BACKGROUND In patients with cirrhosis and ascites,fluid retention may artificially increase their body weight and body mass index(BMI),potentially masking a low BMI and leading to under recognition of malnutrition.AIM...BACKGROUND In patients with cirrhosis and ascites,fluid retention may artificially increase their body weight and body mass index(BMI),potentially masking a low BMI and leading to under recognition of malnutrition.AIM To investigate whether computed tomography(CT)-based correction of the body weight improves malnutrition detection in patients with cirrhosis and ascites.METHODS This retrospective study included 100 hospitalized patients with cirrhosis and ascites.The BMI was evaluated using three approaches:The actual body weight method,the CT-corrected method,and the CT-sensitive corrected method.Low BMI and Global Leadership Initiative on Malnutrition(GLIM)-defined malnutrition were assessed under the three methods,and differences in detection rates were compared.RESULTS Compared with the actual body weight method,both the CT-corrected method and the CT-sensitive corrected method resulted in lower BMI values(both P<0.001),with further reduction observed for the CT-sensitive corrected method(P=0.009).The detection rates of low BMI increased from 6.0%to 10.0%and 12.0%,respectively.The detection rates of GLIM-defined malnutrition increased from 19.0%to 22.0%and 24.0%,respectively.Reclassification was unidirectional,with patients shifting from a non-low BMI or non-malnourished status under the actual body weight method to a low BMI or malnutrition status under the CT-corrected or CT-sensitive corrected method.CONCLUSION CT-based body weight correction reduced BMI estimates and improved the detection of low BMI and GLIMdefined malnutrition in patients with cirrhosis and ascites.This approach may serve as a useful adjunct to nutritional assessment,particularly in patients with significant fluid retention.展开更多
BACKGROUND Hepatocellular carcinoma(HCC)incidence continues to rise,yet treatment options remain limited.Despite recent advances in early screening methods,most patients are still diagnosed at an advanced stage with p...BACKGROUND Hepatocellular carcinoma(HCC)incidence continues to rise,yet treatment options remain limited.Despite recent advances in early screening methods,most patients are still diagnosed at an advanced stage with poor prognosis.Therefore,accurate assessment of prognostic risk using molecular biomarkers is essential for the development of personalized treatment strategies.AIM To investigate the correlation between serum aldo-keto reductase family 1 member B10(AKR1B10)levels and clinicopathological characteristics and prognosis in patients with HCC,and to evaluate the potential clinical value of AKR-1B10 as a prognostic biomarker.METHODS A total of 102 patients with HCC admitted to the Affiliated Hospital of Chengde Medical University between May 2020 and May 2024 were enrolled as study subjects.Complete clinical data and survival follow-up information were collected.Based on the optimal cutoff value for serum AKR1B10 levels in diagnosing HCC(1584.97 pg/mL),determined using receiver operating characteristic curve analysis,patients were divided into a high-level group(≥1584.97 pg/mL,n=75)and a low-level group(<1584.97 pg/mL,n=27).AKR1B10 levels were measured using enzyme-linked immunosorbent assay.In 41 patients who underwent surgical resection,serum AKR1B10 levels were measured preoperatively and at 1-2 days and 3-4 days postoperatively.Theχ2 test or Fisher’s exact test was used to compare categorical data between the two groups.Survival analysis was performed using the Kaplan-Meier method,with intergroup comparisons performed using the log-rank test.Cox univariate and multivariate regression analyses were used to identify independent prognostic factors,and the Friedman test was used to compare serum AKR1B10 levels at different postoperative time points.RESULTS Significant differences were observed between the AKR1B10 high-level and low-level groups in tumor size(χ2=3.999,P<0.05),ascites(χ2=3.902,P<0.05),and Barcelona Clinic Liver Cancer stage(χ2=3.987,P<0.05).The median survival was 17.0 months in the high-level group,significantly shorter than that in the low-level group(32.0 months;χ2=6.172,P<0.05).Multivariate Cox regression analysis indicated that serum AKR1B10 level(hazard ratio=1.819,95%confidence interval:1.009-3.280,P<0.05)and ascites(hazard ratio=1.810,95%confidence interval:1.017-3.223,P<0.05)were independent factors influencing survival in patients with HCC.Additionally,serum AKR1B10 levels were significantly decreased at 1-2 days postoperatively in the 41 patients who underwent surgery,with a continued downward trend at 3-4 days postoperatively(χ2=82.000,P<0.01).CONCLUSION Serum AKR1B10 is an independent risk factor for prognosis in HCC,and high AKR1B10 levels are associated with unfavorable survival outcomes,indicating its potential value as a biomarker for prognostic risk assessment in patients with HCC.展开更多

BACKGROUND Neck pain,a primary symptom of cervical spondylosis,affects patients'physical and mental health,reducing their quality of life.Pain and emotional state interact;however,their longitudinal interrelations...BACKGROUND Neck pain,a primary symptom of cervical spondylosis,affects patients'physical and mental health,reducing their quality of life.Pain and emotional state interact;however,their longitudinal interrelationship remains unclear.In this study,we applied a dual-trajectory model to assess how neck pain and emotional state evolve together over time and how clinical interventions,particularly acupuncture,influence these trajectories.AIM To investigate the longitudinal relationship between neck pain and emotional state in patients with cervical spondylosis.METHODS This prospective cohort study included 472 patients with cervical spondylosis from eight Chinese hospitals.Participants received acupuncture or medication and were followed up at baseline,and at 1,2,4,6,and 8 weeks.Neck pain and emotional distress were assessed using the Northwick Park Neck Pain Questionnaire(NPQ)and the affective subscale of the Short-Form McGill Pain Questionnaire(SF-MPQ),respectively.Group-based trajectory models and dual trajectory analysis were used to identify and correlate pain-emotion trajectories.Multivariate logistic regression identified predictors of group membership.RESULTS Three trajectory groups were identified for NPQ and SF-MPQ scores(low,medium,and high).Higher NPQ trajectory was associated with older age(OR=1.058,P<0.001)and was significantly reduced by acupuncture(OR=0.382,P<0.001).Similarly,acupuncture lowered the odds of high SF-MPQ trajectory membership(OR=0.336,P<0.001),while age increased it(OR=1.037,P<0.001).Dual-trajectory analysis revealed bidirectional associations:69.1%of patients with low NPQ had low SF-MPQ scores,and 42.6%of patients with high SF-MPQ also had high NPQ scores.Gender was a predictor for medium SF-MPQ trajectory(OR=1.629,P=0.094).Occupation and education levels differed significantly across the trajectory groups(P<0.05).CONCLUSION Over time,neck pain and emotional distress are closely associated in patients with cervical spondylosis.Acupuncture alleviates both outcomes significantly,while age is a risk factor.Integrated approaches to pain and emotional management are encouraged.展开更多
BACKGROUND The increasing risk of cardiovascular disease(CVD)associated with worsening environmental exposure is a critical health concern garnering global research attention.AIM To systematically assess the scope and...BACKGROUND The increasing risk of cardiovascular disease(CVD)associated with worsening environmental exposure is a critical health concern garnering global research attention.AIM To systematically assess the scope and characteristics of research on the relationship between environmental exposure and CVD.METHODS A thorough examination of publications on the relationship between environmental exposure and CVD from 1999 to 2022 was carried out by extensively screening the literature using the Web of Science Core Collection.The language of the selected publications was standardized to English.Afterward,different academic tools such as CiteSpace,VOSviewer,HistCite,Python,Matplotlib,and Bibliometrix were utilized to examine the research trends in this field.RESULTS The study’s findings indicated a steady increase in scientific publications among the 1640 analyzed documents,peaking in 2022 with 197 publications.The United States emerged as the leading nation regarding high-quality publications and international collaboration.Harvard University was identified as the most prolific institution.“Environmental research”was the most frequently contributing journal,and Muenzel T was recognized as the top contributor.Current research hotspots are primarily concentrated on themes such as“cardiovascular disease”,“exposure”,“risk”,“mortality”,and“air pollution”.CONCLUSION This study highlights increasing research on the link between environmental exposure and CVD,identifying key exposures and diseases while emphasizing the need for further investigation into underlying mechanisms.展开更多
BACKGROUND Regenerating gene 4(REG4)has been proved to be carcinogenic in some cancers,but its manifestation and possible carcinogenic mechanisms in colorectal cancer(CRC)have not yet been elucidated.Our previous stud...BACKGROUND Regenerating gene 4(REG4)has been proved to be carcinogenic in some cancers,but its manifestation and possible carcinogenic mechanisms in colorectal cancer(CRC)have not yet been elucidated.Our previous study found that the drug resistance of CRC cells may be closely linked to their fat metabolism.AIM To explore the role of REG4 in CRC and its association with lipid droplet formation and chemoresistance.METHODS We conducted a meta-analysis and bioinformatics and pathological analyses of REG4 expression in CRC.The effects of REG4 on the phenotypes and related protein expression were also investigated in CRC cells.We detected the impacts of REG4 on the chemoresistance and lipid droplet formation in CRC cells.Finally,we analyzed how REG4 regulated the transcription and proteasomal degradation of lipogenic enzymes in CRC cells.RESULTS Compared to normal mucosa,REG4 mRNA expression was high in CRC(P<0.05)but protein expression was low.An inverse correlation existed between lymph node and distant metastases,tumor-node-metastasis staging or short overall survival and REG4 mRNA overexpression(P<0.05),but vice versa for REG4 protein expression.REG4-related genes included:Chemokine activity;taste receptors;protein-DNA and DNA packing complexes;nucleosomes and chromatin;generation of second messenger molecules;programmed cell death signals;epigenetic regulation and DNA methylation;transcription repression and activation by DNA binding;insulin signaling pathway;sugar metabolism and transfer;and neurotransmitter receptors(P<0.05).REG4 exposure or overexpression promoted proliferation,antiapoptosis,migration,and invasion of DLD-1 cells in an autocrine or paracrine manner by activating the epidermal growth factor receptor-phosphoinositide 3-kinase-Akt-nuclear factor-κB pathway.REG4 was involved in chemoresistance not through de novo lipogenesis,but lipid droplet assembly.REG4 inhibited the transcription of acetyl-CoA carboxylase 1(ACC1)and ATP-citrate lyase(ACLY)by disassociating the complex formation of anti-acetyl(AC)-acetyl-histone 3-AC-histone 4-inhibitor of growth protein-5-si histone deacetylase;-sterol-regulatory element binding protein 1 in their promoters and induced proteasomal degradation of ACC1 or ACLY.CONCLUSION REG4 may be involved in chemoresistance through lipid droplet assembly.REG4 reduces expression of de novo lipid synthesis key enzymes by inhibiting transcription and promoting ubiquitination-mediated proteasomal degradation.展开更多

