Objective To observe influence of kinetic needling on functional restoration of upper limbs of patients with apoplexy-induced shoulder-hand syndrome. Methods Ninety patients were randomly divided into an observation group (45 cases) and a control group (45 cases). Basic treatments selected according to corresponding stroke units were applied to both groups. Besides, scalp acupuncture at Dingnie Qianxiexian (MS 6, 顶颞前斜线), Dingnie Houxiexian (MS 6, 顶颞前斜线) and Dingzhongxian (NS 5, 顶中线), was applied in the treatment group. In the observation group, active and passive movement was carried out during the process of scalp acupuncture, and then needling plus bloodletting puncturing and cupping was applied. In the control group, only needling plus blood-letting puncturing and cupping was given. Therapeutic effects, pain scoring obtained by visual analogue scales (VAS), FugI-Meyer and FugI-Meyer's scoring reflecting moving functions of upper limbs were made after 4 courses of treatments in both groups. Results The total effective rate was 95.5% and 91.1% in the observation group and control group, respectively, showing superiority of the former to the latter (P〈0.05). After treatment, VAS scores were all obviously reduced in both groups (P〈0.01), and the reduction in the former was significantly more than that in the latter (P〈0.01). Moving functions of upper limbs shown by Fugl-Meyer's scoring were improved after 2 courses of treatment in both groups (P〈0.01). The improvement was more obvious after 4 courses of treatment than that after 2 courses of treatment in both groups (P〈0.05). After 4 courses of treatment, moving functions shown by FugI-Meyer's scoring were more remarkably improved in the observation group than those in the control group(t=3.9, P〈0.01). Conclusion Both kinetic needling combined with needling plus blood-letting puncturing and cupping and simple needling plus blood-letting puncturing and cupping are effective in treating shoulder-hand syndrome after apoplexy, and the former is better than the latter. 展开更多

Puncturing has been recognized as a promising technology to cope with the coexistence problem of enhanced mobile broadband(eMBB) and ultra-reliable low latency communications(URLLC)traffic. However, the steady performance of eMBB traffic while meeting the requirements of URLLC traffic with puncturing is a major challenge in some realistic scenarios. In this paper, we pay attention to the timely and energy-efficient processing for eMBB traffic in the industrial Internet of Things(IIoT), where mobile edge computing(MEC) is employed for data processing. Specifically, the performance of eMBB traffic and URLLC traffic in a MEC-based IIoT system is ensured by setting the threshold of tolerable delay and outage probability, respectively. Furthermore,considering the limited energy supply, an energy minimization problem of eMBB device is formulated under the above constraints, by jointly optimizing the resource blocks(RBs) punctured by URLLC traffic, data offloading and transmit power of eMBB device. With Markov's inequality, the problem is reformulated by transforming the probabilistic outage constraint into a deterministic constraint. Meanwhile, an iterative energy minimization algorithm(IEMA) is proposed.Simulation results demonstrate that our algorithm has a significant reduction in the energy consumption for eMBB device and achieves a better overall effect compared to several benchmarks. 展开更多

Flexible needle puncture path planning in surgical robots faces significant challenges,such as limited adaptability to multi-objective environments and poor real-time performance.These issues affect both the accuracy and efficiency of needle puncture,restricting its application in complex medical scenarios.This paper proposes a deep reinforcement learning-based method to improve flexible needle path planning.To address the limitations of traditional models in accurately capturing needle dynamics,a hierarchical tissue model based on the unicycle framework is designed,which integrates kinematic and mechanical models.This approach considers the varying forces from different tissues on the needle at various positions.A dynamic multi-objective environment and obstacle model are also constructed,along with a target prioritization scheme for multi-objective optimization.Additionally,a prioritized experience replay(PER)mechanism is introduced to improve data efficiency in the learning process.This method enhances the needle's adaptability and robustness in dynamic environments.Simulation results demonstrate that the model improved real-time performance and precision in dynamic multiobjective environments,making intelligent decisions based on target priorities and accelerating the exploration of optimal strategies. 展开更多

