BACKGROUND Laparoscopic cholecystectomy(LC)is widely accepted as the standard surgical treatment for gallbladder diseases due to its advantages:Reduced postoperative pain,shorter hospital stay,and faster recovery.Despite these benefits,port-siterelated complications remain a crucial clinical issue.The umbilicus is commonly used for initial access to the peritoneal cavity;however,its anatomical structure predisposes it to bacterial colonization,raising concerns regarding surgical site infection and incisional hernia,particularly when a transumbilical incision is used.Although transumbilical access offers shorter operative time and superior cosmetic outcomes,its safety remains debated as standard preoperative skin preparation may not adequately eliminate umbilical microflora.Evidence regarding the effectiveness of additional umbilical care in reducing complications associated with transumbilical access is limited.AIM To compare operative time,surgical site infection,length of hospital stay,postoperative cosmetic satisfaction,scar healing,and incisional hernia rates in LC patients receiving either transumbilical incision with additional umbilical care or a periumbilical incision with standard preoperative care.METHODS This prospective randomized controlled study was conducted at a tertiary-level university hospital.Patients aged≥18 years who were scheduled for elective LC and met the inclusion criteria were enrolled.In total,88 patients were assessed,and 80 patients who completed follow-up were randomized into two equal groups.In the intervention group,preoperative umbilical care with chlorhexidine gluconate was applied twice(once 6 hours before surgery and once immediately before skin preparation);trocar entry was performed via a transumbilical incision.In the control group,standard skin preparation was applied,and trocar entry was achieved through a periumbilical incision.The operative time,surgical site infection,the length of hospital stay,cosmetic satisfaction,scar healing,and incisional hernia were evaluated.Cosmetic satisfaction was assessed using a visual analog scale and scar quality using the patient and observer scar assessment scale.Patients were followed during hospitalization and reassessed at the first postoperative month.RESULTS Baseline demographic and clinical characteristics were similar between the two groups.No significant differences were observed in surgical site infection rates,the length of hospital stay,or incisional hernia occurrence between groups.Operative time was significantly shorter in the transumbilical incision group.Postoperative cosmetic satisfaction scores were significantly higher in patients who had a transumbilical incision.Patient-and observerreported scar assessment scores demonstrated significantly better scar healing outcomes in the transumbilical incision group.CONCLUSION Transumbilical access combined with appropriate preoperative umbilical care can be safely used in LC.This approach offers complication rates comparable to the periumbilical technique,with the advantages of shorter operative time,improved scar quality,and higher postoperative cosmetic satisfaction. 展开更多

BACKGROUND Umbilical venous catheterisation(UVC)is frequently used in neonates but may be complicated by malposition or migration.Extravasation is rare and can mimic other abdominal pathologies.CASE SUMMARY We report a preterm infant,born at 27 weeks’gestation,who developed bilateral pneumothorax and spontaneous intestinal perforation requiring drainage.On day 8,the infant clinically deteriorated with persistent high-volume clear peritoneal drain output and worsening dehydration despite fluid escalation.Abdominal radiography confirmed UVC migration into the liver with possible extravasation.Following catheter removal,peritoneal drainage reduced dramatically,and the infant improved rapidly.He was discharged at 38 weeks’corrected age and remained well with age-appropriate development.CONCLUSION UVC extravasation can masquerade as high-output peritoneal drainage in the setting of intestinal perforation,thereby delaying diagnosis.Persistent,unusually high peritoneal drainage should prompt consideration of catheter leakage.This rare presentation expands the spectrum of UVC-related complications described in the literature and emphasizes the importance of prompt recognition and management. 展开更多

