Through integrated transcriptomic, functional, and pharmacological analyses, we demonstrate that hyperbaric oxygen (HBO) exposure triggers pressure-dependent neurotoxicity mediated by the reactive oxygen species (ROS)-lysosome-mechanistic target of rapamycin (mTOR) axis. Key findings include the following: mechanistic hierarchy: Five atmospheres absolute (ATA) HBO induces metabolic dysregulation and cell cycle arrest, whereas six ATA exceeds compensatory thresholds, triggering overt apoptotic signatures;pathway crosstalk: Lysosomal permeabilization activates mTOR complex 1 (mTORC1) via cathepsin release, while mTORC1 hyperactivation suppresses transcription factor EB (TFEB)-mediated lysosomal regeneration, creating a self-amplifying loop;therapeutic potential: Mouse validation confirmed that mTOR inhibition (temsirolimus) attenuates neurotoxicity, with hippocampus-specific efficacy. BACKGROUND Hyperbaric oxygen therapy(HBOT)has been explored as a potential treatment for depression,with various studies indicating its benefits on psychological,cognitive,and functional outcomes.AIM To evaluate the e...BACKGROUND Hyperbaric oxygen therapy(HBOT)has been explored as a potential treatment for depression,with various studies indicating its benefits on psychological,cognitive,and functional outcomes.AIM To evaluate the efficacy of HBOT in treating depression.METHODS A systematic review of randomized controlled trials was conducted to assess the impact of HBOT on depression.Studies were identified through database searches,screened for eligibility,and assessed for bias.Outcome measures included standardized depression scales,cognitive assessments,and functional evaluations.RESULTS It was observed across the 6 included trials that HBOT significantly reduced depressive symptoms and improved functional outcomes across multiple studies.Improvements in psychological and neurological measures were consistently reported.Additionally,substantial relief from post-concussion symptoms and enhancements in cognitive and emotional well-being were noted.Some studies highlighted the correlation between physiological markers,such as hemoglobin A1c levels,and depressive symptoms,suggesting HBOT's broader therapeutic potential.Combining HBOT with pharmacological treatments showed rapid improvements in depression and cognitive function,further supporting HBOT's efficacy.CONCLUSION HBOT demonstrated significant efficacy in reducing depressive symptoms and improving cognitive and functional outcomes.The therapy's potential benefits extended across various measures,indicate its potential as a multifaceted treatment for depression.Further research is recommended to explore HBOT's mechanisms and optimize treatment protocols.展开更多

Methods: From January 2024 to January 2026,80 tracheostomized comatose patients requiring hyperbaric oxygen therapy were enrolled. Patients were randomly assigned into two groups. The control group received standard hyperbaric oxygen therapy protocols and general neurosurgical nursing care;the observation group additionally implemented a comprehensive nursing strategy spanning the entire treatment course, emphasizing patient safety management in the hyperbaric chamber, meticulous respiratory care, continuous monitoring of key physiological parameters, and proactive prevention of complications. Results: Upon completion of the prescribed treatment course, the observation group demonstrated significantly greater improvements in consciousness levels and daily activity scores compared to the control group (P <0.05). Throughout the treatment period, the overall incidence of treatment-or care-related adverse events was markedly lower in the observation group (5.0%) versus the control group (27.5), with a statistically significant difference (P <0.05). Additionally, the observation group exhibited superior clinical outcomes in terms of weaning from mechanical ventilation support and recovery of consciousness. Conclusion: For comatose patients who have undergone tracheostomy, implementing comprehensive and systematic nursing interventions during hyperbaric oxygen therapy can effectively ensure treatment safety, promote neurological function recovery, and significantly reduce complication rates, thereby providing a reliable nursing practice pathway for optimizing the clinical prognosis of such patients.展开更多
BACKGROUND Central retinal artery occlusion(CRAO)is a potentially blinding disease,and hyperbaric oxygen therapy(HBOT)is becoming increasingly popular with the support of scientific evidence.Despite the presence of va...BACKGROUND Central retinal artery occlusion(CRAO)is a potentially blinding disease,and hyperbaric oxygen therapy(HBOT)is becoming increasingly popular with the support of scientific evidence.Despite the presence of various acute management measures,there is no clear evidence on the gold standard treatment for CRAO.AIM To identify factors and imaging parameters associated with good visual outcome,which guide ophthalmologists in the triage of CRAO patients for HBOT.METHODS Patients who suffered from CRAO and had a symptom onset≤6 h were recruited for a course of HBOT in a tertiary hospital after failing bedside treatment.Patient demographics,onset time,CRAO eye parameters,and past medical history were prospectively collected.Visual outcomes after HBOT were also analyzed.RESULTS A total of 26 patients were included;the female-to-male ratio was 1:1.6,and the mean age was 67.5 years±13.3 years(range 44–89 years).The mean duration of follow-up and mean visual acuity(VA)improvement were 10.0 mo±5.3 mo and 0.48 logarithm of minimal angle of resolution(logMAR)±0.57 logMAR(approx-imately 9 letters in ETDRS)(P=0.0001,Z=-3.67),respectively.The 1 mm zone of central macular thickness(CMT)on optical coherence tomography was not associated with VA changes(P=0.119);however,the 1-to-3 mm circular rim of CMT was fairly associated(P=0.02,Spearman's coefficient=0.45).Complete retinal perfusion time during fundus fluorescein angiography(FFA)was mode-rately associated(P=0.01,Spearman's coefficient=0.58)with visual outcome.展开更多

The primary aim of this study was to assess the efficacy and mechanism of hyperbaric oxygen therapy (HBOT) to reduce the keloid recurrence rate after surgical excision and radiotherapy. Methods: (1) A total of 240 patients were randomly divided into two groups. Patients in the HBOT group (O group) received HBOT after surgical excision and radiotherapy. Patients in the other group were treated with only surgical excision and radiotherapy (K group). (2) Scar tissue from recurrent patients was collected after a second operation. Hematoxylin and eosin (H&E) staining was used to observe keloid morphology. Certain inflammatory factors (interleukin-6 (IL-6), hypoxia-inducible factor-1α (HIF-1α), tumor necrosis factor-α (TNF-α), nuclear factor KB (NF-κB), and vascular endothelial growth factor (VEGF)) were measured using immunohistochemical staining. Results: (1) The recurrence rate of the O group (5.97%) was significantly lower than that of the K group (14.15%), P〈0.05. Moreover, patients in the O group reported greater satisfaction than those in the K group (P〈0.05). (2) Compared with the recurrent scar tissue of the K group, the expression levels of the inflammatory factors were lower in the recurrent scar tissue of the O group. Conclusions: Adjunctive HBOT effectively reduces the keloid recurrence rate after surgical excision and radiotherapy by improving the oxygen level of the tissue and alleviating the inflammatory process.展开更多