The coexistence of ultra-reliable low-latency communication(URLLC)and enhanced mobile broadband(eMBB)services in 5G-based industrial wireless networks(IWNs)poses significant resource slicing challenges due to their inherent performance requirement conflicts.To address this challenge,this paper proposes a puncturing method that uses a model-aided deep reinforcement learning(DRL)algorithm for URLLC over eMBB services in uplink 5G networks.First,a puncturing-based optimization problem is formulated to maximize the eMBB accumulated rate under strict URLLC latency and reliability constraints.Next,we design a random repetition coding-based contention(RRCC)scheme for sporadic URLLC traffic and derive its analytical reliability model.To jointly optimize the scheduling parameters of URLLC and eMBB,a DRL solution based on the reliability model is developed,which is capable of dynamically adapting to changing environments.The accelerated convergence of the model-aided DRL algorithm is demonstrated using simulations,and the superiority in resource efficiency of the proposed method over existing approaches is validated. 展开更多

BACKGROUND Atrial electrode lead dislodgment leading to phrenic nerve capture is rare.CASE SUMMARY An elderly man with third-degree atrioventricular block underwent dual-chamber pacemaker implantation.During the procedure,both leads were introduced through the same venous puncture site.A passive lead was positioned in the atrium,and an active lead was positioned in the ventricle.On postoperative day 3,the patient developed diaphragmatic flutter.Electrocardiography demonstrated poor atrial sensing.Repeat chest radiography and computed tomography revealed displacement of the atrial lead compared with its immediate postoperative position,while the ventricular lead remained stable.Device interrogation confirmed abnormal atrial sensing.The frequency of diaphragmatic flutter varied according to the programmed atrial pacing rate,whereas ventricular sensing and pacing thresholds were normal.After reprogramming the device to ventricular inhibited pacing mode,the diaphragmatic flutter resolved completely.CONCLUSION Atrial electrode lead displacement may capture the phrenic nerve,leading to diaphragmatic flutter. 展开更多

BACKGROUND Hydatid cyst disease of the liver remains a significant public health problem in endemic regions.While surgical treatment has traditionally been the mainstay of therapy,minimally invasive percutaneous approaches have emerged as safe and effective alternatives,especially for selected World Health Organization(WHO)cystic echinococcosis(CE)1 and CE3a cysts.Comparative data on efficacy,complication rates,and clinical outcomes between the two methods are essential for guiding optimal treatment selection.AIM To compare and evaluate the efficacy,safety,complication rates,and clinical course of WHO CE1 and CE3a liver hydatid cysts treated with surgical and percutaneous methods.METHODS A total of 989 patients diagnosed with liver hydatid cyst and treated either surgically(n=734)or percutaneously(n=255)between 2005 and 2025 were included in the study.Demographic data,treatment process,complications,and recurrence rates of the retrospectively evaluated patients were recorded.Cyst volume,hospital stay duration,and catheter removal times were compared.Cases with and without fistula development were also analyzed separately.RESULTS There was no significant difference between the surgical(n=734)and percutaneous(n=255)groups in terms of gender(female:76.0%vs 72.2%;P=0.250)and age(38.4±15.9 years vs 38.1±16.1 years;P=0.800),respectively.Operation time(85.6±34.5 minutes vs 40.3±15.7 minutes;P<0.001),hospital stay duration(7.3±6.2 days vs 3.1±2.3 days;P<0.001),catheter removal time(6.6±5.3 days vs 5.5±6.4 days;P=0.014),and intraoperative organ injury rate(2.7%vs 0%;P=0.002)were significantly longer/higher in the surgical group compared to the percutaneous group.Recollection was significantly more frequent in the percutaneous group(4.7%vs 1.2%;P=0.001),as was anaphylaxis(1.6%vs 0.3%;P=0.041).The rate of cysto-biliary fistula was similar in both groups[surgical 14.6%(n=113),percutaneous 14.9%(n=43);P=0.902].However,in patients with fistula,catheter removal time(surgical:8.3±4.9 days vs 5.9±2.7 days and percutaneous:17.8±8.7 days vs 3.5±2.9 days;P<0.001)and initial cyst volumes(surgical:774.8±513.2 mL vs 356.7±95.6 mL and percutaneous:700.9±288.2 mL vs 346.5±279.2 mL;P<0.001)were significantly higher compared to those without fistula.CONCLUSION For treatment of WHO CE1 and CE3a liver cysts,the percutaneous approach is a safe and effective method due to shorter hospital stays,minimal invasiveness,and negligible risk of intraoperative organ injury,whereas surgical methods appear marginally advantageous regarding recollection and anaphylaxis.In both groups,higher cyst volume increases the risk of fistula and may prolong the treatment process.Patient selection should consider these parameters. 展开更多