Over the past decade,the human umbilical cord has been recognized as a significant and ethical source of mesenchymal stem cells(MSCs),particularly Wharton's jelly(WJ).WJ,as a connective tissue of the cord,possesses gelatinous uniformity and is being studied in regenerative medicine for certain modalities.This mucoid tissue provides a remarkable therapeutic potential through its extracellular matrix components,MSCs,and proteoglycans,while serving biochemical and protective functions during fetal development.This review highlights the advancements in WJ's biological structure and function,biomechanical characteristics,and regenerative capacities,with a special focus on WJ-acquired MSCs alongside their clinical translation.Current research reveals that WJ-MSCs may be used in clinical settings as a potential source due to their high proliferation capacity,differentiation capabilities,and an immunomodulatory profile,compared to adult-tissue-derived stem cells.These significant properties make this tissue a good candidate for cell-based interventions. 展开更多

Intrathecal administration of human umbilical cord mesenchymal stem cells may be a promising approach for the treatment of stroke,but its safety,effectiveness,and mechanism remain to be elucidated.In this study,good manufacturing practice-grade human umbilical cord mesenchymal stem cells(5×105 and 1×106 cells)and saline were administered by cerebellomedullary cistern injection 72 hours after stroke induced by middle cerebral artery occlusion in rats.The results showed(1)no significant difference in mortality or general conditions among the three groups.There was no abnormal differentiation or tumor formation in various organs of rats in any group.(2)Compared with saline-treated animals,those treated with human umbilical cord mesenchymal stem cells showed significant functional recovery and reduced infarct volume,with no significant differences between different human umbilical cord mesenchymal stem cell doses.(3)Human umbilical cord mesenchymal stem cells were found in the ischemic brain after 14 and 28 days of follow-up,and the number of positive cells significantly decreased over time.(4)Neuronal nuclei expression in the human umbilical cord mesenchymal stem cell group was greater than that in the saline group,while glial fibrillary acidic protein and ionized calcium binding adaptor molecule 1 expression levels decreased.(5)Human umbilical cord mesenchymal stem cell treatment increased the number of CD31+microvessels and doublecortin-positive cells after ischemic stroke.Human umbilical cord mesenchymal stem cells also upregulated the expression of CD31+/Ki67+.(6)At 14 days after intrathecal administration,brain-derived neurotrophic factor expression in the peri-infarct area and the concentrations of brain-derived neurotrophic factor in the cerebrospinal fluid in both human umbilical cord mesenchymal stem cell groups were significantly greater than those in the saline group and persisted until the 28th day.Taken together,these results indicate that the intrathecal administration of human umbilical cord mesenchymal stem cells via cerebellomedullary cistern injection is safe and effective for the treatment of ischemic stroke in rats.The mechanisms may include alleviating the local inflammatory response in the peri-infarct region,promoting neurogenesis and angiogenesis,and enhancing the production of neurotrophic factors. 展开更多

Cancer-related incomplete bowel obstruction presents a significant challenge in oncology,with conventional palliative treatments often proving insufficient in alleviating symptoms and controlling inflammation.In this issue,Wu et al report that bottle gourd moxibustion combined with umbilical therapy(BGM-UT)leads to notable improvement in gastrointestinal recovery,reduced opioid dependency,and lower key inflammatory cytokine levels.These findings suggest that BGM-UT may help interrupt the cycle of pain,opioid side effects,and bowel dysfunction,and could be associated with systemic anti-inflammatory effects.Beyond its clinical implications,this work highlights the possibility that evidence-supported traditional therapies may be integrated into modern oncology care.However,the retrospective,single-center design highlights the need for future prospective,multicenter trials and mechanistic studies.If validated in future studies,BGM-UT may emerge as a promising complementary treatment,offering a potentially less invasive and patient-centered approach that warrants further scientific evaluation. 展开更多

