Development and implementation of an ecological education model for evidence-based practice competency in undergraduate nursing students:A mixed-methods feasibility study 认领 引用

Objectives This study aimed to develop a competency-oriented ecological education model and evaluate the feasibility and preliminary outcomes of its implementation for enhancing evidence-based practice competencies among undergraduate nursing students.Methods An ecological education model was developed based on educational ecology theory and the input–environment–output(IEO)theoretical framework,and implemented from 2019 to 2024.The model integrated three core components:a four-year progressive competency development pathway(ranging from evidence-based thinking in year one to comprehensive practice application in year four);a three-tier ecological environment(comprising physical,classroom,and practice environments that incorporates six best practice spotlight organization guidelines into eight core courses);and multidimensional practice platforms,including health science communication and guideline translation.The reform was conducted at a nursing school of a double first-class university in Beijing,China.A naturalistic longitudinal cohort design was employed,which included 1,223 undergraduate nursing students at baseline(2019,pre-reform)and 1,237 at post-implementation(2024).A convergent parallel mixed-methods approach was used to evaluate the implementation outcomes.Quantitative data on academic performance and innovation outcomes were collected at both time points,baseline and post-implementation.Semi-structured interviews with 18 students were conducted to supplement the exploration of their learning experiences.Results Academic engagement increased from 78.5%to 97.3%,evidence-based graduation theses rose from 18.6%to 30.0%,and innovation and entrepreneurship projects increased from 40 to 150.Qualitative analysis revealed four themes and twelve subthemes,revealing enhanced critical appraisal skills(88.9%),evidence synthesis capabilities(94.4%),and professional confidence(83.3%)among students.Integrated mixed-methods analysis revealed stage-specific characteristics across academic years:a complementarity pattern in Year 1(foundational awareness building),a convergence pattern in Years 2–3(systematic competency construction),an expansion pattern during the Year 3 transition(role adaptation challenges),and a convergence pattern in Year 4(comprehensive transformation and professional confidence enhancement).Conclusions This feasibility study demonstrated that the ecological education model could be successfully implemented in undergraduate nursing education with promising preliminary outcomes.This study provides an initial framework for nursing education innovation and establishes foundational evidence warranting future rigorous controlled studies to examine the model’s effectiveness. 展开更多

China’s poverty alleviation and elimination campaign(PAEC,2015-2021)aimed to eliminate absolute poverty and reduce social inequality,making improvements in the accessibility and equity of basic educational facilities a key component.However,existing research on the accessibility of educational facilities in China has predomi nantly focused on developed urban areas or specific regions,lacking nationwide spatiotemporal assessments.To address this gap,this study systematically evaluates the accessibility and equity of basic educational facilities(kindergartens,primary,and secondary schools)across mainland of China during the PAEC.Travel time cost,derived using multi-source geospatial datasets and the nearest neighbor method,was adopted as the primary indicator for assessing educational accessibility and equity.The results revealed pronounced disparities in ac cessibility between Eastern and Western China.Overall,both the accessibility and equity of basic educational facilities improved substantially during the campaign,with the average travel time per person reducing by ap proximately 50%.Notably,the rate of improvement in impoverished regions was nearly double that observed in non-impoverished areas.Although widespread improvements in educational equity have occurred,the urban rural disparity persists as a primary barrier to achieving comprehensive educational fairness.This study offers empirical evidence and methodological innovations for optimizing educational resource allocation and provides high-resolution temporal data to support the monitoring and evaluation of progress toward the Sustainable De velopment Goals in education. 展开更多

