Purpose:This study compares the hierarchical structure of scientific teams across countries and investigates factors associated with the observed cross-national differences.Design/methodology/approach:Drawing on 150,817 publications with author contribution statements,we focus on the 15 countries with the largest volume of scientific publications,examine cross-country variations in the proportion of tall teams,and analyze how this proportion correlates with other factors.Findings:Scientific output from East Asia is dominated by tall teams,which persist after controlling for team size,indicating that this pattern cannot be fully accounted for by the prevalence of larger teams in these countries.Cultural factors,measured by Power Distance,as well as the observed funding patterns of major basic science agencies,are associated with the dominance of tall teams in East Asia.Research limitations:This study is limited by its reliance on publications with author contribution statements,which may introduce selection bias;its focus on cultural and funding factors,while leaving other institutional contexts unexamined;and its use of a leadership concentration measure that does not capture other dimensions of hierarchy.Practical implications:Understanding cross-national differences in research team structures and their associated cultural and institutional factors can inform science policy and team management.Originality/value:This study provides a systematic crossnational comparison of team hierarchy and offers a mechanistic understanding of the dominance of tall teams in East Asia,highlighting associations with cultural and funding factors. 展开更多

BACKGROUND Patients with decompensated cirrhosis frequently experience severe psychological distress,anxiety,and depression,yet psychological support is often fragmented in conventional care.AIM To investigate the effect of interdisciplinary team scheduling on psychological outcomes in decompensated cirrhosis.METHODS A randomized,single-blind,single-center trial was conducted from January 2022 to December 2024 in Guangxi Zhuang Autonomous Region.A total of 110 patients with decompensated cirrhosis(Distress Thermometer≥4)were randomized to interdisciplinary team scheduling(n=55)or conventional scheduling(n=55).Psychological distress,anxiety,depression,and quality of life were assessed using the Distress Thermometer,Self-Rating Anxiety Scale,Self-Rating Depression Scale,and World Health Organization Quality of Life 100 questionnaire,respectively.RESULTS Following the intervention,the interdisciplinary group achieved significantly lower psychological distress[3(2-3)vs 3(3-4)],anxiety(41.65±4.29 vs 46.38±4.18),and depression scores(45.79±3.25 vs 50.14±3.69)compared with the control group(all P<0.05).Quality of life scores also improved significantly in the physical,psychological,and social domains(P<0.05).CONCLUSION The interdisciplinary team scheduling model effectively alleviates psychological symptoms and enhances quality of life among patients with decompensated cirrhosis.This model addresses unmet psychosocial needs through early,continuous,and collaborative care,providing a practical framework for integrating psychological support into chronic liver disease management. 展开更多

As the core leader of the nursing team,head nurses play a vital role in improving the professional competence of the team,enhancing team cohesion,upgrading nursing quality,ensuring nursing safety,and promoting nursing innovation.This study takes 15 head nurses from different levels and departments as research subjects,and collects data through semistructured interviews and observation methods.Combined with transformational leadership theory,social learning theory and organizational commitment theory,an“Input-Process-Output(IPO)”analytical framework is constructed.Through thematic induction and cross-case analysis,this paper systematically explores the mechanism and realization paths of head nurses’leadership in nursing team management.The results show that through exemplary leadership,scientific management and effective communication,head nurses not only improve the professional literacy and teamwork ability of nursing staff,but also promote the continuous optimization of nursing quality and safety management.This study further puts forward strategies to strengthen the capacity building of head nurses themselves,improve the management system and focus on personnel training,to provide theoretical support and empirical reference for nursing management practice and policy formulation. 展开更多