Diarrhea-predominant irritable bowel syndrome(IBS-D)is a prevalent functional gastrointestinal disorder characterized by chronic abdominal pain and loose stools,increasingly recognized as a condition driven by low-grade inflammation,epithelial barrier dysfunction,and disturbances of the gut microbiota rather than purely disordered motility.In the recent issue of World Journal of Stem Cells,Zhang et al report that human umbilical cord-derived mesenchymal stem cells(hUCMSCs)ameliorate IBS-D-like disease in rats by attenuating mucosal inflammation,restoring tight junction protein expression,and stabilizing the intestinal microbiome.This editorial critically appraises their work from a translational perspective,highlighting the novelty of targeting the gut-immune-microbiome axis with hUC-MSCs in a functional bowel disorder.We discuss potential mechanisms of action,including paracrine immunomodulation,epithelial repair,and microbiome-short chain fatty acid-immune cross-talk,and relate these to current concepts of IBS-D pathophysiology.Key limitations of the animal data,gaps in human evidence,and challenges for clinical translation-such as patient selection,optimal dosing and delivery,and regulatory demands for advanced therapy medicinal products-are addressed.Finally,we propose priorities for early-phase clinical trials that integrate mechanistic biomarkers with patient-centered outcomes to determine whether hUC-MSCs can transition from experimental therapy to a realistic,disease-modifying option for IBS-D. 展开更多

BACKGROUND Preeclampsia(PE)is a serious complication in pregnancy.It is one of the primary causes of maternal and perinatal mortality.Human amniotic epithelial cells(hAECs)and mesenchymal stem cells(MSCs)derived from human umbilical cord blood(hucbMSCs)are both perinatal stem cells,capable of expressing characteristic stem cell surface markers.MSC-derived exosomes(MSCs-exos)exhibit functional properties comparable to those of MSCs,while offering advantages such as greater biological stability and the ability to circumvent potential complications associated with MSC-based therapy.This study utilized hAEC-derived exosomes(hAECs-exos)and hucbMSC-derived exosomes(hucbMSCs-exos)to treat preeclamptic rats and investigate their therapeutic effects.AIM To investigate the therapeutic effects of hAECs-exos and hucbMSCs-exos on PE in rats.METHODS Thirty-two pregnant rats were divided into four groups(normal pregnancy,PE,and two exosome-treated groups;n=8).The PE model was induced by L-arginine methyl ester.From gestation day 12,the treatment groups received hAECs-exos or hucbMSCs-exos for 7 days,while controls received normal saline.Blood pressure,urinary protein,fetal/placental weight,and tissue analyses were performed.Quantitative data are expressed as the mean±SD.Differences among multiple groups were analyzed by one-way analysis of variance(ANOVA),followed by the LSD-t test for pairwise comparisons.A P value<0.05 was considered statistically significant.RESULTS Compared to the normal pregnancy group,rats in the PE group exhibited significantly elevated blood pressure and 24-hour urinary protein levels,indicating successful model establishment.Furthermore,the PE group showed significantly increased levels of interleukin-6,tumor necrosis factor-α,soluble fms-like tyrosine kinase-1,and malondialdehyde,along with decreased fetal/placental weight,levels of interleukin-10,placental growth factor,vascular endothelial growth factor,superoxide dismutase,and placental CD31 expression(P<0.05).Treatment with both exosomes significantly reversed all these alterations compared to the PE group(P<0.05).Histological analysis further confirmed that the treatments markedly alleviated renal and placental pathological damage induced by PE.CONCLUSION This study demonstrates that both hAECs-exos and hucbMSCs-exos have therapeutic effects in rats with PE,potentially through mechanisms involving the inhibition of oxidative stress and inflammatory responses. 展开更多

