Background Delayed neurocognitive recovery(dNCR)is a prevalent complication in older patients undergoing surgery.It may progress to long-term cognitive impairment and increase the risk of Alzheimer's disease.Aims ...Background Delayed neurocognitive recovery(dNCR)is a prevalent complication in older patients undergoing surgery.It may progress to long-term cognitive impairment and increase the risk of Alzheimer's disease.Aims This study aimed to evaluate the effects of olfactory enrichment on dNCR and to examine the association between olfactory function and dNCR.Methods This sham-controlled,assessor-blind,parallelgroup randomised trial enrolled 149 participants aged65 or older undergoing elective total knee or hip replacement under general anaesthesia.Participants were assigned to either the olfactory enrichment group or the sham group.The intervention group received daily olfactory enrichment from 3 days preoperatively to 7 days postoperatively.Cognitive function was evaluated using a neuropsychological test battery3 days before and 7 days after surgery.Olfactory identification ability was assessed by five-odour olfactory detection arrays.Propensity score matching analysis was employed to mitigate potential confounding and selection bias.Results A total of 131 patients completed the study(62 in the olfactory enrichment group and 69 in the sham group).The overall incidence of dNCR was 26.7%(35 out of 131).In the intention-to-treat analysis,the difference between groups was not statistically significant(19.4%vs.33.3%;χ2=3.259;p=0.071).However,in the 1:1 propensity score-matched cohort(n=82),the incidence of dNCR was significantly lower in the olfactory enrichment group than in the sham group(12.2%vs.39.0%;χ2=7.476;p=0.005).Raw postoperative cognitive scores and individual change scores did not differ between the groups.Participants with decreased olfactory identification scores(n=32)had a significantly higher incidence of dNCR than those with stable or improved scores(40.6%vs.22.2%;χ2=4.183;p=0.041).Conclusions In older patients undergoing major orthopaedic surgery,perioperative olfactory dysfunction is associated with an increased risk of dNCR.Olfactory enrichment may represent a potential nonpharmacological strategy for reducing postoperative cognitive decline in this population.展开更多

Time-delayed blasting is widely utilized in engineering to mitigate induced vibration hazards and enhance fragmentation.The underlying vibration reduction principle is the decrease of the charge weight per delay,while...Time-delayed blasting is widely utilized in engineering to mitigate induced vibration hazards and enhance fragmentation.The underlying vibration reduction principle is the decrease of the charge weight per delay,while the potential for further vibration reduction remains debated,largely due to unclear underlying mechanisms.In light of the popularization of electronic detonators and the representativeness of double-hole configurationsfor multiple blastholes,it is essential to investigate the vibration characteristics induced by time-delayed double blastholes.Therefore,a series of doubleborehole experimental blasts was conducted in an underground roadway to clarify the variation in vibration from single-hole to dual-hole conditions.Based on the experimental data and inherent limitations,an exact full-fieldtheoretical model was further employed to systematically analyze the effects of delay time,charge length,and borehole inclination angle on vibrations induced by various doublehole configurations.The experimental data and theoretical analysis reveal that the general scaled distance effectively predicts vibrations in delayed blasting but does not reflectvibration reduction.Increasing delay time causes fluctuatingPPVs,which stabilize slightly above single-hole PPVs as delay times exceed a certain value.The delayed blasting primarily reduces near-fieldfrequencies.Longer charge lengths in double boreholes increase PPV levels and attenuation rates within a certain length,and the vibration behavior of combined long and short charge lengths is governed by the long blasthole.Larger blasthole inclination angles enhance vibration amplitude and reduce PPV attenuation rates.Optimizing inclination angles is more critical than adjusting delay times,and parallel boreholes offer the best vibration control.展开更多
The increasing demand for flexible displays and wearable electronics has driven extensive efforts to develop stretchable organic lightemitting diodes(OLEDs).A critical challenge in this field is the creation of emissi...The increasing demand for flexible displays and wearable electronics has driven extensive efforts to develop stretchable organic lightemitting diodes(OLEDs).A critical challenge in this field is the creation of emissive layers that combine high efficiency with mechanical robustness.Thermally activated delayed fluorescence(TADF)materials have attracted significant attention as third-generation emitters capable of achieving 100%internal quantum efficiency;however,their application in stretchable OLEDs has been limited.In this study,we propose an elastomer doping strategy.Polyurethane(PU)is incorporated into TADF polymers to improve their mechanical flexibility while maintaining a high luminescent efficiency.The resulting composite films exhibited excellent TADF characteristics and remarkable stretchability(75%).OLEDs fabricated from these materials achieved a maximum external quantum efficiency(EQE)of 14.26%and a peak luminance of 73570 cd·m-2,with the PUdoped devices showing a significantly suppressed efficiency roll-off.Additionally,a fully stretchable OLED architecture was designed and operated under tensile strain to maintain stable electroluminescent performance.These results demonstrate that elastomer doping is an effective strategy for balancing the mechanical compliance with optoelectronic performance,offering a promising pathway for the development of high-performance stretchable OLEDs for flexible electronics.展开更多

