BACKGROUND Post-intensive care syndrome(PICS)is a frequent complication following major gastrointestinal tumor surgery,characterized by persistent physical impairment associated with systemic inflammation,muscle catabolism,and neuromuscular dysfunction.AIM To investigate the impact of multimodal rehabilitation nursing(MRN)on physical functional recovery in post-operative gastrointestinal tumor patients with PICS,using mechanism-related outcome indicators.METHODS This retrospective study enrolled 80 patients with PICS after gastrointestinal tumor surgery between August 2022 and August 2024.Patients who received an 8-week MRN program were compared with those receiving conventional care.Physical performance,inflammatory status,nutritional parameters,and neuromuscular function were evaluated at predefined time points.RESULTS Compared with conventional care,MRN was associated with significant improvements in handgrip strength,phase angle,and compound muscle action potential amplitude/area ratio,along with substantial reductions in interleukin-6 levels,timed up and go time,and time to functional recovery(all P<0.05).CONCLUSION MRN significantly improves physical recovery in post-operative PICS patients by modulating inflammation,muscle metabolism,and neuromuscular function,supporting its use as a mechanism-based rehabilitation strategy in clinical practice. 展开更多

As global strategies shift toward expanding treatment eligibility for chronic hepatitis B,ensuring long-term adherence is critical.Block et al recently published a study in World Journal of Virology,challenging the conventional focus on economic barriers and revealing that compromised physical functioning is a significantly more potent predictor of non-adherence than financial affordability.This finding exposes a profound“clinical discordance”:Patients may achieve viral suppression yet suffer from residual fatigue and functional impairment,potentially driven by immune exhaustion or metabolic comorbidities.We argue that this symptom burden serves as a functional blockade to daily persistence.Therefore,clinical care must evolve from“adherence policing”to active symptom management.Integrating patient-reported outcomes and addressing physical deficits are essential strategies to safeguard the efficacy of antiviral regimens in the elimination era. 展开更多

Objectives:This prospective cohort study examined the change trajectories of cognitive and physical functions of individuals with motoric cognitive risk(MCR)syndrome,as well as the longitudinal associations between MCR syndrome and changes in cognitive and physical functions,to provide a new perspective on preventing dementia.Methods:Participants were selected from the China Health and Retirement Longitudinal Study(CHARLS).Demographic characteristics,health status,and lifestyle variables were assessed in 2011.MCR syndrome was defined as the presence of subjective cognitive complaints and objective slow gait,with preserved activities of daily living and absence of dementia,and assessed in 2011.Cognitive function,including orientation,attention and calculation,episodic memory,and visuospatial ability,was measured from 2011 to 2018.Physical function,including grip strength,balance ability,and repeated chair stand tests,was measured from 2011 to 2015.Generalized estimating equation was employed to analyze the longitudinal associations between MCR syndrome in 2011 and changes in cognitive functions over 7 years and physical functions over 4 years.Results:Among 4,217 participants,475 had MCR syndrome in 2011.Both participants with MCR syndrome and those without exhibited a decline in both cognitive and physical function over 7 years and 4 years of follow-up,except for fluctuations in visuospatial ability.Non-MCR syndrome participants demonstrated significantly better overall cognitive function in 2018 compared to 2011(Group×Time:B=0.44,P=0.035)than those in the MCR syndrome group.However,participants with non-MCR syndrome demonstrated significantly worse visuospatial ability in 2013(Group×time:B=−0.44,P=0.002)and 2018(Group×time:B=−0.34,P=0.016)compared to those in the MCR syndrome group.Non-MCR syndrome participants demonstrated significantly better performance in repeated chair stand tests in 2013(Group×time:B=0.31,P<0.001)and 2015(Group×time:B=0.37,P<0.001)compared to those in the MCR syndrome group in 2011.Conclusions:Older adults with MCR syndrome experience worse overall cognitive and physical function performance,especially in repeated chair stand tests,than individuals without MCR syndrome over 7-year and 4-year follow-up periods.It is suggested that future interventional studies will target both physical and cognitive functions in MCR syndrome individuals,providing insights for the prevention of dementia. 展开更多

