BACKGROUND Dopaminergic neurons are lost in Parkinson’s disease(PD),a degenerative neurological condition.While the effects of PD on motor functions are well understood,its impact on respiratory function is less clear.In PD patients with suspicious pulmonary symptoms,the presence and degree of impairment in lung function often cannot be optimally assessed through pulmonary function tests(PFTs)due to the patients'neurological symptoms.AIM To examine the relationship between diaphragmatic thickness as determined by ultrasonography(US)and conventional PFTs in PD patients.Additionally,we evaluated the potential use of diaphragmatic thickness as an alternative measure for assessing respiratory function in PD patients who are unable to perform PFTs.METHODS A total of 43 PD patients who presented to the clinics between June 15,2021,and June 30,2022,were included.Patients with coronavirus disease 2019 infection history,conditions affecting diaphragmatic motility,recent thoracic or abdominal surgery,or imaging showing restrictive masses were excluded.Demographic data,disease duration,Hoehn-Yahr stage,and medication use were recorded.PFTs were used to evaluate forced vital capacity,forced expiratory volume in the first second,and other parameters,while US was used to measure diaphragmatic thickness.Breathlessness was assessed using the modified medical research council dyspnea scale.RESULTS Significant correlations were found between peak expiratory flow(PEF)and diaphragm fraction(P=0.019),as well as between PEF percentage and diaphragmatic thickness during expiration(P=0.023).In early-stage PD,diaphragmatic thickness during inspiration correlated with oxygen saturation(P=0.048).In advanced-stage PD,PEF and PEF%were significantly correlated with diaphragm thickness during expiration[r=0.405,P=0.049(for PEF);r=0.463,P=0.023(for PEF%)]and diaphragm fraction[r=-0.476,P=0.019(for PEF);r=-0.617,P=0.001(for PEF%)].CONCLUSION Present study assessed the relationship between PFTs and diaphragmatic thickness in PD patients.Diaphragmatic thickness measured by US was found to correlate with certain PFT parameters,suggesting it could be used as an alternative measure for assessing respiratory function in Parkinson’s patients who have difficulty performing PFTs. 展开更多

BACKGROUND Small intestinal bacterial overgrowth(SIBO)is suspected and excluded frequently in functional gastrointestinal(GI)disorders.Children presenting with various esophago-gastro-duodenal(upper GI)symptoms are rarely subjected to investig-ations for SIBO.AIM To estimate the frequency of SIBO in children having functional upper GI sym-ptoms(as cases)and to compare the result of the SIBO status to that of the con-trols.METHODS Children aged 6 to 18 who presented with upper GI symptoms were selected for the study.All children were subjected to upper GI endoscopy before being advised of any proton pump inhibitors(PPIs).Children with normal endoscopy were assigned as cases,and children having any endoscopic lesion were design-ated as controls.Both groups were subjected to a glucose-hydrogen breath test by Bedfont Gastrolyser.RESULTS A total of 129 consecutive children who were naive to PPIs and had normal ba-seline investigations were included in the study.Among them,67 patients had endoscopic lesions and served as the control group,with six cases being excluded due to the presence of Helicobacter pylori in gastric biopsies.Sixty-two children with normal endoscopy results formed the case group.In the case group,35 children(59%)tested positive for hydrogen breath tests,compared to 13 children(21%)in the control group.The calculated odds ratio was 5.38(95%confidence interval:2.41-12.0),which was statistically significant.Further analysis of symptoms revealed that nausea,halitosis,foul-smelling eructation,and epigastric fullness were positive predictors of SIBO.CONCLUSION It is worthwhile to investigate and treat SIBO in all children presenting with upper GI symptoms that are not explained by endoscopy findings. 展开更多

BACKGROUND Insulin resistance is a cardiometabolic risk factor characterized by elevated insulin levels.It is associated with fatty liver disease and elevated liver function tests(LFT)in cross-sectional studies,but data from cohort studies are scarce.AIM To investigate the association between insulin and pathological LFT,liver disease,and cirrhosis in a populationbased retrospective cohort study.METHODS Anthropometric and cardiometabolic factors of 857 men and 1228 women from prospective cohort studies were used.LFT were obtained at two time points 8 years to 24 years after baseline.Liver disease diagnoses were obtained from nationwide registries.The association between insulin levels and the development of elevated LFT or liver disease and cirrhosis was analyzed.RESULTS Total follow-up was 54054 person-years for women and 27556 person-years for men.Insulin levels were positively correlated with elevated LFT during follow-up,whereas physical activity and coffee consumption were negatively correlated.Individuals with both insulin levels in the upper tertile and alcohol consumption above MASLD thresholds had an increased risk for both liver disease,adjusted hazard ratio(aHR)of 4.3(95%CI:1.6-14.6)and cirrhosis(aHR=4.8,95%CI:1.6-14.6).CONCLUSION This population-based study provides evidence that high insulin levels are a risk factor for development of elevated liver enzymes and clinically manifest liver disease.The results support the concept of metabolic dysfunction associated liver disease. 展开更多