Delayed neurocognitive recovery following anesthesia and surgery is a common complication in older adult patients.Synapses are fundamental to cognitive function.The activity of synapses heavily depends on the energy s...Delayed neurocognitive recovery following anesthesia and surgery is a common complication in older adult patients.Synapses are fundamental to cognitive function.The activity of synapses heavily depends on the energy supplied by synaptic mitochondria,which are significantly influenced by oxidative stress.Sirtuin 3 is a histone deacetylase located in the mitochondrial matrix that plays a pivotal role in regulating mitochondrial function.However,it remains unclear whether and how sirtuin 3 is involved in the development of delayed cognitive recovery.Therefore,in this study,we investigated the potential role of sirtuin 3 in synapses during delayed neurocognitive recovery.Our results showed that anesthesia and surgery induced cognitive impairment in mice and reduced sirtuin 3 protein expression.Overexpression of sirtuin 3 inhibited opening of the mitochondrial permeability transition pore by reducing acetylation of K166 on cyclophilin D and also rescued cognitive impairment.Aged mice carrying the cyclophilin D-K166R mutation exhibited significantly reduced cognitive impairment.Similarly,administering the mitochondrial permeability transition pore blocker,cyclosporine A,effectively alleviated the decline in synaptic mitochondrial function and cognitive impairment caused by anesthesia and surgery in aged mice.These results indicate that the sirtuin 3/cyclophilin D-K166/mPTP signaling pathway in hippocampal synaptic mitochondria is involved in delayed neurocognitive recovery of aged mice,suggesting this pathway could serve as a potential target for treatment.展开更多

BACKGROUND Post-polypectomy bleeding is a common adverse event associated with colonoscopic polypectomy.Although various risk factors for delayed post-polypectomy bleeding(DPPB)have been identified,the relationship be...BACKGROUND Post-polypectomy bleeding is a common adverse event associated with colonoscopic polypectomy.Although various risk factors for delayed post-polypectomy bleeding(DPPB)have been identified,the relationship between immediate postpolypectomy bleeding(IPPB)and subsequent DPPB risk remains unclear.Furthermore,risk factors for DPPB specifically among patients who experience IPPB have not been systematically evaluated.AIM To investigate the association between IPPB and DPPB,hypothesizing that patients experiencing IPPB would have a higher risk of subsequent DPPB,and to identify independent risk factors for DPPB among patients who experienced IPPB.METHODS In this retrospective cohort study,we analyzed the data of patients who underwent colonoscopic polypectomies in the Republic of Korea between 2005 and 2022.Information on multiple covariates,including patient-,polyp-,and procedure-related factors,was collected.Patients were categorized based on IPPB occurrence,and DPPB incidence was compared between the groups.The McNemar test was used to assess the association between IPPB and DPPB.Logistic regression analysis was used to identify risk factors for DPPB among patients with IPPB.RESULTS Of the 16684 patients included,933(5.59%)experienced IPPB and 15751(94.41%)did not.Among patients with IPPB,35(3.75%)developed DPPB,whereas 246(1.56%)of patients without IPPB developed DPPB,demonstrating a statistically significant positive association between IPPB and DPPB[paired odds ratio(OR)=3.65,95%confidence interval(CI):3.17-4.20,P<0.001].Following multivariable analysis among patients with IPPB,trainee-performed procedures was identified as the only independent risk factor for DPPB(OR=3.091,95%CI:1.037-9.219,P=0.043).CONCLUSION IPPB occurrence is significantly associated with an increased risk of DPPB,suggesting the need for careful preventive strategies,particularly for procedures performed by trainees.展开更多
Thick-layer organic light-emitting diodes(OLEDs)comprising a single thick film to simultaneously transport carriers and emit light offer significant advantages in simplifying the fabrication process and enhancing oper...Thick-layer organic light-emitting diodes(OLEDs)comprising a single thick film to simultaneously transport carriers and emit light offer significant advantages in simplifying the fabrication process and enhancing operational stability.However,achieving highly efficient thick-layer non-doped OLEDs requires emitters possessing balanced bipolar carrier transport,strong solid-state emission and high exciton utilization,which impose high requirements on designing luminescent materials.In this work,two representative bipolar aggregation-induced delayed fluorescence(AIDF)emitters are employed to fabricate such thick-layer non-doped devices.Excellent electroluminescence(EL)performances are attained with low turn-on voltages(2.5 V),high external quantum efficiencies(19.8%),and negligible efficiency roll-off at high luminance.Furthermore,the operational lifetime of the thick-layer device increases by more than twofold compared to that of the thin-layer devices.In addition,highefficiency,high-color-purity simplified thick-layer hyperfluorescence OLEDs are also achieved by using their thick neat films as sensitizers for multi-resonance emitters.These results indicate that the bipolar AIDF emitters are promising candidates for constructing simple thick-layer OLEDs,which provides a feasible strategy for developing highly efficient and stable OLEDs.展开更多