Surgical resection is the cornerstone of treatment for gastrointestinal tumors,but postoperative recovery is fraught with challenges,including impaired wound healing and significant psychological distress.Standard nursing care often falls short of addressing these multifaceted needs.The recent study by Huang and Qian provides compelling evidence for the efficacy of a multidisciplinary team(MDT)collaborative nursing model.Their retrospective analysis demonstrates that an integrated,holistic approach significantly improves wound healing,alleviates anxiety and depression,enhances quality of life,and reduces complication rates compared to conventional care.This letter discusses the significance of these findings,contextualizes the synergistic benefits of the MDT model,and outlines future directions for research and clinical implementation.It underscores the necessity of adopting such patient-centered,integrated care models as the new standard in postoperative management for gastrointestinal oncology. 展开更多

BACKGROUND Rare and complex clinical cases often lack standardized management protocols,posing significant diagnostic and therapeutic challenges.Multidisciplinary team(MDT)collaboration provides holistic perspectives by integrating surgical,medical,radiological,pathological,and psychosocial expertise.Concurrently,innovative research-including precision medicine,artificial intelligence-driven diagnostics,and novel therapeutics-is bridging the gap between theory and practice.Synthesizing current evidence on MDT-driven approaches and innovative interventions is essential to improve patient outcomes,establish standardized frameworks,and advance global clinical medicine.AIM To systematically review MDT roles and innovative research in managing rare and complex clinical cases.METHODS A systematic search of PubMed and Cochrane Library(2013-2025)was conducted.Eligible studies included case reports,case series,systematic reviews,and clinical trials involving rare or complex cases with MDT involvement.Non-peer-reviewed sources and anecdotal reports were excluded.Data extraction focused on MDT composition,decision-making processes,innovative interventions,and patient outcomes.Study quality was assessed using PRISMA guidelines.RESULTS Evidence demonstrates that MDTs enhance diagnostic accuracy,optimize treatment planning,and improve survival in oncology,cardiology,and neurology.Rare congenital anomalies and complex autoimmune disorders particularly benefit from cross-specialty collaboration.Innovative research achievements include genetic profiling for precision therapies,machine learning models predicting rare disease progression,and novel therapeutics such as immunotherapy and regenerative medicine.Case-based evidence highlights rare sarcomas,congenital heart defects,and atypical autoimmune syndromes,where early MDT involvement and integration of research findings yielded improved patient quality of life and clinical outcomes.CONCLUSION MDT-driven strategies,combined with innovative research,are transforming rare case management,advancing clinical medicine,and contributing to improved global health outcomes. 展开更多

Manned-Unmanned Teaming(MUM-T)is an operational system where manned and unmanned systems perform missions through a collaboration interface,expanding beyond defense into civilian domains.The core of MUM-T lies in the organic interaction between manned and unmanned systems.The Collaboration Interface enabling this interaction becomes a primary target for cyber attacks due to its reliance on wireless networks.Compromising the reliability of the collaboration interface goes beyond simple communication failures;it directly leads to mission failure and aircraft safety issues.Therefore,systematic threat analysis and assessment tailored to this specific domain are essential.This study performs threat modeling based on the MITRE ATT&CK framework for the MUM-T collaboration interface and proposes a behavior-based risk assessment methodology combined with multi-criteria decision making(MCDM)techniques.First,attack techniques reflecting the characteristics of the collaboration interface are derived to structure threat scenarios.The relative importance of each TTP of the scenario is quantified to calculate the overall risk.When applied to GPS and battery spoofing scenarios,the proposed methodology confirmed that by structurally reflecting the likelihood of occurrence and propagation paths of TTP units,it precisely derives the complex threat characteristics of MUM-T environments.By defining collaboration interfaces as the analysis target and presenting a TTP-based threat analysis and quantitative risk assessment methodology,this study provides practical grounds for determining threat-specific response priorities in MUM-T environments. 展开更多