Objectives:This study aimed to explore the impact of innovative health education technologies grounded in the Transtheoretical Model(TTM)on self-care ability,stoma-related complication incidence,and satisfaction among patients with colorectal cancer undergoing enterostomy.Methods:A randomized controlled trial involving 200 patients who underwent enterostomy and were admitted by random sampling between January 2022 and December 2022 was conducted.The patients were divided into a control group(n=100)and an intervention group(n=100).The control group was educated using conventional methods,whereas the intervention group was educated using innovative health education technologies grounded in the TTM.Self-care ability,stoma-related complication incidence,and satisfaction scores were compared between the two groups.Results:All 200 patients completed the study.Before the intervention,there was no significant difference in self-care ability scores between the two groups(t=−0.295,P=0.870).After the intervention,the intervention group(56.31±13.75)had a significantly higher score than the control group(t=22.328,P<0.001).The comparison within each group revealed no significant change in the control group’s score(t=−0.191,P=0.984);however,the intervention group showed a significant difference(t=−22.340,P<0.001).The incidence of complications in the intervention group(11.0%)was significantly lower than that in the control group(38.0%)(χ²=19.71,P<0.010).The satisfaction rate in the intervention group(90.0%)was significantly greater than that in the control group(59.0%)(χ²=25.29,P<0.001).Conclusions:Applying innovative health education technologies based on the TTM for patients with enterostomy can effectively increase educational efficiency,improve patients’self-care ability,reduce the incidence of stoma-related complications,and enhance patients’satisfaction with the plan,thereby promoting innovation in health education. 展开更多

BACKGROUND Enhancing the professionalism of physicians in diagnosing and treating Helicobacter pylori(H.pylori)infection is crucial,given their pivotal role in the eradication therapy.AIM To explore the effectiveness of a single training course on H.pylori eradication therapy in gastroenterologists.METHODS This was a prospective,before-and-after,non-randomized interventional study based on real-world data.From March 2022 to December 2023,each physician enrolled 25 patients and acquired review outcomes,before they underwent professional training.Following this educational intervention,each physician proceeded to enroll an additional cohort of 25 patients and obtained subsequent review results for comparison.The primary outcome was a comparison of H.pylori eradication rate among gastroenterologists before and after training.RESULTS In total,20 physicians and 1000 patients were finally analyzed.After training,the eradication rates improved significantly in both intention-to-treat analysis[58.6%vs 71.6%,odds ratio(OR)=1.785,95%confidence interval(CI):1.370-2.325,P<0.001]and per-protocol analysis(81.2%vs 88.6%,OR=1.788,95%CI:1.191-2.684,P=0.005).The therapy standardization rates(71.0%vs 89.6%,OR=3.519,95%CI:2.490-4.974,P<0.001)and patient review rates(71.4%vs 80.4%,OR=1.643,95%CI:1.225-2.204,P=0.001)also improved significantly.However,no improvement was found in adverse reaction documentation rates(21.0%vs 21.2%,OR=1.010,95%CI:0.743-1.372,P=0.951).Subgroup analysis revealed that physicians with less experience and historically lower eradication rates benefited more from the training course.CONCLUSION A one-time training course on H.pylori eradication in physicians could improve H.pylori eradication rates,as well as therapy standardization rates and patient review rates. 展开更多

The prospective cohort study by Twohig et al evaluates the efficacy of a novel educational video module(EVM)in promoting treatment engagement and reducing alcohol use among hospitalized patients with alcohol-associated liver disease(ALD).Analyzing 42 patients,the study demonstrates that exposure to the EVM significantly increased rates of both pharmacologic(50%vs 22%)and psychosocial(73.8%vs 44%)treatment within 30 days of discharge,while markedly reducing the return to alcohol use(7.9%vs 35.6%)compared to a retrospective control cohort.These findings underscore the potential of a standardized,scalable educational intervention to bridge critical knowledge gaps in alcohol use disorder(AUD)management.While the study highlights the EVM as a powerful tool for patient empowerment and system-level quality improvement,its single-center design and limited sample size necessitate further validation through multicenter randomized trials.This article contextualizes these promising results within the broader challenge of AUD treatment,emphasizing the urgent need to integrate innovative,patient-centered education into standard clinical pathways to alleviate the growing burden of ALD. 展开更多

Traditional Chinese medicine(TCM)is increasingly recognized worldwide as a complementary and alternative medical system,valued for its empirical effectiveness,cost-efficiency,and safety.Australia became the first Western nation to regulate TCM practitioners under a national registration framework in 2012.This highlighted the critical role of TCM education in maintaining a qualified workforce and fostering evidence-based research.Currently,five higher education institutions in Australia offer TCM programs accredited by Ahpra.However,a number of public universities—such as University of Technology Sydney and the Royal Melbourne Institute of Technology—have discontinued their TCM programs due to financial constraints and declining enrollments.This decline in training capacity coincides with growing public demand for TCM services,particularly in chronic disease management and aged care,risking a future shortage of practitioners.Looking ahead,the role of training TCM professionals may increasingly fall to private colleges and institutes,which are not subject to the same external pressures and regulatory constraints as public universities.Meanwhile,an increasing proportion of female TCM practitioners represents a potential positive trend that may help mitigate the effects of a shrinking workforce.By tracing the historical evolution,regulatory milestones,and contemporary challenges of TCM education in Australia,this review aims to illuminate the broader trajectory of the TCM profession in a global context.Confronted with this potential crisis,enhanced integration with mainstream healthcare,policy support to improve practitioner remuneration,and innovative educational models to attract a new generation of students may benefit the development of TCM education. 展开更多