Objective:To investigate the effect of BMSCs transplantation plus hyperbaric oxygen(HBO)on repair of rat SCI.Methods:Seventy five male rats were divided randomly into five groups:sham,vehicle.BMSCs transplantation gro...Objective:To investigate the effect of BMSCs transplantation plus hyperbaric oxygen(HBO)on repair of rat SCI.Methods:Seventy five male rats were divided randomly into five groups:sham,vehicle.BMSCs transplantation group,combination group,15 rats in each group.Every week after the SCI onset,all animals were evaluated for behavior outcome by Basso-BeattleBresnahan(BBB) score and inclined plane test.Axon recovery was examined with focal spinal cord tissue by electron microscope at 6 weeks after the SCI onset.HE staining and BrdU staining were performed to examine the BMSCs and lesion post injury.Somatosensory evoked potential(SEP) testing was performed to detect the recovery of neural conduction.Results from the behavior tests from combination group were significant higher than rats which received only transplantation or HBO treatment.Results from histopathology showed favorable recovery from combination group than other treatment groups.The number of BrdU+ in combination group were measureable more than transplantation group(P<0.05).The greatest decrease in TNF-α,IL-1β,IL-6.IFN-α determined by Elisa assay in combination group were evident too.Conclusions:BMSCs transplantation can promote the functional recovery of rat hind limbs after SCI,and its combination with HBO has a synergistic effect.展开更多
Welding polarity has influence on welding stability to some extent,but the specific relationship between welding polarity and weld quality has not been found,especially under the hyperbaric environment.Based on a hype...Welding polarity has influence on welding stability to some extent,but the specific relationship between welding polarity and weld quality has not been found,especially under the hyperbaric environment.Based on a hyperbaric dry welding experiment system,gas metal arc welding(GMAW)experiments with direct current electrode positive(DCEP)and direct current electrode negative(DCEN)operations are carried out under the ambient pressures of 0.1 MPa,0.4 MPa,0.7 MPa and 1.0 MPa to find the influence rule of different welding polarities on welding spatters and weld bead geometry.The effects of welding polarities on the weld bead geometry such as the reinforcement,the weld width and the penetration are discussed.The experimental results show that the welding spatters gradually grow in quantity and size for GMAW with DCEP,while GMAW with DCEN can produce fewer spatters comparatively with the increase of the ambient pressure.Compared with DCEP,the welding current and arc voltage waveforms for DCEN is more stable and the distribution of welding current probability density for DCEN is more concentrated under the hyperbaric environment.When the ambient pressure is increased from 0.1 MPa to 1.0 MPa,the effects of welding polarities on the reinforcement,the weld width and the penetration are as follows:an increase of 0.8 mm for the weld reinforcement is produced by GMAW with DCEN and 1.3 mm by GMAW with DCEP,a decrease of 7.2 mm for the weld width is produced by DCEN and 6.1 mm by DCEP;and an increase of 3.9 mm for the penetration is produced by DCEN and 1.9 mm by DCEP.The proposed research indicates that the desirable stability in the welding procedure can be achieved by GMAW with DCEN operation under the hyperbaric environment.展开更多

In addition, the mechanism of this potential ...Hyperbaric oxygen therapy for the treatment of neonatal hypoxic-ischemic brain damage has been used clinically for many years, but its effectiveness remains controversial. In addition, the mechanism of this potential neuroprotective effect remains unclear. This study aimed to investigate the influence of hyperbaric oxygen on the proliferation of neural stem cells in the subventricular zone of neonatal Sprague-Dawley rats (7 days old) subjected to hypoxic-ischemic brain damage. Six hours after modeling, rats were treated with hyperbaric oxygen once daily for 7 days. Immunohistochemistry revealed that the number of 5-bromo-2'-deoxyuridine positive and nestin positive cells in the subventricular zone of neonatal rats increased at day 3 after hypoxic-ischemic brain damage and peaked at day 5. After hyperbaric oxygen treatment, the number of 5-bromo-2'- deoxyuddine positive and nestin positive cells began to increase at day 1, and was significantly higher than that in normal rats and model rats until day 21. Hematoxylin-eosin staining showed that hyperbaric oxygen treatment could attenuate pathological changes to brain tissue in neonatal rats, and reduce the number of degenerating and necrotic nerve cells. Our experimental findings indicate that hyperbaric oxygen treatment enhances the proliferation of neural stem cells in the subventricular zone of neonatal rats with hypoxic-ischemic brain damage, and has therapeutic potential for promoting neurological recovery following brain injury.展开更多

OBJECTIVE: To observe the effects and clinical significance of C-reactive protein in patients with craniocerebral injury after hyperbaric oxygenation. DESIGN: Randomized controlled study. SETTING: Departments of Neurosurgery, Laboratory and Hyperbaric Oxygen, the Second Affiliated Hospital, Medical College of Shantou University. PARTICIPANTS: A total of 60 patients with craniocerebral injury were selected from Department of Neurosurgery, the Second Affiliated Hospital, Medical College of Shantou University from October 2006 to April 2007. There were 37 males and 23 females and the mean age was 26 years. All subjects were certainly diagnosed as history of craniocerebral injury. Patients hospitalized at 24 hours after injury, Glasgow Coma Score ranged from 3 to 12 points, and all patients were certainly diagnosed with CT or MR scanning. Patients and their relatives provided confirmed consent. All the subjects were randomly divided into hyperbaric oxygenation group and control group with 30 in each group. METHODS: Patients in the control group were treated with routinely neurosurgical therapy after hospitalization; however, based the same basic treatment in the control group, patients in the hyperbaric oxygenation group received hyperbaric oxygenation by using iced-wheel four-door 2-cabin air-compression chamber (made in Yantai) from 24 hours to 10 days after operation or injury. After entering the cabin, patients who had a clear consciousness breathed the oxygen by using face mask; contrarily, patients directly breathed the oxygen. Therapeutic project: Expression was increased for about 15–20 minutes, maintained for about 70–80 minutes, and decreased for 20 minutes. Otherwise, pressure was maintained from 0.2 to 0.25 MPa. Hyperbaric oxygenation took an hour for once a day and 10 times were regarded as a course. Venous blood was collected before treatment and on the next day of the first course end. Content of C-reactive protein in plasma was measured with immune turbidimetry in hyperbaric oxygenation group; in addition, content of C-reactive protein in plasma was directly measured with the same method at the corresponding time in the control group. If the content was less or equal to 8 mg/L, it was regarded as normal value. Effects of the two groups were evaluated based on Glasgow Coma Score before and after treatment. MAIN OUTCOME MEASURES: Content of plasma C-reactive protein and Glasgow Coma Score in the two groups before and after treatment. RESULTS: All 60 patients were involved in the final analysis. ① Content of plasma C-reactive protein: The two contents were obviously higher than normal value after craniocerebral injury. There was no significant difference in the two groups before treatment (P 〉 0.05), but both contents were decreased after treatment, and there was significant difference between HBOT group and control group after treatment (t =4.756, P 〈 0.01). In addition, there was significant difference in hyperbaric oxygen therapy group before and after treatment (t =5.236, P 〈 0.01). ② Glasgow Coma Score: There was no significant difference in the two groups before treatment (P 〉 0.05), but scores were increased in both groups after treatment (t =9.92, 2.51, P 〈 0.01, 0.05); on the other hand, therefore, there was significant difference between the two groupsafter treatment (t =9.21, P 〈 0.01). CONCLUSION: Hyperbaric oxygenation can remarkably decrease content of plasma C-reactive protein in patients with craniocerebral injury at the phase of stress.展开更多

