In recent years,researchers have extensively investigated the Hankel determinant,which consists of coefficients appearing in a holomorphic function’s Taylor-Maclaurin series.Hankel matrices are widely used in Markov processes,non-stationary signals,and other mathematical disciplines.The aim of the current research article is to first improve the bounds of coefficient-related problems by employing the well-known Carathéodory function.The problems that we are going to improve were obtained by Tang et al.The sharp estimates of the most difficult problem of geometric function theory known as the third-order Hankel determinant are also contributed here.Zalcman and Fekete-Szegöinequalities are also studied here for the defined family of holomorphic functions. 展开更多

Air in wastewater treatment plants(WWTPs)contains high concentrations of pathogens and antimicrobial resistance determinants(ARDs).However,limited information on their particle-size distribution hinders assessment of occupational inhalation exposure risks in WWTPs.In this study,air samples above the aeration tanks and the secondary sedimentation tanks of WWTPs were collected using Anderson eight-stage air samplers,and the size-resolved bacterial community was characterized by 16S rRNA gene sequencing.The inhalation deposition of ARDs and pathogens in the human respiratory tract was quantitatively evaluated by droplet digital PCR.Results showed that airborne bacterial concentrations above the aeration tank increased with particle size,ranging from 2914 to 5645 copies/m3.By contrast,concentrations above the secondary sedimentation tank exhibited a bimodal distribution,with peaks at 5.8-9.0μm(5592 copies/m3)and 0.65-1.1μum(5302 copies/m3).The most prevalent airborne antibiotic resistance genes were tetW and floR,associated with aerodynamic diameters of 2.64μm and 3.43μm,respectively.The International Commission on Radiological Protection deposition model indicated that airborne pathogens and ARDs were primarily deposited in the upper respiratory tract(17.73-165.63 copies/h)and alveoli(1.84-14.63 copies/h).Notably,deposition densities of Escherichia coli and Enterococcus faecium reached 2.26-36.20 copies/hand 1.62-31.31 copies/h,respectively.Co-occurrence of these pathogens with floR and sulⅡin fine particulate matter may pose significant health risks.This study elucidates the size distribution and respiratory deposition behavior of ARDs and pathogens in WWTP air,addressing critical knowledge gaps for comprehensive assessment of airborne antimicrobial resistance and associated occupational health risks. 展开更多

Psychological state and body image disturbance in head and neck cancer patients:Prevalence and determinants 认领 引用

BACKGROUND Head and neck cancer(HNC)usually imposes a psychological burden on patients.In particular,treatment-related facial changes often predispose them to body image disturbance(BID),thereby exacerbating psychological stress.AIM To characterize the psychological state(anxiety,depression)and BID status in patients with HNC and to identify determinants associated with BID.METHODS A total of 100 patients with HNC treated between January 2020 and January 2025 were analyzed.The psychological state was assessed using the Hospital Anxiety and Depression Scale-Anxiety(HADS-A)and Hospital Anxiety and Depression Scale-Depression(HADS-D)subscales,while body image-related distress was evaluated using the Body Image Scale(BIS).Linear associations among scales were examined using Pearson’s or Spearman’s correlation coefficients.Potential predictors of BID were evaluated using univariate and multivariable regression models.RESULTS Anxiety and depressive symptoms were identified in 25%and 30%of patients,respectively.Mean HADS-A and HADS-D scores were(5.62±2.89)and 5.00(3.00,8.00),respectively,while the BIS total score was 16.00(14.00,18.00).Global BIS scores and all BIS domains(affective,cognitive,and behavioral)correlated positively with both HADS-A and HADS-D scores.Regression analyses identified multimodal treatment[odds ratio(OR)=6.730,P=0.011],female sex(OR=5.206,P=0.003),disease progression(OR=4.316,P=0.012),unemployment(OR=3.461,P=0.042),anxiety(HADS-A;OR=4.031,P=0.036),and depression(HADS-D;OR=3.690,P=0.039).CONCLUSION Patients with HNC commonly present with mild psychological issues accompanied by moderate BID.Female patients,those with late-stage cancer(III-IV),recipients of multimodal therapy,unemployed individuals,and patients with anxiety or depressive symptoms are at greater risk of BID. 展开更多

