariate analysis showed that cSEMS placement independently reduced the risk of residual stones(OR=0.31,95%CI:0.11-0.98,P=0.039),whereas larger stone size predicted treatment failure(OR=1.73,95%CI:1.44-3.17,P=0.041).CONCLUSION In patients with DBDS and incomplete initial ductal clearance,temporary cSEMS placement achieved higher bile duct clearance and lower reintervention rates compared with PS,without increasing complications.Prospective randomized studies are needed to confirm these findings. 展开更多

BACKGROUND Hemorrhage following pancreaticobiliary surgery is a high-risk complication,with a mortality rate of 16%-38%.At present,minimally invasive endovascular intervention comprising superselective arterial embolization(SAE)and covered stent implantation(CSI)is the treatment of choice.However,in certain cases,both SAE and CSI become infeasible.AIM To evaluate the effectiveness of coil-assisted N-butyl cyanoacrylate(NBCA)embolization in comparison with that of CSI in managing delayed hemorrhage after hepatobiliary–pancreatic surgery when SAE is infeasible.METHODS Ninety-eight continuous patients(n=105 cases;mean age,58.4 years)with delayed massive hemorrhage who were treated with coil-assisted NBCA embolization(NBCA group,n=45)and/or CSI(CSI group,n=60)were retrospectively evaluated between March 2014 and December 2023.Data on technical and clinical success,30-day mortality,and severe intervention-related adverse events were collected and analyzed.RESULTS The technical and clinical success rates in the NBCA group(100%and 93.3%,respectively)were significantly higher than those in the CSI group(88.3%and 73.3%,respectively),with a statistically significant difference between the two groups(P=0.019 and 0.010,respectively).The 30-day mortality rates and major intervention-related complications were 17.8%and 0%,respectively,in the NBCA group and 18.3%and 1.7%in the CSI group,respectively,with no statistically significant difference between the two groups.CONCLUSION In terms of technical and clinical success,coil-assisted NBCA embolization was more effective than CSI for managing delayed hemorrhage after hepatobiliary–pancreatic surgery when SAE was not feasible. 展开更多

BACKGROUND The treatment of difficult common bile duct stones(CBDS)remains a big challenge around the world.Biliary stenting is a widely accepted rescue method in patients with failed stone extraction under endoscopic retrograde cholangiopancreatography.Fully covered self-expanding metal stent(FCSEMS)has gained increasing attention in the management of difficult CBDS.AIM To manufacture a drug-eluting FCSEMS,which can achieve controlled release of stone-dissolving agents and speed up the dissolution of CBDS.METHODS Customized covered nitinol stents were adopted.Sodium cholate(SC)and disodium ethylene diamine tetraacetic acid(EDTA disodium,EDTA for short)were used as stone-dissolving agents.Three different types of drug-eluting stents were manufactured by dip coating(Stent I),coaxial electrospinning(Stent II),and dip coating combined with electrospinning(Stent III),respectively.The drugrelease behavior and stone-dissolving efficacy of these stents were evaluated in vitro to sort out the best manufacturing method.And the selected stonedissolving stents were further put into porcine CBD to evaluate their biosecurity.RESULTS Stent I and Stent II had obvious burst release of drugs in the first 5 d while Stent III presented controlled and sustainable drug release for 30 d.In still buffer,the final stone mass-loss rate of each group was 5.19%±0.69%for naked FCSEMS,20.37%±2.13%for Stent I,24.57%±1.45%for Stent II,and 33.72%±0.67%for Stent III.In flowing bile,the final stone mass-loss rate of each group was 5.87%±0.25%for naked FCSEMS,6.36%±0.48%for Stent I,6.38%±0.37%for Stent II,and 8.15%±0.27%for Stent III.Stent III caused the most stone mass-loss no matter in still buffer or in flowing bile,which was significantly higher than those of other groups(P0.05)and histopathological examination(P>0.05).CONCLUSION The novel SC and EDTA-eluting FCSEMS is efficient in diminishing CBDS in vitro.When conventional endoscopic techniques fail to remove difficult CBDS,SC and EDTA-eluting FCSEMS implantation may be considered a promising alternative. 展开更多

Objective To evaluate the safety and efficacy of EVT of BBAs in the ICA with a Willis covered stent. Methods 20 consecutive patients(5 men and 15 women) with ruptured BBAs underwent EVT using a Willis covered stent in two institutions from March 2013 to March 2018. Clinical observations, angiographic characteristics, and procedural and follow-up outcomes were retrospectively evaluated. Results 20 consecutive patients(5 men and 15 women) with ruptured BBAs underwent EVT using a Willis covered stent in two institutions from March 2013 to March 2018. Clinical observations, angiographic characteristics, and procedural and follow-up outcomes were retrospectively evaluated. Conclusion Our initial results demonstrate that reconstructive EVT using a Willis covered stent provides a viable approach to treat ICA BBAs. However, an expanded clinical evaluation and larger cohort are needed to confirm the results. 展开更多