BACKGROUND Acute intestinal obstruction is a common surgical emergency with high mortality,requiring efficient multidisciplinary collaboration to improve outcomes.AIM To explore the optimization of nursing pathways for patients with acute intestinal obstruction using the multidisciplinary team(MDT)model.METHODS A total of 176 patients with acute intestinal obstruction admitted to the Emergency Department of our hospital between December 2023 and February 2025 were randomly divided into a control group(n=88)and an intervention group(n=88)using a random number table.The control group received routine emergency nursing interventions,whereas the intervention group received emergency nursing pathway interventions under the MDT model.The groups were compared in terms of emergency treatment efficiency,improvement of clinical symptoms,36-Item Short Form Health Survey(SF-36)scores,nursing satisfaction,and early(within 72 hours)intestinal obstruction–related complications-specifically,electrolyte disturbance,strangulated obstruction,and intestinal necrosis/perforation.RESULTS The intervention group had significantly shorter triage,condition assessment,and emergency treatment times(all P<0.001),faster relief of abdominal pain and vomiting,and earlier return of bowel function(all P<0.001).The rate of early complications was lower in the intervention group(2.27%vs 10.23%,P=0.029).Nursing satisfaction was higher(96.59%vs 81.82%,P=0.002),and post-intervention SF-36 scores were significantly higher(P<0.05).CONCLUSION The MDT-based emergency nursing pathway improves emergency efficiency,relieves clinical symptoms,enhances quality of life and satisfaction,and reduces early intestinal obstruction–related complications in patients with acute intestinal obstruction. 展开更多

Objective:To explore the application effect of the stratified teaching model in multidisciplinary team(MDT)teaching for gastric cancer,providing practical evidence for improving the quality of medical talent training in gastric cancer MDT diagnosis and treatment capabilities.Methods:Sixty learners who participated in gastric cancer MDT teaching at our hospital from July 2024 to July 2025 were selected as the research subjects and randomly divided into an observation group(n=30)and a control group(n=30)using a random number table method.The control group adopted the traditional“one-size-fits-all”teaching model,while the observation group employed a stratified teaching model.Students in the observation group were divided into basic,intermediate,and advanced levels based on their foundational knowledge and learning abilities,with differentiated teaching objectives,content,and evaluation systems implemented accordingly.The theoretical assessment scores,clinical skill operation scores,MDT case discussion ability ratings,and teaching satisfaction of students in both groups were compared after instruction.Results:After the teaching period,students in the observation group scored significantly higher than those in the control group in theoretical assessments,clinical skill operations,and MDT case discussion ability(P<0.001).Teaching satisfaction was also significantly higher in the observation group compared to the control group(P<0.05).Within the observation group,advanced-level students scored significantly higher than intermediate-and basic-level students in theoretical assessments,clinical skill operations,and MDT case discussion ability,while intermediate-level students outperformed basic-level students in all areas(all P<0.001).Conclusion:The stratified teaching model effectively enhances the quality of MDT teaching for gastric cancer,improving students’theoretical knowledge,clinical skills,and MDT case discussion abilities,as well as increasing teaching satisfaction.It is worthy of promotion and application in medical education. 展开更多

BACKGROUND Intestinal stoma closure can lead to severe gut microbiome dysbiosis,adversely affecting postoperative recovery and quality of life.An integrative approach combining traditional Chinese medicine(TCM)and Western medicine within a multi-disciplinary team(MDT)framework presents a promising strategy to enhance gut health.However,its efficacy has not been systematically evaluated.AIM To investigate the efficacy of an MDT approach combining TCM and Western medicine in promoting gut microbiome recovery after intestinal stoma closure.METHODS A retrospective study design was employed,analyzing data from patients who underwent intestinal stoma closure between January 2020 and December 2023.Patients were divided into two groups:The control group,which received standard Western medical care,and the MDT group,which received an integrative approach combining TCM and Western medicine.Medical records were reviewed to collect data on patient demographics,clinical characteristics,pre-and postoperative gut microbiome analysis,gastrointestinal symptoms,hospital stay duration,psychological assessments,immunological indicators,and nutritional status.RESULTS The study included 251 patients,with 122 in the control group and 129 in the MDT group.Baseline characteristics were similar between the groups.The MDT group showed significantly higher alpha diversity(Shannon index)postoper-atively compared with the control group.The MDT group had higher operational taxonomic unit counts for Bacteroidetes and Proteobacteria than the control group,whereas the control group had higher operational taxonomic unit counts for Firmicutes than the MDT group.The MDT group demonstrated lower prevalence rates of abdominal pain,bloating,and diarrhea postoperatively and shorter hospital stays than the control group.Patients in the MDT group reported improvements in chronic condition management,psychological well-being,immunological indicators,and nutritional status.The MDT group exhibited enhanced immune function,improved intestinal permeability,and better quality of life than the control group.CONCLUSION This retrospective study suggests that the MDT approach,combining TCM and Western medicine,has the potential to promote gut microbiome recovery and improve overall outcomes after intestinal stoma closure. 展开更多