In the present study, we explored the mechanisms associated with the recovery of neurological fun...Clinical studies have shown that hyperbaric oxygen therapy improves motor function in patients with spinal cord injury. In the present study, we explored the mechanisms associated with the recovery of neurological function after hyperbaric oxygen therapy in a rat model of spinal cord injury. We established an acute spinal cord injury model using a modification of the free-falling object method, and treated the animals with oxygen at 0.2 MPa for 45 minutes, 4 hours after injury. The treatment was administered four times per day, for 3 days. Compared with model rats that did not receive the treatment, rats exposed to hyperbaric oxygen had fewer apoptotic cells in spinal cord tissue, lower expression levels of aquaporin 4/9 mRNA and protein, and more NF-200 positive nerve fibers. Furthermore, they had smaller spinal cord cavities, rapid recovery of somatosensory and motor evoked potentials, and notably better recovery of hindlimb motor function than model rats. Our findings indicate that hyperbaric oxygen therapy reduces apoptosis, downregulates aquaporin 4/9 mRNA and protein expression in injured spinal cord tissue, improves the local microenvironment for nerve regeneration, and protects and repairs the spinal cord after injury.展开更多
AIM: To investigate the individual and combined effects of allopurinol and hyperbaric oxygen (HBO) therapy on biochemical and histopathological changes, oxidative stress, and bacterial translocation (BT) in the e...AIM: To investigate the individual and combined effects of allopurinol and hyperbaric oxygen (HBO) therapy on biochemical and histopathological changes, oxidative stress, and bacterial translocation (BT) in the experimental rat acute pancreatitis (AP). METHODS: Eighty-five Sprague-Dawley rats were included in the study. Fifteen of the eighty-five rats were used as controls (sham, Group I ). AP was induced via intraductal taurocholate infusion in the remaining seventy rats. Rats that survived to induction of acute necrotizing pancreatitis were randomized into four groups. Group H received saline, Group m allopurinol, Group IV allopurinol plus HBO and Group v HBO alone. Serum amylase levels, oxidative stress parameters, BT and histopathologic scores were determined. RESULTS: Serum amylase levels were lower in Groups Ⅲ, Ⅳ and v compared to Group H (974 ± 110, 384 ± 40, 851 + 56, and 1664 Ⅳ 234 U/L, respectively, P 〈 0.05, for all). Combining the two treatment optionsrevealed significantly lower median [25-75 percentiles] histopathological scores when compared to individual administrations (13 [12.5-15] in allopurinol group, 9.5 [7-11.75] in HBO group, and 6 [4.5-7.5] in combined group, P 〈 0.01). Oxidative stress markers were significantly better in all treatment groups compared to the controls. Bacterial translocation into the pancreas and mesenteric lymph nodes was lower in Groups m, iV and v compared to Group H (54%, 23%, 50% vs 100% for translocation to pancreas, and 62%, 46%, 58% vs 100% for translocation to mesenteric lymph nodes, respectively, P 〈 0.05 for all). CONCLUSION: The present study confirms the benefit of HBO and allopurinol treatment when administered separately in experimental rat AP. Combination of these treatment options appears to prevent progression of pancreatic injury parameters more effectively.展开更多
Accumulating studies have demonstrated that hyperbaric oxygen(HBO)treatment alleviates spinal cord injury(SCI).However,the underlying mechanism by which HBO alleviates SCI remains to be elucidated.In this study,we per...Accumulating studies have demonstrated that hyperbaric oxygen(HBO)treatment alleviates spinal cord injury(SCI).However,the underlying mechanism by which HBO alleviates SCI remains to be elucidated.In this study,we performed genome-wide transcriptional profiling of the spinal cord between SCI mice and mice that received HBO treatment by high-throughput RNA sequencing at 1 week after SCI.We also compared genome-wide transcriptional profiles from SCI mice and sham-operated mice.We found 76 differentially co-expressed genes in sham-operated mice,SCI mice,and HBO-treated SCI mice.Using Gene Ontology and Kyoto Encyclopedia of Genes and Genomes enrichment analysis,we identified the biological characteristics of these differentially expressed genes from the perspectives of cell component,biological process,and molecular function.We also found enriched functional pathways including ferroptosis,calcium signaling pathway,serotonergic synapse,hypoxia-inducible factor-1 signaling pathway,cholinergic synapse,and neuroactive ligand-receptor interaction.We performed quantitative reverse transcription-polymerase chain reaction and validated that HBO treatment decreased the expression of Hspb1(heat shock protein beta 1),Hmox1(heme oxygenase 1),Ftl1(ferritin light polypeptide 1),Tnc(tenascin C)and Igfbp3(insulin-like growth factor binding protein 3)and increased the expression of Slc5a7(solute carrier family 5 choline transporter member 7)after SCI.These results revealed the genome-wide transcriptional profile of the injured spinal cord after HBO treatment.Our findings contribute to a better understanding of the mechanism by which HBO treats SCI and may provide new targets for SCI intervention.展开更多