Combined studies on the behavior of reactive fragment initiating covered explosive are performed theoretically and experimentally. The results in- dicate that the response of the covered explosive subjected to reactive fragment is not consistent with the classical shock initiation model. When impacting and penetrating into the covered explosive, the reactive fragment releases great amounts of thermo-chemical energy up to 5. 68 times of its kinetic energy into the explosive due to violent chemical reactions. This impact-induced chemical energy re- lease behavior of the reactive fragment significantly enhances the initiation capability and damage effects on the covered explosive. 展开更多

Background:Symptomatic vertebral artery dissecting aneurysm(VADA)is a challenging disease with controversy on treatment strategy due to anatomic configuration and their nature.Moreover,the outcomes of reconstructive treatment have not been well established.Objective:To evaluate the safety and efficacy of reconstructive endovascular treatment(EVT)for symptomatic VADAs with Willis covered stent.Methods:We evaluated retrospectively 13 patients with symptomatic VADAs who treated with Willis covered stent,compared with stent-assisted coiling(SAC)on the characteristics,posttreatment course,angiographic and clinical follow-up outcomes at an average of 14.4 months(range,3-48 months).Results:A total of 33 patients with symptomatic VADAs were reviewed,23 of these patients with ruptured VADAs.The technical successful rate is 100%respectively in Willis covered stent(Group A)and SAC(Group B,n=20).The initial complete occlusion rate was significant higher in group A(100%)than group B(30%)(p0.05).No obvious in-stent stenosis and no re-hemorrhage and delayed ischemic symptoms during the follow-up period.The final angiograms of all survived patients demonstrated the complete occlusion rate was higher in group A(100%)than group B(80%),but no significant statistical difference(p>0.05).Clinical outcomes were favorable in 31(93.9%),severe disability occurred in one in group B,and only one death in group A.The final clinical outcomes were also not significant difference in the two groups(p>0.05).Conclusions:Our initial result demonstrated reconstructive EVT with Willis covered stent provides a viable approach for selected symptomatic VADAs involving the intracranial and extracranial segments,which is similar to favorable results with SAC.However,an expanded clinical experiences and larger cohort studies are needed. 展开更多

Remote sensing data have been widely applied to extract minerals in geologic exploration, however, in areas covered by vegetation, extracted mineral information has mostly been small targets bearing little information. In this paper, we present a new method for mineral extraction aimed at solving the difficulty of mineral identification in vegetation covered areas. The method selected six sets of spectral difference coupling between soil and plant(SVSCD). These sets have the same vegetation spectra reflectance and a maximum different reflectance of soil and mineral spectra from Hyperion image based on spectral reflectance characteristics of measured spectra. The central wavelengths of the six, selected band pairs were 2314 and 701 nm, 1699 and 721 nm, 1336 and 742 nm, 2203 and 681 nm, 2183 and 671 nm, and 2072 and 548 nm. Each data set's reflectance was used to calculate the difference value. After band difference calculation, vegetation information was suppressed and mineral abnormal information was enhanced compared to the scatter plot of original band. Six spectral difference couplings, after vegetation inhibition, were arranged in a new data set that requires two components that have the largest eigenvalue difference from principal component analysis(PCA). The spatial geometric structure features of PC1 and PC2 was used to identify altered minerals by spectral feature fitting(SFF). The collecting rocks from the 10 points that were selected in the concentration of mineral extraction were analyzed under a high-resolution microscope to identify metal minerals and nonmetallic minerals. Results indicated that the extracted minerals were well matched with the verified samples, especially with the sample 2, 4, 5 and 8. It demonstrated that the method can effectively detect altered minerals in vegetation covered area in Hyperion image. 展开更多

Delayed intra-abdominal hemorrhage after pancreatic surgery is a potentially lethal complication.Transarterial coil embolization and/or the placing of an endovascular stent are minimally invasive and effective procedures.An artery that is extensively eroded and rendered friable due to operative skeletonization or postoperative inflammation sometimes contributes to delayed intra-abdominal hemorrhage or rebleeding after coil embolization.This report presents a case of successful management of postoperative hemorrhage in a-74-year-old Japanese male.He experienced bleeding from a pseudoaneurysm of the brittle hepatic artery following total pancreatectomy.Initially the pseudoaneurysm was successfully treated with covered coronary stent-grafts,but rebleeding occurred 1 mo later due to the brittleness of the artery.Rebleeding was definitively managed by the complete packing of the stent by coil embolization.He remains stable at 18 mo following the f inal embolization.A stent graft can be used for protecting a brittle artery to avoid injury by coil embolization. 展开更多