METHODS: Fusarium solani was induced in the right eye of 30 New Zealand rabbits. Forty-eight hours after inoculation, the animals were divided into 3 different treatment groups, 10 rabbit eyes of each group: Group 1 (Natamycin) treated with topical Natamycin, group 2 (Natacyn) treated with topical Natacyn, group 3 (control) treated with topical saline solution. The eyes of each group was examined clinically with slit lamp using ulcer scoring system on day 4, 10, 15, and 21 for status of healing, corneal vascularisation, iritis, hypopyon and macular nebula. The findings were recorded on day 10 and day 21. RESULTS: Ulcer score on day 10, day 15, day 21: The score of Natamycin group are 1.45 +/- 0.16, 1.08 +/- 0.11, 0.70 +/- 0.40. The score of Natacyn group are 1.35 +/- 0.12, 1.10 +/- 0.12, 0.65 +/- 0.35. the score of control group are 1.30 +/- 0.08, 3.63 +/- 0.28, 3.80 +/- 0.16. Natamycin group and Natacyn group were different from control group (P <0.01). There is no difference between Natamycin group and Natacyn group. Status of healing on day 10 and day 21: The cure rate of the Natamycin group is 90% on day 10, and 100% on day 21. The cure rate of the Natacyn group is 80% on day 10, and 100% on day 21.Natamycin group and Natacyn group were different from control group (P<0.01). There is no difference between Natamycin group and Natacyn group. Corneal vascularisation, iritis, hypopyon and macular nebula on day 10 and day 21: in Natamycin group, the number of the eyes which have Corner vascularisation, iritis, hypopyon and macular nebula are 2,0,0,2. In Natacyn group, the number of the eyes which have Corner vascularisation, iritis, hypopyon and macular nebula are 1,0,0,2. In control group, the number of the eyes which have Corner vascularisation, iritis, hypopyon and macular nebula are 9,9,8,9.Natamycin group and Natacyn group were different from control group (P<0.01). There is no difference between Natamycin group and Natacyn group. CONCLUSION: Natamycin was found to be effective in fungal keratitis, similar to Natacyn, and it can stop the corner vascularisation, iritis, hypopyon and macular nebula to happen. Natamyin manufactured in China is effective against fungal keratitis, with esay availability and low toxicity in its use.展开更多
Previous studies have demonstrated that sericin effectively reduces blood glucose,and protects islet cells,as well as the gonads and kidneys.However,whether sericin improves diabetes mellitus-induced structural and fu...Previous studies have demonstrated that sericin effectively reduces blood glucose,and protects islet cells,as well as the gonads and kidneys.However,whether sericin improves diabetes mellitus-induced structural and functional problems in the central nervous system remains poorly understood.Rat models of type 2 diabetes mellitus were established by intraperitoneal injection of streptozotocin.The present study observed histological changes in the hippocampus and cerebral cortex,as well as heme oxygenase-1 expression,and explored sericin effects on the central nervous system in diabetic rats.Pathological damage to neural cells in the rat hippocampus and cerebral cortex was relieved following intragastric administration of sericin at a dose of 2.4 g/kg for 35 consecutive days.Heme oxygenase-1 protein and mRNA expressions were decreased in the hippocampus and cerebral cortex of diabetes mellitus rats after sericin treatment.The results suggest that sericin plays a protective effect on the nervous system by decreasing the high expression of heme oxygenase-1 following diabetes mellitus.展开更多
In this study,we investigated the effects of mobile phone radiation on spatial learning,reference memory,and morphology in related brain regions.After the near-field radiation(0.521.08 W/kg)was delivered to 8-week-old...In this study,we investigated the effects of mobile phone radiation on spatial learning,reference memory,and morphology in related brain regions.After the near-field radiation(0.521.08 W/kg)was delivered to 8-week-old Wistar rats 2 hours per day for 1 month,behavioral changes were examined using the Morris water maze.Compared with the sham-irradiated rats,the irradiated rats exhibited impaired performance.Morphological changes were investigated by examining synaptic ultrastructural changes in the hippocampus.Using the physical dissector technique,the number of pyramidal neurons,the synaptic profiles,and the length of postsynaptic densities in the CA1 region were quantified stereologically.The morphological changes included mitochondrial degenerations,fewer synapses,and shorter postsynaptic densities in the radiated rats.These findings indicate that mobile phone radiation can significantly impair spatial learning and reference memory and induce morphological changes in the hippocampal CA1 region.展开更多
BACKGROUND Vascular complications of transradial percutaneous coronary intervention(PCI)are rare and usually occur at the access site below the elbow.Life-threatening vascular complications during transradial PCI ther...BACKGROUND Vascular complications of transradial percutaneous coronary intervention(PCI)are rare and usually occur at the access site below the elbow.Life-threatening vascular complications during transradial PCI therapy,such as vessel perforation and dissection in the brachiocephalic,subclavian,internal mammary,and thyrocervical arteries,are rarely reported.Subclavian artery bleeding is a potentially serious complication of vascular interventional procedures leading to tracheal obstruction,hemothorax,respiratory failure,hemorrhagic shock,and death if not diagnosed early and treated promptly.CASE SUMMARY A male patient with typical angina pectoris underwent coronary angiography and stent implantation.During the procedure,the patient felt pharyngeal pain and tightness,which we mistook for myocardial ischemia.After PCI,swelling in the right neck and supraclavicular area was observed.The patient experienced dyspnea,emergency endotracheal intubation was performed,and then a sudden drop in blood pressure was observed.Ultrasound and contrast-enhanced computed tomography scans demonstrated a cervical hematoma severely compressing the trachea due to subclavian artery bleeding.Brachiocephalic angiography revealed a vascular injury site at the root of the right subclavian artery at the intersection of the right common carotid artery.A covered stent was deployed to the right subclavian artery with successful sealing of the perforation,and a bare stent was implanted in the junction of the right common carotid and brachiocephalic arteries to prevent obstruction of blood flow to the brain.CONCLUSION Subclavian artery bleeding is a lifethreatening complication of PCI.Early prevention,rapid recognition,and prompt treatment may improve the prognosis.展开更多
BACKGROUND The pathological type of simple squamous carcinoma in colorectal malignancies is rare.Simple squamous cell carcinoma of the colorectum occurs most frequently in the rectum.The clinicopathological features a...BACKGROUND The pathological type of simple squamous carcinoma in colorectal malignancies is rare.Simple squamous cell carcinoma of the colorectum occurs most frequently in the rectum.The clinicopathological features and biological behaviors of squamous colorectal carcinoma are unclear,and its prognosis may be worse than that of simple adenocarcinoma.Studies on squamous colorectal cancer are currently limited to case reports,and there is no standard treatment protocol.Therefore,more case reports are required to fully understand squamous colorectal cancer.CASE SUMMARY We reported the case of a 56-year-old woman who complained of constipation for 2 years.Colonoscopy revealed a sigmoid colon tumor,and the pathological result of colonoscopy was squamous carcinoma.After completing the relevant assessment,the patient was clinically diagnosed with cT4aN0M0,stage IIB,and surgery was performed.Based on postoperative pathological results,the patient was diagnosed with pT4bN0M0,stage IIC.Six cycles of adjuvant chemotherapy were administered after surgery.Liver metastasis and abdominal wall mass were found more than 1 mo after the end of the last chemotherapy session.Targeted local treatment was not performed because the liver had multiple metastases,but I125 particle implantation of the abdominal wall mass was performed.Two cycles of first-line chemotherapy were administered after the surgery.The patient underwent 14 mo of treatment and eventually died from the tumor.CONCLUSION Squamous carcinoma of sigmoid colon is a rare tumor with unclear pathogenesis.Its clinicopathological diagnosis should be paid close attention.展开更多
BACKGROUND Surgery for thyroid carcinoma offers a good prognosis;however,cervical lymph node metastasis may occur in the early stage.An effective diagnostic method can accurately guide clinical surgical planning and t...BACKGROUND Surgery for thyroid carcinoma offers a good prognosis;however,cervical lymph node metastasis may occur in the early stage.An effective diagnostic method can accurately guide clinical surgical planning and the scope of lymph node dissection,ultimately improving patient prognosis.AIM To explore the diagnostic value of fine-needle aspiration of thyroglobulin(FNATg)combined with ultrasound(US)-guided fine-needle aspiration cytology for cervical lymph node metastasis in thyroid carcinoma.METHODS We enrolled 209 pathologically confirmed thyroid carcinoma patients who visited our hospital between Jan 2017 and Dec 2020.Patients were tentatively diagnosed with cervical lymph node enlargement using preoperative US.They underwent US-guided fine-needle aspiration cytology and FNA-Tg.The value of single and combined application of the two methods for the diagnosis of cervical lymph node metastasis was calculated.The factors affecting FNA-Tg for diagnosis were analyzed using univariate and multivariate methods.RESULTS FNA-Tg values were significantly higher among patients with positive cervical lymph node metastasis.The sensitivity and specificity of US-guided fine-needle aspiration cytology,FNA-Tg,and US-guided fine-needle aspiration cytology+FNA-Tg were 85.48%and 90.59%,83.06%and 87.06%,and 96.77%and 91.76%,respectively.The area under the receiver operating characteristic curve for USguided fine-needle aspiration cytology,FNA-Tg,and the two combined,was 0.880,0.851,and 0.943,respectively.A long diameter/short diameter ratio<2,an insufficient number of acquired cells,a low serum thyroglobulin level,and an absence of typical metastatic US features increased the risk of cervical lymph node metastasis in thyroid carcinoma patients misdiagnosed using FNA-Tg.CONCLUSION The diagnostic value of FNA-Tg for detecting cervical lymph node metastasis is not high;however,combined with US-guided fine-needle aspiration cytology,it is significantly improved.展开更多
BACKGROUND Collision carcinoma is a rare histological pattern,and includes two or more different types of tumors coexisting in the same organ as one neoplasm.Different to the combined type,the two adjacent tumors of c...BACKGROUND Collision carcinoma is a rare histological pattern,and includes two or more different types of tumors coexisting in the same organ as one neoplasm.Different to the combined type,the two adjacent tumors of collision carcinoma are histologically distinct.Collision carcinoma may occur from any origin or organ,including the cecum,liver,cervix,thyroid,stomach,kidney,and esophagus.In the rectum,adenocarcinoma is the most common pathological type,the combined type is rare,and collision tumors are even rarer.To date,only a limited number of collision carcinoma cases originating from the rectum have been reported.Due to the scarcity of rectal collision carcinoma,more cases need to be reported to fully understand the clinico-pathological features and biological behavior of the tumor.CASE SUMMARY Here we report a 40-year-old female who presented with the chief complaints of persistent changes in bowel habits and hematochezia for 10 d.She underwent Miles'operation which revealed a collision carcinoma of the rectum,showing a“side by side”pattern,composed of a high grade neuroendocrine carcinoma,(small cell carcinoma)and moderately differentiated adenocarcinoma,based on its clinico-pathological features and biological behavior.The patient remained disease-free at 12 mo follow-up.We also focused on the related literature and expert opinion.CONCLUSION Collision carcinoma is a rare tumor with ambiguous biological behavior.Greater attention should be paid to its clinico-pathologic diagnosis.Regular and adequate follow-up is essential to help rule out metastasis and assess the prognosis.展开更多