Background Transcatheter aortic valve implantation(TAVI)is the principal treatment for aortic stenosis in older and high-risk patients.Whilst important in holistic decision making,frailty indices vary in ease of use,o...Background Transcatheter aortic valve implantation(TAVI)is the principal treatment for aortic stenosis in older and high-risk patients.Whilst important in holistic decision making,frailty indices vary in ease of use,objectivity,and accuracy.We assess prognostic ability of the Clinical Frailty Scale(CFS)and Essential Frailty Toolset(EFT)in predicting delayed discharge and 1-year mortality following TAVI.Methods Prospective,single centre,observational cohort study of patients undergoing TAVI at a tertiary centre in the UK from 2020-2022.Clinical characteristics,CFS,and EFT were collected pre-procedurally.Primary outcomes were length of hospital stay following TAVI and mortality at 1-year post-procedure.Generalized regression with elastic net is used to assess the additive discriminative ability of frailty scores to predict outcomes.Stepwise regression is used to identify informative outcome predictors.Results One hundred and four patients were included with mean age 81.3±5.9 years,47%female.Median post-procedural stay length was 2 days(IQR 2.75).Use of EFT and CFS resulted in a statistically significant prediction of survival at 1 year post TAVI(EFT:ΔLogL of 2.68,ChiSq P=0.021;RCFSΔLogL:1.42,ChiSq P=0.092)compared to predictive model using conventional clinical characteristics.EuroSCORE II and TAVI2SCORe were poor predictors of outcomes.CFS and access site(femoral vs.non-femoral)significantly improved prediction of delayed discharge.Conclusions EFT informs decision making on survival 1-year post-TAVI,beyond conventional measures.Use of non-femoral access site and high CFS are the best predictors of delayed discharge.Surgical risk stratification scores poorly predict mediumterm TAVI survival and delayed discharge.展开更多

Neuronal oscillations arise from the interplay between intrinsic neuronal dynamics and network connectivity.In this work,we investigate the effects of network topology on oscillatory behavior in a FitzHugh–Nagumo(FHN...Neuronal oscillations arise from the interplay between intrinsic neuronal dynamics and network connectivity.In this work,we investigate the effects of network topology on oscillatory behavior in a FitzHugh–Nagumo(FHN)neuronal model with distributed delay,representing ion-channel memory effects,and diffusive delay,accounting for axonal transmission delays.The neurons are coupled through quasi-Laplacian interactions on Barab´asi–Albert(BA)scale-free networks.Using the multiple-time-scales(MTS)method,we derive amplitude equations near the Hopf bifurcation point and establish explicit relationships between oscillatory dynamics and network topology.The analysis shows that the smallest negative eigenvalue of the network governs the critical delay threshold for oscillation onset,while the distributed-delay parameterσand diffusive delayτjointly regulate this threshold.The resulting oscillation frequencies are confined to the beta band(15–30 Hz),a frequency range often associated with pathological neural activity in Parkinson's disease.Extensive numerical simulations over 50 network realizations confirm the theoretical predictions.Hub nodes with higher degrees exhibit lower critical delays and larger oscillation amplitudes,whereas peripheral nodes display weaker and more heterogeneous responses.Statistical analysis further reveals a negative correlation between node degree and critical delay and a positive correlation between node degree and oscillation amplitude.These results demonstrate how delay effects and network topology jointly shape the emergence and spatial organization of collective oscillations,providing insights into synchronization phenomena in complex neuronal networks.展开更多
BACKGROUND Traumatic subserosa hematoma of the proximal jejunum with delayed rupture and hemorrhage is exceedingly rare,especially without anticoagulation.No prior reports document spontaneous rupture precisely on day...BACKGROUND Traumatic subserosa hematoma of the proximal jejunum with delayed rupture and hemorrhage is exceedingly rare,especially without anticoagulation.No prior reports document spontaneous rupture precisely on day 5 post-blunt trauma.This case uniquely demonstrates this delayed pattern,initial computed tomography(CT)misdiagnosis as a retroperitoneal neoplasm due to Treitz ligament proximity,and conservative management failure,offering novel insights into its unpredictable course and the need for prompt surgical intervention in non-coagulopathic patients.CASE SUMMARY A 21-year-old male delivery courier sustained blunt upper abdominal trauma from a tricycle handlebar impact during a motor vehicle accident.Initial symptoms were mild abdominal pain and distension.Symptoms worsened on day 2 with nausea and vomiting;admission CT showed a retroperitoneal cystic mass with hemorrhage,misdiagnosed as duodenal gastrointestinal stromal tumor.Conservative management was initiated.On hospital day 4(post-injury day 5),sudden severe pain,hypotension(90/45 mmHg),and tachycardia developed.Bedside ultrasound confirmed massive hemoperitoneum.Emergency laparotomy revealed a 10-cm ruptured subserosal hematoma at the proximal jejunum near the ligament of Treitz with active oozing;hematoma evacuation and serosal repair were performed.Postoperative course was uneventful;oral intake resumed on day 3,no contrast swallow X-ray was performed due to clinical stability,and he was discharged on day 7.One-month follow-up CT showed complete resolution.CONCLUSION This rare traumatic proximal jejunal subserosal hematoma ruptured on day 5 without anticoagulation,highlighting the need for early CT with Treitz focus,serial monitoring,and prompt laparotomy.This study is limited by its single-case,retrospective design and the lack of long-term follow-up data.展开更多
This study aimed to identify biomarkers associated with intracranial tumors in children undergoing high-dose methotrexate(HDMTX)chemotherapy,using untargeted metabolomics to enable early detection of delayed drug elim...This study aimed to identify biomarkers associated with intracranial tumors in children undergoing high-dose methotrexate(HDMTX)chemotherapy,using untargeted metabolomics to enable early detection of delayed drug elimination.Leveraging ultra-performance liquid chromatography coupled with quadrupole time-of-flight tandem mass spectrometry(UPLC-QTOF-MS/MS),serum samples from children with medulloblastoma were analyzed prior to drug administration.We developed an orthogonal partial least squares discriminant analysis(OPLS-DA)-based pattern recognition approach to characterize differences in metabolic profiles between children exhibiting normal MTX elimination and those with delayed elimination.A total of 48 distinct serum metabolites were identified.Metabolic pathway analysis revealed significant perturbations in sphingolipid and glycerophospholipid metabolism between the two groups.Among these,10 differential metabolites demonstrated an AUC>0.8 and were thus selected as candidate biomarkers for further validation.The biomarkers identified in this study effectively distinguished between normal and delayed MTX elimination,providing a potential early-warning tool for delayed drug clearance.展开更多