Coxsackievirus B4(CVB4)is a highly pathogenic enterovirus associated with severe neurological disease and mortality.To establish research models that recapitulate severe CVB4 infection,two clinical isolates with distinct neurovirulence-the high-virulence strain GZ-HFM01 and low-virulence strain GZ-R6-were used.An in vitro neurocytotoxicity model with human neuroblastoma SH-SY5Y cells showed that GZ-HFM01 produced significantly larger plaques than GZ-R6,reflecting its increased capacity to damage neuronal cells.Concurrently,an optimized in vivo severe infection model was established in 3-day-old ICR mice through intraperitoneal inoculation,which reproduced key clinical features of severe disease.Compared with GZ-R6,GZ-HFM01 infection resulted in significantly reduced survival,progressive neurological impairment,time-dependent viral accumulation in brain tissue,pronounced histopathological injury,and elevated serum pro-inflammatory cytokine levels.To map genomic determinants of neurovirulence,a chimeric virus panel was generated by replacing individual genome segments(5′untranslated region[UTR],P1,P2,P3,or-3′UTR)of GZ-HFM01 with the corresponding regions from GZ-R6.Evaluation with the established models demonstrated that replacement of the P2 region significantly attenuated both the cytopathic effect in SH-SY5Y cells and pathogenicity in 3-day-old ICR mice.Animal studies further indicated that substitution of the 5′UTR or P1 region also reduced virulence-a phenotype absent from the cellbased model-underscoring the multifactorial regulation of CVB4 pathogenesis.In conclusion,this study provides validated in vitro and in vivo models of severe CVB4 infection and identifies key genomic segments that contribute to neurovirulence,offering a foundation for mechanistic research and the development of targeted interventions against severe CVB4-induced disease. 展开更多

Background:Early childhood malnutrition is a serious public health issue in India,contributing to stunting,wasting,and underweight among children under 5 years.The study aims to explore the factors influencing these conditions and highlight the region most affected.Methods:This study uses secondary data collected from National Family Health Survey 2019–21.Multivariate logistic regression has been run to identify the risk factors associated with malnutrition conditions among children under age five including stunting,wasting,and underweight based on Composite Index of Anthropometric Failure(CIAF)measures.To explore the hot spot zone for stunting,wasting and underweight,cluster analysis has been done applying univariate local Moran's I.Results:The study explores a significant proportion of children in India suffering from malnutrition,particularly stunting is more prevalent than of underweight and wasting.Out of 25 indicators,age,sex,birth size of child and the mother's body mass index and current employment status have been identified as key predictors of malnutrition status.The children aged 25 to 36 months are more likely to be stunted,wasted and underweight than the other age groups which is consistent with CIAF with adjusted odds ratio(AOR)of 3.92(95%confidence interval[CI]:1.92–7.99,p<0.01).A sex-based analysis shows females are less likely to be malnourished than male child which is also evident in CIAF(AOR:0.66;95%CI:0.55–0.80,p<0.01).The children having normal or larger sized at birth are less likely to suffer from stunting,wasting and underweight and even in CIAF than the small size children at birth.The children of underweight mother are more likely to be malnourished than the children of mother having normal body mass index which is also reported in CIAF(AOR:1.34;95%CI:1.05–1.72,p=0.02).Additionally,economic status and geographical region significantly impact malnutrition with children with wealthier households faring better which is consistent in CIAF(AOR:0.63;95%CI:0.45–0.88 for richest).Univariate local indicators of spatial association cluster analysis reveals that the western,eastern,and central zones are particularly vulnerable to child malnutrition.Conclusion:Combating prevailing malnutrition in India requires collective responsibility and collaboration among policy-makers,geographers,healthcare professionals,and communities. 展开更多