Pulmonary function test(PFT)is a vital noninvasive method for evaluating respiratory system function and is widely used in the diagnosis,surgical risk assessment,and prognostic prediction of chronic airway diseases and interstitial lung diseases.In 2021,the American Thoracic Society and the European Respiratory Society updated their pulmonary function interpretation guidelines,choosing to adopt z-scores instead of percentage of predicted forced expiratory volume in 1 second(FEV1),thereby significantly improving sensitivity and prognostic evaluation.However,conventional PFT relies on patient cooperation and is limited in its ability to effectively capture the heterogeneity of localized lesions.In recent years,the use of multimodal medical imaging in pulmonary function prediction has progressed rapidly.When combined with radiomics and deep learning techniques,these imaging methods enable more accurate and quantitative functional assessments.This review introduces that pulmonary function assessment should integrate multimodal imaging features with clinical functional indicators.This approach establishes cross-scale linkages between morphology and function and provides new perspectives for the early screening of high-risk individuals with accelerated pulmonary aging,evaluating the effectiveness of interventions,and enabling personalized intervention strategies.We review the classification systems and clinical value of PFT,discuss the advances and limitations of multimodal imaging in predicting lung function,and explore future directions in standardized data acquisition and multicenter validation to promote precise pulmonary function assessment and clinical application. 展开更多

End-stage liver disease(ESLD)is a debilitating condition that often leads to death.Once diagnosed with ESLD,a patient’s quality of life decreases significantly.Well-established markers,such as the model for end-stage liver disease and Child-Pugh scores,are used to diagnose,monitor,and predict survival.Another tool,the 6-minute walk test(6MWT),has shown promise as an adjunct measure but remains underutilized despite its strong results and ease of reproducibility.Previous research has demonstrated that the 6MWT,as in many other conditions,helps predict survival,mortality,and outcomes in individuals with ESLD.A recent observational study by Corrêa et al found that the 6MWT is a reliable measure of functional capacity in patients with liver cirrhosis.Although the study’s small sample size of 20 patients limits its generalizability,the findings align with prior evidence showing that the 6MWT is an independent predictor of mortality and that poor test performance is associated with greater liver dysfunction.However,the 6MWT is a nonspecific test,and patient performance can be influenced by comorbidities such as heart failure,pulmonary disease,and frailty.Additionally,the optimal timing for administering the test has not been clearly established in the literature.Overall,although well-validated markers already exist for diagnosing and assessing outcomes in liver disease,the addition of the 6MWT-a low-cost,reliable,easily reproducible,and objective measure-may provide valuable prognostic and functional information if standardized protocols and covariates are better controlled. 展开更多

BACKGROUND Functional abdominal pain syndrome(FAPS)is a subset of functional gastrointestinal disorder characterized by chronic recurrent abdominal pain condition that is not attributable to structural,organic,or metabolic diseases.The etiology and pathophysiology of FAPS remain poorly understood;however,brain-gut interactions and food intolerance,particularly to fermentable oligo-,di-,and monosaccharides and polyols,have been implicated.AIM To determine the frequency of fructose malabsorption(FM)in patients with FAPS and to evaluate the occurrence of resultant symptoms.METHODS This was a prospective,case-control study conducted over 22 months,from December 2016 to September 2018,at the Department of Gastroenterology,Govind Ballabh Pant Institute of Postgraduate Medical Education and Research,New Delhi,India.In this study,61 patients with FAPS and 31 healthy controls underwent fructose hydrogen breath testing(FHBT)after excluding small intestinal bacterial overgrowth.RESULTS FM was detected in 26%of patients with FAPS vs 3%of controls(P=0.009).Symptoms during the FHBT were significantly more prevalent in patients with FAPS(39.3%)than in the controls(12.9%)(P=0.009).Among patients with FAPS,those with positive FHBT results had a higher prevalence of symptoms(62.5%)than those with negative results(31%)(P=0.03).Multivariate analysis identified younger age(P=0.012)and male sex(P=0.049)as predictive of a positive FHBT result.Methane producers were present in 37%of patients with FAPS,with no difference between those with and without FM.CONCLUSION This study demonstrated a high prevalence of FM and intolerance in patients with FAPS,with younger age and male sex as predictors. 展开更多