摘要Background Delayed neurocognitive recovery(dNCR)is a prevalent complication in older patients undergoing surgery.It may progress to long-term cognitive impairment and increase the risk of Alzheimer's disease.Aims This study aimed to evaluate the effects of olfactory enrichment on dNCR and to examine the association between olfactory function and dNCR.Methods This sham-controlled,assessor-blind,parallelgroup randomised trial enrolled 149 participants aged65 or older undergoing elective total knee or hip replacement under general anaesthesia.Participants were assigned to either the olfactory enrichment group or the sham group.The intervention group received daily olfactory enrichment from 3 days preoperatively to 7 days postoperatively.Cognitive function was evaluated using a neuropsychological test battery3 days before and 7 days after surgery.Olfactory identification ability was assessed by five-odour olfactory detection arrays.Propensity score matching analysis was employed to mitigate potential confounding and selection bias.Results A total of 131 patients completed the study(62 in the olfactory enrichment group and 69 in the sham group).The overall incidence of dNCR was 26.7%(35 out of 131).In the intention-to-treat analysis,the difference between groups was not statistically significant(19.4%vs.33.3%;χ2=3.259;p=0.071).However,in the 1:1 propensity score-matched cohort(n=82),the incidence of dNCR was significantly lower in the olfactory enrichment group than in the sham group(12.2%vs.39.0%;χ2=7.476;p=0.005).Raw postoperative cognitive scores and individual change scores did not differ between the groups.Participants with decreased olfactory identification scores(n=32)had a significantly higher incidence of dNCR than those with stable or improved scores(40.6%vs.22.2%;χ2=4.183;p=0.041).Conclusions In older patients undergoing major orthopaedic surgery,perioperative olfactory dysfunction is associated with an increased risk of dNCR.Olfactory enrichment may represent a potential nonpharmacological strategy for reducing postoperative cognitive decline in this population.

摘要Dear Editor,This letter studies the finite-time stability(FTS)problem of nonlinear impulsive systems with delayed impulses.The system under consideration is subjected to a flexible class of time-varying delays in impulsive signals,allowing them to arbitrarily span one or more impulse instants.Sufficient Lyapunov-based conditions for FTS of such systems are presented,where a novel class of impulse time sequences is established for all admissible uncertainties of delayed impulses.

摘要X-ray delayed imaging technology leverages an"Trradiation-storage-thermally stimulated readout"mode,demonstrating unique advantages in intermittent monitoring scenarios such as high-temperature/high-radiation environments.This typically requires materials combining abundant carrier-trapping energy levels with structural thermostability,Herein,Cr3+-doped ZnGa2O4glass ceramics(ZGC GCs)serve as a functional platform,utilizing their deep trap en ergy levels(main peak 197.7℃)to efficiently capture and store X-ray imaging information,and achieve high-brightness X-ray delayed imaging by modulating the thermally stimulated detrapping rates.Under its exceptional structural thermostability and deep trap characteristics,ZGC GCs maintain high-quality delayed imaging capability by thermal stimulation even after 180℃irradiation,highlighting their potential for high-temperature nondestructive tes ting and surpassing the high-temperature failure threshold of conventional scintillators.This work pioneers functional applications of glass ceramics in high-temperature irradiation scenarios and offers a novel perspective for selecting harsh-environment-adapted X-ray imaging materials.