Robotic-assisted medical technology has long been a key area of research in modern surgical medicine.Roboticassisted puncture techniques,both theoretically and practically,hold significant potential to improve puncture precision and overall surgical outcomes in clinical practice.This paper presents a model-free variable impedance control(MFVIC)method for robotic soft tissue puncture tasks,enabling high-precision puncture of soft tissues using variable impedance control without requiring model information.Conventional position-or forcebased control methods often fail to ensure the precision of puncture or maintain an appropriate puncture force,both of which are critical for the task.The proposed variable impedance control approach allows for accurate puncture to the desired location while maintaining low puncture force throughout the puncture process,thus effectively meeting the demands of the puncture task.Additionally,a Jacobian matrix estimator is designed to estimate the Jacobian matrix of the redundant robotic arm in real-time during operation.This enables precise robot control using sensor data,without the need for prior knowledge of the robot model. 展开更多

Mechanochemically responsive effects show promise in revealing the underlying microscale deformation mechanism of polymeric materials.In this work,we conducted puncture experiments on multiple network elastomeric membranes embedded with ring-opening mechanophores.During the puncture process,fluorescence signals reveal that the region surrounding the indenter tip sustains the dominant stress concentration,and this activated area expands progressively with increasing indentation depth.A mechanochemical constitutive model is developed to couple the fluorescence response and mechanical deformation.The model quantitatively predicts the mechanical and mechanochemical responses.The simulated fluorescence distributions through finite element analysis show agreement with experimental observations,capturing both the spatial extent and intensity evolution of activated zones.This work offers new insights into applying puncture experiments to characterize the mechanical behaviors of soft materials. 展开更多

This study presents a computational and experimental framework for assessing magnetic resonance imaging(MRI)-induced radiofrequency(RF)heating risks for a transcranial puncture needle at 3 T MRI.Due to the one-dimensional structure of the puncture needle,as well as its clinical interventional use,the transfer function method was used to evaluate the needle’s electromagnetic model.Multi-variable computational simulations encompassing over 100000 tests were performed using five anatomical virtual human models,including various coil configurations and clinical implantation trajectories.The in vivo RF heating was predicted by convolving the simulated tangential electric field along each path with the measured transfer function,scaled by an experimentally validated factor.The maximum temperature rise of the puncture needle in the human body does not exceed 6℃ after a 15-min scan under a given power limit exposure under MRI.This integrated approach provides a robust safety assessment for transcranial MRI-guided intervention. 展开更多

BACKGROUND Postdural puncture headache(PDPH)is a significant complication of neuraxial procedures.Although conservative treatments and the invasive epidural blood patch(EBP)are currently standard approaches,the sphenopalatine ganglion(SPG)block is emerging as a promising,less-invasive alternative.The pathophysiology of PDPH involves cerebrospinal fluid leakage,resulting in reduced intracranial pressure and compensatory cerebral vasodilation,which is partially mediated by SPG stimulation.The SPG block aims to interrupt this vasodilation using local anesthetics,thereby alleviating headache symptoms.AIM To provide a comprehensive overview of current evidence regarding the use of the SPG block in treating PDPH,explores the anatomical and physiological basis of this intervention,describes various administration techniques for administering the block,and critically assesses the efficacy and safety data from clinical studies.METHODS A systematic literature search was conducted on PubMed and the Cochrane Database to identify systematic reviews and meta-analyses published up to April 2025,using the keywords“sphenopalatine ganglion block”and“post-dural puncture headache”.RESULTS The SPG block is a simple,noninvasive,and effective bedside procedure.Clinical studies have demonstrated that it provides rapid pain relief,with high success rates and an onset of action typically within 10-30 minutes.The most used technique is the transnasal approach,which is associated with minimal and transient adverse effects such as nasal discomfort and bitter taste.CONCLUSION Although EBP remains the preferred treatment for severe PDPH,the SPG block is a viable alternative for mild-tomoderate cases,often allowing patients to postpone or avoid EBP.Comparative studies suggest that SPG block has a quicker onset than EBP,though in some cases,it provides shorter duration relief.Overall,the SPG block is a safe,effective,and readily accessible treatment for PDPH.Its minimally invasive nature and high success rate in providing rapid pain relief make it an excellent first-line alternative to more invasive procedures.Further largescale,standardized randomized controlled trials are required to optimize protocols and fully integrate the SPG block into mainstream clinical practice. 展开更多