AIM:To investigate the role of growth hormone(GH),hyperbaric oxygen therapy(HBOT)and combined therapy on the intestinal neomucosa formation of the gastric serosa.METHODS:Forty-eight male Wistar-albino rats,weighing 25...AIM:To investigate the role of growth hormone(GH),hyperbaric oxygen therapy(HBOT)and combined therapy on the intestinal neomucosa formation of the gastric serosa.METHODS:Forty-eight male Wistar-albino rats,weighing 250-280 g,were used in this study.The rats were divided into four groups(n=12):Group 1,control,gastric serosal patch;Group 2,gastric serosal patch+GH;Group 3,gastric serosal patch+HBOT;and Group 4,gastric serosal patch+GH+HBOT.Abdominal access was achieved through a midline incision,and after the 1-cm-long defect was created in the jejunum,a 1 cm×1 cm patch of the gastric corpus was anastomosed to the jejunal defect.Venous blood samples were taken to determine the insulin-like growth factor 1(IGF-1)and insulin-like growth factor binding protein 3(IGFBP-3)basal levels.HBOT was performed in Groups 3 and 4.In Groups 2 and 4,human GH was given subcutaneously at a dose of 2 mg per kg/d for 28 d,beginning on the operation day.All animals were sacrificed 60 d after surgery.The jejunal segment and the gastric anastomotic area were excised for histological examination.The inflammatory process,granulation,collagen deposition and fibroblast activity at the neomucosa formation were studied and scored.Additionally,the villus density,villus height,and crypt depth were counted and recorded.The measurements of villus height and crypt depth were calculated with an ocular micrometer.New vessel growth was determined by calculatingeach new vessel in a 1 mm 2 area.RESULTS:In the histological comparison of groups,no significant differences were observed between the control group and Groups 2 and 3 with respect to epithelialization,granulation,fibroblastic activity and the inflammatory process,but significant differences were present between the control group and all others groups(Groups 2-4)with respect to angiogenesis(P<0.01)and collagen deposition(P<0.05,P<0.01).Significant differences between the control group and Group 4 were also observed with respect to epithelialization and fibroblastic activity(P<0.01 and P<0.05,respectively).There were significant differences in villus density in all of groups compared with the control group(P<0.05).Crypt depth was significantly greater in Group 4 than in the control group(P<0.05),but no other groups had deeper crypts.However,villus height was significantly longer in Groups 2 and 4 than in the control group(P<0.05).The comparison of groups revealed,significant difference between control group and Groups 2 and 4)with respect to the levels of IGF-1 and IGFBP-3(P<0.01)3 wk after the operation.CONCLUSION:HBOT or GH and combined therapy augmented on neomucosal formation.The use of combined therapy produced a synergistic effect on the histological,morphological and functional parameters.展开更多

OBJECTIVE: To investigate the dynamic changes of Na^+-K^+-ATPase and Ca^2+-ATPase activity in the rat brain following transient global cerebral ischemia-reperfusion (IR), as well as the effects of hyperbaric oxygen (HBO) treatment. DESIGN, TIME AND SETTING: A randomized and controlled animal study was performed in the Department of Biochemistry and Molecular Biology, Capital Medical University between February and December 2006. MATERIALS: Clean-grade, female, Sprague Dawley rats were provided by the Animal Research Department of Capital Medical University (License number: SYXK11-00-0047). Na^+-K^+-ATPase and Ca^2+-ATPase kits were provided by Nanjing Jiancheng Bioengineering Institute (Nanjing, China). A hyperbaric oxygen chamber (DWC150-300) was supplied by Shanghai 701 Medical Oxygen Chamber Factory (Shanghai, China). METHODS: Sixty-three rats were randomly divided into nine groups: sham operated group (sham-O) as control, groups of IR, and groups treated with hyperbaric oxygen (HBO) after IR. Animal from the IR and HBO groups were sacrificed after four different survival intervals of 6, 24, 48 and 96 hours, respectively. Each group consisted of seven rats. The rats of HBO groups were placed into the hyperbaric chamber. The HBO chamber was flushed with pure oxygen for 5 minutes, followed by a gradual rise in pressure over 5 minutes and stabilization at 0.2 MPa. Then, pure oxygen was supplied for 45 minutes in stabilized pressure, followed by gradually reduced pressure over 15 minutes. The rats of the 6-h HBO group were placed into the HBO chamber following reperfusion for 3 hours on the first day, which was repeated on three consecutive days, always at the same time. Rats in the sham-O group and IR group remained under normal atmospheric pressure. MAIN OUTCOME MEASURES: The Na^+-K^+-ATPase and Ca^2+-ATPase activity in rat brain homogenate was detected by the ammonium molybdate assay method. RESULTS: All 63 rats were included in the final analysis. Alter 6 hours, Na^+-K^+-ATPase activity was significantly greater in HBO animals, compared with IR animals (P 〈 0.05) and sham-O controls (P〈 0.01). In both, the HBO group and IR group, Na^+-K^+-ATPase activity returned to normal levels after 24 hours (P 〉 0.05). At 48 and 96 hours, Na^+-K^+-ATPase activity was significantly greater in HBO and IR animals, compared with sham-O animals (P 〈 0.05). Ca^2+-ATPase activity was significantly greater in the HBO group after 6 hours, compared with the sham-O group (P 〈 0.01 ), and returned to normal levels at 24 and 96 hours (P 〉 0.05). In the IR group, Ca^2+-ATPase activity was significantly higher after 6 hours than in the sharn-O group (P〈 0.01), and returned to normal levels after 24 hours (P 〉 0.05). CONCLUSION: The Na^+-K^+-ATPase and Ca^2+-ATPase activity in IR groups increased during the acute and the delayed phase following transient global cerebral IR. HBO treatment not only increased Na^+-K^+-ATPase activity at the acute stage, but also induced a faster recovery of Ca^2+-ATPase activity.展开更多