摘要Time-delayed blasting is widely utilized in engineering to mitigate induced vibration hazards and enhance fragmentation.The underlying vibration reduction principle is the decrease of the charge weight per delay,while the potential for further vibration reduction remains debated,largely due to unclear underlying mechanisms.In light of the popularization of electronic detonators and the representativeness of double-hole configurationsfor multiple blastholes,it is essential to investigate the vibration characteristics induced by time-delayed double blastholes.Therefore,a series of doubleborehole experimental blasts was conducted in an underground roadway to clarify the variation in vibration from single-hole to dual-hole conditions.Based on the experimental data and inherent limitations,an exact full-fieldtheoretical model was further employed to systematically analyze the effects of delay time,charge length,and borehole inclination angle on vibrations induced by various doublehole configurations.The experimental data and theoretical analysis reveal that the general scaled distance effectively predicts vibrations in delayed blasting but does not reflectvibration reduction.Increasing delay time causes fluctuatingPPVs,which stabilize slightly above single-hole PPVs as delay times exceed a certain value.The delayed blasting primarily reduces near-fieldfrequencies.Longer charge lengths in double boreholes increase PPV levels and attenuation rates within a certain length,and the vibration behavior of combined long and short charge lengths is governed by the long blasthole.Larger blasthole inclination angles enhance vibration amplitude and reduce PPV attenuation rates.Optimizing inclination angles is more critical than adjusting delay times,and parallel boreholes offer the best vibration control.

摘要The increasing demand for flexible displays and wearable electronics has driven extensive efforts to develop stretchable organic lightemitting diodes(OLEDs).A critical challenge in this field is the creation of emissive layers that combine high efficiency with mechanical robustness.Thermally activated delayed fluorescence(TADF)materials have attracted significant attention as third-generation emitters capable of achieving 100%internal quantum efficiency;however,their application in stretchable OLEDs has been limited.In this study,we propose an elastomer doping strategy.Polyurethane(PU)is incorporated into TADF polymers to improve their mechanical flexibility while maintaining a high luminescent efficiency.The resulting composite films exhibited excellent TADF characteristics and remarkable stretchability(75%).OLEDs fabricated from these materials achieved a maximum external quantum efficiency(EQE)of 14.26%and a peak luminance of 73570 cd·m-2,with the PUdoped devices showing a significantly suppressed efficiency roll-off.Additionally,a fully stretchable OLED architecture was designed and operated under tensile strain to maintain stable electroluminescent performance.These results demonstrate that elastomer doping is an effective strategy for balancing the mechanical compliance with optoelectronic performance,offering a promising pathway for the development of high-performance stretchable OLEDs for flexible electronics.

摘要Systematic optimization of the delayed self-heterodyne method for laser frequency noise characterization is investigated across an extensive linewidth range(100 Hz to 10 MHz).By evaluating various fiber lengths,window functions,and five demodulation algorithms,we identify a critical trade-off:long fibers enhance sensitivity for narrow-linewidth lasers but exacerbate spectral leakage in broad-linewidth sources.Our findings demonstrate that Hanning and Blackman windows effectively suppress this leakage,ensuring measurement consistency across different delay lengths.Among the evaluated algorithms,the Hilbert transform offers the superior balance of high-frequency accuracy and computational efficiency.Validated under lowpower conditions,this optimized framework provides a robust and power-independent methodology for precise FN analysis,offering significant guidance for high-performance laser development.

摘要Delayed neurocognitive recovery following anesthesia and surgery is a common complication in older adult patients.Synapses are fundamental to cognitive function.The activity of synapses heavily depends on the energy supplied by synaptic mitochondria,which are significantly influenced by oxidative stress.Sirtuin 3 is a histone deacetylase located in the mitochondrial matrix that plays a pivotal role in regulating mitochondrial function.However,it remains unclear whether and how sirtuin 3 is involved in the development of delayed cognitive recovery.Therefore,in this study,we investigated the potential role of sirtuin 3 in synapses during delayed neurocognitive recovery.Our results showed that anesthesia and surgery induced cognitive impairment in mice and reduced sirtuin 3 protein expression.Overexpression of sirtuin 3 inhibited opening of the mitochondrial permeability transition pore by reducing acetylation of K166 on cyclophilin D and also rescued cognitive impairment.Aged mice carrying the cyclophilin D-K166R mutation exhibited significantly reduced cognitive impairment.Similarly,administering the mitochondrial permeability transition pore blocker,cyclosporine A,effectively alleviated the decline in synaptic mitochondrial function and cognitive impairment caused by anesthesia and surgery in aged mice.These results indicate that the sirtuin 3/cyclophilin D-K166/mPTP signaling pathway in hippocampal synaptic mitochondria is involved in delayed neurocognitive recovery of aged mice,suggesting this pathway could serve as a potential target for treatment.