BACKGROUND Timely identification and monitoring of lung disease progression are key components of effective management in patients with cystic fibrosis(CF).Pulmonary function tests(PFTs)and high-resolution computed tomography(HRCT)scans are commonly used to assess disease progression in these patients.Determining which modality provides the most sensitive and reliable assessment is crucial for establishing effective disease monitoring guidelines,particularly in India,where national guidelines are lacking.AIM To assess the progression of lung disease in patients with CF using a combination of HRCT and PFTs.METHODS A total of 32 patients with CF were prospectively enrolled from the outpatient clinic and followed longitudinally.Clinical and physiological parameters,including age,spirometric indices,and HRCT based Bhalla scores,were evaluated at baseline and after one-year followup.Paired comparisons between baseline and follow-up HRCT scores,as well as baseline and follow-up PFT parameters,were performed using a two-tailed paired t-test to assess statistically significant changes over the follow-up period.Pearson correlation coefficient(r)was used to evaluate the relationship between HRCT scores and spirometric parameters at each individual time point(baseline and follow-up).A P value0.05).CONCLUSION Despite significant radiological progression of structural lung damage detected by HRCT,pulmonary function parameters remained relatively stable.These findings suggest that PFTs may be less sensitive in detecting progressive structural deterioration in CF,underscoring the value of HRCT as a sensitive tool for disease monitoring and early detection of worsening lung disease. 展开更多

The six-minute walk test(6MWT)remains a tool for assessing exercise capacity in heart failure,with six-minute walk distance as the endpoint.To enhance interpretability,vital signs such as heart rate and blood pressure may also be recorded in accordance with technical statements.However,beta-blockers often blunt heart-rate-based readouts during the 6MWT,thereby reducing the interpretability of six-minute walk distance.Akimova et al recently published a study in World Journal of Experimental Medicine,which identified blood pressure variability(BPV)as a minutes-scale recovery signal during a paired 6MWT.The index targets the 20-minute post-exercise recovery window and uses point-to-point systolic blood pressure change to help characterize cardiac reserve and to enhance interpretation of the 6MWT.The measure is low cost and easy to implement,and BPV remains associated with imaging indices such as left ventricular ejection fraction in betablocked heart failure populations.Importantly,this should be viewed as an early,proof-of-concept insight.Future work should confirm these findings in multicenter prospective studies,define operational protocols and thresholds for BPV,and explore relationships with clinical outcomes,thereby further enhancing the value of minutes-scale BPV. 展开更多

Correlation Analysis Between Serum Homocysteine Levels and Executive Function Impairment in Patients with Schizophrenia 认领 引用

Methods: Eighty schizophrenia patients in a stable disease stage were enrolled in this cross-sectional study. Participants were classified into a high Hcy concentration group (n=38) and a normal Hcy concentration group (n=42) based on fasting serum Hcy levels. Executive function was assessed using three standardized neuropsychological tests: the Wisconsin Card Classification Test (WCST), the Stroop Color Word Test, and the test. Data were analyzed using independent-sample t-tests, chi-square tests, Pearson correlation analysis, and multiple linear regression models. Results: The high Hcy group demonstrated significantly fewer correct responses in the WCST and significantly more persistent errors compared to the normal group. The high Hcy group also exhibited longer reaction times in both the SCWT and TMT-B tasks. Correlation analysis revealed a negative association between serum Hcy concentration and WCST correct response count, and a positive association with TMT-B completion time. Regression analysis further confirmed that serum Hcy was an independent variable influencing both WCST correct response count and TMT-B completion time. Conclusion: Elevated serum homocysteine levels in schizophrenia patients are associated with impaired specific components of executive function, suggesting that Hcy may serve as a biomarker for related cognitive impairments.This biomarker is involved in the pathological process underlying the impaired disease control capacity. 展开更多

