Cardiometabolic response to high intensity functional training versus rowing-based high intensity interval training 认领 引用

This study compared the cardiometabolic response between rowing high intensity interval training(HIIT)and high intensity functional training(HIFT).Twenty two adults(age and.max=[25±7]yr and[40±9]mL/kg/min)underwent incremental exercise on a rowing ergometer to assess.max and peak power output(PPO).Subsequently,they underwent rowing HIIT(six 1 minutes[min]efforts at 85%PPO with 75 seconds[s]recovery)or HIFT(six 1 min efforts including pushups,jump squats,mountain climbers,and air squats with 75 s recovery).Gas exchange data,heart rate(HR),and blood lactate concentration(BLa)were acquired during exercise and postexercise.These regimens elicited 92%-96%HRmax,with higher peak HR([174±14]b/min vs.[167±15]b/min,p=0.002)and time spent≥85%HRmax([6.1±2.2]min vs.[5.0±2.8]min,p=0.03)for HIFT versus rowing HIIT.Results showed similar peak.between HIFT and HIIT([81±9]vs.[82±8].max,p=0.72),yet lower mean.([1.73±0.38]L/min vs.[1.93±0.50]L/min,p<0.001).Mean and peak ventilation(.)was significantly higher(p<0.05)with HIFT([62±13]L/min and[84±18]L/min vs.[53±15]L/min and[73±19]L/min).BLa was significantly higher with HIFT versus rowing HIIT(p<0.001)and was elevated 15 min post-exercise([7.0±3.0]mM vs.[3.5±1.1]mM).In healthy,active adults,HIFT elicits vigorous intensities similar to traditional HIIT. 展开更多

Pathological specimen sampling is the primary and key link of pathological diagnosis,as well as the most core and basic fundamental skill in the standardized training of resident pathologists.The standardization of sampling directly determines the quality of pathological sections,the accuracy of diagnosis,and the construction of trainees’clinical pathological thinking.At present,most resident trainees have common problems such as weak sampling concepts,non-standard operations,insufficient lesion identification,and sampling omissions,which constitute the main bottleneck restricting the quality of pathological residency training.Based on the growth law of pathological talents,the Pathology Residency Training Base of Baoding First Central Hospital integrates standardized sampling teaching into the whole three-year residency training,and constructs a characteristic sampling training system of“admission desensitization–ladder-based practical operation–subspecialty competency–quality control review–closedloop improvement”to target and solve trainees’problems including weak foundation,random operation and unclear key points.This paper focuses on expounding the core value of standardized sampling in consolidating the foundation of pathological residency training,and summarizes the base’s hierarchical sampling training mode,quality control management measures and practical effects,so as to provide a promotable practical scheme for domestic pathological residency training bases to strengthen basic teaching and improve trainees’core professional competencies. 展开更多

Background Obesity is a risk factor for developing cardiometabolic disease.Exercise training is pivotal in the treatment of obesity and is associated with reduced cardiovascular mortality.This study examined the effect of high-fat feeding on cardiac morphology and mitochondrial function,alongside the mitigating effects of voluntary exercise training.Methods Six-week-old male C57Bl/6 J mice commenced a high fat diet(HFD)or chow diet and were randomized to receive locked(sedentary)or unlocked(voluntary exercise training(VET))running wheels at 10 weeks of age.Mice were monitored until 30 weeks of age and euthanized for collection of tissues.Magnetic resonance imaging was performed to assess body composition,and echocardiography was performed to assess cardiac function.Results Compared with chow-fed animals,the HFD increased body weight and adiposity and decreased cardiolipins(CL)in the heart,which are required for maintaining adequate mitochondrial respiration.Importantly,VET reversed these effects and induced physiological cardiac hypertrophy.Cardiac mitochondrial respiratory chain analysis revealed decreased complexes II and IV activity following high fat feeding,while VET enhanced complex I activity,emphasizing the cardioprotective effect of exercise training in obesity.Conclusion This study uncovers mechanisms by which obesity and exercise impact cardiac mitochondrial health and suggests the mitochondria is a therapeutic target in obesity-related cardiovascular diseases. 展开更多