摘要A feedback control,based on delayed discrete observations,is proposed for hybrid neutral stochastic differential systems with mixed delay.Unlike conventional methods that rely on continuous mode observations,it addresses the difficulty and cost of mode identification and considers system delay.This paper successfully stabilizes unstable systems by integrating discrete state observations,discrete mode observations,and delay factors into the controller design.It also verifies the effectiveness of the theory through an example.

摘要The development of polymeric materials that exhibit blue thermally activated delayed fluorescence(TADF)is of great interest for optoelectronic applications.However,achieving TADF in polymers often requires an elaborate monomer design.The high-energy local triplet state(3LE)of carbazole complicates its application despite the molecular orbital arrangement being suitable for blue emission.Here,we present an approach to polymer design that makes it possible to solve this problem.We demonstrate the in situ formation of a TADF donor-acceptor system during Suzuki polycondensation,creating an extended carbazole-based donor matrix coupled with a triazine acceptor.The resulting polymer exhibited efficient TADF with a low energy gap(ΔEST)value if a phenyl N-substituent,enabling essential electron delocalization,was present in the carbazole moiety.This work establishes a versatile platform for developing carbazole-based TADF polymers,overcoming the fundamental limitations that hinder their widespread application.
摘要BACKGROUND Post-polypectomy bleeding is a common adverse event associated with colonoscopic polypectomy.Although various risk factors for delayed post-polypectomy bleeding(DPPB)have been identified,the relationship between immediate postpolypectomy bleeding(IPPB)and subsequent DPPB risk remains unclear.Furthermore,risk factors for DPPB specifically among patients who experience IPPB have not been systematically evaluated.AIM To investigate the association between IPPB and DPPB,hypothesizing that patients experiencing IPPB would have a higher risk of subsequent DPPB,and to identify independent risk factors for DPPB among patients who experienced IPPB.METHODS In this retrospective cohort study,we analyzed the data of patients who underwent colonoscopic polypectomies in the Republic of Korea between 2005 and 2022.Information on multiple covariates,including patient-,polyp-,and procedure-related factors,was collected.Patients were categorized based on IPPB occurrence,and DPPB incidence was compared between the groups.The McNemar test was used to assess the association between IPPB and DPPB.Logistic regression analysis was used to identify risk factors for DPPB among patients with IPPB.RESULTS Of the 16684 patients included,933(5.59%)experienced IPPB and 15751(94.41%)did not.Among patients with IPPB,35(3.75%)developed DPPB,whereas 246(1.56%)of patients without IPPB developed DPPB,demonstrating a statistically significant positive association between IPPB and DPPB[paired odds ratio(OR)=3.65,95%confidence interval(CI):3.17-4.20,P<0.001].Following multivariable analysis among patients with IPPB,trainee-performed procedures was identified as the only independent risk factor for DPPB(OR=3.091,95%CI:1.037-9.219,P=0.043).CONCLUSION IPPB occurrence is significantly associated with an increased risk of DPPB,suggesting the need for careful preventive strategies,particularly for procedures performed by trainees.
摘要Thick-layer organic light-emitting diodes(OLEDs)comprising a single thick film to simultaneously transport carriers and emit light offer significant advantages in simplifying the fabrication process and enhancing operational stability.However,achieving highly efficient thick-layer non-doped OLEDs requires emitters possessing balanced bipolar carrier transport,strong solid-state emission and high exciton utilization,which impose high requirements on designing luminescent materials.In this work,two representative bipolar aggregation-induced delayed fluorescence(AIDF)emitters are employed to fabricate such thick-layer non-doped devices.Excellent electroluminescence(EL)performances are attained with low turn-on voltages(2.5 V),high external quantum efficiencies(19.8%),and negligible efficiency roll-off at high luminance.Furthermore,the operational lifetime of the thick-layer device increases by more than twofold compared to that of the thin-layer devices.In addition,highefficiency,high-color-purity simplified thick-layer hyperfluorescence OLEDs are also achieved by using their thick neat films as sensitizers for multi-resonance emitters.These results indicate that the bipolar AIDF emitters are promising candidates for constructing simple thick-layer OLEDs,which provides a feasible strategy for developing highly efficient and stable OLEDs.