Objectives:There is a growing global concern regarding mental health and well-being in educational settings,especially in contexts of socio-educational exclusion.Active educational interventions offer an effective approach to enhance students’mental health and personal and social well-being.This study aimed to evaluate the effects of a psychosocial intervention program based on the Sports Education model(SEM)on subjective well-being(positive and negative affect)and externalizing problems(aggressiveness,hyperactivity,and behavioral problems)in youth students from a highly vulnerable socio-cultural community belonging to an ethnic minority with significant socio-educational support needs associated with socio-educational exclusion.Methods:The intervention took place during school hours,consisting of 17 sessions of 45 min each,at a publicly owned school serving disadvantaged communities.The ethnic minority sample,comprising all preadolescent students,consisted of 28 participants(13 males and 15 females)aged 10 to 15(Mean=10.19;SD=1.39),all of whom were part of the experimental group.The present study employed a single-group pre-test/post-test design due to the unique conditions of the school.Results:Subjective well-being was assessed using the PANAS Positive and Negative Affect Scale for Children and Adolescents(PANASN),while the teacher version of the Behavior Assessment System for Children(BASC-2;TRS)was employed to evaluate externalizing problems.The results suggested significant improvements in subjective well-being,with a significant increase in positive affect(d=0.80)and a significant decrease in negative affect(d=1.14),both exhibiting large effect sizes.Similarly,aggressiveness(d=0.62)and behavioral problems(d=0.79)showed significant reductions,both reflecting medium effect sizes.Conclusions:These findings suggest the suitability and effectiveness of this Sports Education program for promoting socio-affective improvements in students from socio-culturally disadvantaged backgrounds and academic environments.SEM-based programs are recommended,as they support mental health and social well-being,offer evidence-based benefits for teachers,and foster holistic student development. 展开更多

Digitalization and the Urban-Rural Education Gap:Evidence from China’s Rural Distance Compulsory Education Project 认领 引用

Public investment in the information and telecommunications technology(ICT)for rural basic education is crucial for promoting the sharing of highquality educational resources urban and rural areas and advancing educational equity.Utilizing data from the 2015 China 1%Population Sample Survey,this study evaluates the impact of the“Pilot Project for Modern Distance Education in Rural Primary and Secondary Schools”on educational equity.We analyze these effects through two lenses:the urban-rural education gap and inter-generational educational mobility within rural areas.Our findings reveal a dual effect:while a 100-yuan increase in per-student funding from the project narrows the urban-rural attainment gap by approximately 12.80%,it simultaneously diminishes relative inter-generational mobility within rural communities.Specifically,the correlation between the educational rank-orders of parents and their children increases by roughly 9.52%,suggesting a strengthening of educational persistence.To explore the mechanisms behind these divergent results,we conduct an analysis based on theoretical models of public education investment and human capital accumulation.The analysis demonstrates that although the project promotes overall human capital formation in rural areas,individuals from advantaged family backgrounds benefit disproportionately.This skewed distribution of benefits stems primarily from uneven improvements in educational quality resulting from the project’s implementation strategies.Heterogeneity analysis further indicates that females and children from wealthier households experience greater gains,while local fiscal preferences and managerial incentives influence the extent to which policy effects are realized.We conclude that while advancing rural distance education remains vital,policy focus should prioritize equitable fund allocation and the establishment of robust oversight mechanisms to ensure that educational informatics investments realize their potential for inclusive growth. 展开更多

Impact of“four-step”death education on life quality and negative emotions in advanced esophageal cancer patients and families 认领 引用