Clinically,individualized training improves post-stroke motor function rehabilitation efficiency.However,the mechanisms underlying how individualized training facilitates recovery remain relatively unclear.Here,we explored the cortical and corticomuscular rehabilitative effects of post-stroke motor function recovery during individualized training using a rat model of intracerebral hemorrhage.Forced training or individualized fatigue-controlled training was provided from days 2 to 14 post-stroke.The fatigue-controlled training group exhibited superior motor function recovery and less central fatigue compared with the forced training group.Electroencephalograph power spectrum density slope analysis demonstrated better inter-hemispheric balance in the fatigue-controlled training group than in the forced training group.Directed corticomuscular coherence analysis indicated that training-induced fatigue led to a short-term downregulation of descending directed corticomuscular coherence and an upregulation of ascending directed corticomuscular coherence.In the long term,excessive fatigue hindered the recovery of descending control in the affected hemisphere.In conclusion,the individualized strategy of peripheral fatigue-controlled training achieved better motor function recovery,which may be attributed to the mitigation of central fatigue,optimization of inter-hemispheric balance,and enhancement of descending control in the affected hemisphere.This is the first study to investigate the mechanisms underlying individualized rehabilitative effects at the cortical and corticomuscular levels.Our findings suggest that personalized training protocols that are tailored to manage fatigue levels may substantially enhance post-stroke motor efficiency.They also provide a mechanistic foundation for developing fatigue-monitored,individualized rehabilitation programs in clinical practice. 展开更多

BACKGROUND Clinical psychological nursing plays a vital role in clinical practice,and“mental health nurses”face greater stress than general nurses.Assessing the stress responses of“psychological nurses”during the implementation of psychological care and developing corresponding stress reduction strategies will enhance their capacity for stress management.AIM To explore the effect of pressure adjustment on mental health nurses in graded psychological nursing departments of general hospitals and to enhance their stress-coping ability through the Stress Management and Resiliency Trainingclinical(SMART-C)program.METHODS A total of 40 mental health nurses engaged in graded psychological nursing assessment and implementation in a hospital-wide pilot department from January 2019 to June 2020 were enrolled.The SMART-C stress management training was implemented,and stressors and psychosomatic responses were compared before and after the intervention.RESULTS Before mental health nursing participated in group training,the psychological stressor assessment results showed that“task”stress was significantly higher than“responsibility”stress,and“interpersonal”stress was close to the domestic norm.After 8 weeks of SMART-C stress management training,the“responsibility”and“task”stress decreased significantly.Before the implementation of the plan,stomachaches,headaches,and sleep issues were the three most severe physical symptoms troubling mental health nurses.The self-rated psychological stress and somatic symptom scores among the mental health nurses decreased significantly following SMART-C training.CONCLUSION SMART-C stress management training can improve stress-adjustment ability and reduce adverse somatic reactions in mental health nurses. 展开更多

BACKGROUND:This study aims to evaluate the immediate and 12-month effects of communitybased first-aid training on public knowledge and attitude,assess satisfaction,and identify factors associated with score changes.METHODS:This was a prospective study.In 2022-2023,a total of 2,010 community residents in Hainan Province received first-aid training and completed structured questionnaires at baseline,immediately after training,and at 3,6,and 12 months after training.First-aid knowledge was assessed through 33 items,with a maximum total score of 33.First-aid attitude was evaluated using seven items,totaling a maximum score of 21.Satisfaction was measured on a 5-point Likert scale.Paired-sample t-tests were used to compare baseline and post-training scores,repeated-measures analysis of variance(ANOVA)was used to examine time effects,and multiple linear regression was used to analyze factors associated with score changes.RESULTS:The satisfaction score with the training was high(mean score>4.3 for all items).The first-aid knowledge and attitude scores increased significantly after training(first-aid knowledge at baseline,14.90±7.63;immediately after training,20.70±5.72,P<0.001;and attitude at baseline,17.52±2.29;immediately after training,17.89±1.54,P<0.001).At 12 months after training,knowledge scores declined slightly compared with those immediately after training but remained above baseline(time effect P<0.001),whereas attitude scores remained stable(time effect P<0.001).Knowledge improvement was greater among middle-income participants and less among participants with lower levels of education,those in professional occupations,widowed individuals,or those who had previously received first-aid training.Attitude improvements were more pronounced among male participants,younger participants,and those in the agriculture or sales/service sectors.CONCLUSION:Community-based first-aid training improved public first-aid knowledge and attitude and was well received by participants.While knowledge levels declined somewhat over time,attitude remained relatively stable,highlighting the importance of continuous reinforcement training.Personalized reinforcement strategies may be particularly beneficial for individuals with lower levels of education,specific professional backgrounds,widowhood,or prior training experience to further enhance training effectiveness. 展开更多