BACKGROUND Gastroesophageal reflux disease(GERD)is a widely occurring digestive disorder associated with persistent esophageal inflammation and compromised epithelial barrier integrity.Increasing evidence suggests that ion channels,particularly the epithelial sodium channel(ENaC),play a role in mucosal homeostasis and inflammatory regulation.Theδsubunit of ENaC(ENaCδ),which is expressed in human tissues but absent in conventional rodent models,may contribute to disease pathogenesis.However,its role in GERD remains unclear,and current treatments such as proton pump inhibitors are often insufficient in addressing inflammation and barrier dysfunction.Human umbilical cord mesenchymal stem cells(hUCMSCs)have emerged as a promising therapeutic strategy due to their anti-inflammatory and tissue-repair properties.AIM To investigate the role of human ENaCδin the development and progression of GERD using a humanized ENaCδ(hENaCδ)mouse model,and to evaluate the therapeutic potential of hUC-MSCs in alleviating esophageal inflammation and restoring epithelial barrier function.METHODS In the present study,a total of 48 mice were randomly assigned to eight experimental groups to investigate the role of hENaCδin GERD and to evaluate the therapeutic potential of hUC-MSCs.A hENaCδmouse model was established using CRISPR/Cas9 technology,followed by esophageal acid perfusion to induce GERD.RESULTS hENaCδexacerbated inflammatory infiltration in the submucosa of the esophagus in GERD mice,promoted the expression of interleukin(IL)-1,IL-1β,IL-17 and inducible nitric oxide synthase in mice(P<0.001),upregulated the relative expression and phosphorylation levels of phosphoinositide 3-kinase(PI3K),protein kinase B(AKT),and mammalian target of rapamycin(mTOR)(P<0.05),and downregulated the expression of myosin phosphatase target subunit 1(MYPT1)and claudin-1(P<0.001).hUC-MSCs could alleviate the pathological changes of GERD mice with hENaCδ,inhibit the expression of IL-1,IL-1β,IL-17 and inducible nitric oxide synthase in mice(P<0.001),downregulate the expression of PI3K,AKT and mTOR in esophageal tissue(P<0.001),and upregulate the expression of MYPT1 and claudin-1(P<0.001).Although both hUC-MSCs and proton pump inhibitor treatments could effectively alleviate the inflammatory response of GERD,hUC-MSCs had a more significant effect in inhibiting IL-17 and restoring the expression of MYPT1 and claudin-1.CONCLUSION hENaCδexacerbates GERD-related inflammation and barrier injury in mice by activating the PI3K/AKT/mTOR pathway and downregulating MYPT1 and claudin-1,while hUC-MSCs treatment can effectively block this pathway and reduce inflammation and barrier injury. 展开更多

Endometrial disorders,notably intrauterine adhesions and thin endometrium,are major causes of female infertility and are often poorly managed by conventional therapies.Human umbilical cord-derived mesenchymal stem cells and their exosomes have emerged as promising regenerative alternatives,leveraging their immunomodulatory,anti-fibrotic,and pro-angiogenic properties.This review provides a critical synthesis of evidence from 125 studies(2007-2025),focusing not only on therapeutic mechanisms and clinical progress but also on the specific hurdles that impede clinical translation.Their mechanistic actions converge on key repair processes,including immune modulation,inhibition of transforming growth factor-β/Smad-mediated fibrosis,and promotion of vascularization.Early phase clinical trials have reported improved endometrial thickness and pregnancy rates,including live births in 30.8%-38.5%of severe intrauterine adhesion cases,with initially favorable safety profiles.The efficacy is further augmented by combining it with biomaterial carriers or estrogen.However,clinical translation is significantly constrained by inherent challenges,including small-scale,heterogeneous trials with short follow-ups;manufacturing complexities due to donor variability and a critical lack of potency assays linking cell phenotype to in vivo efficacy;and unresolved safety concerns,particularly regarding long-term and intergenerational risks,ectopic tissue formation,and context-dependent protumorigenic potential in conditions such as endometriosis and cancer.Therefore,future advancements hinge on addressing these interconnected fronts:Conducting mechanism-informed,large-scale clinical trials,establishing manufacturing standards integrated with biologically relevant potency assays,and resolving context-dependent efficacy and safety paradoxes. 展开更多