BACKGROUND Cri du chat syndrome(CdCS),also known as 5p deletion syndrome(5p-)is a syndrome caused by partial deletion of the 5p chromosome in human beings.The incidence accounts for 1/50000 and the cause of CdCS is re...BACKGROUND Cri du chat syndrome(CdCS),also known as 5p deletion syndrome(5p-)is a syndrome caused by partial deletion of the 5p chromosome in human beings.The incidence accounts for 1/50000 and the cause of CdCS is related to partial deletion of chromosome 5 short arm(p).CdCS is a sporadic event.Only one case of CdCS was detected by chromosome screening in 125 and 170 pregnant Iranian women[1].The most prominent clinical manifestations of CdCS are typical high-pitched cat calls,severe mental retardation or mental retardation and is most harmful to both language and growth retardation[2].CdCS is a chromosome mutation disease which occurs during embryonic development and the symptoms of some cases are extremely atypical.It is difficult to make an early diagnosis and screening in clinic.We can suspect the disease from its atypical manifestations in the weak crying of cats,and chromosome karyotype analysis can find some questionable gene deletion fragments to assist the clinical diagnosis and prognosis of CdCS.CASE SUMMARY A 2-d-old male child who was admitted to our hospital with a poor postnatal reaction and poor milk intake.The baby’s crying and sucking is weak,reaction and feeding time is poor and the baby has nausea and vomiting.Karyotype analysis showed that the chromosomes were 46,XY,deletion(5)p15.Whole genome microarray analysis(named ISCN2013)showed that the chromosomes of the child were male karyotypes and contained three chromosomal abnormalities.Among them,loss of 5p15.2pter(113576-13464559)was associated with cat call syndrome.After 3 mo of follow-up,the child still vomited repeatedly,had poor milk intake,did not return to normal growth,had developmental retardation and a poor directional response.CONCLUSION Therefore,when cat crying and laryngeal sounds occur in the neonatal period,it should be considered that they are related to CdCS.Chromosome karyotype and genome analysis are helpful for the diagnosis of CdCS.展开更多