Quaternary biogenic gas reservoirs are extensively developed in the Sanhu Depression of the Qaidam Basin,where the largest domestic biogenic gas production base has been built up.In recent years,however,the exploration of biogenic gas there encounters a variety of difficulties,such as the identification of true and false seismic abnormality,the determination of micro-relief structure and the identification and description of lithologic traps,which are the bottlenecks restricting its exploration breakthrough.In this paper,hydrocarbon accumulation patterns of biogenic gas were studied.Then,based on fine structure interpretation results,combined with laboratory experiments,the sealing capacity,distribution range,formation time and genesis of salt crust were discussed,and the salt crust covered hydrocarbon accumulation pattern of biogenic gas was put forward.Finally,combined with the conditions of source rocks and reservoirs,the exploration prospect of salt crust covered biogenic gas reservoirs was predicted.And the following research results were obtained.First,the Quaternary in the Sanhu Depression is composed of sandstone-mudstone interbeds of shore-shallow lake facies,with superior conditions of source rocks and reservoirs.Many source-reservoir-caprock assemblages are vertically formed.The biogenic gas has the characteristics of dynamic hydrocarbon accumulation,i.e.,continuous migration,accumulation,diffusion and re-accumulation.Second,under the effect of Himalayan movement,the climate changes frequently and the salinity of the ancient lake is zoned in the Sanhu Depression.A freshwater area is formed at the inlet of the river in the south and a brine area is formed in the north.Under the influence of evaporation,the phreatic water in the north is salinized continuously,and a set of extensively distributed salt crust with steady thickness and a strong sealing capacity is formed at the eastern part of Lingjian fault.Third,the biogenic gas generated by dark mudstone of lacustrine facies in the depression center migrates laterally and vertically to Lingjian fault zone and gets accumulated under the sealing of salt crust.In conclusion,high-quality source rocks and reservoirs and the salt crust with a strong sealing capacity in the Sanhu Depression constitute a good spatial-temporal configuration relationship of source rocks,reservoirs and cap rocks,so salt crust covered biogenic gas reservoirs can be formed to provide abundant natural gas resources with a broad exploration prospect. 展开更多

Several techniques have been reported to address different endovascular device failures.We report the case of a premature deployment of a covered balloon mounted stent during endovascular repair of a posttraumatic carotid-cavernous fistula(CCF).A 50-year-old male suffered a fall resulting in loss of consciousness and multiple facial fractures.Five weeks later,he developed decreased left visual acuity,proptosis,chemosis,limited eye movements and cranial/orbit bruit.Cerebral angiography demonstrated a direct left CCF and endovascular repair with a 5.0 mm×19 mm covered stent was planned.Once in the lacerum segment,increased resistance was encountered and the stent was withdrawn resulting in premature deployment.A 3 mm×9 mm balloon was advanced over an exchange length microwire and through the stent lumen.Once distal to the stent,the balloon was inflated and slowly pulled back in contact with the stent.All devices were successfully withdrawn as a unit.The use of a balloon to retrieve a prematurely deployed balloon mounted stent is a potential rescue option if leaving the stent in situ carries risks. 展开更多

Biliary fully covered self-expanding metal stents (FCSEMS) are now being used to treat several benign biliary conditions. Advantages include small predeployment and large postexpansion diameters in addition to an easy insertion technique. Lack of imbedding of the metal into the bile duct wall enables removability. In benign biliary strictures that usually require multiple procedures, despite the substantially higher cost of FCSEMS compared with plastic stents, the use of FCSEMS is offset by the reduced number of endoscopic retrograde cholangiopancreatography interventions required to achieve stricture resolution. In the same way, FCSEMS have also been employed to treat complex bile leaks, perforation and bleeding after endoscopic biliary sphincterotomy and as an aid to maintain permanent drainage tracts obtained by means of Endoscopic Ultrasound-guided biliary drainage. Good success rates have been achieved in all these conditions with an acceptable number of complications. FCSEMS were successfully removed in all patients. Comparative studies of FCSEMS and plastic stents are needed to demonstrate efficacy and cost-effectiveness 展开更多

Based on the model of a contaminated sea surface that was proposed by Lombardini et al., the influence of the damping effect of oil films on the sea surface roughness spectrum and the geometrical structure of the sea surface is examined in detail by comparing with a clean sea surface. Fhrthermore, based on a quasi-stationary algorithm, a time series of backscattered echoes from a time-evolving sea surface covered by oil slicks is obtained by utilizing the frequency-domain numerical method of the parallel fast multiple method. Then, the Doppler spectrum is evaluated by performing a standard spectral estimation technique. Finally, the influence of the oil film damping effect on the Doppler spectrum of the backscattered echoes from time-evolving sea surface is investigated in detail by making a comparison of the Doppler spectrum of an oil-covered sea surface with the Doppler spectrum of a dean sea surface. The numerical simulations show that the damping effect of oil films has an influence on the Doppler spectrum signature for both horizontal-to-horizontal and vertical-to-vertical polarizations. 展开更多