Objective:The objective of this study was to assess patient satisfaction and its determinants among adult patients attending the University of Calabar Teaching Hospital(UCTH),Cross River State,Nigeria.Materials and Methods:A descriptive cross-sectional study was conducted among 424 adult patients across major outpatient clinics and selected inpatient wards.Participants were selected using a multi-stage,proportionate stratified sampling method.Data were collected via a structured interviewer-administered questionnaire adapted from the Patient Satisfaction Questionnaire Short-Form-18 and SERVQUAL instruments.Data were analyzed using SPSS version 25.Descriptive statistics summarized participant characteristics and satisfaction scores.Bivariate associations were assessed using Chi-square tests,and significant variables were included in a multivariable logistic regression to identify independent determinants of satisfaction.Results:The study population had a mean age of 37.8±11.6 years,and 55.2%were female.Overall,62.3%(n=264)of participants were satisfied with the services provided.Satisfaction was higher among outpatients(68.4%)and participants with tertiary education(69.8%).In multivariable analysis,significant independent predictors of satisfaction were:good communication(adjusted odds ratios[aOR]=2.98;95%confidence interval[CI]:1.75-5.07),adequate staff availability(aOR=2.45;95%CI:1.40-4.30),short waiting time<60 min(aOR=1.87;95%CI:1.12-3.12),and tertiary education(aOR=3.12;95%CI:1.48-6.56).Type of patient(inpatient vs.outpatient)was not statistically significant(aOR=1.42;P=0.150).The regression model demonstrated good fit(Hosmer-Lemeshow P=0.61)and strong predictive ability(AUC=0.81).Conclusion:Patient satisfaction at UCTH is moderate,with process-related factors,particularly communication,staffing,and waiting time,playing a more critical role than structural or demographic characteristics. 展开更多

BACKGROUND Patients with brain tumors(BTs)are often accompanied by anxiety and depression,which are not conducive to improved clinical outcomes.AIM To investigate anxiety and depression in relation to sleep quality and post-traumatic growth(PTG)in patients with BTs and identified associated determinants.METHODS A total of 169 patients with BTs admitted between October 2022 and January 2025 were enrolled.Data on anxiety and depression(Hospital Anxiety and Depression Scale,HADS),sleep quality(Pittsburgh Sleep Quality Index,PSQI),and PTG was collected.Correlation analysis was conducted to evaluate associations between HADS subscales-HADS-Anxiety and HADS-Depression-and PSQI and PTG Inventory(PTGI)scores.Anxiety or depression-associated determinants were identified using univariate screening followed by binary logistic regression analysis.RESULTS Among the 169 patients with BTs,the prevalence of anxiety,depression,anxiety-depression comorbidity,and either anxiety or depression was 34.91%,30.18%,10.65%,and 54.44%,respectively.The four most PSQI dimensions were sleep latency,sleep duration,daytime dysfunction,and sleep quality.The mean PTGI score was 59.89±8.83,with 47.93%of patients scoring<60.The correlation analysis demonstrated positive correlations between HADSAnxiety and HADS-Depression scores and PSQI,and negative correlations with PTGI.Regression analysis showed that age,educational level,PSQI,and PTGI independently influenced anxiety or depression in patients with BTs.CONCLUSION Anxiety and depression in patients with BTs are closely associated with sleep quality and PTG.Age≥50 years,high school education,and PTGI≥60 exert protective effects against anxiety or depression,whereas PSQI≥11 independently increases the risk. 展开更多

Introduction:Arterial hypertension is a major public health issue due to its increasing morbidity and mortality,largely linked to inadequate control.Objective:To determine the prevalence and determinants of blood pressure control in adults in Gabon.Patients and Methods:A multicenter,cross-sectional,descriptive,and analytical study was conducted in a hospital setting over six months in seven provinces of Gabon,including 338 hypertensive patients.Exclusion criteria were gestational hypertension,suspected secondary hypertension,acute illness,concurrent treatments,and refusal to participate.A physical examination was performed to assess blood pressure,heart rate,and anthropometric parameters.A questionnaire collected socioeconomic data,characteristics of care received,follow-up location,and health insurance status.Hypertension was considered controlled when blood pressure measured on three readings was below 140/90 mmHg.Statistical analysis was performed using Epi-Info 3.5,with significance set at 0.05.Results:The prevalence of controlled hypertension was 35%.The main factors associated with poor control included low socioeconomic status and education level,insufficient knowledge of the disease,poor adherence to treatment,and comorbidities such as diabetes.Follow-up by a cardiologist and the use of a fixed-dose combination of antihypertensive drugs were associated with better control.Conclusion:In Gabon,blood pressure control remains relatively low.This therefore necessitates improving the socio-economic status of patients,the quality of practitioners,and access to health insurance. 展开更多