BACKGROUND:Leptin preserves reproductive functions by stimulating hypothalamic-pituitary-gonadal axis activities at different levels.Some interneurons play an important role in leptin regulation of the gonadal axis.I...BACKGROUND:Leptin preserves reproductive functions by stimulating hypothalamic-pituitary-gonadal axis activities at different levels.Some interneurons play an important role in leptin regulation of the gonadal axis.It remains uncertain whether leptin regulates reproductive functions by activating proopiomelanocortin (POMC) neurons.OBJECTIVE:To investigate leptin effects on secretory function of the neuroendocrine-gonadal axis by activating POMC neurons and to observe and verify the relationship between leptin effects and various time points.DESIGN,TIME AND SETTING:The randomized,controlled,animal study was performed at the Basic Institute of Chengde Medical University and the Research Room of Reproductive Immunology of National Research Institute for Family Planning from June to September 2008.MATERIALS:Leptin (Peprotech,USA),a-melanocyte stimulating hormone and rabbit anti-POMC polyclonal antibody (SC-20148) (Santa Cruz Biotechnology,USA),follicle stimulating hormone,luteinizing hormone,and gonadotropin releasing hormone enzyme-labeled immunosorbent assay (ELISA) kit (ADL,USA) were used in the present study.METHODS:A total of 60 healthy,female,adult,Wistar rats received 17 (3-estradiol for 5 consecutive days at 15 days after ovariectomy.The rats were randomly assigned to physiological saline (n= 35),leptin (n = 35),and a-melanocyte stimulating hormone (n = 20) groups.MAIN OUTCOME MEASURES:Changes in gonadotropin releasing hormone,luteinizing hormone,and follicle stimulating hormone were compared following intraventricular injection of physiological saline,leptin,and a-melanocyte stimulating hormone at various time points.Changes in POMC mRNA and protein expression in the hypothalamus were measured following physiological saline and leptin injection via the lateral ventricle.RESULTS:Compared to the physiological saline group,leptin and a-melanocyte stimulating hormone affected secretion in the hypothalamus-pituitary axis.Leptin affected secretion of gonadotropin releasing hormone,luteinizing hormone,and follicle stimulating hormone,whereas a-melanocyte stimulating hormone inhibited secretion of these hormones.Compared to the physiological saline group,POMC mRNA expression was significantly increased in the hypothalamus at 2 and 4 hours after leptin injection (P〈 0.05),but expression recovered to physiological saline group levels at 6 hours after injection (P 〉 0.05).POMC protein expression was significantly increased in the hypothalamus at 4 and 6 hours after leptin injection (P〈 0.05).CONCLUSION:Leptin affects secretory function of the neuroendocrine-gonadal axis through combined effects on POMC neurons and other pathways.Results suggested that the regulatory effects of POMC neurons were later compared to other neurons.展开更多
BACKGROUND Pheochromocytoma is a rare catecholamines-secreting tumor arising from chromaffin cells in the adrenal medulla.It classically presents with paroxysmal hypertension,headaches,palpitations,sweating,and metabo...BACKGROUND Pheochromocytoma is a rare catecholamines-secreting tumor arising from chromaffin cells in the adrenal medulla.It classically presents with paroxysmal hypertension,headaches,palpitations,sweating,and metabolic disorders.Atypical presentations such as acute myocardial infarction,heart failure,cardiomyopathy,stroke,and transient erythrocytosis have been infrequently documented.CASE SUMMARY We describe the case of a 72-year-old man diagnosed with pheochromocytoma presenting with non-ST segment elevation myocardial infarction,heart failure,and transient erythrocytosis with nonobstructed coronary arteries.This was his second heart attack.The patient was previously diagnosed with myocardial infarction,and an immense mass was found on the left adrenal gland 3 years prior.Based on clinical and laboratory findings,a diagnosis of pheochromocytoma was confirmed.His coronary angiogram showed nonobstructed coronary arteries except for a myocardial bridge in the left anterior descending branch.This was a form of type-2 myocardial infarction.The myocardial cell lesions were caused by sudden secretion of catecholamines by the pheochromocytoma.Even more atypically,his hemoglobin level was obviously elevated at admission,but after a few days of treatment with an alpha-adrenergic receptor blocker,it dropped to normal levels without additional treatment.CONCLUSION Pheochromocytoma may be a cause of acute myocardial infarction,heart failure,and transient erythrocytosis.展开更多
BACKGROUND Obesity is associated with a better prognosis in patients with community-acquired pneumonia(the so-called obesity survival paradox),but conflicting results have been found.AIM To investigate the relationshi...BACKGROUND Obesity is associated with a better prognosis in patients with community-acquired pneumonia(the so-called obesity survival paradox),but conflicting results have been found.AIM To investigate the relationship between all-cause mortality and body mass index in patients with community-acquired pneumonia.METHODS This retrospective study included patients with community-acquired pneumonia hospitalized in the First Hospital of Qinhuangdao from June 2013 to November 2018.The patients were grouped as underweight(<18.5 kg/m2),normal weight(18.5-23.9 kg/m2),and overweight/obesity(≥24 kg/m2).The primary outcome was all-cause hospital mortality.RESULTS Among 2327 patients,297(12.8%)were underweight,1013(43.5%)normal weight,and 1017(43.7%)overweight/obesity.The all-cause hospital mortality was 4.6%(106/2327).Mortality was lowest in the overweight/obesity group and highest in the underweight group(2.8%,vs 5.0%,vs 9.1%,P<0.001).All-cause mortality of overweight/obesity patients was lower than normal-weight patients[odds ratio(OR)=0.535,95%confidence interval(CI)=0.334-0.855,P=0.009],while the allcause mortality of underweight patients was higher than that of normal-weight patients(OR=1.886,95%CI:1.161-3.066,P=0.010).Multivariable analysis showed that abnormal neutrophil counts(OR=2.38,95%CI:1.55-3.65,P<0.001),abnormal albumin levels(OR=0.20,95%CI:0.06-0.72,P=0.014),high-risk Confusion-Urea-Respiration-Blood pressure-65 score(OR=2.89,95%CI:1.48-5.64,P=0.002),and intensive care unit admission(OR=3.11,95%CI:1.77-5.49,P<0.001)were independently associated with mortality.CONCLUSION All-cause mortality of normal-weight patients was higher than overweight/obesity patients,lower than that of underweight patients.Neutrophil counts,albumin levels,Confusion-Urea-Respiration-Blood pressure-65 score,and intensive care unit admission were independently associated with mortality in patients with community-acquired pneumonia.展开更多
BACKGROUND For resectable hepatocellular carcinoma(HCC),radical hepatectomy is commonly used as a curative treatment.However,postoperative recurrence significantly diminishes the overall survival(OS)of HCC patients,es...BACKGROUND For resectable hepatocellular carcinoma(HCC),radical hepatectomy is commonly used as a curative treatment.However,postoperative recurrence significantly diminishes the overall survival(OS)of HCC patients,especially with microva-scular invasion(MVI)as an independent high-risk factor for recurrence.While some studies suggest that postoperative adjuvant therapy may decrease the risk of recurrence following liver resection in HCC patients,the specific role of adju-vant therapies in those with MVI remains unclear.AIM To conduct a network meta-analysis(NMA)to evaluate the efficacy of various adjuvant therapies and determine the optimal adjuvant regimen.METHODS A systematic literature search was conducted on PubMed,EMBASE,and Web of Science until April 6,2023.Studies comparing different adjuvant therapies or comparing adjuvant therapy with hepatectomy alone were included.Hazard ratios(HRs)with 95%confidence intervals were used to combine data on recurrence free survival and OS in both pairwise meta-analyses and NMA.RESULTS Fourteen eligible trials(2268 patients)reporting five different therapies were included.In terms of reducing the risk of recurrence,radiotherapy(RT)[HR=0.34(0.23,0.5);surface under the cumulative ranking curve(SUCRA)=97.7%]was found to be the most effective adjuvant therapy,followed by hepatic artery infusion chemotherapy[HR=0.52(0.35,0.76);SUCRA=65.1%].Regarding OS improvement,RT[HR:0.35(0.2,0.61);SUCRA=93.1%]demonstrated the highest effectiveness,followed by sorafenib[HR=0.48(0.32,0.69);SUCRA=70.9%].INTRODUCTION Hepatocellular carcinoma(HCC)is the sixth most common malignant tumor in the world and ranks third in terms of worldwide malignant tumor mortality rates in 2020[1].Curative treatments for HCC include ablation,radical hepatectomy,and liver transplantation.However,ablation is suitable only for early-stage HCC patients,who represent a small percentage of the overall HCC population.Although liver transplantation serves as the optimal treatment for HCC patients,the scarcity of donor organs restricts the availability of this procedure.Therefore,hepatectomy is the most commonly employed curative treatment for resectable HCC.Unfortunately,the 5-year recurrence rate for patients who undergoing hepatectomy ranges from 50%to 70%[2,3].Recurrence of HCC is associated with several risk factors[4],including single nodule>5 cm,vascular invasion,and multiple nodules.Among these factors,microvascular invasion(MVI)is an independent risk factor for recurrence.MVI is defined as the presence of cancer cells in the lumen of endothelium-lined vessels,typically in the small branches of the portal and hepatic veins of the paracancerous liver tissue,visible only under the microscope[5].Previous studies have shown that among HCC patients who underwent hepatectomy,those with MVI had a higher risk of recurrence and shorter overall survival(OS)than those without MVI[6].Several studies have indicated that adjuvant therapy following curative hepatectomy can prevent recurrence and improve OS in HCC patients with MVI.These postoperative adjuvant therapies include transarterial chemoembolization(TACE)[7],sorafenib[8],hepatic artery infusion chemotherapy(HAIC)[9],and radiotherapy(RT)[10].However,the existing studies mostly compare individual adjuvant therapy with hepatectomy alone.Direct or indirect comparisons between the various adjuvant therapies are lacking.Therefore,we performed the network meta-analysis(NMA)to compare the relative efficacy of each adjuvant therapy to determine the optimal treatment.展开更多