Human umbilical cord mesenchymal stem cells(hUC-MSCs)have emerged as highly promising seed cells in regenerative medicine due to advantages such as wide availability,noninvasive collection,strong proliferative capacity,and low immunogenicity.However,the inevitable cellular senescence and loss of stemness during in vitro expansion critically compromise their clinical efficacy.Recent research has shifted focus to the supportive microenvironment.This review elaborates on an emerging strategy,the engineered microenvironment,which aims to effectively maintain and enhance hUC-MSCs stemness by mimicking and manipulating the physical,chemical,and biological signals of the stem cell niche.The article details the key roles and molecular mechanisms of physical factors(e.g.,hypoxia,mechanical stimulation,and three-dimensional culture),chemical signals(small molecules and cytokines),and biomaterial scaffolds in regulating hUC-MSCs stemness.It further explores the application potential and enhanced therapeutic efficacy of this strategy in disease models such as bone repair,nerve regeneration,and endometrial repair.Finally,the review outlines challenges,including standardization and scalability.We propose future directions integrating cutting-edge technologies such as artificial intelligence,multi-omics analysis,and smart responsive materials.The aim is to provide new perspectives and a theoretical basis for developing more efficient hUC-MSCs clinical application strategies. 展开更多

Mesenchymal stem cell-based therapies have emerged as promising candidates for the treatment of steroid-induced avascular necrosis of the femoral head(SANFH).A study by Lv et al published in the recent issue of the World Journal of Stem Cells,investigated a novel strategy aimed at enhancing the therapeutic potential of human umbilical cord mesenchymal stem cells(hUCMSCs)for SANFH through the genetic silencing of sclerostin(SOST),a key inhibitor of bone formation.In a murine model of SANFH,transplantation of SOST-silenced hUCMSCs yielded superior structural and histological outcomes compared with unmodified hUCMSCs.Micro-computed tomography confirmed marked restoration of the femoral head microarchitecture,including increased bone volume,trabecular number,and trabecular thickness.Histological examination revealed reduced adipocyte infiltration and fewer empty lacunae following SOST-silenced hUCMSCs treatment.Mechanistically,these improvements were associated with a shift in bone metabolism toward osteogenesis,reflected by an elevated serum osteoprotegerineceptor activator of nuclear factor-kappa B ligand ratio,activation of the Wnt/β-catenin pathway,and downregulation of adipogenic markers(peroxisome proliferator-activated receptor gamma and CCAAT/enhancerbinding protein)in bone tissue.Collectively,these findings demonstrate that SOST knockdown corrects the osteogenic-adipogenic imbalance by modulating the Wnt/β-catenin pathway,thereby significantly augmenting the therapeutic efficacy of hUCMSCs in SANFH.However,this approach warrants further translational studies. 展开更多

Stem cell treatment may enhance erectile dysfunction(ED)in individuals with cavernous nerve injury(CNI).Nevertheless,no investigations have directly ascertained the implications of varying amounts of human umbilical cord-derived mesenchymal stem cells(HUC-MSCs)on ED.We compare the efficacy of three various doses of HUC-MSCs as a therapeutic strategy for ED.Sprague–Dawley rats(total=175)were randomly allocated into five groups.A total of 35 rats underwent sham surgery and 140 rats endured bilateral CNI and were treated with vehicles or doses of HUC-MSCs(1×106 cells,5×106 cells,and 1×107 cells in 0.1 ml,respectively).Penile tissues were harvested for histological analysis on 1 day,3 days,7 days,14 days,28 days,60 days,and 90 days postsurgery.It was found that varying dosages of HUC-MSCs enhanced the erectile function of rats with bilateral CNI and ED.Moreover,there was no significant disparity in the effectiveness of various dosages of HUC-MSCs.However,the expression of endothelial markers(rat endothelial cell antigen-1[RECA-1]and endothelial nitric oxide synthase[eNOS]),smooth muscle markers(alpha smooth muscle actin[α-SMA]and desmin),and neural markers(neurofilament[RECA-1]and neurogenic nitric oxide synthase[nNOS])increased significantly with prolonged treatment time.Masson’s staining demonstrated an increased in the smooth muscle cell(SMC)/collagen ratio.Significant changes were detected in the microstructures of various types of cells.In vivo imaging system(IVIS)analysis showed that at the 1st day,the HUC-MSCs implanted moved to the site of damage.Additionally,the oxidative stress levels were dramatically reduced in the penises of rats administered with HUC-MSCs. 展开更多