Adult-born oligodendrocytes are continuously produced in the brains of rodents.The functional role of these cells has been linked to the motor-related activities of healthy animals and is vital for acquiring new motor skills.However,the relationship between these cells and the control of motor-related activities has not been investigated in pathological conditions.Therefore,the aim of this study is to investigate the role of oligodendrocytes in depression-related motor deficits and the effects of training.Psychomotor retardation is a key symptom of depression.Consistent with the impairments observed in rodent motor performance,the proliferation and activation of adult-born oligodendrocytes are altered in a corticosterone-induced stress paradigm.Therapeutic rotarod training can alleviate these symptoms by reversing the aforementioned changes.Notably,these alterations are particularly pronounced in layer I of the motor cortex.Thus,this study provides evidence of the potential functional involvement of adult-born oligodendrocytes in the motor impairments observed in the depressed animals.Additionally,it offers preliminary results for further investigation into layer I of the motor cortex in relation to these pathological conditions. 展开更多

Wideband extremely large-scale multipleinput multiple-output(XL-MIMO)is a promising technology for sixth-generation(6G)communications due to its high spectral efficiency and data rates.However,as the number of antennas increase in XLMIMO,users are more likely to be in the near-field region,where beam training becomes more complex.Additionally,the expanding bandwidth introduces beam split and grating lobe effects.In this paper,we analyze the conditions for grating lobe generation,demonstrating the relationship between the grating lobe angle and bandwidth.As the bandwidth increases,a large portion of the visible angle range will produce grating lobes.The analysis shows that the grating lobe location always lies on the distance ring of the main lobe,and the grating lobe distance can be uniquely determined by the grating lobe angle on a single distance ring.Then we propose a wideband grating lobe beam training method based on controllable grating lobe generation mechanism,which applies to both the angle and distance dimensions to cover the user range.Simulation results show that this method achieves comparable rate performance with low overhead. 展开更多

Objective Cerebral venous outflow disorders(CVOD)can impair cerebral perfusion and produce diverse,often debilitating symptoms,substantially reducing quality of life.Intermittent hypoxiahyperoxia training(IHHT)has demonstrated therapeutic potential across various pathologies and may represent a promising non-pharmacological approach for CVOD management.Methods Patients with imaging-confirmed CVOD underwent 14 IHHT sessions,each comprising four cycles of 10-minute hypoxia(11%O2)stimulation and 20-minute hyperoxia(38%O2).Physiological parameters and adverse events were monitored throughout the intervention.Clinical scales,24-hour ambulatory blood pressure,blood tests,jugular ultrasound,and perfusion imaging were assessed preand post-intervention.Results No participants experienced intolerable discomfort or severe adverse events;vital signs remained within normal ranges.No significant changes were observed in 24-hour blood pressure,blood cell counts,lipid profiles,or other blood markers.Notably,60%of patients(n=12)reported overall symptom improvement on the Patient Global Impression of Change scale.Headache severity,as measured by the visual analogue scale,significantly decreased(6.33±1.22 vs.4.89±2.03,P=0.016).In patients with internal jugular vein(IJV)stenosis,significant improvements were observed in regional cerebral blood flow(including the insula,occipital lobe,internal capsule,and lenticula)and left J3-segment IJV flow volume(107.27[47.50,160.00]vs.140.83[55.00,210.00]mL/min,P=0.011).Conclusion The current IHHT protocol is safe and well-tolerated in patients with CVOD.IHHT may alleviate CVOD-related symptoms by improving oxygen saturation,cerebral perfusion,and venous outflow pattern,supporting its potential as a non-invasive therapeutic strategy. 展开更多

Objectives This study aimed to identify barriers and facilitators influencing the implementation of a leadership training program based on the Developing and Sustaining Nursing Leadership Best Practice Guideline for nurse managers.Methods The descriptive qualitative study was conducted through semi-structured individual interviews at a tertiary hospital in Beijing,China.We selected 16 nurse managers who had participated in a leadership training program using a purposive sampling method with a maximum variation strategy from March to August 2024.The collected data were analyzed using thematic analysis.Results This study identified 7 barriers and 17 facilitators guided by the Consolidated Framework for Implementation Research(CFIR).Barriers were identified as:intervention characteristics(complexity of content,excessive content,challenging assignments after training,gap between theory and practice,and time-consuming),and inner setting(unclear training goals,mentor incompetence).Key facilitators emerged from:intervention characteristics(internally developed content,Best Practice Guideline with high level of evidence,focusing on leadership competency for manager,logically connected design),outer setting(meeting participants’needs,networking with leadership authority,peer pressure),inner setting(effective communication,supported teamwork,tension for change,fitting the participants’value),characteristics of individuals(intrinsic motivation,self-efficacy,beginning or mid-stage of change),and process(planning developed in advance,full motivation,feedback and team debriefing)domains.Conclusions This study identified drivers of implementation for a leadership training program in the Chinese context.These findings support the successful delivery of this program and can inform future leadership training and support improvements in nurse managers’leadership. 展开更多