The accelerating digitalization of global business and the growing prevalence of Virtual International Teams(VITs)have transformed organizational design and managerial practice.This paper develops an integrated analytical framework combining Principal-Agent Theory(PAT)and Business Process Reengineering(BPR)to address the key challenges of virtual collaboration-information asymmetry,trust,motivation,and control.While PAT explains behavioral dynamics of delegation,incentives,and accountability,BPR provides structural and technological tools for process optimization and transparency.Through theoretical synthesis and comparative analysis of leading corporations such as IBM,SAP,Google,and Deloitte,the study demonstrates that integrating agency mechanisms with reengineered digital processes enables organizations to balance autonomy with accountability and flexibility with control.The proposed model conceptualizes virtual management as a dual-governance system in which trust,technology,and transparency interact to create adaptive,learning-oriented teams.The research highlights that virtual leadership must evolve from supervision to system design,emphasizing empathy,data fluency,and ethical governance.Beyond its theoretical contribution,the study offers practical guidelines for implementing incentive-compatible digital architectures that foster collaboration,innovation,and resilience in hybrid and global environments.The synthesis of PAT and BPR provides a foundation for developing a broader theory of Collaborative Digital Governance,positioning virtual teamwork as a defining feature of modern enterprises in the Fourth Industrial Revolution. 展开更多

Objective:To explore the application effect of the airway management team combined with the evidence-based nursing model in the early rehabilitation of patients with acute respiratory distress syndrome(ARDS).Methods:A total of 76 elderly patients with ARDS admitted from January 2024 to January 2025 were selected and randomly divided into a control group(receiving conventional nursing)and an observation group(receiving airway management team combined with evidence-based nursing).Blood gas indicators such as arterial partial pressure of oxygen(PaO2)and oxygenation index(PaO2/FiO2)before and after intervention were compared between the two groups.The quality of life of patients after intervention was evaluated using the Short Form Health Survey(SF-36),and satisfaction with nursing services was assessed using a self-made questionnaire.Results:After intervention,both PaO2and PaO2/FiO2in the observation group were significantly higher than those in the control group(both p<0.05).The scores of the observation group in the 11dimensions of physical functioning,role-physical,role-emotional,and social functioning on the SF-36 scale were significantly higher than those of the control group(p<0.05).The overall satisfaction rate with nursing services in the observation group was 94.74%,significantly higher than the 76.32%in the control group(p<0.05).Conclusion:The intervention model of the airway management team combined with evidence-based nursing can effectively improve the oxygenation status and quality of life of elderly patients with ARDS and enhance their satisfaction with nursing services.It is an optimized critical care nursing plan worthy of clinical promotion. 展开更多