BACKGROUND Diabetes mellitus(DM)and metabolic-associated fatty liver disease(MAFLD)are common metabolic disorders,and their coexistence can exacerbate the progression of either disease.Human umbilical cord mesenchymal stem cell(hUCMSC)therapy has shown promising potential in the treatment of several metabolic diseases.AIM To investigate how hUC-MSCs affect liver metabolism in diabetic rats with MAFLD and assess their therapeutic potential and underlying mechanisms.METHODS A streptozotocin-induced rat model of DM with MAFLD was established,and hUC-MSCs were administered via tail vein injection.Changes in body weight,fasting blood glucose(FBG),and serum triglyceride(TG),alanine aminotransferase,aspartate aminotransferase levels,and pathological changes of liver were evaluated.Receiver operating characteristic analysis was used to assess the diagnostic value of differential metabolites and their ability to predict the therapeutic effects of hUC-MSCs.Spearman correlation was employed to analyze the relationships between liver metabolites and key biochemical markers.RESULTS hUC-MSC treatment significantly reduced FBG and TG levels in diabetic rats with MAFLD and improved histological steatosis and injury in the liver.Metabolomic analysis indicated that hUC-MSCs significantly ameliorated liver metabolic disturbances via their regulatory effect on several key metabolic pathways related to carbohydrate,amino acid,and lipid metabolism.Receiver operating characteristic curve analysis revealed that 70 differential metabolites had good diagnostic value for DM with MAFLD and could effectively predict the therapeutic effect of hUC-MSCs.Moreover,Spearman correlation analysis confirmed that significant correlations existed between differential liver metabolites and the concentrations of biochemical markers(FBG,TG,alanine aminotransferase,aspartate aminotransferase).CONCLUSION hUC-MSCs alleviate liver metabolic disturbances in diabetic rats with MAFLD,thereby mitigating the pathological state of DM and slowing the progression of MAFLD. 展开更多

BACKGROUND There is currently no effective treatment for osteoarthritis(OA),which is the most common joint disorder leading to disability.Although human umbilical cord mesenchymal stem cells(hUC-MSCs)are promising OA treatments,their use is limited by the condition itself,and understanding of the underlying mechanisms of OA is lacking.AIM To explore the specific molecular mechanism by which hUC-MSC-derived exosomal miR-199a-3p improves OA.METHODS Sodium iodoacetate was injected into rat articulations to construct an animal model of OA.Interleukin(IL)-1βwas used to induce human chondrocytes(CHON-001)to construct an OA chondrocyte model.Exosomes in hUC-MSCs were isolated using Ribo™Exosome Isolation Reagent.Real-time reverse transcriptase-polymerase chain reaction and western blotting were used to detect the expression of related genes and proteins,and damage to CHON-001 cells and rat articular cartilage tissue was evaluated by enzyme-linked immunosorbent assay,terminal deoxynucleotidyl transferase-mediated deoxyuridine tripho-sphate-nick end labelling staining and hematoxylin and eosin staining.RESULTS hUC-MSC-derived exosomes(hUC-MSC-Exos)inhibited the expression of IL-1β-induced inflammatory cytokines,namely,IL-6,IL-8 and tumor necrosis factor-α.hUC-MSC-Exos also improved the viability but inhibited the apoptosis of CHON-001 cells,improved the pathological condition of articular cartilage tissue and alleviated the development of OA in vivo.Mechanistically,hUC-MSC-Exos downregulated the expression of mitogen-activated protein kinase 4 by delivering miR-199a-3p,thereby inhibiting the activation of the nuclear factor-kappaB signaling pathway,alleviating IL-1β-induced chondrocyte inflammation and apoptosis,and ultimately improving the development of OA.CONCLUSION hUC-MSC-derived exosomal miR-199a-3p alleviates OA by inhibiting the mitogen-activated protein kinase 4uclear factor-kappaB signaling pathway.The present findings suggest that miR-199a-3p delivery by hUC-MSCExos may be a novel strategy for the treatment of OA. 展开更多