In recent years,physical unclonable function(PUF)has emerged as a lightweight solution in the Internet of Things security.However,conventional PUFs based on complementary metal oxide semiconductor(CMOS)present challenges such as insufficient randomness,significant power and area overhead,and vulnerability to environmental factors,leading to reduced reliability.In this study,we realize a strong,highly reliable and reconfigurable PUF with resistance against machine-learning attacks in a 1 kb spinorbit torque magnetic random access memory fabricated using a 180 nm CMOS process.This strong PUF achieves a challenge-response pair capacity of 109 through a computing-in-memory approach.The results demonstrate that the proposed PUF exhibits near-ideal performance metrics:50.07% uniformity,50% diffuseness,49.89% uniqueness,and a bit error rate of 0%,even in a 375 K environment.The reconfigurability of PUF is demonstrated by a reconfigurable Hamming distance of 49.31% and a correlation coefficient of less than 0.2,making it difficult to extract output keys through side-channel analysis.Furthermore,resistance to machine-learning modeling attacks is confirmed by achieving an ideal accuracy prediction of approximately 50% in the test set. 展开更多

Confronting the escalating global challenge of counterfeit products,developing advanced anticounterfeiting materials and structures with physical unclonable functions(PUFs)has become imperative.All-optical PUFs,distinguished by their high output complexity and expansive response space,offer a promising alternative to conventional electronic counterparts.For practical authentications,the expansion of optical PUF keys usually involves intricate spatial or spectral shaping of excitation light using bulky external apparatus,which largely hinders the applications of optical PUFs.Here,we report a plasmonic PUF system based on heterogeneous nanostructures.The template-assisted shadow deposition technique was employed to adjust the morphological diversity of densely packed metal nanoparticles in individual PUFs.Transmission images were processed via a hash algorithm,and the generated PUF keys with a scalable capacity from 2875 to 243401 exhibit excellent uniqueness,randomness,and reproducibility.Furthermore,the wavelength and the polarization state of the excitation light are harnessed as two distinct expanding strategies,offering the potential for multiscenario applications via a single PUF.Overall,our reported plasmonic PUFs operated with the multidimensional expanding strategy are envisaged to serve as easy-to-integrate,easy-to-use systems and promise efficacy across a broad spectrum of applications,from anticounterfeiting to data encryption and authentication. 展开更多

Background This study compared knee osteoarthritis(OA)outcomes specific to pain,physical function,and quality of life in later life based on strength training(ST)participation over a lifetime.Methods Participants from the Osteoarthritis Initiative(n=3192)were grouped by ST engagement during ages 12–18 years,19–34 years,35–49 years,and 50+years.Participants were categorized as:No ST(no ST at any point;61.7±9.0 years(mean±SD)),Some ST(engaged in ST during 1–3 life stages;58.9±8.7 years),and Lifelong ST(consistently engaged in ST across all life stages;55.6±8.1 years).Measures were collected at baseline and Year 4:Western Ontario and McMaster Universities Osteoarthritis Index Scores(WOMAC;pain,daily activities),Knee Injury and Osteoarthritis Outcome Score(KOOS;sports,recreation),Physical Activity Score for the Elderly(PASE),Short Form-12 Physical Component Score(SF-12 PCS),mobility disability,chair rise time,and walking speed(20 m and 400 m).Results At Year 4,the Lifelong ST group reported better WOMAC activity scores in the right knee along with better WOMAC pain,KOOS sportsecreation,and PASE scores compared to other groups(p<0.05).The Lifelong ST group had the lowest incidence of mobility disability of all groups(0.8%vs.2.3%–4.1%;p=0.015)and maintained the fastest walking speeds in Year 4.Conclusion For those with knee OA,ST throughout life may help preserve function and mobility,allowing for greater physical activity engagement while keeping pain levels relatively lower. 展开更多