摘要BACKGROUND In patients with cirrhosis and ascites,fluid retention may artificially increase their body weight and body mass index(BMI),potentially masking a low BMI and leading to under recognition of malnutrition.AIM To investigate whether computed tomography(CT)-based correction of the body weight improves malnutrition detection in patients with cirrhosis and ascites.METHODS This retrospective study included 100 hospitalized patients with cirrhosis and ascites.The BMI was evaluated using three approaches:The actual body weight method,the CT-corrected method,and the CT-sensitive corrected method.Low BMI and Global Leadership Initiative on Malnutrition(GLIM)-defined malnutrition were assessed under the three methods,and differences in detection rates were compared.RESULTS Compared with the actual body weight method,both the CT-corrected method and the CT-sensitive corrected method resulted in lower BMI values(both P<0.001),with further reduction observed for the CT-sensitive corrected method(P=0.009).The detection rates of low BMI increased from 6.0%to 10.0%and 12.0%,respectively.The detection rates of GLIM-defined malnutrition increased from 19.0%to 22.0%and 24.0%,respectively.Reclassification was unidirectional,with patients shifting from a non-low BMI or non-malnourished status under the actual body weight method to a low BMI or malnutrition status under the CT-corrected or CT-sensitive corrected method.CONCLUSION CT-based body weight correction reduced BMI estimates and improved the detection of low BMI and GLIMdefined malnutrition in patients with cirrhosis and ascites.This approach may serve as a useful adjunct to nutritional assessment,particularly in patients with significant fluid retention.

摘要BACKGROUND Hepatocellular carcinoma(HCC)incidence continues to rise,yet treatment options remain limited.Despite recent advances in early screening methods,most patients are still diagnosed at an advanced stage with poor prognosis.Therefore,accurate assessment of prognostic risk using molecular biomarkers is essential for the development of personalized treatment strategies.AIM To investigate the correlation between serum aldo-keto reductase family 1 member B10(AKR1B10)levels and clinicopathological characteristics and prognosis in patients with HCC,and to evaluate the potential clinical value of AKR-1B10 as a prognostic biomarker.METHODS A total of 102 patients with HCC admitted to the Affiliated Hospital of Chengde Medical University between May 2020 and May 2024 were enrolled as study subjects.Complete clinical data and survival follow-up information were collected.Based on the optimal cutoff value for serum AKR1B10 levels in diagnosing HCC(1584.97 pg/mL),determined using receiver operating characteristic curve analysis,patients were divided into a high-level group(≥1584.97 pg/mL,n=75)and a low-level group(<1584.97 pg/mL,n=27).AKR1B10 levels were measured using enzyme-linked immunosorbent assay.In 41 patients who underwent surgical resection,serum AKR1B10 levels were measured preoperatively and at 1-2 days and 3-4 days postoperatively.Theχ2 test or Fisher’s exact test was used to compare categorical data between the two groups.Survival analysis was performed using the Kaplan-Meier method,with intergroup comparisons performed using the log-rank test.Cox univariate and multivariate regression analyses were used to identify independent prognostic factors,and the Friedman test was used to compare serum AKR1B10 levels at different postoperative time points.RESULTS Significant differences were observed between the AKR1B10 high-level and low-level groups in tumor size(χ2=3.999,P<0.05),ascites(χ2=3.902,P<0.05),and Barcelona Clinic Liver Cancer stage(χ2=3.987,P<0.05).The median survival was 17.0 months in the high-level group,significantly shorter than that in the low-level group(32.0 months;χ2=6.172,P<0.05).Multivariate Cox regression analysis indicated that serum AKR1B10 level(hazard ratio=1.819,95%confidence interval:1.009-3.280,P<0.05)and ascites(hazard ratio=1.810,95%confidence interval:1.017-3.223,P<0.05)were independent factors influencing survival in patients with HCC.Additionally,serum AKR1B10 levels were significantly decreased at 1-2 days postoperatively in the 41 patients who underwent surgery,with a continued downward trend at 3-4 days postoperatively(χ2=82.000,P<0.01).CONCLUSION Serum AKR1B10 is an independent risk factor for prognosis in HCC,and high AKR1B10 levels are associated with unfavorable survival outcomes,indicating its potential value as a biomarker for prognostic risk assessment in patients with HCC.
摘要With the rapid advancement of technology,artificial intelligence(AI)has emerged as a transformative force in gastroenterology,particularly in diagnosing upper gastrointestinal diseases such as Barrett's esophagus(BE),esophageal cancer,gastroesophageal reflux disease(GERD),and esophagogastric varices.AI's capabilities in image analysis,classification,detection,and segmentation have significantly improved diagnostic accuracy and efficiency.For BE,AI models achieve high sensitivity and specificity in detecting early neoplastic changes and guiding targeted biopsies.In esophageal cancer,AI enhances early lesion detection,improving intervention success rates.For GERD,AI classifies disease severity based on the Los Angeles grading system and accurately segments lesions.Additionally,AI detects esophagogastric varices and predicts bleeding risks more effectively than traditional methods.Despite these advancements,challenges remain,including the need for high-quality data,multi-center validation,and ensuring AI model interpretability.Future research should address these issues and further integrate AI into clinical practice to optimize patient outcomes.This review highlights AI's transformative impact on upper gastrointestinal disease diagnosis,emphasizing its potential to revolutionize endoscopic practice and improve patient care.

摘要BACKGROUND Neck pain,a primary symptom of cervical spondylosis,affects patients'physical and mental health,reducing their quality of life.Pain and emotional state interact;however,their longitudinal interrelationship remains unclear.In this study,we applied a dual-trajectory model to assess how neck pain and emotional state evolve together over time and how clinical interventions,particularly acupuncture,influence these trajectories.AIM To investigate the longitudinal relationship between neck pain and emotional state in patients with cervical spondylosis.METHODS This prospective cohort study included 472 patients with cervical spondylosis from eight Chinese hospitals.Participants received acupuncture or medication and were followed up at baseline,and at 1,2,4,6,and 8 weeks.Neck pain and emotional distress were assessed using the Northwick Park Neck Pain Questionnaire(NPQ)and the affective subscale of the Short-Form McGill Pain Questionnaire(SF-MPQ),respectively.Group-based trajectory models and dual trajectory analysis were used to identify and correlate pain-emotion trajectories.Multivariate logistic regression identified predictors of group membership.RESULTS Three trajectory groups were identified for NPQ and SF-MPQ scores(low,medium,and high).Higher NPQ trajectory was associated with older age(OR=1.058,P<0.001)and was significantly reduced by acupuncture(OR=0.382,P<0.001).Similarly,acupuncture lowered the odds of high SF-MPQ trajectory membership(OR=0.336,P<0.001),while age increased it(OR=1.037,P<0.001).Dual-trajectory analysis revealed bidirectional associations:69.1%of patients with low NPQ had low SF-MPQ scores,and 42.6%of patients with high SF-MPQ also had high NPQ scores.Gender was a predictor for medium SF-MPQ trajectory(OR=1.629,P=0.094).Occupation and education levels differed significantly across the trajectory groups(P<0.05).CONCLUSION Over time,neck pain and emotional distress are closely associated in patients with cervical spondylosis.Acupuncture alleviates both outcomes significantly,while age is a risk factor.Integrated approaches to pain and emotional management are encouraged.
摘要BACKGROUND The increasing risk of cardiovascular disease(CVD)associated with worsening environmental exposure is a critical health concern garnering global research attention.AIM To systematically assess the scope and characteristics of research on the relationship between environmental exposure and CVD.METHODS A thorough examination of publications on the relationship between environmental exposure and CVD from 1999 to 2022 was carried out by extensively screening the literature using the Web of Science Core Collection.The language of the selected publications was standardized to English.Afterward,different academic tools such as CiteSpace,VOSviewer,HistCite,Python,Matplotlib,and Bibliometrix were utilized to examine the research trends in this field.RESULTS The study’s findings indicated a steady increase in scientific publications among the 1640 analyzed documents,peaking in 2022 with 197 publications.The United States emerged as the leading nation regarding high-quality publications and international collaboration.Harvard University was identified as the most prolific institution.“Environmental research”was the most frequently contributing journal,and Muenzel T was recognized as the top contributor.Current research hotspots are primarily concentrated on themes such as“cardiovascular disease”,“exposure”,“risk”,“mortality”,and“air pollution”.CONCLUSION This study highlights increasing research on the link between environmental exposure and CVD,identifying key exposures and diseases while emphasizing the need for further investigation into underlying mechanisms.