Adversarial training(AT)is widely regarded as a crucial defense method for deep neural networks against adversarial attacks.Most of the existing AT methods suffer from the problems of insufficient coverage of perturbation space and robust overfitting.In view of this,we propose an AT framework with adaptive example reuse(AT-AER)to help improve the adversarial robustness of deep models.In AT-AER,a new concept named 2nd-order adversarial example(AE)is proposed by adaptively filtering AEs generated during the historical training phase,which achieves sufficient coverage of diverse attack directions.Meanwhile,by analysing the fundamental causes of robust overfitting,we propose the strategies of wave descending learning rate(WDLR),cosine increasing weight decay(CIWD)and cosine increasing attack strength(CIAS)in collaboration with AT-AER to optimise models.In addition,the Stochastic Weight Averaging(SWA)technique is introduced to further improve the stability of training.Finally,experiments on three benchmark datasets show that AT-AER exhibits significant advantages in the face of strong adversarial attacks.Its adaptive mechanism effectively alleviates the phenomenon of robust overfitting where the performance difference between the best model and the last model is less than 1%.The study further reveals that using traditional weak attacks(e.g.,FGSM)to evaluate the robustness of models may lead to a false sense of reliability,indicating the necessity of using strong attacks for robustness evaluation.This study provides a solution for AT that balances efficiency and performance. 展开更多

Heart failure is a major global health problem resulting in reduced exercise tolerance,functional capacity,and health-related quality of life(HRQoL).In addition to central cardiac dysfunction,peripheral skeletal muscle abnormalities,particularly involving the knee extensors,substantially contribute to physical limitation.This narrative review synthesizes current evidence regarding the effects of isolated knee extension resistance training(KERT)on muscle strength,exercise tolerance,and HRQoL in patients with chronic heart failure.A comprehensive but non-systematic search of PubMed,Scopus,and Google Scholar was conducted to identify English-language clinical studies evaluating isolated or predominantly knee extensor-based resistance training with relevant functional outcomes.Evidence indicates that isolated KERT is a safe and clinically meaningful intervention that consistently improves muscle strength and submaximal functional capacity,primarily through peripheral adaptations such as enhanced metabolic efficiency and reduced symptom burden during daily activities.However,substantial heterogeneity in training protocols may partly explain variability in reported outcomes.Changes in peak oxygen uptake are inconsistent,reflecting the specificity of local muscular adaptations.Improvements in HRQoL,particularly in physical functioning domains,are frequently reported.Overall,isolated KERT represents a valuable complementary component of cardiac reha-bilitation programs,although further research is needed to optimize protocols and determine long-term effects. 展开更多

Background Regular physical training induces adaptive effects across multiple organ systems,highlighting the existence of inter-organ communication networks.However,the molecular mechanisms underlying both exercise-induced adaptations and organ-to-organ signaling are not fully characterized.Circulating extracellular vesicles(EVs),including exosomes,carry molecules like microRNAs(miRNAs)that may mediate tissue crosstalk.This study aimed to identify specific exercise training-responsive miRNAs that affect skeletal muscle function.Methods miRNA expression profiles of serum-derived EVs were analyzed in healthy young individuals before and after 3 weeks endurance exercise training.Exercise training-responsive miRNAs were then validated for a functional role in cellular metabolic processes in human myotubes.Results We identified several exercise training-responsive miRNAs within exosome-rich EVs in serum,including miR-136-3p.In human myotubes,miR-136-3p enhanced glucose uptake and targeted the nardilysin convertase(NRDC)gene.Transfection of miR-136-3p or silencing of NRDC induced a shift towards glycolytic metabolism in mitochondria and modulated gene expressions related to myogenesis.Pancreatic islets were identified as a potential source of miR-136-3p based on in silico analysis of gene expression and a molecular analysis of conditioned media from isolated pancreatic islets.Conclusion MiR-136-3p is an endurance training-responsive molecular transducer that modulates glucose metabolism and cellular proliferation in myocytes.Associated with EVs,extracellular miR-136-3p may serve as a molecular messenger to communicate islet–skeletal muscle crosstalk after exercise.Extracellular miR-136-3p may serve as a molecular messenger to communicate islet–skeletal muscle crosstalk.Our results highlight a miRNA-mediated mechanism that participates in inter-organ communication to fine tune the metabolic adaptations to exercise. 展开更多