BACKGROUND Patients with advanced esophageal cancer and their families often experience reduced quality of life(QoL)and heightened negative emotions.Death education may offer benefits,but evidence in this context is limited.AIM To investigate the impact of a systematic“four-step death education”intervention on the QoL and negative emotions of postoperative patients with advanced esophageal cancer and their primary caregivers.METHODS A retrospective cohort study was conducted involving 235 patients with advanced esophageal cancer who underwent surgery at The Fourth Hospital of Hebei Medical University from June 2021 to June 2024.The participants were assigned to either the standard care group(SCG,n=127)or the“four-step death education”group(FDEG,n=108)on the basis of the received intervention.SCG received standard care and basic psychological support,and FDEG additionally received a four-stage death education program encompassing information provision,emotional support,life review and meaning exploration,and end-of-life care preparation.QoL and negative emotion levels were measured using validated instruments:The European Organization for Research and Treatment of Cancer QoL Questionnaire Core 30 for patient QoL,the Family QoL Survey,the Zarit Burden Interview for caregiver burden,the Pittsburgh Sleep Quality Index,the Self-Rating Anxiety Scale,the Self-Rating Depression Scale,and the Attitudes Toward Life and Death Scale.Assessments were conducted at baseline and 3 months post-intervention.RESULTS Baseline characteristics were comparable between groups.At 3 months,FDEG demonstrated significantly improved symptom and functional domain QoL scores in several areas,including fatigue,nausea,insomnia,loss of appetite,and physical and emotional functioning.FDEG reported increased family QoL and reduced caregiver burden.The patients and caregivers in FDEG showed superior improvements in sleep quality and greater reductions in anxiety and depression scores than those in SCG.Post-intervention,FDEG exhibited more positive attitudes toward death.CONCLUSION The postoperative“four-step death education”intervention enhances QoL,relieves caregiver burden,and reduces negative emotions among patients with advanced esophageal cancer and their caregivers,supporting broadened implementation of structured death education programs in palliative care settings. 展开更多

Objectives This scoping review aimed to identify and summarize the current research on virtual reality(VR)technologies used for health education in cancer patients,as well as to identify key areas of application.Methods In accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews guidelines,a comprehensive literature search was performed across 11 electronic databases and gray literature sources from inception to 12 September 2025.Studies employing immersive VR tools to improve health education outcomes in cancer patients were included.Data extraction and thematic synthesis were conducted to map evidence regarding VR modalities,educational applications,and outcome measures.Results Twenty-eight studies met the inclusion criteria.VR was applied across four primary educational scenarios,including radiotherapy,chemotherapy,surgery,and healthy behavior(including rehabilitation,smoking cessation,and self-management).Eight distinct VR modalities were identified,namely VR videos,virtual environments,virtual environment for radiotherapy training(VERT),VR interactions,3D models,VR games,VR non-player characters(VR NPCs),and virtual libraries.Among these,VR videos(50.0%),virtual environments(46.4%),and VR interactions(28.6%)were the most frequently employed.The interventions led to significant improvements in patient knowledge,skills,attitudes,health behaviors,and psychological well-being.A clear evolution in VR educational approaches has been observed,shifting from static environmental familiarization toward interactive,gamified,and intelligence-driven experiences.Nevertheless,notable gaps remain regarding safety protocols and data privacy protections,with only a minority of studies addressing these issues.Conclusions VR technologies demonstrate considerable promise as an innovative educational tool in oncology care,enhancing patient understanding,psychological preparedness,and engagement throughout the cancer journey.Future implementation must address infrastructural,ethical,and user-centered design barriers to facilitate the scalable and sustainable integration of this approach into clinical practice. 展开更多