摘要Macrocyclic cascade supramolecular assembly could significantly enhance the fluorescence/phosphorescence resonance energy transfer(F/PRET)efficiency through macrocyclic and spatial dual confinement effect.Herein,we reported a cascade supramolecular assembly containing6-bromoisoquinolinium-modified permethylated cyclodextrin(BQ-PCD),cucurbit[7]uril(CB[7]),and tetra(4-sulfonatophenyl)porphyrin(TPPS),in which the enhanced PRET from 6-bromoisoquinolinium(BQ)to TPPS could be achieved through the dual macrocyclic confinement for multicolor delayed luminescence and information encryption.In TPPS⊂BQ-PCD⊂CB[7],pure organic room temperature phosphorescence of BQ-PCD at 530 nm is induced by CB[7]macrocyclic confinement,which further transferred to TPPS via spatial confinement,achieving delayed fluorescence at 645 and 715 nm with high PRET efficiency and quantum yield(17.9%).Meanwhile,reversible TPPS concentration-dependent multicolor luminescence was achieved in presence of competitive guest(methionine peptide),followed by porphyrin-photosensitization process,being applied in information encryption.This research presents a facile strategy for efficient PRET through macrocyclic cascade confinement assembly.
摘要Delayed wound healing following radical gastrectomy remains an important yet underappreciated complication that prolongs hospitalization,increases costs,and undermines patient recovery.In An et al’s recent study,the authors present a machine learning-based risk prediction approach using routinely available clinical and laboratory parameters.Among the evaluated algorithms,a decision tree model demonstrated excellent discrimination,achieving an area under the curve of 0.951 in the validation set and notably identifying all true cases of delayed wound healing at the Youden index threshold.The inclusion of variables such as drainage duration,preoperative white blood cell and neutrophil counts,alongside age and sex,highlights the pragmatic appeal of the model for early postoperative monitoring.Nevertheless,several aspects warrant critical reflection,including the reliance on a postoperative variable(drainage duration),internal validation only,and certain reporting inconsistencies.This letter underscores both the promise and the limitations of adopting interpretable machine learning models in perioperative care.We advocate for transparent reporting,external validation,and careful consideration of clinically actionable timepoints before integration into practice.Ultimately,this work represents a valuable step toward precision risk stratification in gastric cancer surgery,and sets the stage for multicenter,prospective evaluations.

摘要Background Transcatheter aortic valve implantation(TAVI)is the principal treatment for aortic stenosis in older and high-risk patients.Whilst important in holistic decision making,frailty indices vary in ease of use,objectivity,and accuracy.We assess prognostic ability of the Clinical Frailty Scale(CFS)and Essential Frailty Toolset(EFT)in predicting delayed discharge and 1-year mortality following TAVI.Methods Prospective,single centre,observational cohort study of patients undergoing TAVI at a tertiary centre in the UK from 2020-2022.Clinical characteristics,CFS,and EFT were collected pre-procedurally.Primary outcomes were length of hospital stay following TAVI and mortality at 1-year post-procedure.Generalized regression with elastic net is used to assess the additive discriminative ability of frailty scores to predict outcomes.Stepwise regression is used to identify informative outcome predictors.Results One hundred and four patients were included with mean age 81.3±5.9 years,47%female.Median post-procedural stay length was 2 days(IQR 2.75).Use of EFT and CFS resulted in a statistically significant prediction of survival at 1 year post TAVI(EFT:ΔLogL of 2.68,ChiSq P=0.021;RCFSΔLogL:1.42,ChiSq P=0.092)compared to predictive model using conventional clinical characteristics.EuroSCORE II and TAVI2SCORe were poor predictors of outcomes.CFS and access site(femoral vs.non-femoral)significantly improved prediction of delayed discharge.Conclusions EFT informs decision making on survival 1-year post-TAVI,beyond conventional measures.Use of non-femoral access site and high CFS are the best predictors of delayed discharge.Surgical risk stratification scores poorly predict mediumterm TAVI survival and delayed discharge.
摘要A series of blue and blue‑green Ir(Ⅲ)complexes has been investigated theoretically to explore their electronic structures,photophysical properties,efficiency roll‑off effect,and thermal activation delayed fluorescence(TADF)properties.All calculations were performed using density functional theory(DFT)and time‑dependent density functional theory(TDDFT).Calculations for electronic structures,frontier molecular orbital characteristics(which determine the efficiency roll‑off effect of the complexes),and photophysical properties were conducted using the Gaussian 09 software package.The calculation of spin‑orbit coupling matrix elements,which determine the TADF properties of the complexes,was performed using the ORCA software package.The calculation results show that the auxiliary ligand tetraphenylimidodiphosphinate(tpip),a strong electron‑withdrawing group,can mitigate the efficiency roll‑off effect of the complex.Furthermore,TADF is observed in one of the designed complexes,(F3Phppy)2Ir(tpip),where F3Phppy=2‑[4‑(2,4,6‑trifluorophenyl)phenyl]pyridine.