摘要This study established human cerebral organoids as a promising platform for investigating central nervous system oxygen toxicity (CNS-OT). Through integrated transcriptomic, functional, and pharmacological analyses, we demonstrate that hyperbaric oxygen (HBO) exposure triggers pressure-dependent neurotoxicity mediated by the reactive oxygen species (ROS)-lysosome-mechanistic target of rapamycin (mTOR) axis. Key findings include the following: mechanistic hierarchy: Five atmospheres absolute (ATA) HBO induces metabolic dysregulation and cell cycle arrest, whereas six ATA exceeds compensatory thresholds, triggering overt apoptotic signatures;pathway crosstalk: Lysosomal permeabilization activates mTOR complex 1 (mTORC1) via cathepsin release, while mTORC1 hyperactivation suppresses transcription factor EB (TFEB)-mediated lysosomal regeneration, creating a self-amplifying loop;therapeutic potential: Mouse validation confirmed that mTOR inhibition (temsirolimus) attenuates neurotoxicity, with hippocampus-specific efficacy. The cerebral organoid model offers a human-relevant system to overcome species limitations in neurotoxicity research, facilitating mechanistic discovery and therapeutic target identification.
摘要BACKGROUND Hyperbaric oxygen therapy(HBOT)has been explored as a potential treatment for depression,with various studies indicating its benefits on psychological,cognitive,and functional outcomes.AIM To evaluate the efficacy of HBOT in treating depression.METHODS A systematic review of randomized controlled trials was conducted to assess the impact of HBOT on depression.Studies were identified through database searches,screened for eligibility,and assessed for bias.Outcome measures included standardized depression scales,cognitive assessments,and functional evaluations.RESULTS It was observed across the 6 included trials that HBOT significantly reduced depressive symptoms and improved functional outcomes across multiple studies.Improvements in psychological and neurological measures were consistently reported.Additionally,substantial relief from post-concussion symptoms and enhancements in cognitive and emotional well-being were noted.Some studies highlighted the correlation between physiological markers,such as hemoglobin A1c levels,and depressive symptoms,suggesting HBOT's broader therapeutic potential.Combining HBOT with pharmacological treatments showed rapid improvements in depression and cognitive function,further supporting HBOT's efficacy.CONCLUSION HBOT demonstrated significant efficacy in reducing depressive symptoms and improving cognitive and functional outcomes.The therapy's potential benefits extended across various measures,indicate its potential as a multifaceted treatment for depression.Further research is recommended to explore HBOT's mechanisms and optimize treatment protocols.
摘要Objective: To evaluate the efficacy of an integrated nursing care protocol in tracheostomized comatose patients undergoing hyperbaric oxygen therapy, with a focus on its impact on neurological outcomes and treatment safety. Methods: From January 2024 to January 2026,80 tracheostomized comatose patients requiring hyperbaric oxygen therapy were enrolled. Patients were randomly assigned into two groups. The control group received standard hyperbaric oxygen therapy protocols and general neurosurgical nursing care;the observation group additionally implemented a comprehensive nursing strategy spanning the entire treatment course, emphasizing patient safety management in the hyperbaric chamber, meticulous respiratory care, continuous monitoring of key physiological parameters, and proactive prevention of complications. Results: Upon completion of the prescribed treatment course, the observation group demonstrated significantly greater improvements in consciousness levels and daily activity scores compared to the control group (P <0.05). Throughout the treatment period, the overall incidence of treatment-or care-related adverse events was markedly lower in the observation group (5.0%) versus the control group (27.5), with a statistically significant difference (P <0.05). Additionally, the observation group exhibited superior clinical outcomes in terms of weaning from mechanical ventilation support and recovery of consciousness. Conclusion: For comatose patients who have undergone tracheostomy, implementing comprehensive and systematic nursing interventions during hyperbaric oxygen therapy can effectively ensure treatment safety, promote neurological function recovery, and significantly reduce complication rates, thereby providing a reliable nursing practice pathway for optimizing the clinical prognosis of such patients.
摘要BACKGROUND Central retinal artery occlusion(CRAO)is a potentially blinding disease,and hyperbaric oxygen therapy(HBOT)is becoming increasingly popular with the support of scientific evidence.Despite the presence of various acute management measures,there is no clear evidence on the gold standard treatment for CRAO.AIM To identify factors and imaging parameters associated with good visual outcome,which guide ophthalmologists in the triage of CRAO patients for HBOT.METHODS Patients who suffered from CRAO and had a symptom onset≤6 h were recruited for a course of HBOT in a tertiary hospital after failing bedside treatment.Patient demographics,onset time,CRAO eye parameters,and past medical history were prospectively collected.Visual outcomes after HBOT were also analyzed.RESULTS A total of 26 patients were included;the female-to-male ratio was 1:1.6,and the mean age was 67.5 years±13.3 years(range 44–89 years).The mean duration of follow-up and mean visual acuity(VA)improvement were 10.0 mo±5.3 mo and 0.48 logarithm of minimal angle of resolution(logMAR)±0.57 logMAR(approx-imately 9 letters in ETDRS)(P=0.0001,Z=-3.67),respectively.The 1 mm zone of central macular thickness(CMT)on optical coherence tomography was not associated with VA changes(P=0.119);however,the 1-to-3 mm circular rim of CMT was fairly associated(P=0.02,Spearman's coefficient=0.45).Complete retinal perfusion time during fundus fluorescein angiography(FFA)was mode-rately associated(P=0.01,Spearman's coefficient=0.58)with visual outcome.
摘要BACKGROUND Endoscopic therapy is the primary approach for treating Mallory-Weiss syndro-me,particularly under conditions of mucosal protection and gastric acid suppre-ssion.However,for a subset of patients who cannot undergo endoscopic interven-tion or for whom such treatment proves ineffective,alternative measures like arterial embolization or surgical intervention may be required.While hyperbaric oxygen therapy(HBOT)has been applied across a range of medical conditions,its application in managing hemorrhage due to gastric tears remains undocumented.CASE SUMMARY A 52-year-old patient was admitted with symptoms of hematemesis and melena,and an endoscopy revealed a gastric fundus tear approximately 4 cm×5 cm in size.The lesion was considered unsuitable for endoscopic repair by the attending endoscopist.Despite conservative measures,including fasting and acid sup-pression,the patient experienced persistent bleeding and a substantial decrease in hemoglobin levels relative to admission values.Following a multidisciplinary consultation,HBOT was initiated,resulting in the cessation of bleeding and rapid wound healing.CONCLUSION For patients with gastric tears presenting with active hemorrhage,HBOT might offer an effective alternative when conventional endoscopic therapies are not viable or have been unsuccessful.
摘要Radiotherapy(RT)is the backbone of multimodality treatment of more than half of cancer cases.Despite new modern RT techniques,late complications may occur such as radiation proctitis(RP).The natural history of RP is unpredictable.Minor symptoms may resolve spontaneously or require conservative treatment.On the other hand,for similar and uncomplicated clinical contexts,symptoms may persist and can even be refractory to the progressive increase in treatment measures.Over the last decades,an enormous therapeutic armamentarium has been considered in RP,including hyperbaric oxygen therapy(HBOT).Currently,the evidence regarding the impact of HBOT on RP and its benefits is conflicting.Additional prospective and randomised studies are necessary to validate HBOT’s effectiveness in the‘real world’clinical practice.This article reviewed the relevant literature on pathophysiology,clinical presentation,different classifications and discuss RP management including a proposal for a therapeutic algorithm with a focus on HBOT.

摘要Objective: Keloids are exuberant cutaneous scars that form due to abnormal growth of fibrous tissue fol- lowing an injury. The primary aim of this study was to assess the efficacy and mechanism of hyperbaric oxygen therapy (HBOT) to reduce the keloid recurrence rate after surgical excision and radiotherapy. Methods: (1) A total of 240 patients were randomly divided into two groups. Patients in the HBOT group (O group) received HBOT after surgical excision and radiotherapy. Patients in the other group were treated with only surgical excision and radiotherapy (K group). (2) Scar tissue from recurrent patients was collected after a second operation. Hematoxylin and eosin (H&E) staining was used to observe keloid morphology. Certain inflammatory factors (interleukin-6 (IL-6), hypoxia-inducible factor-1α (HIF-1α), tumor necrosis factor-α (TNF-α), nuclear factor KB (NF-κB), and vascular endothelial growth factor (VEGF)) were measured using immunohistochemical staining. Results: (1) The recurrence rate of the O group (5.97%) was significantly lower than that of the K group (14.15%), P〈0.05. Moreover, patients in the O group reported greater satisfaction than those in the K group (P〈0.05). (2) Compared with the recurrent scar tissue of the K group, the expression levels of the inflammatory factors were lower in the recurrent scar tissue of the O group. Conclusions: Adjunctive HBOT effectively reduces the keloid recurrence rate after surgical excision and radiotherapy by improving the oxygen level of the tissue and alleviating the inflammatory process.