With the rapid development of the Internet of Things(IoT),the widespread adoption of applications such as smart homes and industrial IoT has raised the demand for secure authentication and key agreement among resource-constrained devices over open communication channels.Traditional authentication protocols often rely on centralized servers for key distribution,which results in high communication overhead and exposes systems to single-point-of-failure risks.Moreover,IoT devices are typically constrained in computational resources and are vulnerable to hardware cloning.These limitations necessitate lightweight yet robust security mechanisms.To address these challenges,we propose a lightweight peer-to-peer authentication protocol based on Physically Unclonable Function(PUF)and Multiple Reference Fuzzy Extractor(MRFE).The proposed protocol enables direct mutual authentication and key agreement between IoT devices without the participation of a trusted third-party server.Formal security analysis,along with evaluations of computation and communication costs,demonstrates that the protocol achieves strong security guarantees while maintaining high efficiency.Therefore,the proposed protocol is well-suited for lightweight peer-to-peer authentication scenarios in IoT environments. 展开更多

Background:Studies have revealed the protective effect of DL-3-n-butylphthalide(NBP)against diseases associated with ischemic hypoxia.However,the role of NBP in animals with hypobaric hypoxia has not been elucidated.This study investigated the effects of NBP on rodents with acute and chronic hypobaric hypoxia.Methods:Sprague-Dwaley rats and Kunming mice administered with NBP(0,60,120,and 240 mg/kg for rats and 0,90,180,and 360 mg/kg for mice)were placed in a hypobaric hypoxia chamber at 10,000 m and the survival percentages at 30 min were determined.Then,the time and distance to exhaustion of drug-treated rodents were evaluated during treadmill running and motor-driven wheel-track treadmill experiments,conducted at 5800 m for 3 days or 20 days,to evaluate changes in physical functions.The frequency of active escapes and duration of active escapes were also determined for rats in a shuttle-box experiment,conducted at 5800 m for 6 days or 27 days,to evaluate changes in learning and memory function.ATP levels were measured in the gastrocnemius muscle and malonaldehyde(MDA),superoxide dismutase(SOD),hydrogen peroxide(H2O2),glutathione peroxidase(GSH-Px),and lactate were detected in sera of rats,and routine blood tests were also performed.Results:Survival analysis at 10,000 m indicated NBP could improve hypoxia tolerance ability.The time and distance to exhaustion for mice(NBP,90 mg/kg)and time to exhaustion for rats(NBP,120 and 240 mg/kg)significantly increased under conditions of acute hypoxia compared with control group.NBP treatment also significantly increased the time to exhaustion for rats when exposed to chronic hypoxia.Moreover,240 mg/kg NBP significantly increased the frequency of active escapes under conditions of acute hypoxia.Furthermore,the levels of MDA and H2O2 decreased but those of SOD and GSH-Px in the sera of rats increased under conditions of acute and chronic hypoxia.Additionally,ATP levels in the gastrocnemius muscle significantly increased,while lactate levels in sera significantly decreased.Conclusion:NBP improved physical and learning and memory functions in rodents exposed to acute or chronic hypobaric hypoxia by increasing their anti-oxidative capacity and energy supply. 展开更多

IoT devices rely on authentication mechanisms to render secure message exchange.During data transmission,scalability,data integrity,and processing time have been considered challenging aspects for a system constituted by IoT devices.The application of physical unclonable functions(PUFs)ensures secure data transmission among the internet of things(IoT)devices in a simplified network with an efficient time-stamped agreement.This paper proposes a secure,lightweight,cost-efficient reinforcement machine learning framework(SLCR-MLF)to achieve decentralization and security,thus enabling scalability,data integrity,and optimized processing time in IoT devices.PUF has been integrated into SLCR-MLF to improve the security of the cluster head node in the IoT platform during transmission by providing the authentication service for device-to-device communication.An IoT network gathers information of interest from multiple cluster members selected by the proposed framework.In addition,the software-defined secured(SDS)technique is integrated with SLCR-MLF to improve data integrity and optimize processing time in the IoT platform.Simulation analysis shows that the proposed framework outperforms conventional methods regarding the network’s lifetime,energy,secured data retrieval rate,and performance ratio.By enabling the proposed framework,number of residual nodes is reduced to 16%,energy consumption is reduced by up to 50%,almost 30%improvement in data retrieval rate,and network lifetime is improved by up to 1000 msec. 展开更多