摘要BACKGROUND Regenerating gene 4(REG4)has been proved to be carcinogenic in some cancers,but its manifestation and possible carcinogenic mechanisms in colorectal cancer(CRC)have not yet been elucidated.Our previous study found that the drug resistance of CRC cells may be closely linked to their fat metabolism.AIM To explore the role of REG4 in CRC and its association with lipid droplet formation and chemoresistance.METHODS We conducted a meta-analysis and bioinformatics and pathological analyses of REG4 expression in CRC.The effects of REG4 on the phenotypes and related protein expression were also investigated in CRC cells.We detected the impacts of REG4 on the chemoresistance and lipid droplet formation in CRC cells.Finally,we analyzed how REG4 regulated the transcription and proteasomal degradation of lipogenic enzymes in CRC cells.RESULTS Compared to normal mucosa,REG4 mRNA expression was high in CRC(P<0.05)but protein expression was low.An inverse correlation existed between lymph node and distant metastases,tumor-node-metastasis staging or short overall survival and REG4 mRNA overexpression(P<0.05),but vice versa for REG4 protein expression.REG4-related genes included:Chemokine activity;taste receptors;protein-DNA and DNA packing complexes;nucleosomes and chromatin;generation of second messenger molecules;programmed cell death signals;epigenetic regulation and DNA methylation;transcription repression and activation by DNA binding;insulin signaling pathway;sugar metabolism and transfer;and neurotransmitter receptors(P<0.05).REG4 exposure or overexpression promoted proliferation,antiapoptosis,migration,and invasion of DLD-1 cells in an autocrine or paracrine manner by activating the epidermal growth factor receptor-phosphoinositide 3-kinase-Akt-nuclear factor-κB pathway.REG4 was involved in chemoresistance not through de novo lipogenesis,but lipid droplet assembly.REG4 inhibited the transcription of acetyl-CoA carboxylase 1(ACC1)and ATP-citrate lyase(ACLY)by disassociating the complex formation of anti-acetyl(AC)-acetyl-histone 3-AC-histone 4-inhibitor of growth protein-5-si histone deacetylase;-sterol-regulatory element binding protein 1 in their promoters and induced proteasomal degradation of ACC1 or ACLY.CONCLUSION REG4 may be involved in chemoresistance through lipid droplet assembly.REG4 reduces expression of de novo lipid synthesis key enzymes by inhibiting transcription and promoting ubiquitination-mediated proteasomal degradation.

摘要AIM: To evaluate the efficacy of topical administration Natamycin, which is produced by China, in an experimental rabbit model of Fusarium solani keratitis, to provide experimental basis for the application of clinical safety. METHODS: Fusarium solani was induced in the right eye of 30 New Zealand rabbits. Forty-eight hours after inoculation, the animals were divided into 3 different treatment groups, 10 rabbit eyes of each group: Group 1 (Natamycin) treated with topical Natamycin, group 2 (Natacyn) treated with topical Natacyn, group 3 (control) treated with topical saline solution. The eyes of each group was examined clinically with slit lamp using ulcer scoring system on day 4, 10, 15, and 21 for status of healing, corneal vascularisation, iritis, hypopyon and macular nebula. The findings were recorded on day 10 and day 21. RESULTS: Ulcer score on day 10, day 15, day 21: The score of Natamycin group are 1.45 +/- 0.16, 1.08 +/- 0.11, 0.70 +/- 0.40. The score of Natacyn group are 1.35 +/- 0.12, 1.10 +/- 0.12, 0.65 +/- 0.35. the score of control group are 1.30 +/- 0.08, 3.63 +/- 0.28, 3.80 +/- 0.16. Natamycin group and Natacyn group were different from control group (P <0.01). There is no difference between Natamycin group and Natacyn group. Status of healing on day 10 and day 21: The cure rate of the Natamycin group is 90% on day 10, and 100% on day 21. The cure rate of the Natacyn group is 80% on day 10, and 100% on day 21.Natamycin group and Natacyn group were different from control group (P<0.01). There is no difference between Natamycin group and Natacyn group. Corneal vascularisation, iritis, hypopyon and macular nebula on day 10 and day 21: in Natamycin group, the number of the eyes which have Corner vascularisation, iritis, hypopyon and macular nebula are 2,0,0,2. In Natacyn group, the number of the eyes which have Corner vascularisation, iritis, hypopyon and macular nebula are 1,0,0,2. In control group, the number of the eyes which have Corner vascularisation, iritis, hypopyon and macular nebula are 9,9,8,9.Natamycin group and Natacyn group were different from control group (P<0.01). There is no difference between Natamycin group and Natacyn group. CONCLUSION: Natamycin was found to be effective in fungal keratitis, similar to Natacyn, and it can stop the corner vascularisation, iritis, hypopyon and macular nebula to happen. Natamyin manufactured in China is effective against fungal keratitis, with esay availability and low toxicity in its use.

摘要Previous studies have demonstrated that sericin effectively reduces blood glucose,and protects islet cells,as well as the gonads and kidneys.However,whether sericin improves diabetes mellitus-induced structural and functional problems in the central nervous system remains poorly understood.Rat models of type 2 diabetes mellitus were established by intraperitoneal injection of streptozotocin.The present study observed histological changes in the hippocampus and cerebral cortex,as well as heme oxygenase-1 expression,and explored sericin effects on the central nervous system in diabetic rats.Pathological damage to neural cells in the rat hippocampus and cerebral cortex was relieved following intragastric administration of sericin at a dose of 2.4 g/kg for 35 consecutive days.Heme oxygenase-1 protein and mRNA expressions were decreased in the hippocampus and cerebral cortex of diabetes mellitus rats after sericin treatment.The results suggest that sericin plays a protective effect on the nervous system by decreasing the high expression of heme oxygenase-1 following diabetes mellitus.

摘要In this study,we investigated the effects of mobile phone radiation on spatial learning,reference memory,and morphology in related brain regions.After the near-field radiation(0.521.08 W/kg)was delivered to 8-week-old Wistar rats 2 hours per day for 1 month,behavioral changes were examined using the Morris water maze.Compared with the sham-irradiated rats,the irradiated rats exhibited impaired performance.Morphological changes were investigated by examining synaptic ultrastructural changes in the hippocampus.Using the physical dissector technique,the number of pyramidal neurons,the synaptic profiles,and the length of postsynaptic densities in the CA1 region were quantified stereologically.The morphological changes included mitochondrial degenerations,fewer synapses,and shorter postsynaptic densities in the radiated rats.These findings indicate that mobile phone radiation can significantly impair spatial learning and reference memory and induce morphological changes in the hippocampal CA1 region.

摘要BACKGROUND Vascular complications of transradial percutaneous coronary intervention(PCI)are rare and usually occur at the access site below the elbow.Life-threatening vascular complications during transradial PCI therapy,such as vessel perforation and dissection in the brachiocephalic,subclavian,internal mammary,and thyrocervical arteries,are rarely reported.Subclavian artery bleeding is a potentially serious complication of vascular interventional procedures leading to tracheal obstruction,hemothorax,respiratory failure,hemorrhagic shock,and death if not diagnosed early and treated promptly.CASE SUMMARY A male patient with typical angina pectoris underwent coronary angiography and stent implantation.During the procedure,the patient felt pharyngeal pain and tightness,which we mistook for myocardial ischemia.After PCI,swelling in the right neck and supraclavicular area was observed.The patient experienced dyspnea,emergency endotracheal intubation was performed,and then a sudden drop in blood pressure was observed.Ultrasound and contrast-enhanced computed tomography scans demonstrated a cervical hematoma severely compressing the trachea due to subclavian artery bleeding.Brachiocephalic angiography revealed a vascular injury site at the root of the right subclavian artery at the intersection of the right common carotid artery.A covered stent was deployed to the right subclavian artery with successful sealing of the perforation,and a bare stent was implanted in the junction of the right common carotid and brachiocephalic arteries to prevent obstruction of blood flow to the brain.CONCLUSION Subclavian artery bleeding is a lifethreatening complication of PCI.Early prevention,rapid recognition,and prompt treatment may improve the prognosis.