By connecting with practical cases from multiple higher vocational colleges, it explores the practical paths of organizational collaboration, content collaboration, and method collaboration, and builds a collaborative guarantee system of institutional guarantee, platform guarantee, and cultural infiltration. The digital economy is driving the transformation of the financial accounting industry,giving rise to new trends such as the integration of business and finance as well as intelligent finance.These developments pose higher demands on the teaching faculty in financial accounting education.However,the traditional“dual-qualification”teacher training model exhibits several limitations,including an imbalanced faculty structure,superficial industry-university collaboration,outdated competency development,and flaws in the evaluation system.Adopting a combined theoretical and empirical approach,this study proposes a new construction pathway guided by the integration of industry and education.This pathway involves establishing a multidimensional faculty structure,innovating industry-university collaborative mechanisms,upgrading competency systems,and reconstructing the evaluation system.The effectiveness of this approach is validated through a case study of the“digital-twin dual-qualification”model implemented at a higher education institution.Research indicates that this new model enhances the alignment between education and industry.The study provides a replicable practical framework for constructing“dual-qualification”teaching teams in financial accounting education. 展开更多

Objective:Interpersonal brain synchronization(IBS)has emerged as a significant concept in understanding collaborative team dynamics,with functional near-infrared spectroscopy(fNIRS)proving to be a vital tool in its assessment.This review aims to collate and analyze the literature on the application of fNIRS in various team settings,emphasizing its potential utility in surgical environments.Methods:A thorough search and screening process across multiple databases resulted in 17 studies being reviewed,with a focus on the utilization of fNIRS to measure IBS in different collaborative tasks.This review examined the tasks employed,participant demographics,organizational structures of teams,methodologies for IBS measurement,and correlations between brain synchronization and behavioral measurements.Results:fNIRS emerged as a non-invasive,cost-effective,and portable tool,predominantly used to assess IBS in pair-based tasks with a variety of participant demographics.Wavelet transform coherence was the primary method used for measuring synchronization,particularly in the prefrontal brain region.A consistent correlation was found between increased brain synchronization and enhanced team performance,underscoring the potential of fNIRS in understanding and optimizing team dynamics.Conclusion:This review establishes fNIRS as a promising tool for investigating the neural mechanisms underlying team cooperation,providing invaluable insights for potential applications in surgical settings.While acknowledging the limitations in the current literature,the review highlights the need for further research with larger sample sizes and varied task complexities to solidify the understanding of IBS and its impact on team performance.The ultimate goal is to leverage fNIRS in assessing and improving surgical team dynamics,contributing to improved patient outcomes and safety. 展开更多

目的探讨非急救转运过程中团队协作水平对患者安全与满意度的影响,为构建标准化转运管理体系提供依据。方法采用回顾性观察研究设计,选取2022年1月—2024年12月长沙市某非急救转运平台完成的750例转运任务。基于团队急救评估量表(TEAM)... 目的探讨非急救转运过程中团队协作水平对患者安全与满意度的影响,为构建标准化转运管理体系提供依据。方法采用回顾性观察研究设计,选取2022年1月—2024年12月长沙市某非急救转运平台完成的750例转运任务。基于团队急救评估量表(TEAM)评估转运团队的协作水平,结合客户满意度问卷-8(CSQ-8)评价患者满意度,并收集转运过程中的安全事件发生情况,分析其相关性。结果750例患者中,92.67%需实施心电监护,82.80%接受呼吸支持,13.87%使用血管活性药物。TEAM评分为(34.87±5.67)分,患者满意度3个核心维度11个条目均分3.17分,Q12全面评价评分为(6.93±2.03)分。CSQ-8患者满意度评分为(26.54±1.95)分,得分≥26分者占64.13%。TEAM总分与突发事件发生率呈负相关(r≈-0.583,P<0.001),与满意度评分呈正相关(r≈0.624,P<0.01)。结论非急救转运过程风险较高,高度依赖团队协作。良好的团队协作能力可显著降低安全事件发生率,提高患者满意度。建议加强非技术技能培训,构建以TEAM为基础的标准化转运流程与质量改进机制,以保障转运安全和服务质量。 展开更多