In order to reduce physical unclonable fixnction (PUF) response instability and imbalance caused by the metastability and the bias of arbiter, this paper uses an improved balanced D flip-plop (DFF) based on the unbalanced DFF to reduce the bias in response output and enhances the security of PUF by adopting two balanced DFFs in series. The experimental results show that two cascaded balanced DFFs improve the stability of the DFF, and the output of two balanced DFFs is more reliable. The entropy of output is fixed at 98.7%. 展开更多

BACKGROUND Studies on varicose veins have focused its effects on physical function;however,whether nonsurgical treatments alter muscle oxygenation or physical function remains unclear.Moreover,the differences in such functions between individuals with varicose veins and healthy individuals remain unclear.AIM To investigate changes in physical function and the quality of life(QOL)following nonsurgical treatment of patients with varicose veins and determine the changes in their muscle oxygenation during activity.METHODS We enrolled 37 participants(those with varicose veins,n=17;healthy individuals,n=20).We performed the following measurements pre-and post-nonsurgical treatment in the varicose vein patients and healthy individuals:Calf muscle oxygenation during the two-minute step test,open eyes one-leg stance,30 s sit-to-stand test,visual analog scale(VAS)for pain,Pittsburgh sleep quality index,physical activity assessment,and QOL assessment.RESULTS Varicose veins patients and healthy individuals differ in most variables(physical function,sleep quality,and QOL).Varicose veins patients showed significant differences between pre-and post-nonsurgical treatment—results in the 30 sit-to-stand test[14.41(2.45)to 16.35(4.11),P=0.018],two-minute step test[162.29(25.98)to 170.65(23.80),P=0.037],VAS for pain[5.35(1.90)to 3.88(1.73),P=0.004],and QOL[39.34(19.98)to 26.69(17.02),P=0.005];however,no significant difference was observed for muscle oxygenation.CONCLUSION Nonsurgical treatment improved lower extremity function and QOL in varicose veins patients,bringing their condition close to that of healthy individuals.Future studies should include patients with severe varicose veins requiring surgery to confirm our findings. 展开更多

In the emerging Industrial Internet of Things(IIoT),authentication problems have become an urgent issue for massive resource-constrained devices because traditional costly security mechanisms are not suitable for them.The security protocol designed for resource-constrained systems should not only be secure but also efficient in terms of usage of energy,storage,and processing.Although recently many lightweight schemes have been proposed,to the best of our knowledge,they are unable to address the problem of privacy preservation with the resistance of Denial of Service(DoS)attacks in a practical way.In this paper,we propose a lightweight authentication protocol based on the Physically Unclonable Function(PUF)to overcome the limitations of existing schemes.The protocol provides an ingenious authentication and synchronization mechanism to solve the contradictions amount forward secrecy,DoS attacks,and resource-constrained.The performance analysis and comparison show that the proposed scheme can better improve the authentication security and efficiency for resource-constrained systems in IIoT. 展开更多

Method: 100 elderly patients with cerebral hemorrhage treated in our hospital from January 2024 to September 2025 were selected and randomly divided into an observation group (n=50, comprehensive nursing centered on functional rehabilitation) and a control group (n=50, routine nursing). The HINSS score of the observation group after intervention was significantly lower than that of the control group (P<0.05). The FMA score and GQOLI-74 score of the observation group after intervention were significantly higher than those of the control group (P<0.05). Conclusion: Comprehensive nursing interventions centered on functional rehabilitation can improve the neurological function of elderly patients with cerebral hemorrhage, thereby enhancing their limb function and quality of life. 展开更多