摘要Objective Hyperbaric oxygen treatment(HBOT)has demonstrated efficacy in improving hearing levels of patients with idiopathic sudden sensorineural hearing loss(ISSHL);however,the underlying mechanisms are not well understood.HBOT alleviates the inflammatory response,which is mediated by Toll-like receptor(TLR)4 and nuclear factor(NF)-κB.In this study we investigated whether HBOT attenuates inflammation in ISHHL patients via alteration of TLR4 and NF-κB expression.Methods ISHHL patients(n=120)and healthy control subjects(n=20)were enrolled in this study.Patients were randomly divided into medicine group treated with medicine only(n=60)and HBO group receiving both HBOT and medicine(n=60).Audiometric testing was performed pre-and posttreatment.TLR4,NF-кB,and TNF-αexpression in peripheral blood of ISSHL patients and healthy control subjects was assessed by ELISA before and after treatment.Results TLR4,NF-κB,and TNF-αlevels were upregulated in ISSHL patients relative to healthy control subjects;the levels were decreased following treatment and were lower in the HBO group than that in the medicine group post-treatment(P<0.05 and P<0.01).Conclusion HBOT alleviates hearing loss in ISSHL patients by suppressing the inflammatory response induced by TLR4 and NF-κB signaling.

摘要Objective:To investigate the effect of BMSCs transplantation plus hyperbaric oxygen(HBO)on repair of rat SCI.Methods:Seventy five male rats were divided randomly into five groups:sham,vehicle.BMSCs transplantation group,combination group,15 rats in each group.Every week after the SCI onset,all animals were evaluated for behavior outcome by Basso-BeattleBresnahan(BBB) score and inclined plane test.Axon recovery was examined with focal spinal cord tissue by electron microscope at 6 weeks after the SCI onset.HE staining and BrdU staining were performed to examine the BMSCs and lesion post injury.Somatosensory evoked potential(SEP) testing was performed to detect the recovery of neural conduction.Results from the behavior tests from combination group were significant higher than rats which received only transplantation or HBO treatment.Results from histopathology showed favorable recovery from combination group than other treatment groups.The number of BrdU+ in combination group were measureable more than transplantation group(P<0.05).The greatest decrease in TNF-α,IL-1β,IL-6.IFN-α determined by Elisa assay in combination group were evident too.Conclusions:BMSCs transplantation can promote the functional recovery of rat hind limbs after SCI,and its combination with HBO has a synergistic effect.

摘要Welding polarity has influence on welding stability to some extent,but the specific relationship between welding polarity and weld quality has not been found,especially under the hyperbaric environment.Based on a hyperbaric dry welding experiment system,gas metal arc welding(GMAW)experiments with direct current electrode positive(DCEP)and direct current electrode negative(DCEN)operations are carried out under the ambient pressures of 0.1 MPa,0.4 MPa,0.7 MPa and 1.0 MPa to find the influence rule of different welding polarities on welding spatters and weld bead geometry.The effects of welding polarities on the weld bead geometry such as the reinforcement,the weld width and the penetration are discussed.The experimental results show that the welding spatters gradually grow in quantity and size for GMAW with DCEP,while GMAW with DCEN can produce fewer spatters comparatively with the increase of the ambient pressure.Compared with DCEP,the welding current and arc voltage waveforms for DCEN is more stable and the distribution of welding current probability density for DCEN is more concentrated under the hyperbaric environment.When the ambient pressure is increased from 0.1 MPa to 1.0 MPa,the effects of welding polarities on the reinforcement,the weld width and the penetration are as follows:an increase of 0.8 mm for the weld reinforcement is produced by GMAW with DCEN and 1.3 mm by GMAW with DCEP,a decrease of 7.2 mm for the weld width is produced by DCEN and 6.1 mm by DCEP;and an increase of 3.9 mm for the penetration is produced by DCEN and 1.9 mm by DCEP.The proposed research indicates that the desirable stability in the welding procedure can be achieved by GMAW with DCEN operation under the hyperbaric environment.

摘要Hyperbaric oxygen therapy for the treatment of neonatal hypoxic-ischemic brain damage has been used clinically for many years, but its effectiveness remains controversial. In addition, the mechanism of this potential neuroprotective effect remains unclear. This study aimed to investigate the influence of hyperbaric oxygen on the proliferation of neural stem cells in the subventricular zone of neonatal Sprague-Dawley rats (7 days old) subjected to hypoxic-ischemic brain damage. Six hours after modeling, rats were treated with hyperbaric oxygen once daily for 7 days. Immunohistochemistry revealed that the number of 5-bromo-2'-deoxyuridine positive and nestin positive cells in the subventricular zone of neonatal rats increased at day 3 after hypoxic-ischemic brain damage and peaked at day 5. After hyperbaric oxygen treatment, the number of 5-bromo-2'- deoxyuddine positive and nestin positive cells began to increase at day 1, and was significantly higher than that in normal rats and model rats until day 21. Hematoxylin-eosin staining showed that hyperbaric oxygen treatment could attenuate pathological changes to brain tissue in neonatal rats, and reduce the number of degenerating and necrotic nerve cells. Our experimental findings indicate that hyperbaric oxygen treatment enhances the proliferation of neural stem cells in the subventricular zone of neonatal rats with hypoxic-ischemic brain damage, and has therapeutic potential for promoting neurological recovery following brain injury.
摘要Changes of plasma C-reactive protein in patients with craniocerebral injury before and after hyperbaric oxygenation: A randomly controlled study BACKGROUND: Plasma inflammatory factor, such as C-reactive protein, whose content is regarded as a sensitively pathological marked protein and quantitative indexes of central nervous system injury, has been paid more and more attention in clinic. OBJECTIVE: To observe the effects and clinical significance of C-reactive protein in patients with craniocerebral injury after hyperbaric oxygenation. DESIGN: Randomized controlled study. SETTING: Departments of Neurosurgery, Laboratory and Hyperbaric Oxygen, the Second Affiliated Hospital, Medical College of Shantou University. PARTICIPANTS: A total of 60 patients with craniocerebral injury were selected from Department of Neurosurgery, the Second Affiliated Hospital, Medical College of Shantou University from October 2006 to April 2007. There were 37 males and 23 females and the mean age was 26 years. All subjects were certainly diagnosed as history of craniocerebral injury. Patients hospitalized at 24 hours after injury, Glasgow Coma Score ranged from 3 to 12 points, and all patients were certainly diagnosed with CT or MR scanning. Patients and their relatives provided confirmed consent. All the subjects were randomly divided into hyperbaric oxygenation group and control group with 30 in each group. METHODS: Patients in the control group were treated with routinely neurosurgical therapy after hospitalization; however, based the same basic treatment in the control group, patients in the hyperbaric oxygenation group received hyperbaric oxygenation by using iced-wheel four-door 2-cabin air-compression chamber (made in Yantai) from 24 hours to 10 days after operation or injury. After entering the cabin, patients who had a clear consciousness breathed the oxygen by using face mask; contrarily, patients directly breathed the oxygen. Therapeutic project: Expression was increased for about 15–20 minutes, maintained for about 70–80 minutes, and decreased for 20 minutes. Otherwise, pressure was maintained from 0.2 to 0.25 MPa. Hyperbaric oxygenation took an hour for once a day and 10 times were regarded as a course. Venous blood was collected before treatment and on the next day of the first course end. Content of C-reactive protein in plasma was measured with immune turbidimetry in hyperbaric oxygenation group; in addition, content of C-reactive protein in plasma was directly measured with the same method at the corresponding time in the control group. If the content was less or equal to 8 mg/L, it was regarded as normal value. Effects of the two groups were evaluated based on Glasgow Coma Score before and after treatment. MAIN OUTCOME MEASURES: Content of plasma C-reactive protein and Glasgow Coma Score in the two groups before and after treatment. RESULTS: All 60 patients were involved in the final analysis. ① Content of plasma C-reactive protein: The two contents were obviously higher than normal value after craniocerebral injury. There was no significant difference in the two groups before treatment (P 〉 0.05), but both contents were decreased after treatment, and there was significant difference between HBOT group and control group after treatment (t =4.756, P 〈 0.01). In addition, there was significant difference in hyperbaric oxygen therapy group before and after treatment (t =5.236, P 〈 0.01). ② Glasgow Coma Score: There was no significant difference in the two groups before treatment (P 〉 0.05), but scores were increased in both groups after treatment (t =9.92, 2.51, P 〈 0.01, 0.05); on the other hand, therefore, there was significant difference between the two groupsafter treatment (t =9.21, P 〈 0.01). CONCLUSION: Hyperbaric oxygenation can remarkably decrease content of plasma C-reactive protein in patients with craniocerebral injury at the phase of stress.