Objective Data on homocysteine(Hcy) status and its determinants are limited among women during pregnancy and postpartum. This cross-sectional study aimed to investigate Hcy levels during pregnancy and postpartum, and to explore the determinants like geographic factor.Methods This study was conducted in women at mid-pregnancy, late-pregnancy and postpartum from southern, central and northern China. Approximately 132 women were included in each stratum by the three phases and regions. Plasma Hcy concentrations were assessed using high-performance liquid chromatography(HPLC), with hyperhomocysteinemia defined as > 10.0 μmol/L. Quantile regression was to estimate medians and interquartile ranges(IQRs), and logistic regression to examine the determinants of hyperhomocysteinemia.Results For 1,190 women included, the median(IQR) Hcy concentration was 5.66(4.62, 7.37) μmol/L.The adjusted median in mid-pregnancy, late-pregnancy and postpartum women was 4.75(4.13, 5.54),5.72(4.81, 6.85) and 7.09(5.65, 8.75) μmol/L, respectively, showing an increasing trend(P < 0.001). This increasing trend persisted across the three regions. Higher Hcy concentrations were observed in women residing in northern region and those with younger age or lower economic status. A total of 106(8.9%)women had hyperhomocysteinemia, with a higher prevalence in those residing in northern region(16.0%), or in postpartum women(16.5%).Conclusion Hcy levels, varying with geographic region, maternal age and economic status, are increased from mid-pregnancy to late-pregnancy and postpartum, indicating a need to monitor Hcy levels in pregnant and postpartum women to control potential risks related to elevated Hcy levels. 展开更多

Objectives:This scoping review aims to methodically identify and analyse the determinants which influence maternal mortality and healthcare utilization in women of childbearing years in Nigeria.The review specifically addresses the difficulties faced in assessing medical care,and the ongoing initiatives to lower the maternal mortality ratio.Methods:Primary studies(published after year 2000)focused on maternal mortality,health determinants,and the utilization of maternal healthcare services in Nigeria were retrieved following a systematic search across multiple databases,including Scopus,PubMed,Google Scholar,and Web of Science.These were screened using defined inclusion and exclusion criteria.Data from these studies were extracted and thematically analysed to identify common themes and trends.Results:A total of 21 publications were included in this review comprising 2 qualitative,17 quantitative,and2 mixed study designs.Major health determinants identified in these studies include hypertensive disorders of pregnancy,haemorrhage,and sepsis/septicaemia,contributing as much as 29%,24%,and 14.2%of maternal deaths respectively in over 10%of the reviewed studies.Similarly,social-economic determinants including poverty,maternal education,health system issues and culture significantly impacted the utilization of maternal healthcare services,inadvertently impacting maternal mortality.Despite various maternal health interventions such as the"Abiye"Safe Motherhood Initiative,the Integrated Maternal,Newborn,and Child Health Strategy and strengthening emergency obstetric care by governmental and non-governmental agencies,early childbearing,unemployment,large family sizes,and dependence on male spouses continued to result in suboptimal utilization of services and adverse maternal outcomes.Conclusion:This scoping review comprehensively integrated existing data from published studies to provide a detailed picture of maternal mortality,its health determinants,and factors influencing care utilization among women of childbearing age in Nigeria and approaches to reduce maternal mortality in the country and possibly in the West African sub-region. 展开更多