摘要BACKGROUND The pathological type of simple squamous carcinoma in colorectal malignancies is rare.Simple squamous cell carcinoma of the colorectum occurs most frequently in the rectum.The clinicopathological features and biological behaviors of squamous colorectal carcinoma are unclear,and its prognosis may be worse than that of simple adenocarcinoma.Studies on squamous colorectal cancer are currently limited to case reports,and there is no standard treatment protocol.Therefore,more case reports are required to fully understand squamous colorectal cancer.CASE SUMMARY We reported the case of a 56-year-old woman who complained of constipation for 2 years.Colonoscopy revealed a sigmoid colon tumor,and the pathological result of colonoscopy was squamous carcinoma.After completing the relevant assessment,the patient was clinically diagnosed with cT4aN0M0,stage IIB,and surgery was performed.Based on postoperative pathological results,the patient was diagnosed with pT4bN0M0,stage IIC.Six cycles of adjuvant chemotherapy were administered after surgery.Liver metastasis and abdominal wall mass were found more than 1 mo after the end of the last chemotherapy session.Targeted local treatment was not performed because the liver had multiple metastases,but I125 particle implantation of the abdominal wall mass was performed.Two cycles of first-line chemotherapy were administered after the surgery.The patient underwent 14 mo of treatment and eventually died from the tumor.CONCLUSION Squamous carcinoma of sigmoid colon is a rare tumor with unclear pathogenesis.Its clinicopathological diagnosis should be paid close attention.

摘要BACKGROUND Surgery for thyroid carcinoma offers a good prognosis;however,cervical lymph node metastasis may occur in the early stage.An effective diagnostic method can accurately guide clinical surgical planning and the scope of lymph node dissection,ultimately improving patient prognosis.AIM To explore the diagnostic value of fine-needle aspiration of thyroglobulin(FNATg)combined with ultrasound(US)-guided fine-needle aspiration cytology for cervical lymph node metastasis in thyroid carcinoma.METHODS We enrolled 209 pathologically confirmed thyroid carcinoma patients who visited our hospital between Jan 2017 and Dec 2020.Patients were tentatively diagnosed with cervical lymph node enlargement using preoperative US.They underwent US-guided fine-needle aspiration cytology and FNA-Tg.The value of single and combined application of the two methods for the diagnosis of cervical lymph node metastasis was calculated.The factors affecting FNA-Tg for diagnosis were analyzed using univariate and multivariate methods.RESULTS FNA-Tg values were significantly higher among patients with positive cervical lymph node metastasis.The sensitivity and specificity of US-guided fine-needle aspiration cytology,FNA-Tg,and US-guided fine-needle aspiration cytology+FNA-Tg were 85.48%and 90.59%,83.06%and 87.06%,and 96.77%and 91.76%,respectively.The area under the receiver operating characteristic curve for USguided fine-needle aspiration cytology,FNA-Tg,and the two combined,was 0.880,0.851,and 0.943,respectively.A long diameter/short diameter ratio<2,an insufficient number of acquired cells,a low serum thyroglobulin level,and an absence of typical metastatic US features increased the risk of cervical lymph node metastasis in thyroid carcinoma patients misdiagnosed using FNA-Tg.CONCLUSION The diagnostic value of FNA-Tg for detecting cervical lymph node metastasis is not high;however,combined with US-guided fine-needle aspiration cytology,it is significantly improved.
摘要BACKGROUND Collision carcinoma is a rare histological pattern,and includes two or more different types of tumors coexisting in the same organ as one neoplasm.Different to the combined type,the two adjacent tumors of collision carcinoma are histologically distinct.Collision carcinoma may occur from any origin or organ,including the cecum,liver,cervix,thyroid,stomach,kidney,and esophagus.In the rectum,adenocarcinoma is the most common pathological type,the combined type is rare,and collision tumors are even rarer.To date,only a limited number of collision carcinoma cases originating from the rectum have been reported.Due to the scarcity of rectal collision carcinoma,more cases need to be reported to fully understand the clinico-pathological features and biological behavior of the tumor.CASE SUMMARY Here we report a 40-year-old female who presented with the chief complaints of persistent changes in bowel habits and hematochezia for 10 d.She underwent Miles'operation which revealed a collision carcinoma of the rectum,showing a“side by side”pattern,composed of a high grade neuroendocrine carcinoma,(small cell carcinoma)and moderately differentiated adenocarcinoma,based on its clinico-pathological features and biological behavior.The patient remained disease-free at 12 mo follow-up.We also focused on the related literature and expert opinion.CONCLUSION Collision carcinoma is a rare tumor with ambiguous biological behavior.Greater attention should be paid to its clinico-pathologic diagnosis.Regular and adequate follow-up is essential to help rule out metastasis and assess the prognosis.

摘要This article reviews of the original research published by Wu et al in the World Journal of Gastroenterology,delving into the pivotal role of the gut microbiota in the pathogenesis of Crohn's disease(CD).Insights were gained from fecal microbiota transplantation(FMT)in mouse models,revealing the intricate interplay between the gut microbiota,mesenteric adipose tissue(MAT),and creeping fat.The study uncovered the characteristics of inflammation and fibrosis in the MAT and intestinal tissues of patients with CD;moreover,through the FMT mouse model,it observed the impact of samples from healthy patients and those with CD on symptoms.The pathogenesis of CD is complex,and its etiology remains unclear;however,it is widely believed that gut microbiota dysbiosis plays a significant role.Recently,with the development and application of next-generation sequen-cing technology,research on the role of fungi in the pathogenesis and chronicity of CD has deepened.This editorial serves as a supplement to the research by Wu et al who discussed advances related to the study of fungi in CD.
摘要BACKGROUND Cri du chat syndrome(CdCS),also known as 5p deletion syndrome(5p-)is a syndrome caused by partial deletion of the 5p chromosome in human beings.The incidence accounts for 1/50000 and the cause of CdCS is related to partial deletion of chromosome 5 short arm(p).CdCS is a sporadic event.Only one case of CdCS was detected by chromosome screening in 125 and 170 pregnant Iranian women[1].The most prominent clinical manifestations of CdCS are typical high-pitched cat calls,severe mental retardation or mental retardation and is most harmful to both language and growth retardation[2].CdCS is a chromosome mutation disease which occurs during embryonic development and the symptoms of some cases are extremely atypical.It is difficult to make an early diagnosis and screening in clinic.We can suspect the disease from its atypical manifestations in the weak crying of cats,and chromosome karyotype analysis can find some questionable gene deletion fragments to assist the clinical diagnosis and prognosis of CdCS.CASE SUMMARY A 2-d-old male child who was admitted to our hospital with a poor postnatal reaction and poor milk intake.The baby’s crying and sucking is weak,reaction and feeding time is poor and the baby has nausea and vomiting.Karyotype analysis showed that the chromosomes were 46,XY,deletion(5)p15.Whole genome microarray analysis(named ISCN2013)showed that the chromosomes of the child were male karyotypes and contained three chromosomal abnormalities.Among them,loss of 5p15.2pter(113576-13464559)was associated with cat call syndrome.After 3 mo of follow-up,the child still vomited repeatedly,had poor milk intake,did not return to normal growth,had developmental retardation and a poor directional response.CONCLUSION Therefore,when cat crying and laryngeal sounds occur in the neonatal period,it should be considered that they are related to CdCS.Chromosome karyotype and genome analysis are helpful for the diagnosis of CdCS.

摘要BACKGROUND Severe skeletal class II malocclusion is the indication for combined orthodontic and orthognathic treatment.CASE SUMMARY A woman with a chief complaint of a protruding chin and an inability to close her lips requested orthodontic camouflage.The treatment plan consisted of extracting the right upper third molar,right lower third molar,left lower second molar,and left upper third molar and moving the maxillary dentition distally using a convenient method involving microimplant nail anchors,push springs,long arm traction hooks,and elastic traction chains.After 52 months of treatment,her overbite and overjet were normal,and her facial profile was favorable.CONCLUSION This method can be used for distal movement of the maxillary dentition and to correct severe skeletal class II malocclusion in adults.