A 78-year-old male was admitted to our hospital because of choledocholithiasis.ERC demonstrated choledocholithiases with a maximum diameter of 13 mm, and we performed endoscopic papillary large balloon dilation(EPLBD) with a size of 15 mm.Immediately following the balloon deflation, spurting hemorrhage occurred from the orifice of the duodenal papilla.Although we performed endoscopic hemostasis by compressing the bleeding point with the large balloon catheter, we could not achieve hemostasis.Therefore, we placed a 10 mm fully covered selfexpandable metallic stent(SEMS) across the duodenal papilla, and the hemorrhage stopped immediately.After 1 wk of SEMS placement, duodenal endoscopy revealed ulcerative lesions in both the orifice of the duodenal papilla and the lower bile duct.A direct peroral cholangioscopy using an ultra-slim upper endoscope revealed a visible vessel with a longitudinal mucosal tear in the ulceration of the lower bile duct.We believe that the mucosal tear and subsequent ruptured vessel were caused by the EPLBD procedure. 展开更多

ObjectiveTo evaluate the long-term efficacy of covered stent implantation in the treatment of elderly patients with coronary perforation while undergoing percutaneous coronary intervention(PCI).MethodsFrom June 2004 to June 2012,our center has followed ten elderly patients(age≥60 years)who sustained coronary perforation during PCI.The major adverse cardiac events(MACE)were observed as well.The patients were advised to take 75 mg/day Clopidogrel for two years,and indefinite use of 100 mg/day enteric-coated aspirin.ResultsSix out of the 10 patients aged from 60 to 76 years old(mean 68.6±5.2 years)were male,four were female.The average diameter of the implanted stents was 3.3±0.3 mm,and the average length was 22.1±3.7 mm.All the ruptures were successfully sealed without intra-procedural death.The follow-up duration ranged from 0.6 to 67 months(mean 31.7±24.5 months).One patient died of multiple organ failure due to lung infection in 19 days after PCI;one died of cardiac sudden death in 13 months after PCI;one had angina pectoris in 53 months after PCI;one underwent multi-slice CT examination in six months after PCI,and no in-stent restenosis was found.The other four patients received angiography follow-up,and the results showed that three patients had no intra-stent restenosis,while one had left anterior descending(LAD)restenosis in the covered stent in 67 months after PCI.The in-hospital mortality was 10%(1/10).The MACE rate in 12 months after PCI was 10%(1/10).During the entire followed-up period,the restenosis rate in target vessels was 20%(1/5),mortality was 20%(2/10),and the MACE rate was 40%(4/10).ConclusionTreatment of coronary perforation by using covered stents can achieve favorable long-term results;a two-year dual antiplatelet therapy(DAPT)after PCI can effectively prevent intra-stent thrombosis. 展开更多

Objective Antiretroviral drugs covered by medical insurance have been gradually used by people living with human immunodeficiency virus(PLWH)in recent years in China.This study aimed to analyze their willingness to pay(WTP)for antiretroviral drugs.Methods A mixed-methods study design involving a cross-sectional survey and in-depth interviews was conducted.A cross-sectional survey was performed to collect data on the general characteristics,economic status,antiretroviral therapy(ART)status,and WTP of PLWH in 18 Chinese cities from August 2022 to February 2023.Multivariate logistic regression was used to analyze the factors associated with WTP.Representatives of PLWH were interviewed via in-depth interviews,and the data were thematically analyzed.Results Among the 941 PLWH,271(28.80%)were willing to pay for antiretroviral drugs covered by medical insurance.For basic medical insurance for urban and rural residents,PLWH with the following characteristics were more willing to pay:an educational level of senior high school or technical secondary school,having an undergraduate degree or higher,frequently working away from their hometowns,and homosexual transmission.Off-farm workers and recipients of government medical aid were more unwilling to pay.For basic medical insurance for urban employees,PLWH with the following characteristics were more willing to pay:frequently working away from their hometowns;homosexual transmission;personal annual income≥100,000 CNY;and adverse events of antiretroviral drugs.The main reasons for PLWH’s WTP for antiretroviral drugs covered by medical insurance were that the drugs had fewer adverse events and were easier to administer.The main reasons for PLWH’s unwillingness to pay were financial difficulties and privacy concerns.Conclusion Nearly one-third of PLWH are willing to pay for antiretroviral drugs covered by medical insurance.In the future,PLWH with a high WTP can be guided to use these drugs. 展开更多