Objective This study aimed to evaluate the potential of a collagen-coated,3D-printed tracheal scaffold(3D-TechTra)integrated with human umbilical cord mesenchymal stem cells(hUC-MSCs)for tracheal tissue regeneration.Methods The thermoplastic polyurethane/polylactic acid(TPU/PLA)scaffold was engineered to optimize mechanical properties and biocompatibility,with the goal of mimicking the structural and tensile characteristics of native tracheal tissue.Subsequently,preclinical experiments were conducted using rabbit models:the performance of the collagen-coated TPU/PLA scaffold with hUC-MSCs was compared with that of uncoated scaffolds and collagen-only scaffolds.In vitro tests were also performed to assess the adhesion,proliferation,and differentiation of hUC-MSCs on the scaffold.For in vivo evaluation,multiple analytical methods were employed,including immunohistological analysis(to detect glycosaminoglycan deposition and extracellular matrix remodeling),radiographic and endoscopic evaluations(to assess tracheal contour and airway obstruction),and survival analysis(to monitor animal outcomes and systemic toxicity).Results In vitro,hUC-MSCs successfully adhered to and proliferated on the TPU/PLA scaffold,and differentiated into adipogenic,osteogenic,and chondrogenic lineages,which supported the potential for tissue-specific regeneration;in vivo,compared with uncoated or collagen-only scaffolds,the collagen-coated TPU/PLA scaffold integrated with hUC-MSCs exhibited enhanced integration with host tissues,superior biocompatibility,and reduced tracheal stenosis,while also preserving airway patency,alleviating inflammation,and facilitating epithelial regeneration,smooth muscle formation,and vascularization.Immunohistological analysis further revealed significant glycosaminoglycan deposition and extracellular matrix remodeling in the hUC-MSC-treated group,and radiographic and endoscopic evaluations confirmed preserved tracheal contour and reduced airway obstruction;additionally,survival analysis showed significantly improved outcomes in animals treated with the collagen-coated TPU/PLA scaffold containing hUC-MSCs,with no systemic toxicity observed.Conclusions This study demonstrated the synergistic potential of TPU/PLA scaffolds,collagen coatings,and hUC-MSCs,providing valuable evidence for advancing the application of these components in tracheal tissue engineering. 展开更多

Buckling failure in submarine cables presents a prevalent challenge in ocean engineering.This work aims to explore the buckling behavior of umbilical cables with damaged sheaths subjected to compression and bending cyclic loads.A finite element model is devised,incorporating a singular armor wire,a rigid core,and a damaged sheath.To scrutinize the buckling progression and corresponding deformation,axial compression and bending cyclic loads are introduced.The observations reveal that a reduction in axial compression results in a larger number of cycles before buckling ensues and progressively shifts the buckling position toward the extrados and fixed end.Decreasing the bending radius precipitates a reduction in the buckling cycle number and minimizes the deformation in the armor wire.Furthermore,an empirical model is presented to predict the occurrence of birdcage buckling,providing a means to anticipate buckling events and to estimate the requisite number of cycles leading to buckling. 展开更多