摘要BACKGROUND:Leptin preserves reproductive functions by stimulating hypothalamic-pituitary-gonadal axis activities at different levels.Some interneurons play an important role in leptin regulation of the gonadal axis.It remains uncertain whether leptin regulates reproductive functions by activating proopiomelanocortin (POMC) neurons.OBJECTIVE:To investigate leptin effects on secretory function of the neuroendocrine-gonadal axis by activating POMC neurons and to observe and verify the relationship between leptin effects and various time points.DESIGN,TIME AND SETTING:The randomized,controlled,animal study was performed at the Basic Institute of Chengde Medical University and the Research Room of Reproductive Immunology of National Research Institute for Family Planning from June to September 2008.MATERIALS:Leptin (Peprotech,USA),a-melanocyte stimulating hormone and rabbit anti-POMC polyclonal antibody (SC-20148) (Santa Cruz Biotechnology,USA),follicle stimulating hormone,luteinizing hormone,and gonadotropin releasing hormone enzyme-labeled immunosorbent assay (ELISA) kit (ADL,USA) were used in the present study.METHODS:A total of 60 healthy,female,adult,Wistar rats received 17 (3-estradiol for 5 consecutive days at 15 days after ovariectomy.The rats were randomly assigned to physiological saline (n= 35),leptin (n = 35),and a-melanocyte stimulating hormone (n = 20) groups.MAIN OUTCOME MEASURES:Changes in gonadotropin releasing hormone,luteinizing hormone,and follicle stimulating hormone were compared following intraventricular injection of physiological saline,leptin,and a-melanocyte stimulating hormone at various time points.Changes in POMC mRNA and protein expression in the hypothalamus were measured following physiological saline and leptin injection via the lateral ventricle.RESULTS:Compared to the physiological saline group,leptin and a-melanocyte stimulating hormone affected secretion in the hypothalamus-pituitary axis.Leptin affected secretion of gonadotropin releasing hormone,luteinizing hormone,and follicle stimulating hormone,whereas a-melanocyte stimulating hormone inhibited secretion of these hormones.Compared to the physiological saline group,POMC mRNA expression was significantly increased in the hypothalamus at 2 and 4 hours after leptin injection (P〈 0.05),but expression recovered to physiological saline group levels at 6 hours after injection (P 〉 0.05).POMC protein expression was significantly increased in the hypothalamus at 4 and 6 hours after leptin injection (P〈 0.05).CONCLUSION:Leptin affects secretory function of the neuroendocrine-gonadal axis through combined effects on POMC neurons and other pathways.Results suggested that the regulatory effects of POMC neurons were later compared to other neurons.

摘要BACKGROUND Pheochromocytoma is a rare catecholamines-secreting tumor arising from chromaffin cells in the adrenal medulla.It classically presents with paroxysmal hypertension,headaches,palpitations,sweating,and metabolic disorders.Atypical presentations such as acute myocardial infarction,heart failure,cardiomyopathy,stroke,and transient erythrocytosis have been infrequently documented.CASE SUMMARY We describe the case of a 72-year-old man diagnosed with pheochromocytoma presenting with non-ST segment elevation myocardial infarction,heart failure,and transient erythrocytosis with nonobstructed coronary arteries.This was his second heart attack.The patient was previously diagnosed with myocardial infarction,and an immense mass was found on the left adrenal gland 3 years prior.Based on clinical and laboratory findings,a diagnosis of pheochromocytoma was confirmed.His coronary angiogram showed nonobstructed coronary arteries except for a myocardial bridge in the left anterior descending branch.This was a form of type-2 myocardial infarction.The myocardial cell lesions were caused by sudden secretion of catecholamines by the pheochromocytoma.Even more atypically,his hemoglobin level was obviously elevated at admission,but after a few days of treatment with an alpha-adrenergic receptor blocker,it dropped to normal levels without additional treatment.CONCLUSION Pheochromocytoma may be a cause of acute myocardial infarction,heart failure,and transient erythrocytosis.
摘要BACKGROUND Obesity is associated with a better prognosis in patients with community-acquired pneumonia(the so-called obesity survival paradox),but conflicting results have been found.AIM To investigate the relationship between all-cause mortality and body mass index in patients with community-acquired pneumonia.METHODS This retrospective study included patients with community-acquired pneumonia hospitalized in the First Hospital of Qinhuangdao from June 2013 to November 2018.The patients were grouped as underweight(<18.5 kg/m2),normal weight(18.5-23.9 kg/m2),and overweight/obesity(≥24 kg/m2).The primary outcome was all-cause hospital mortality.RESULTS Among 2327 patients,297(12.8%)were underweight,1013(43.5%)normal weight,and 1017(43.7%)overweight/obesity.The all-cause hospital mortality was 4.6%(106/2327).Mortality was lowest in the overweight/obesity group and highest in the underweight group(2.8%,vs 5.0%,vs 9.1%,P<0.001).All-cause mortality of overweight/obesity patients was lower than normal-weight patients[odds ratio(OR)=0.535,95%confidence interval(CI)=0.334-0.855,P=0.009],while the allcause mortality of underweight patients was higher than that of normal-weight patients(OR=1.886,95%CI:1.161-3.066,P=0.010).Multivariable analysis showed that abnormal neutrophil counts(OR=2.38,95%CI:1.55-3.65,P<0.001),abnormal albumin levels(OR=0.20,95%CI:0.06-0.72,P=0.014),high-risk Confusion-Urea-Respiration-Blood pressure-65 score(OR=2.89,95%CI:1.48-5.64,P=0.002),and intensive care unit admission(OR=3.11,95%CI:1.77-5.49,P<0.001)were independently associated with mortality.CONCLUSION All-cause mortality of normal-weight patients was higher than overweight/obesity patients,lower than that of underweight patients.Neutrophil counts,albumin levels,Confusion-Urea-Respiration-Blood pressure-65 score,and intensive care unit admission were independently associated with mortality in patients with community-acquired pneumonia.
摘要BACKGROUND For resectable hepatocellular carcinoma(HCC),radical hepatectomy is commonly used as a curative treatment.However,postoperative recurrence significantly diminishes the overall survival(OS)of HCC patients,especially with microva-scular invasion(MVI)as an independent high-risk factor for recurrence.While some studies suggest that postoperative adjuvant therapy may decrease the risk of recurrence following liver resection in HCC patients,the specific role of adju-vant therapies in those with MVI remains unclear.AIM To conduct a network meta-analysis(NMA)to evaluate the efficacy of various adjuvant therapies and determine the optimal adjuvant regimen.METHODS A systematic literature search was conducted on PubMed,EMBASE,and Web of Science until April 6,2023.Studies comparing different adjuvant therapies or comparing adjuvant therapy with hepatectomy alone were included.Hazard ratios(HRs)with 95%confidence intervals were used to combine data on recurrence free survival and OS in both pairwise meta-analyses and NMA.RESULTS Fourteen eligible trials(2268 patients)reporting five different therapies were included.In terms of reducing the risk of recurrence,radiotherapy(RT)[HR=0.34(0.23,0.5);surface under the cumulative ranking curve(SUCRA)=97.7%]was found to be the most effective adjuvant therapy,followed by hepatic artery infusion chemotherapy[HR=0.52(0.35,0.76);SUCRA=65.1%].Regarding OS improvement,RT[HR:0.35(0.2,0.61);SUCRA=93.1%]demonstrated the highest effectiveness,followed by sorafenib[HR=0.48(0.32,0.69);SUCRA=70.9%].INTRODUCTION Hepatocellular carcinoma(HCC)is the sixth most common malignant tumor in the world and ranks third in terms of worldwide malignant tumor mortality rates in 2020[1].Curative treatments for HCC include ablation,radical hepatectomy,and liver transplantation.However,ablation is suitable only for early-stage HCC patients,who represent a small percentage of the overall HCC population.Although liver transplantation serves as the optimal treatment for HCC patients,the scarcity of donor organs restricts the availability of this procedure.Therefore,hepatectomy is the most commonly employed curative treatment for resectable HCC.Unfortunately,the 5-year recurrence rate for patients who undergoing hepatectomy ranges from 50%to 70%[2,3].Recurrence of HCC is associated with several risk factors[4],including single nodule>5 cm,vascular invasion,and multiple nodules.Among these factors,microvascular invasion(MVI)is an independent risk factor for recurrence.MVI is defined as the presence of cancer cells in the lumen of endothelium-lined vessels,typically in the small branches of the portal and hepatic veins of the paracancerous liver tissue,visible only under the microscope[5].Previous studies have shown that among HCC patients who underwent hepatectomy,those with MVI had a higher risk of recurrence and shorter overall survival(OS)than those without MVI[6].Several studies have indicated that adjuvant therapy following curative hepatectomy can prevent recurrence and improve OS in HCC patients with MVI.These postoperative adjuvant therapies include transarterial chemoembolization(TACE)[7],sorafenib[8],hepatic artery infusion chemotherapy(HAIC)[9],and radiotherapy(RT)[10].However,the existing studies mostly compare individual adjuvant therapy with hepatectomy alone.Direct or indirect comparisons between the various adjuvant therapies are lacking.Therefore,we performed the network meta-analysis(NMA)to compare the relative efficacy of each adjuvant therapy to determine the optimal treatment.