摘要Clinical studies have shown that hyperbaric oxygen therapy improves motor function in patients with spinal cord injury. In the present study, we explored the mechanisms associated with the recovery of neurological function after hyperbaric oxygen therapy in a rat model of spinal cord injury. We established an acute spinal cord injury model using a modification of the free-falling object method, and treated the animals with oxygen at 0.2 MPa for 45 minutes, 4 hours after injury. The treatment was administered four times per day, for 3 days. Compared with model rats that did not receive the treatment, rats exposed to hyperbaric oxygen had fewer apoptotic cells in spinal cord tissue, lower expression levels of aquaporin 4/9 mRNA and protein, and more NF-200 positive nerve fibers. Furthermore, they had smaller spinal cord cavities, rapid recovery of somatosensory and motor evoked potentials, and notably better recovery of hindlimb motor function than model rats. Our findings indicate that hyperbaric oxygen therapy reduces apoptosis, downregulates aquaporin 4/9 mRNA and protein expression in injured spinal cord tissue, improves the local microenvironment for nerve regeneration, and protects and repairs the spinal cord after injury.
摘要AIM: To investigate the individual and combined effects of allopurinol and hyperbaric oxygen (HBO) therapy on biochemical and histopathological changes, oxidative stress, and bacterial translocation (BT) in the experimental rat acute pancreatitis (AP). METHODS: Eighty-five Sprague-Dawley rats were included in the study. Fifteen of the eighty-five rats were used as controls (sham, Group I ). AP was induced via intraductal taurocholate infusion in the remaining seventy rats. Rats that survived to induction of acute necrotizing pancreatitis were randomized into four groups. Group H received saline, Group m allopurinol, Group IV allopurinol plus HBO and Group v HBO alone. Serum amylase levels, oxidative stress parameters, BT and histopathologic scores were determined. RESULTS: Serum amylase levels were lower in Groups Ⅲ, Ⅳ and v compared to Group H (974 ± 110, 384 ± 40, 851 + 56, and 1664 Ⅳ 234 U/L, respectively, P 〈 0.05, for all). Combining the two treatment optionsrevealed significantly lower median [25-75 percentiles] histopathological scores when compared to individual administrations (13 [12.5-15] in allopurinol group, 9.5 [7-11.75] in HBO group, and 6 [4.5-7.5] in combined group, P 〈 0.01). Oxidative stress markers were significantly better in all treatment groups compared to the controls. Bacterial translocation into the pancreas and mesenteric lymph nodes was lower in Groups m, iV and v compared to Group H (54%, 23%, 50% vs 100% for translocation to pancreas, and 62%, 46%, 58% vs 100% for translocation to mesenteric lymph nodes, respectively, P 〈 0.05 for all). CONCLUSION: The present study confirms the benefit of HBO and allopurinol treatment when administered separately in experimental rat AP. Combination of these treatment options appears to prevent progression of pancreatic injury parameters more effectively.

摘要Accumulating studies have demonstrated that hyperbaric oxygen(HBO)treatment alleviates spinal cord injury(SCI).However,the underlying mechanism by which HBO alleviates SCI remains to be elucidated.In this study,we performed genome-wide transcriptional profiling of the spinal cord between SCI mice and mice that received HBO treatment by high-throughput RNA sequencing at 1 week after SCI.We also compared genome-wide transcriptional profiles from SCI mice and sham-operated mice.We found 76 differentially co-expressed genes in sham-operated mice,SCI mice,and HBO-treated SCI mice.Using Gene Ontology and Kyoto Encyclopedia of Genes and Genomes enrichment analysis,we identified the biological characteristics of these differentially expressed genes from the perspectives of cell component,biological process,and molecular function.We also found enriched functional pathways including ferroptosis,calcium signaling pathway,serotonergic synapse,hypoxia-inducible factor-1 signaling pathway,cholinergic synapse,and neuroactive ligand-receptor interaction.We performed quantitative reverse transcription-polymerase chain reaction and validated that HBO treatment decreased the expression of Hspb1(heat shock protein beta 1),Hmox1(heme oxygenase 1),Ftl1(ferritin light polypeptide 1),Tnc(tenascin C)and Igfbp3(insulin-like growth factor binding protein 3)and increased the expression of Slc5a7(solute carrier family 5 choline transporter member 7)after SCI.These results revealed the genome-wide transcriptional profile of the injured spinal cord after HBO treatment.Our findings contribute to a better understanding of the mechanism by which HBO treats SCI and may provide new targets for SCI intervention.