Hydatid disease,caused by the larval stage of Echinococcus granulosus,is a significant public health problem in endemic regions and worldwide.The liver is the most frequently affected organ,followed by the lungs,in adults.Hepatic hydatid cysts(HHC)may remain asymptomatic for years and are often incidentally detected on imaging.Radiological evaluation plays a pivotal role in the diagnosis and classification of hydatid cysts into World Health Organization types(cystic lesion and cystic echinococcus 1 through cystic echinococcus 5).Further,imaging also helps plan appropriate management and assists in image-guided percutaneous interventions when appropriate.Treatment strategies for HHC include anthelmintic therapy,surgery,percutaneous interventions,and a“watch and wait”approach.With advances in interventional radiology,minimally invasive percutaneous techniques have emerged as effective and safer alternatives to surgery in select cases.Procedures such as puncture,aspiration,injection,and reaspiration,standard catheterization technique,modified catheterization technique,percutaneous evacuation,andÖrmeci technique have demonstrated high cure rates,with reduced morbidity and mortality.Surgical management remains reserved for cysts that are ruptured or show communication with biliary ducts,and for cysts located in challenging anatomical locations.This review discusses the role of interventional radiology in HHC,highlighting the indications,techniques,and recent advancements in percutaneous management strategies. 展开更多

Objective:To explore the therapeutic effect of double-puncture tympanic membrane puncture and tympanic cavity drug injection in patients with acute secretory otitis media.Methods:A total of 84 patients with acute secretory otitis media admitted to our hospital from June 2024 to June 2025 were selected and randomly divided into two groups by drawing lots.The control group(42 cases)was treated with the traditional single-puncture tympanic membrane puncture and tympanic cavity drug injection method,while the observation group(42 cases)was treated with the double-puncture tympanic membrane puncture and tympanic cavity drug injection method.The therapeutic effects of the two groups were compared.Results:The overall treatment response rate,overall complication rate,time to symptom relief,and improvement in hearing threshold in the observation group were all superior to those in the control group,with statistically significant differences(P<0.05).Conclusion:For acute secretory otitis media,the treatment method of double-puncture tympanic membrane puncture and tympanic cavity drug injection demonstrates definite efficacy,significantly reducing the incidence of complications,accelerating symptom relief,and improving hearing function,making it worthy of promotion. 展开更多

Objective:To investigate the clinical effect of precision nursing in pediatric intravenous infusion.Methods:A total of 120 children who received intravenous infusion in The Second Affiliated Hospital of Guilin Medical University from October 2025 to December 2025 were selected and divided into a control group and an observation group,with 60 children in each group.The control group received routine nursing,while the observation group received precision nursing.The nursing effects of the two groups were compared.Results:The compliance of children during infusion in the observation group was higher than that in the control group,the incidence of adverse events was lower,the success rate of one-time puncture was higher,and the nursing satisfaction was higher than that in the control group.Conclusion:Precision nursing can eliminate children’s resistance to intravenous infusion,make them cooperate better with nurses’operations,reduce children’s pain,improve the success rate of one-time puncture in intravenous infusion,and increase parents’satisfaction with intravenous infusion nursing.It is worthy of promotion in clinical practice. 展开更多

The Clinical Application Value of Child-Friendly Psychological Nursing in Pediatric Patients Receiving Intravenous Infusion 认领 引用