Objective:This study aimed to develop and preliminary validate an entrustable professional activities(EPAs)framework for undergraduate midwifery students in China,providing a reference for undergraduate midwifery education and competency assessment.Methods:An initial EPA framework was constructed through a systematic literature review,qualitative research,and participant observation.From January to March 2022,semi-structured interviews were conducted with 27 stakeholders,followed by participant observation involving six individuals from April to May 2022.Subsequently,two rounds of Delphi consultation were conducted from June to October 2022 with 24 experts from 11 tertiary hospitals and six universities offering midwifery programs across eight provinces in China.Experts evaluated the importance,appropriateness,and entrustment level settings of each item,and the framework was revised accordingly.The finalized framework was then converted into a questionnaire and administered to undergraduate midwifery interns from three universities in Fujian Province to conduct a preliminary validation of its application.Results:Response rates for the two Delphi rounds were 87.5% and 83.3%,respectively.Kendall’s coefficients of concordance were 0.126 and 0.147(P<0.001),and expert authority coefficients(Cr)were 0.864 and 0.873.The finalized framework comprised three confidence levels,11 core EPAs,and 60 nested EPAs.In the preliminary validation phase,81 valid questionnaires were collected.The overall Cronbach’s α coefficient was 0.967,and the test-retest reliability(ICC)was 0.675,indicating good internal consistency and stability.Conclusion:The developed EPAs framework for undergraduate midwifery students demonstrates sound scientific rigor and feasibility and may provide theoretical and practical guidance for reforming undergraduate midwifery education and evaluating competency in China. 展开更多

AIM:To evaluate the ability of six advanced large language models(LLMs)—in providing accurate,comprehensive,and readable patient education on corneal refractive surgeries[laser in-situ keratomileusis(LASIK),keratorefractive lenticule extraction(KLEx),and photorefractive keratectomy(PRK)]in both English and Chinese.METHODS:This is a cross-sectional,comparative study.Twenty-six questions,compiled from authoritative ophthalmologic sources and covering four domains(procedure basics and eligibility;safety,risks and longterm stability;recovery and postoperative experience;and practical concerns),were administered in both English and Chinese via fresh chat sessions with each LLM,respectively.Five performance metrics were evaluated:accuracy,comprehensiveness,word count,readability,and reproducibility,using appropriate statistical tests.RESULTS:OpenAI o1 and DeepSeek-R1 consistently achieved the highest accuracy and most comprehensive responses,significantly outperforming ChatGPT-4o,Gemini Advanced,Claude Sonnet,and Tongyi Qwen(Friedman P<0.001).Although overall accuracy and comprehensiveness were similar across languages,Chinese responses were significantly longer.Readability varied among the models,with Claude Sonnet generally producing the most readable English texts.Reproducibility analysis revealed moderate consistency,reflecting inherent variability in outputs to identical prompts.CONCLUSION:Reasoning-augmented LLMs,particularly OpenAI o1 and DeepSeek-R1,demonstrate superior performance in delivering bilingual patient education for corneal refractive surgery,with high accuracy and comprehensiveness.However,variations in response length,readability,and reproducibility indicate that further refinement is necessary before these tools can be reliably integrated into clinical practice. 展开更多

BACKGROUND Orthopaedic surgical education has traditionally depended on the apprenticeship model of“see one,do one,teach one”.However,reduced operative exposure,stricter work-hour regulations,medicolegal constraints,and patient safety concerns have constrained its practicality.Simulation-based training has become a reliable,safe,and cost-efficient alternative.Dry lab techniques,especially virtual and augmented reality,make up 78%of current dry lab research,whereas wet labs still set the standard for anatomical realism.AIM To evaluate the effectiveness,limitations,and future directions of wet and dry lab simulation in orthopaedic training.METHODS A scoping review was carried out across four databases-PubMed,Cochrane Library,Web of Science,and EBSCOhost-up to 2025.Medical Subject Headings included:"Orthopaedic Education","Wet Lab","Dry Lab","Simulation Training","Virtual Reality",and"Surgical Procedure".Eligible studies focused on orthopaedic or spinal surgical education,employed wet or dry lab techniques,and assessed training effectiveness.Exclusion criteria consisted of non-English publications,abstracts only,non-orthopaedic research,and studies unrelated to simulation.Two reviewers independently screened titles,abstracts,and full texts,resolving discrepancies with a third reviewer.RESULTS From 1851 records,101 studies met inclusion:78 on dry labs,7 on wet labs,4 on both.Virtual reality(VR)simulations were most common,with AI increasingly used for feedback and assessment.Cadaveric training remains the gold standard for accuracy and tactile feedback,while dry labs-especially VR-offer scalability,lower cost(40%-60%savings in five studies),and accessibility for novices.Senior residents prefer wet labs for complex tasks;juniors favour dry labs for basics.Challenges include limited transferability data,lack of standard outcome metrics,and ethical concerns about cadaver use and AI assessment.CONCLUSION Wet and dry labs each have unique strengths in orthopaedic training.A hybrid approach combining both,supported by standardised assessments and outcome studies,is most effective.Future efforts should aim for uniform reporting,integrating new technologies,and policy support for hybrid curricula to enhance skills and patient care. 展开更多