Background Despite being included in the Millennium Development Goals(MDGs)and now the Sustainable Development Goals(SDGs),stillbirths remain overlooked with limited regional research,highlighting an ongoing gap in addressing this issue.However,a staggering 2 million stillbirths occur each year,equivalent to one every 16 s.Furthermore,approximately 98%of these stillbirths take place in developing countries,particularly in sub-Saharan Africa(SSA).In light of these statistics and the need to address the lack of data,methodological approaches,and population gaps,this study aims to assess the prevalence and determinants of stillbirths in SSA from 2016 to 2023,aligning with the SDGs.Methods This study used data from the Demographic and Health Survey(DHS)conducted in SSA.The analysis included a weighted sample of 212,194 pregnancies of at least 28 weeks'gestation collected from 2016 to 2023,using R-4.4.0 software.Descriptive data,such as frequencies,were performed.Stillbirth prevalence was visualized using a forest plot.A multilevel modeling analysis was used by considering individual-level factors and community level factors.The multilevel model was employed to account for clustering within countries and allow for the examination of both fixed and random effects that influence stillbirths.For the multivariable analysis,variables with a p value≤0.2 in the bivariate analysis were considered.The Adjusted Odds Ratio(AOR)with a 95%Confidence Interval(CI)and a p value<0.05 were reported to indicate the statistical significance and the degree of association in the final model.Results The pooled prevalence of stillbirths was found to be 1.54%per 100[95%CI 1.19-2.01].Factors positively associated with stillbirths in SSA included maternal age(25-34 years,35-49 years),marital status(married,divorced or widowed),antenatal care visits,age at first birth(before age 20),short birth intervals,long birth intervals,birth order(second or third),residence in rural areas,country income level(lower middle income),and low literacy rate.Factors negatively associated with stillbirth mortality included maternal education(primary education,secondary or higher education),wealth index(higher economic status),access to mass media,access to improved drinking water,distance to health facilities,and country income level(upper middle income).Conclusions Stillbirth rates fall significantly short of achieving Every Newborn Action Plan target by 2030 in SSA.The analysis of factors that affect stillbirth mortality reveals important connections.It is essential to improve maternal education,economic status,and healthcare infrastructure to decrease stillbirth rates and enhance the health outcomes of mothers and children in the region.To effectively address these risks,efforts should concentrate on increasing access to antenatal care,raising awareness,and improving socio-economic conditions.By improving access to healthcare and education,these disparities could potentially lead to a decrease in stillbirth rates in the region. 展开更多

Background The rapid aging in the Philippines presents significant challenges,including high rates of activities of daily living(ADL)disability among older adults.While research has identified various social determinants of ADL disability,there is a gap in understanding how their joint impact on disability among older Filipinos.We adopted the polysocial score approach to assess these joint associations of multiple social factors with disability among older adults in the Philippines.Methods Individuals included in the analysis were from the Longitudinal Study of Ageing and Health in the Philippines.Twenty-nine social factors from five domains were included.The Light Gradient Boosting Machine(LightGBM)model was employed to identify and rank key social determinants,which were then incorporated into a logistic regression model to derive the polysocial score,both continuously and in categories and assessed its association with ADL disability.Model performances were assessed by discrimination,calibration,and reclassification(compared to a reference model).All analyses were separated for men and women.Results We included 5,000 participants(37.0%men)with an average age of 68.3 years.The polysocial score included 19 social factors for men and 18 for women.Among men,the most significant predictors were the frequency of engaging in social activities and the number of friends they had contact with.Attending religious services outside the home and the frequency of attending social activities were the most important factors for women.Eleven social factors overlapped between men and women.Individuals with moderate or high polysocial scores exhibited reduced likelihood of experiencing ADL disability relative to those with low scores.We observed satisfactory model performance among men and women.Conclusions We identified important social factors for men and women and their joint association with ADL disability.The polysocial score could be used to design person-centered social interventions that promote health and support independence. 展开更多

Objective To explore the health determinants of all-cancer mortality from a health-ecological perspective.Methods Using the health-ecological model,we identified 15 health determinants across five domains:personal innate,behavioral,interpersonal network,socioeconomic,and macroenvironmental characteristics.Data on cancer outcomes and health determinants from 185 countries,classified using the Human Development Index(HDI),were extracted from GLOBOCAN 2022,the World Health Organization Global Health Observatory,the Global Burden of Disease Study 2021,the United Nations,and the World Bank.A multivariate linear regression model was used to estimate the association between health determinants and cancer outcomes.Results A total of 153 countries with complete information on cancer outcomes and health determinants were enrolled in the study.A higher all-cancer age-standardized mortality rate(ASMR)was associated with current tobacco use,alcohol consumption,obesity,agricultural employment,and lag-distributed income.In women aged 15–49 years,being married or in a union was associated with a lower ASMR.Early-onset ASMR was positively associated with agricultural employment,lag-distributed income,and unsafe sanitation.Notable sex differences were observed,although tobacco use and obesity were identified as risk factors in both sexes.Conclusion We underscore the crucial need to incorporate cross-sectoral interventions within a resilient healthcare system to effectively mitigate the cancer mortality burden. 展开更多