Objective: This study investigated the effects of cardiac rehabilitation exercise protocols on physical function (PF) in patients with chronic heart failure (CHF). Study Design and Setting: This randomized controlled trial recruited 70 patients who are in stage II CHF with ejection fraction (≤40%) from a Nigerian university teaching hospital. They were randomly assigned into Exercise Group (EG: n = 35) or Control Group (CG: n = 35). Physical function, activity of daily living (ADL), distance walked in six minutes and grip strength were assessed using a validated ADL questionnaire, six minute walk test and a hand dynamometer respectively. In addition to medication, EG underwent aerobic and upper extremity resistance exercises thrice weekly for eight weeks while CG used medications only. Data were analyzed using descriptive and inferential statistics. Alpha level was at p p > 0.05). Significant improvements were noticed at fourth week among participants’ ADL (30.0% ± 6.0%), 6MWD (321.7 ± 26.3 m) and VO2 max (8.9 ± 0.4 mL/kg/min) variables within the exercise EG but no significant changes were observed in the CG (p > 0.05). Participants in EG demonstrated more significant improvements in ADL (15.0% ± 5.0%), 6MWD (406.0 ± 29.7 m) and VO2 max (10.3 ± 0.5 mL/kg/min) (p p > 0.05). Conclusions: Cardiac rehabilitation exercise protocols involving self-paced walking, sit-to-stand and upper extremity dynamic strength training improved activity of daily living, walking and functional capacity in patients with stable chronic heart failure. 展开更多

The number of older adults (individuals ≥65 years), particularly women, in our society is increasing and understanding the impact of exercise on muscle capacity (e.g., strength and power) and subsequently physical function is of utmost importance to prevent disability and maintain independence. Muscle capacity declines with age and this change negatively impacts physical function in older women. Exercise, specifically resistance training, is recommended to counteract these declines; however, the synergistic relationships between exercise, muscle capacity, and physical function are poorly understood. This review will summarize the literature regarding age-related changes in the aforementioned variables and review the research on the impact of resistance training interventions on muscle capacity and physical function in older women. Recommendations for future research in this area will be discussed. 展开更多

An improved coupling of numerical and physical models for simulating 2D wave propagation is developed in this paper. In the proposed model, an unstructured finite element model (FEM) based Boussinesq equations is applied for the numerical wave simulation, and a 2D piston-type wavemaker is used for the physical wave generation. An innovative scheme combining fourth-order Lagrange interpolation and Runge-Kutta scheme is described for solving the coupling equation. A Transfer function modulation method is presented to minimize the errors induced from the hydrodynamic invalidity of the coupling model and/or the mechanical capability of the wavemaker in area where nonlinearities or dispersion predominate. The overall performance and applicability of the coupling model has been experimentally validated by accounting for both regular and irregular waves and varying bathymetry. Experimental results show that the proposed numerical scheme and transfer function modulation method are efficient for the data transfer from the numerical model to the physical model up to a deterministic level. 展开更多

This study aimed to evaluate the efficacy of an individualized remote exercise program on the improvement of body composition and physical fitness of a heterogeneous group of patients who completed breast cancer treatment.This prospective study included 107 women aged 18 to 60,shortly after curative treatment for localized breast cancer,at the Erasto Gaertner Cancer Hospital(HEG)in Curitiba,PR,Brazil.Body composition,maximal oxygen consumption,and muscle resistance were evaluated after nine months of intervention while considering adherence to the program,level of physical activity,presence of binge eating disorder,tumor classification,and treatment type.Seventy-eight women(72.8%)adhered to the training program.Adherent participants showed significant changes in body mass([-4.3±3.6]kg;p<0.0001),body mass index([-1.6±1.5]kg·m−2;p<0.0001),body fat(−3.4%±3.1%;p<0.0001),maximal oxygen consumption([7.5±2.0]ml·kg−1·min−1);p<0.0001),and abdominal resistance([11.2±2.8]reps;p<0.0001).In contrast,these variables did not change significantly in the non-adherent group.Among the adherent participants,those subclassified in the severe binge group showed a more noticeable reduction in body mass,body mass index,and body fat(p<0.05)than those in the non-binge group.Individualized remotely-guided physical exercise programs can improve the body composition and physical fitness of women undergoing post-breast cancer surveillance,regardless of pathological history or treatment. 展开更多