Objective: To thoroughly investigate the clinical application value of child-friendly guided psychological nursing in the treatment of pediatric patients receiving intravenous infusion. Methods: A total of 86 pediatric patients receiving intravenous infusion in the outpatient department of Yangzhong People's Hospital from January to December 2024 were selected as study subjects. Using a 1:1 alternating grouping method, they were divided into a control group (receiving traditional nursing care, n=43) and an observation group (receiving child-friendly guided psychological nursing, n=43) to evaluate the intervention effects of different nursing approaches. Results: The observation group demonstrated significantly better infusion compliance and puncture success rates compared to the control group (P<0.05). All relevant scores in the observation group were lower than those in the control group (P<0.05). At 30 minutes before infusion, 30 minutes after puncture, and at the end of infusion, the RCADS (Child Anxiety and Distress Scale) scores in the observation group were lower than those in the control group (P<0.05). The observation group also showed higher frequency and quality of nurse-patient interactions (P<0.05). Additionally, all relevant scores in the observation group were significantly higher than those in the control group at each time point (P<0.05). Conclusion: Child-friendly guided psychological nursing applied to pediatric patients receiving intravenous infusion yields significant clinical outcomes and possesses high potential for widespread implementation. 展开更多

Most Aloe species are used as new food or functional food ingredient.Even though widely known for its health benefits,the anti-inflammatory effects and underlying mechanisms of Aloin(Alo),an anthraquinone compound isolated from plant species of the genus Aloe,remain unidentified.Here,we investigated the protective effects of Alo against cecal ligation and puncture(CLP)-induced sepsis and microflora in mice.Alo significantly improved CLP-induced sepsis and the survival rate of septic mice,downregulated the expression of proinflammatory factors,and decreased the infiltration of inflammatory cells in tissues.Alo upregulated the proportion of peritoneal macrophages,reduced the number of peritoneal bacteria,decreased the content of short-chain fatty acids and bile acids in the abdominal cavity,and suppressed Toll-like receptor(TLR)-2/4uclear factor kappa-B(NF-κB)/NOD-like receptor thermal protein domain associated protein 3(NLRP3)/Caspase-1/3/8 signaling.Furthermore,Alo altered the composition of the microbiome and promoted the growth of Lactobacillus,which showed a stronger anti-inflammatory effect.Whole-genome analysis identified the genes Saa3,Il10,Fpr1,and Eif4a1 associated with the protective effects of Alo in mice with CLP-induced sepsis.Overall,our results provide novel insights into the therapeutic potential and mechanism of action of Alo in the treatment of sepsis. 展开更多

Acute lung injury(ALI)is a significant complication of sepsis,characterized by high morbidity,mortality,and poor prognosis.Neutrophils,as critical intrinsic immune cells in the lung,play a fundamental role in the development and progression of ALI.During ALI,neutrophils generate neutrophil extracellular traps(NETs),and excessive NETs can intensify inflammatory injury.Research indicates that Taohe Chengqi decoction(THCQD)can ameliorate sepsis-induced lung inflammation and modulate immune function.This study aimed to investigate the mechanisms by which THCQD improves ALI and its relationship with NETs in sepsis patients,seeking to provide novel perspectives and interventions for clinical treatment.The findings demonstrate that THCQD enhanced survival rates and reduced lung injury in the cecum ligation and puncture(CLP)-induced ALI mouse model.Furthermore,THCQD diminished neutrophil and macrophage infiltration,inflammatory responses,and the production of pro-inflammatory cytokines,including interleukin-1β(IL-1β),IL-6,and tumor necrosis factorα(TNF-α).Notably,subsequent experiments confirmed that THCQD inhibits NET formation both in vivo and in vitro.Moreover,THCQD significantly decreased the expression of peptidyl arginine deiminase 4(PAD4)protein,and molecular docking predicted that certain active compounds in THCQD could bind tightly to PAD4.PAD4 overexpression partially reversed THCQD’s inhibitory effects on PAD4.These findings strongly indicate that THCQD mitigates CLP-induced ALI by inhibiting PAD4-mediated NETs. 展开更多