Background This study compared knee osteoarthritis(OA)outcomes specific to pain,physical function,and quality of life in later life based on strength training(ST)participation over a lifetime.Methods Participants from the Osteoarthritis Initiative(n=3192)were grouped by ST engagement during ages 12–18 years,19–34 years,35–49 years,and 50+years.Participants were categorized as:No ST(no ST at any point;61.7±9.0 years(mean±SD)),Some ST(engaged in ST during 1–3 life stages;58.9±8.7 years),and Lifelong ST(consistently engaged in ST across all life stages;55.6±8.1 years).Measures were collected at baseline and Year 4:Western Ontario and McMaster Universities Osteoarthritis Index Scores(WOMAC;pain,daily activities),Knee Injury and Osteoarthritis Outcome Score(KOOS;sports,recreation),Physical Activity Score for the Elderly(PASE),Short Form-12 Physical Component Score(SF-12 PCS),mobility disability,chair rise time,and walking speed(20 m and 400 m).Results At Year 4,the Lifelong ST group reported better WOMAC activity scores in the right knee along with better WOMAC pain,KOOS sportsecreation,and PASE scores compared to other groups(p<0.05).The Lifelong ST group had the lowest incidence of mobility disability of all groups(0.8%vs.2.3%–4.1%;p=0.015)and maintained the fastest walking speeds in Year 4.Conclusion For those with knee OA,ST throughout life may help preserve function and mobility,allowing for greater physical activity engagement while keeping pain levels relatively lower. 展开更多

Background:Zone II flexor tendon repair-the slick,multistranded core sutures,the delicate balance between gapping and tightness-is precisely the kind of procedure that makes hand surgery rotations a proving ground for junior residents,whether they come from orthopedics,plastics,or the emergency room.However,traditional bedside apprenticeship has never really solved the exposure problem.Supervising faculty vary widely in their preferred techniques,and the trainee rarely gets to make-and learn from-catastrophic mistakes before the real tissue is already under the needle.This bottleneck has become particularly obstructive in our own tiered residency cohorts,where the gap between simulator-naive and simulator-trained performance kept widening.So instead of patching the old curriculum with sporadic dry-lab sessions,we decided to flip the sequence:put the Touch Surgery module right at the front end of the block,used it as a deliberate cognitive warm-up,and then immediately reinforced that virtual rehearsal with hands-on work on cadaveric specimens and silicone models.Using a randomized controlled design,we compared our hybrid pipeline against the conventional teaching stream.The simulation-first group demonstrated fewer technical deviations on objective checklists,and their attending physicians also consistently noted less trembling and hesitation during the first live-tissue pass.Methods:Fifty surgical residents(34 males,16 females)from the surgical standardized training base of the First Affiliated Hospital of the University of South China were selected from December 2024 to June 2025.Using a random number table method,they were randomly divided into an experimental group(n=25)and a control group(n=25),receiving virtual-assisted and entity-integrated teaching and traditional teaching methods,respectively.After practical exercises,both groups underwent theoretical and operational assessments,and a multidimensional evaluation was conducted,with the results of the two groups compared and analyzed.Results:After training,the experimental group showed varying degrees of improvement in supervising expert scores,peer teacher scores,instructor scores,resident scores,SEEQ teaching satisfaction scores,theoretical knowledge mastery scores,operational skill level scores,and surgical guideline mastery scores(P<0.05).Conclusion:The virtual-assisted and entity-integrated training model achieved good training outcomes in the training of resident doctors in tendon suturing,significantly improving their theoretical level,operational skills,and teaching satisfaction. 展开更多

Objective:To explore the effects of tongue muscle training combined with respiratory training intervention on patients with dysphagia after stroke.Methods:A total of 70 patients with dysphagia after stroke admitted to our hospital from January 2025 to January 2026 were selected for the study and randomly divided into two groups.The control group(35 cases)underwent conventional rehabilitation training,while the observation group(35 cases)focused on tongue muscle training combined with respiratory training intervention.The intervention effects were compared between the two groups.Results:Before the intervention,there were no significant differences in swallowing function,respiratory function,and nutritional indicators between the two groups(p>0.05).After the intervention,the observation group showed greater improvements in tongue muscle strength and swallowing laryngoscopy scores,as well as lower dysphagia scores and better respiratory function,with significant differences compared to the control group(p<0.05).Conclusion:Tongue muscle training combined with respiratory training intervention can improve swallowing function and overall quality of life in patients with dysphagia after stroke,making it worthy of promotion. 展开更多