BACKGROUND Current drugs primarily target inflammation control but do not reverse tissue remodeling changes for asthma.Human mesenchymal stem cells are known for their anti-inflammatory and tissue remodeling capabilities.However,limited research has explored the therapeutic impact of varying doses and frequencies of human umbilical cord blood-derived mesenchymal stem cells(HUC-MSCs)on established airway remodeling in experimental asthma.AIM To explore and optimize the dosage and administration frequency of HUC-MSCs in experimental models of ovalbumin(OVA)-induced asthma.METHODS BALB/c mice underwent sensitization and were challenged using OVA.Control animals were administered a saline solution following the same protocol.HUC-MSCs were identified using flow cytometry.HUC-MSCs at incremental dosages(1×105,2×105,4×105)were injected via tail veins on day 30(the second after the final stimulation).After comparing each group and determining the optimal dose,supplement the optimal dose twice on day 30 and day 33(the second and fifth day after the final stimulation).Bronchoalveolar lavage fluid(BALF)and serum were harvested for analysis of concentrations of interleukin-4(IL-4),IL-13,immunoglobulin E and interferon-gamma(IFN-γ)by enzyme-linked immunosorbent assay.Pharmacology of airways and lung functions were also evaluated to identify the optimal group.RESULTS The study shows that HUC-MSC transplantation ameliorates OVA-induced asthma by significantly reducing airway inflammation and obstruction in preclinical models.This effect is associated with decreased Th2 cytokines IL-4 and IL-13,and increased Th1 cytokine IFN-γ.The optimal dose of 2×105 cells/mouse was identified as the most effective in reducing local asthmatic airway inflammation and changing levels of IL-4,IL-13,and IFN-γin serum and BALF compared to other single doses of HUC-MSC.Multiple treatments with the medium dose(2×105 cells)of HUC-MSCs on days 30 and 33 yield the best pathological and lung function outcomes.However,double treatments do not reduce IL-4 and IL-13 expression or enhance IFN-γproduction in serum or BALF more effectively than a single medium dose.CONCLUSION HUC-MSCs effectively regulate pro-inflammatory mediators in serum and BALF,modulating airway remodeling and lung function.In this acute mouse asthma model,a single dosage of 2×105 is optimal,with more significant effects of decreasing airway obstruction requiring repeated administration. 展开更多

BACKGROUND Inflammatory bowel disease(IBD)is a chronic,progressive inflammatory condition of the intestine.Mesenchymal stem cell(MSC)therapy for IBD has made significant progress in recent years.To better exploit the therapeutic potential of MSCs,pretreatment strategies are employed to enhance their therapeutic capabilities.As a compound with diverse pharmacological effects,quercetin(QUR)is applied to pretreat human umbilical cord-derived MSCs(hUCMSCs)in this study,thereby augmenting their immunotherapeutic potential.AIM To evaluate the therapeutic efficacy of QUR-pretreated hUCMSCs.METHODS We induced colitis in a mouse model using a 2,4,6-trinitrobenzenesulfonic acid solution.Intraperitoneal injection of QUR-pretreated hUCMSCs significantly improved clinical and pathological manifestations of colitis compared to the model group.Interestingly,the therapeutic effect was superior to that of untreated hUCMSCs.Mice exhibited significantly reduced weight loss,diminished infiltration of inflammatory cells observed in hematoxylin and eosin staining,improved Disease Activity Index and Histological Activity Index scores.Furthermore,colonic tissue analysis revealed a significant upregulation of the anti-inflammatory cytokine interleukin 10(IL-10),accompanied by a downregulation of the pro-inflammatory cytokine IL-6.Further tests also suggested that QUR pretreatment led to inhibition of Janus kinase/signal transducer and activator of transcription(STAT)phosphorylation.RESULTS Our study demonstrated that QUR pretreatment of hUCMSCs significantly enhanced their immune-regulatory capacity.This approach effectively mitigated colonic inflammation in a mouse colitis model by modulating the IL-10/Janus kinase/STAT signaling pathway.CONCLUSION These findings suggest that QUR pretreatment acts synergistically to augment the inherent anti-inflammatory and immune-regulatory properties of hUCMSCs,resulting in enhanced therapeutic efficacy for IBD treatment. 展开更多

Pulmonary fibrosis,a chronic,fatal lung disease affecting millions worldwide,urgently needs more effective treatments.This article comments on the study by Wang et al,which proposed that human umbilical cord mesenchymal stromal cellderived exosomes alleviate rats radiation induced pulmonary fibrosis.The study demonstrated that these exosomes suppressed inflammation,extracellular matrix deposition,and epithelial-mesenchymal transition by inhibition of AKT signaling in radiation-exposed alveolar epithelial cells.Despite these observations,aspects of the study merit further discussion.Most importantly,further confirmation is needed to prove that the therapeutic effect is exerted through the AKT signaling pathway.Moreover,the definitions of both mesenchymal stem cell and exosomes require further refinement,more rigorous terms should be mesenchymal stromal cell and extracellular vesicles.It seems apparent that this therapy will develop into one of great clinical value. 展开更多