AIM: To evaluate and compare the clinical usefulness of 13C-phenylalanine and 13C-methacetin breath tests in quantitating functional hepatic mass in patients with chronic liver disease and to further compare these results with those of conventional tests, Child-Pugh score and serum bile acid levels.METHODS: One hundred and forty patients (50 HCV-related chronic hepatitis, 90 liver cirrhosis patients) and 40 matched healthy controls were studied. Both breath test and routine liver test, serum levels of cholic and chenodeoxycholic acid conjugates were evaluated.RESULTS: Methacetin breath test, expressed as 60 min cumulative percent of oxidation, discriminated the hepatic functional capacity not only between controls and liver disease patients, but also between different categories of chronic liver disease patients. Methacetin breath test was correlated with liver function tests and serum bile acids.Furthermore, methacetin breath test, as well as serum bile acids, were highly predictive of Child-Pugh scores. The diagnostic power of phenylalanine breath test was always less than that of methacetin breath test.CONCLUSION: Methacetin breath test represents a safe and accurate diagnostic tool in the evaluation of hepatic functional mass in chronic liver disease patients. 展开更多

BACKGROUND: Prognostic value of cortisol and thyroid function tests(TFTs) has previously been evaluated in medical ICUs. We aimed to evaluate prognostic efficacy of cortisol and TFTs in critically ill poisoned patients admitted to toxicology intensive care unit(ICU).METHODS: In a prospective study of consecutively enrolled subjects admitted to the toxicology ICU, lab analyses included TFTs(total T3 and T4 as well as TSH) and cortisol levels drawn between 8 am–10 am during period of the first 24 hours post-ingestion/exposure. Simplified Acute Physiology Score Ⅱ(SAPS Ⅱ) and Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ) were recorded. All scores were compared to detect the best prognostic factor. Type of poisoning was also included.RESULTS: In 200 patients evaluated, 129 were male and mean age was 31 years. In general, SAPS Ⅱ, T4, and cortisol could prognosticate death. After regression analysis, only cortisol had such efficacy(P=0.04; OR=1.06; 95%CI=1.05–1.08; cut-off=42 μg/d L; sensitivity=70%; specificity=82%). Between aluminium phosphide(ALP)-and non ALP-poisoned patients, level of consciousness, mean arterial pressure, and cortisol level could prognosticate death in ALP poisoning(all Ps<0.001 in both uni and multivariate analyses). Median(interquartile range; IQR) GCS was 7(6, 10) and 15(8, 15) in non-ALP and ALP-poisoned patients(P<0.003). SAPS Ⅱ and APACHE Ⅱ could not prognosticate death at all.CONCLUSION: Cortisol best prognosticated outcomes for subjects admitted to the toxicology ICU. Its level is higher in ALP-poisoned patients probably due to the higher stress while they remain conscious till the final stages of toxicity and are aware of deterioration of their clinical condition or may be due to their significantly lower blood pressures. 展开更多

AIM To assess intestinal barrier function during human intestinal ischemia and reperfusion(IR).METHODS In a human experimental model,6 cm of jejunum was selectively exposed to 30 min of ischemia(I) followed by 30 and 120 min of reperfusion(R). A sham procedure was also performed. Blood and tissue was sampled at all-time points. Functional barrier function was assessed using dual-sugar absorption tests with lactulose(L) and rhamnose(R). Plasma concentrations of citrulline,an amino acid described as marker for enterocyte function were measured as marker of metabolic enterocytes restoration. Damage to the epithelial lining was assessed by immunohistochemistry for tight junctions( TJs),by plasma marker for enterocytes damage(I-FABP) and analyzed by electron microscopy(EM) using lanthanum nitrate as an electrondense marker.RESULTS Plasma L/R ratio's were significantly increased after 30 min of ischemia(30 I) followed by 30 min of reperfusion(30 R) compared to control(0.75 ± 0.10 vs 0.20 ± 0.09,P < 0.05). At 120 min of reperfusion(120 R),ratio's normalized(0.17 ± 0.06) and were not significantly different from control. Plasma levels of I-FABP correlated with plasma L/R ratios measured at the same time points(correlation: 0.467,P < 0.01). TJs staining shows distortion of staining at 30 I. An intact lining of TJs was again observed at 30 I120 R. Electron microscopy analysis revealed disrupted TJs after 30 I with paracellular leakage of lanthanum nitrate,which restored after 30 I120 R. Furthermore,citrulline concentrations closely paralleled the histological perturbations during intestinal IR.CONCLUSION This study directly correlates histological data with intestinal permeability tests,revealing that the human gut has the ability of to withstand short episodes of ischemia,with morphological and functional recovery of the intestinal barrier within 120 min of reperfusion. 展开更多