The physical function of even a person getting on a wheelchair could be tested in all subjects. Blood was collected by venipuncture and the serum 1, 25-hydroxy vitamin D (1, 25OHD) concentration was measured. The Mini-Mental State Examination (MMSE) and the Montreal Cognitive Assessment-Japanese version (MoCA-J) was used for the cognitive function test. Physical function was measured objectively using the Timed UP and Go (TUG) and 4-m walking test (4MWS). A significant positive correlation was found between serum 1, 25OHD and MMSE or MoCA-J. It is expected that an elderly person can maintain a mean serum 1, 25OHD level of about 100 pg/mL for preventing early cognitive disorder. In the present study, a significant positive correlation was found between urinary 25-hydroxy vitamin D (25OHD)/creatinine and MMSE or MoCA-J. Our results showed that urinary 25OHD might be a useful biomarker for predicting cognitive disorder. There was a significant negative correlation between serum 1, 25OHD and TUG or 4MWS. These findings suggest that serum 1, 25OHD levels might serve as a useful index to improve cognitive and physical functional impairment. 展开更多

OBJECTIVE To investigate possible associations between physical function assessment scales,such as Short Physical Performance Battery(SPPB)and Berg Balance Scale(BBS),with all-cause mortality in acute decompensated heart failure(ADHF)patients.METHODS A total of 108 ADHF patients were analyzed from October 2020 to October 2022,and followed up to May 2023.The association between baseline clinical characteristics and all-cause mortality was analyzed by univariate Cox regression analysis,while for SPPB and BBS,univariate Cox regression analysis was followed by receiver operating characteristic curves,in which the area under the curve represented their predictive accuracy for all-cause mortality.Incremental predictive values for both physical function assessments were measured by calculating net reclassification index and integrated discrimination improvement scores.Optimal cutoff value for BBS was then identified using restricted cubic spline plots,and survival differences below and above that cut-off were compared using Kaplan-Meier survival curves and the log-rank test.The clinical utility of BBS was measured using decision curve analysis.RESULTS For baseline characteristics,age,female,blood urea nitrogen,as well as statins,angiotensin-converting enzyme inhibitors,angiotensin II receptor blockers,or angiotensin receptor-neprilysin inhibitors,were predictive for all-cause mortality for ADHF patients.With respect to SPPB and BBS,higher scores were associated with lower all-cause mortality rates for both assessments;similar area under the curves were measured for both(0.774 for SPPB and 0.776 for BBS).Furthermore,BBS≤36.5 was associated with significantly higher mortality,which was still applicable even adjusting for confounding factors;BBS was also found to have great clinical utility under decision curve analysis.CONCLUSIONS BBS or SPPB could be used as tools to assess physical function in ageing ADHF patients,as well as prognosticate on all-cause mortality.Moreover,prioritizing the improvement of balance capabilities of ADHF patients in cardiac rehabilitation regimens could aid in lowering mortality risk. 展开更多

Objective: Oral diadochokinesis (ODK) has been shown to be associated with oral frailty. In this study, we investigate the relationship between physical function, swallowing function, nutritional evaluation, and each syllable of ODK. Methods: The subjects are 26 elderly people living in an area where they can walk. ODK, tongue pressure, modified water swallowing test, EAT-10, grip strength, MNA-SF, skeletal muscle index, height, weight, and BMI were evaluated. For statistical analysis based on ODK values, the subjects were divided into two groups on the basis of the number of times the syllables /pa/, a/, and /ka/ were repeated: 1) The first group with >6 repetitions exceeded six; 2) the second group with ≤5 repetitions. Both groups were compared using the Mann-Whitney U test (Model 1). In addition, the syllables /pa/, a/, and /ka/ were also divided among the groups and a comparison was performed (Model 2). Results: In Model 1, the tongue pressure value was significantly higher in the good group (p Conclusion: In community-dwelling elderly people, tongue pressure was positively associated with ODK values for the syllables /pa/ and a/, suggesting that tongue pressure may be associated with strength of the lip and tongue tip muscles. Furthermore, it was suggested that the ODK values may even be lower than normal values in the elderly who maintain skeletal muscle mass and thus have adequate tongue pressure;therefore, these indicators may be used as a guideline to assess oral frailty. 展开更多