摘要Neuronal oscillations arise from the interplay between intrinsic neuronal dynamics and network connectivity.In this work,we investigate the effects of network topology on oscillatory behavior in a FitzHugh–Nagumo(FHN)neuronal model with distributed delay,representing ion-channel memory effects,and diffusive delay,accounting for axonal transmission delays.The neurons are coupled through quasi-Laplacian interactions on Barab´asi–Albert(BA)scale-free networks.Using the multiple-time-scales(MTS)method,we derive amplitude equations near the Hopf bifurcation point and establish explicit relationships between oscillatory dynamics and network topology.The analysis shows that the smallest negative eigenvalue of the network governs the critical delay threshold for oscillation onset,while the distributed-delay parameterσand diffusive delayτjointly regulate this threshold.The resulting oscillation frequencies are confined to the beta band(15–30 Hz),a frequency range often associated with pathological neural activity in Parkinson's disease.Extensive numerical simulations over 50 network realizations confirm the theoretical predictions.Hub nodes with higher degrees exhibit lower critical delays and larger oscillation amplitudes,whereas peripheral nodes display weaker and more heterogeneous responses.Statistical analysis further reveals a negative correlation between node degree and critical delay and a positive correlation between node degree and oscillation amplitude.These results demonstrate how delay effects and network topology jointly shape the emergence and spatial organization of collective oscillations,providing insights into synchronization phenomena in complex neuronal networks.

摘要BACKGROUND Traumatic subserosa hematoma of the proximal jejunum with delayed rupture and hemorrhage is exceedingly rare,especially without anticoagulation.No prior reports document spontaneous rupture precisely on day 5 post-blunt trauma.This case uniquely demonstrates this delayed pattern,initial computed tomography(CT)misdiagnosis as a retroperitoneal neoplasm due to Treitz ligament proximity,and conservative management failure,offering novel insights into its unpredictable course and the need for prompt surgical intervention in non-coagulopathic patients.CASE SUMMARY A 21-year-old male delivery courier sustained blunt upper abdominal trauma from a tricycle handlebar impact during a motor vehicle accident.Initial symptoms were mild abdominal pain and distension.Symptoms worsened on day 2 with nausea and vomiting;admission CT showed a retroperitoneal cystic mass with hemorrhage,misdiagnosed as duodenal gastrointestinal stromal tumor.Conservative management was initiated.On hospital day 4(post-injury day 5),sudden severe pain,hypotension(90/45 mmHg),and tachycardia developed.Bedside ultrasound confirmed massive hemoperitoneum.Emergency laparotomy revealed a 10-cm ruptured subserosal hematoma at the proximal jejunum near the ligament of Treitz with active oozing;hematoma evacuation and serosal repair were performed.Postoperative course was uneventful;oral intake resumed on day 3,no contrast swallow X-ray was performed due to clinical stability,and he was discharged on day 7.One-month follow-up CT showed complete resolution.CONCLUSION This rare traumatic proximal jejunal subserosal hematoma ruptured on day 5 without anticoagulation,highlighting the need for early CT with Treitz focus,serial monitoring,and prompt laparotomy.This study is limited by its single-case,retrospective design and the lack of long-term follow-up data.

摘要This study aimed to identify biomarkers associated with intracranial tumors in children undergoing high-dose methotrexate(HDMTX)chemotherapy,using untargeted metabolomics to enable early detection of delayed drug elimination.Leveraging ultra-performance liquid chromatography coupled with quadrupole time-of-flight tandem mass spectrometry(UPLC-QTOF-MS/MS),serum samples from children with medulloblastoma were analyzed prior to drug administration.We developed an orthogonal partial least squares discriminant analysis(OPLS-DA)-based pattern recognition approach to characterize differences in metabolic profiles between children exhibiting normal MTX elimination and those with delayed elimination.A total of 48 distinct serum metabolites were identified.Metabolic pathway analysis revealed significant perturbations in sphingolipid and glycerophospholipid metabolism between the two groups.Among these,10 differential metabolites demonstrated an AUC>0.8 and were thus selected as candidate biomarkers for further validation.The biomarkers identified in this study effectively distinguished between normal and delayed MTX elimination,providing a potential early-warning tool for delayed drug clearance.
摘要BACKGROUND Portal hypertension is typically associated with cirrhosis,but acute etiologies must be considered in atypical cases.Post-splenectomy arteriovenous fistula(AVF)is a rare vascular complication that can lead to acute portal hypertension with nonspecific symptoms.CASE SUMMARY A 43-year-old man presented with acute abdominal pain,hematemesis,and melena,eight years after open splenectomy.Computed tomography angiography revealed a splenic AVF.Emergency laparotomy with splenic artery ligation was performed.The patient recovered uneventfully and remained asymptomatic at five-year follow-up.CONCLUSION Splenic AVF should be considered in patients with new-onset portal hypertension,a history of splenectomy,and no liver disease.Prompt diagnosis and surgical intervention yield excellent outcomes.