“Bodily education”作为马克思的“体育”元概念,对理解马克思主义体育观具有基础性意义。通过不同文献版本的对比可知,“bodily education”在翻译上存在着一定缺陷,对理解和把握马克思的体育意旨产生了一定的影响。从马克思主义哲... “Bodily education”作为马克思的“体育”元概念,对理解马克思主义体育观具有基础性意义。通过不同文献版本的对比可知,“bodily education”在翻译上存在着一定缺陷,对理解和把握马克思的体育意旨产生了一定的影响。从马克思主义哲学视域下身体与劳动关系的逻辑演进分析得出,马克思所言之“体育”始终站在工人阶级立场,围绕工人阶级的身体展开。因而不能将“bodily education”泛化为一般意义上的“体育”,应翻译和理解为“身体教育”。从外在形式来看,其具体指以19世纪德国和瑞典等为代表的体操学校及军事训练所涉及的身体教育内容。从内涵来看,其不仅仅局限于激发个人身体潜能,培养某一方面的运动之能力以达到自我实现之目的,更高维的意义在于提升工人阶级的身体能力,进而服务于阶级斗争,打破资本主义的枷锁,消除异化劳动对人的束缚,为实现人的全面发展创造条件。 展开更多

This rapid communication article highlights a groundbreaking 24-month technical residency programme in forensic sciences designed for recent graduates from diverse fields.This pioneering initiative,supported and funded by the Secretariat of Public Security(Secretaria de Estado da Segurança Pública,SESP)of the state of Paranáin southern Brazil,aimed to bridge the gap between academic education and training and practical forensic expertise.A group of 46 candidates was selected to integrate into the Scientific Police,working under the direct supervision of experienced forensic expert officers while concurrently participating in a specialized programme at the State University of Maringá(Universidade Estadual de Maringá,UEM).Throughout the residency,participants were immersed in real-world forensic investigations,applying advanced methodologies and contributing to the efficiency and accuracy of forensic examinations.By the programme’s conclusion,33 residents had successfully graduated,earning official certifications that enhanced their qualifications for careers in forensic science.Beyond individual professional development,this residency programme significantly strengthened forensic capabilities in Paraná,setting a precedent for structured,high-level forensic education and training in Brazil.Its impact extends to the broader criminal justice system by fostering a new generation of well-trained forensic professionals,ultimately improving the quality and reliability of forensic investigations nationwide.For future editions,we recommend refining the participant selection process to prioritize candidates with strong scientific knowledge.To minimize attrition from the programme,we recommend increasing stipends and benefits for those who complete the residency. 展开更多

Objectives:This study aimed to identify the key patient safety competencies required by nursing students,to assess the effectiveness of current training methods and to examine the barriers to integrating safety principles into undergraduate curricula.Methods:The following databases were systematically searched for peer-reviewed studies published between 2015 and 2024:Scopus,PubMed,Web of Science and Springer Link.Studies written in English that focus on patient safety education in undergraduate nursing were included.The Johns Hopkins Nursing Evidence-Based Practice(JHNEBP)model and the Mixed Methods Appraisal Tool(MMAT)were used to conduct a methodological quality assessment of the articles.Results:Out of a total of 6,889 articles,20 studies were included for analysis.The results were categorized into four key themes:1)the imperative shift from implicit to explicit competency frameworks;2)the efficacy of active,immersive learning in fostering integrated competencies;3)systemic and infrastructural barriers as the primary impediment to integration;4)geographic and resource disparities necessitate context-adaptive pedagogical strategies.Conclusions:Immersive,longitudinal approaches(especially high-fidelity simulation and interprofessional clinical engagements)are particularly effective in bridging the gap between theory and practice.They can incorporate clear and comprehensive patient safety education into the nursing curriculum.Moving forward,we need a paradigm shift in nursing education,where patient safety becomes a fundamental aspect of curriculum design and delivery,rather than just an ancillary topic. 展开更多