Aim:To evaluate the sperm function after vasovasostomy.Methods:Semen samples from 42 subjects after vasovasostomy(Group A:1-6 months,Group B:6-12 months;Group C:12-18 months after vasectomy rever-sal)were investigated.Semen from 34 normal fertile men was used as controls.Spem function tests,including hypo-osmotic swelling test(HOST),acridine orange(AO)fluorescence,acrosome reaction(triple-stain),cervical mucus penetration test(CMPT),etc were done.Results:A After vasectomy reversal,the percentage of HOST was signifi-cantly lower than that of the normal fertile men.In regard to AO,there were no significant differences between the three vasovasostomy groups and between these 3 groups and the controls.With triple-stain,the percentage of nommal acrosome reaction was significantly lower in Group A as compared with the controls,but not in Groups Band C.There were no significant differences in the results of CMPT between the vasovasostomy groups and the controls.However,the number of"poor”type was significantly higher in Groups A and C than in the controls;the percentage of"nega-tive”type were higher in Groups A and B than in the controls.Conclusion:After vasovasostomy a lower level of HOST remained for one year and gradually recovered after one year.Six months after vasectomy reversal,the percent-age of acrosome reaction could be changed from lower level to nommal range.The data of AO indicated that the genetic material(double-stranded DNA)in spermatozoa was not affected by vasovasostomy.To evaluate the result of CMPT after vasectomy reversal,not only the nomal results but also the abnormal results(“poor”and“negative”types)should also be considered. 展开更多

The diagnosis of Chronic Pancreatitis (CP) is based on the detection of abnormal structure or function of the diseased pancreas. The pancreatic function tests more accurately determine the presence of CP than tests of structure, especially for early stage disease. The function tests can be divided into two categories: non- invasive and invasive. The invasive "tube" tests can reliably detect mild, early CP, but are only available at a few referral centers and tend to be poorly tolerated by patients. The non-invasive tests are easy to obtain, but tend to perform poorly in patients with early, mild disease. Therefore, no one test is useful in all clinical situations, and a detailed understanding of the rational, pathophysiologic basis, strengths, and limitations of various tests is needed. This review highlights the role of various pancreatic function tests in the diagnosis of CP including fecal fat analysis, fecal elastase, fecal chymotrypsin, serum trypsin, the secretin stimulation test, the cholecystokinin (CCK) stimulation test, the combined secretin-CCK stimulation test, the intraductal and endoscopic secretin stimulation tests, and the functional magnetic resonance imaging of the pancreas after secretin stimulation. 展开更多

Hormone-stimulated pancreatic function tests (PFTs) are considered the gold standard for measuring pancreatic exocrine function. PFTs involve the administration of intravenous secretin or cholecystokinin, followed by collection and analysis of pancreatic secretions. Because exocrine function may decline in the earliest phase of pancreatic fibrosis, PFTs are considered accurate for diagnosing chronic pancreatitis. Unfortunately, these potentially valuable tests are infrequently performed except at specialized centers, because they are time consuming and complicated. To overcome these limitations, endoscopic PFT methods have been developed which include aspiration of pancreatic secretions through the suction channel of the endoscope. The secretin endoscopic pancreatic function test (ePFT) involves collection of duodenal aspirates at 15, 30, 45 and 60 min after secretin stimulation. A bicarbonate concentration greater than 80 mmol/L in any of the samples is considered a normal result. The secretin ePFT has demonstrated good sensitivity and specificity compared with various reference standards, including the "Dreiling tube" secretin PFT, endoscopic ultrasound, and surgical histology. Furthermore, a standard autoanalyzer can be used for bicarbonate analysis, which allows the secretin ePFT to be performed at any hospital. The secretin ePFT may complement imaging tests like endoscopic ultrasound (EUS) in the diagnosis of early chronic pancreatitis.This paper will review the literature validating the use of ePFT in the diagnosis of exocrine insufficiency and chronic pancreatitis. Newer developments will also be discussed, including the feasibility of combined EUS/ ePFT, the use of cholecystokinin alone or in combination with secretin, and the discovery of new protein and lipid pancreatic juice biomarkers which may complement traditional fluid analysis. 展开更多