摘要Automatic weld seam tracking technology to be used in hyperbaric underwater damaged pipeline repair welding is much more important,because of poor bevel preparation and severe working condition.A weld seam tracking system based on digital signal processing(DSP)passive light weld image processing technology has been established.A convenient charge coupled device(CCD)camera system was used in the high pressure environment with the help of an aperture and focus altering mechanism to guarantee overall image visibility in the scope of pressure below 0.7 MPa.The system can be used in the hyperbaric environment to pick up the real welding image of both the welding arc and the welding pool.The newly developed DSP technology was adopted to achieve the goal of system real time characteristics.An effective weld groove edge recognition technique including narrow interesting window opening,middle value wave filtering,Sobel operator weld edge detecting and edge searching in a defined narrow area was proposed to remove the guide error and system accuracy was ensured.The results of tracking simulation and real tracking application with arc striking have proved the validity and the accuracy of the mentioned system and the image processing method.
摘要AIM:To investigate the role of growth hormone(GH),hyperbaric oxygen therapy(HBOT)and combined therapy on the intestinal neomucosa formation of the gastric serosa.METHODS:Forty-eight male Wistar-albino rats,weighing 250-280 g,were used in this study.The rats were divided into four groups(n=12):Group 1,control,gastric serosal patch;Group 2,gastric serosal patch+GH;Group 3,gastric serosal patch+HBOT;and Group 4,gastric serosal patch+GH+HBOT.Abdominal access was achieved through a midline incision,and after the 1-cm-long defect was created in the jejunum,a 1 cm×1 cm patch of the gastric corpus was anastomosed to the jejunal defect.Venous blood samples were taken to determine the insulin-like growth factor 1(IGF-1)and insulin-like growth factor binding protein 3(IGFBP-3)basal levels.HBOT was performed in Groups 3 and 4.In Groups 2 and 4,human GH was given subcutaneously at a dose of 2 mg per kg/d for 28 d,beginning on the operation day.All animals were sacrificed 60 d after surgery.The jejunal segment and the gastric anastomotic area were excised for histological examination.The inflammatory process,granulation,collagen deposition and fibroblast activity at the neomucosa formation were studied and scored.Additionally,the villus density,villus height,and crypt depth were counted and recorded.The measurements of villus height and crypt depth were calculated with an ocular micrometer.New vessel growth was determined by calculatingeach new vessel in a 1 mm 2 area.RESULTS:In the histological comparison of groups,no significant differences were observed between the control group and Groups 2 and 3 with respect to epithelialization,granulation,fibroblastic activity and the inflammatory process,but significant differences were present between the control group and all others groups(Groups 2-4)with respect to angiogenesis(P<0.01)and collagen deposition(P<0.05,P<0.01).Significant differences between the control group and Group 4 were also observed with respect to epithelialization and fibroblastic activity(P<0.01 and P<0.05,respectively).There were significant differences in villus density in all of groups compared with the control group(P<0.05).Crypt depth was significantly greater in Group 4 than in the control group(P<0.05),but no other groups had deeper crypts.However,villus height was significantly longer in Groups 2 and 4 than in the control group(P<0.05).The comparison of groups revealed,significant difference between control group and Groups 2 and 4)with respect to the levels of IGF-1 and IGFBP-3(P<0.01)3 wk after the operation.CONCLUSION:HBOT or GH and combined therapy augmented on neomucosal formation.The use of combined therapy produced a synergistic effect on the histological,morphological and functional parameters.
摘要BACKGROUND Whether hyperbaric oxygen therapy(HBOT)can cause paradoxical herniation is still unclear.CASE SUMMARY A 65-year-old patient who was comatose due to brain trauma underwent decompressive craniotomy and gradually regained consciousness after surgery.HBOT was administered 22 d after surgery due to speech impairment.Paradoxical herniation appeared on the second day after treatment,and the patient’s condition worsened after receiving mannitol treatment at the rehabilitation hospital.After timely skull repair,the paradoxical herniation was resolved,and the patient regained consciousness and had a good recovery as observed at the follow-up visit.CONCLUSION Paradoxical herniation is rare and may be caused by HBOT.However,the underlying mechanism is unknown,and the understanding of this phenomenon is insufficient.The use of mannitol may worsen this condition.Timely skull repair can treat paradoxical herniation and prevent serious complications.

摘要BACKGROUND: Energy depletion, induced by ischemia or hypoxia, is one of the first events in neuronal injury. OBJECTIVE: To investigate the dynamic changes of Na^+-K^+-ATPase and Ca^2+-ATPase activity in the rat brain following transient global cerebral ischemia-reperfusion (IR), as well as the effects of hyperbaric oxygen (HBO) treatment. DESIGN, TIME AND SETTING: A randomized and controlled animal study was performed in the Department of Biochemistry and Molecular Biology, Capital Medical University between February and December 2006. MATERIALS: Clean-grade, female, Sprague Dawley rats were provided by the Animal Research Department of Capital Medical University (License number: SYXK11-00-0047). Na^+-K^+-ATPase and Ca^2+-ATPase kits were provided by Nanjing Jiancheng Bioengineering Institute (Nanjing, China). A hyperbaric oxygen chamber (DWC150-300) was supplied by Shanghai 701 Medical Oxygen Chamber Factory (Shanghai, China). METHODS: Sixty-three rats were randomly divided into nine groups: sham operated group (sham-O) as control, groups of IR, and groups treated with hyperbaric oxygen (HBO) after IR. Animal from the IR and HBO groups were sacrificed after four different survival intervals of 6, 24, 48 and 96 hours, respectively. Each group consisted of seven rats. The rats of HBO groups were placed into the hyperbaric chamber. The HBO chamber was flushed with pure oxygen for 5 minutes, followed by a gradual rise in pressure over 5 minutes and stabilization at 0.2 MPa. Then, pure oxygen was supplied for 45 minutes in stabilized pressure, followed by gradually reduced pressure over 15 minutes. The rats of the 6-h HBO group were placed into the HBO chamber following reperfusion for 3 hours on the first day, which was repeated on three consecutive days, always at the same time. Rats in the sham-O group and IR group remained under normal atmospheric pressure. MAIN OUTCOME MEASURES: The Na^+-K^+-ATPase and Ca^2+-ATPase activity in rat brain homogenate was detected by the ammonium molybdate assay method. RESULTS: All 63 rats were included in the final analysis. Alter 6 hours, Na^+-K^+-ATPase activity was significantly greater in HBO animals, compared with IR animals (P 〈 0.05) and sham-O controls (P〈 0.01). In both, the HBO group and IR group, Na^+-K^+-ATPase activity returned to normal levels after 24 hours (P 〉 0.05). At 48 and 96 hours, Na^+-K^+-ATPase activity was significantly greater in HBO and IR animals, compared with sham-O animals (P 〈 0.05). Ca^2+-ATPase activity was significantly greater in the HBO group after 6 hours, compared with the sham-O group (P 〈 0.01 ), and returned to normal levels at 24 and 96 hours (P 〉 0.05). In the IR group, Ca^2+-ATPase activity was significantly higher after 6 hours than in the sharn-O group (P〈 0.01), and returned to normal levels after 24 hours (P 〉 0.05). CONCLUSION: The Na^+-K^+-ATPase and Ca^2+-ATPase activity in IR groups increased during the acute and the delayed phase following transient global cerebral IR. HBO treatment not only increased Na^+-K^+-ATPase activity at the acute stage, but also induced a faster recovery of Ca^2+-ATPase activity.