Botanical gardens play a crucial role in maintaining urban ecological balance,providing essential ecosystem services(ES)that enhance environmental quality and residents'well-being.However,the social evaluation of the ecosystem services the Gulele Botanic Garden(GBG)provides remains underexplored.This study assesses the ecosystem services,visitor perceptions,and key influencing factors associated with GBG in Addis Ababa,Ethiopia.A total of 316 visitors were conveniently selected and surveyed through semi-structured interviews and questionnaires conducted from June 1–30,2023.Data were analyzed using descriptive statistics and binary logistic regression analysis in SPSS software(v26).The findings indicate that GBG offers ecotourism opportunities and various other services.Among the respondents,92.1%expressed high awareness of cultural ecosystem services,86%held positive perceptions of regulating and supporting services,and 70.1%viewed provisioning ecosystem services favorably.The study also revealed that higher education levels,age,and frequency of visits were positively correlated with favorable perceptions of ecosystem services.In contrast,perceptions tended to decline with increasing distance from GBG.Visitor perceptions varied significantly based on age,education,distance,and visit frequency,underscoring the need for targeted awareness initiatives.In conclusion,GBG provides multiple ecosystem services that contribute to urban residents'well-being,particularly cultural services as perceived by visitors.To further enhance these services,diversifying recreational areas within the garden and implementing community-wide awareness campaigns are essential. 展开更多

BACKGROUND The global prevalence of diabetes among adults aged 29-79 years was found to be 10.5%.It is a global public health threat with a rising trend in morbidity and mortality.Poor glycemic control(GC)among patients with type 2 diabetes mellitus(T2DM)is a major determinant of diabetes-related complications.There are limited data on GC and associated factors among patients with T2DM in South West Region,Cameroon.AIM To assess GC and identify contributing factors among patients with T2DM in a regional hospital in South West Region,Cameroon.METHODS A cross-sectional study was conducted from February 2022 to July 2022 among 131 participants in Limbe Regional Hospital who were selected by convenience.Glycated hemoglobin(HbA1c)was measured by ion-exchange chromatography.Sociodemographic,clinical,and lifestyle data were collected,entered into Excel,and exported to Statistical Package for Social Sciences version 22 for analysis.A multivariate logistic regression analysis was conducted to assess the association between explanatory variables and GC.The level of significance was set at P<0.05.RESULTS The mean age was 56±5.1 years.Eighty-eight(67.2%)patients were female.The mean HbA1c was 8.8%±1.8%.Poor GC(HbA1c≥7%)was registered in 106(80.9%;95%confidence interval:73.1%-87.3%)participants.Lack of self-monitoring of blood glucose at home was associated with poor GC(adjusted odds ratio:3.858,95%confidence interval:1.262-11.800;P=0.018).CONCLUSION The majority of patients with T2DM had poor GC.Absence of self-monitoring of blood glucose at home was the main contributing factor for poor GC. 展开更多

BACKGROUND Social determinants of health are social and economic factors that influence health intervention outcomes.Type 2 diabetes is a highly prevalent disease,primarily affecting individuals in low-to-middle-income countries.However,the association between social determinants and cardiovascular complications in type 2 diabetes has not been widely studied.AIM To examine the relationship between social determinants of health and cardiovascular complications in individuals with type 2 diabetes.METHODS We conducted a retrospective cohort study with an analytical component at a national-level referral hospital for military personnel in Bogota,Colombia.Patients treated at the diabetes clinic between September 2021 and December 2022 who met the inclusion criteria were included.A total of 583 patients participated in the study.We performed descriptive,bivariate,and binary logistic regression analyses,adjusting for confounding variables.RESULTS Among the 583 patients included,urban residency[odds ratio(OR)=3.05,95%confidence interval(CI):1.01-9.20]and a middle or high educational level(OR=2.33,95%CI:1.14-4.72)were associated with an increased risk of coronary artery disease.Additionally,receiving diabetes education beyond that provided by the clinic(OR=2.15,95%CI:1.14-4.05)and lack of access to spaces for physical activity(OR=4.05,95%CI:1.31-12.5)were associated with a higher risk of diabetic nephropathy and cerebrovascular disease,respectively.CONCLUSION Programs for diabetes management should account for social determinants of health that contribute to cardiovascular complications and increased healthcare costs.Population-based studies are needed to guide targeted interventions and clarify causal relationships. 展开更多