Instead of the equi-biaxial tensile test, the plane strain test carried out with the traditional uniaxialtesting machine is suggested to provide the experimental data for calibration of anisotropic yield function. This simplified method byusing plane strain test was adopted to identify the parameters of Yld2000-2d yield function for 5xxx aluminum alloy and AlMgSialloy sheets. The predicted results of yield stresses, anisotropic coefficients and yield loci by the proposed method were very similarwith the experimental data and those by the equi-biaxial tensile test. It is validated that the plane strain test is effective to provideexperimental data instead of equi-biaxial tensile test for calibration of Yld2000-2d yield function. 展开更多

After stroke,even high-functioning individuals may experience compromised bimanual coordination and fine motor dexterity,leading to reduced functional independence.Bilateral arm training has been proposed as a promising intervention to address these deficits.However,the neural basis of the impairment of functional fine motor skills and their relationship to bimanual coordination performance in stroke patients remains unclear,limiting the development of more targeted interventions.To address this gap,our study employed functional near-infrared spectroscopy to investigate cortical responses in patients after stroke as they perform functional tasks that engage fine motor control and coordination.Twenty-four high-functioning patients with ischemic stroke(7 women,17 men;mean age 64.75±10.84 years)participated in this cross-sectional observational study and completed four subtasks from the Purdue Pegboard Test,which measures unimanual and bimanual finger and hand dexterity.We found significant bilateral activation of the sensorimotor cortices during all Purdue Pegboard Test subtasks,with bimanual tasks inducing higher cortical activation than the assembly subtask.Importantly,patients with better bimanual coordination exhibited lower cortical activation during the other three Purdue Pegboard Test subtasks.Notably,the observed neural response patterns varied depending on the specific subtask.In the unaffected hand task,the differences were primarily observed in the ipsilesional hemisphere.In contrast,the bilateral sensorimotor cortices and the contralesional hemisphere played a more prominent role in the bimanual task and assembly task,respectively.While significant correlations were found between cortical activation and unimanual tasks,no significant correlations were observed with bimanual tasks.This study provides insights into the neural basis of bimanual coordination and fine motor skills in high-functioning patients after stroke,highlighting task-dependent neural responses.The findings also suggest that patients who exhibit better bimanual performance demonstrate more efficient cortical activation.Therefore,incorporating bilateral arm training in post-stroke rehabilitation is important for better outcomes.The combination of functional near-infrared spectroscopy with functional motor paradigms is valuable for assessing skills and developing targeted interventions in stroke rehabilitation. 展开更多

In order to deal with the limitations during the register transfer level verification, a new functional verification method based on the random testing for the system-level of system-on-chip is proposed.The validity of this method is proven theoretically.Specifically, testcases are generated according to many approaches of randomization.Moreover, the testbench for the system-level verification according to the proposed method is designed by using advanced modeling language.Therefore, under the circumstances that the testbench generates testcases quickly, the hardware/software co-simulation and co-verification can be implemented and the hardware/software partitioning planning can be evaluated easily.The comparison method is put to use in the evaluation approach of the testing validity.The evaluation result indicates that the efficiency of the partition testing is better than that of the random testing only when one or more subdomains are covered over with the area of errors, although the efficiency of the random testing is generally better than that of the partition testing.The experimental result indicates that this method has a good performance in the functional coverage and the cost of testing and can discover the functional errors as soon as possible. 展开更多

Rhabdomyolysis is a syndrome of skeletal muscle injury with release of cellular constituents such as potassium,phosphate,urate and intracellular proteins such as myoglobin into the circulation,which may cause complications including acute kidney injury,electrolyte disturbance and cardiac instability.Abnormal liver function tests are frequently observed in cases of severe rhabdomyolysis.Typically,there is an increase in serum aminotransferases,namely aspartate aminotransferase and alanine aminotransferase.This raises the question of liver injury and often triggers a pathway of investigation which may lead to a liver biopsy.However,muscle can also be a source of the increased aminotransferase activity.This review discusses the dilemma of finding abnormal liver function tests in the setting of muscle injury and the potential implications of such an association.It delves into some of the clinical and experimental evidence for correlating muscle injury to raised aminotransferases,and discusses pathophysiological mechanisms such as oxidative stress which may cause actual liver injury.Serum aminotransferases lack tissue specificity to allow clinicians to distinguish primary liver injury from muscle injury.This review also explores potential approaches to improve the accuracy of our diagnostic tools,so that excessive or unnecessary liver investigations can be avoided. 展开更多
